Ms. Betty Usher is an experienced personal care home provider and caregiver. She has provided care for the elderly and disabled for
over 11 years.

Caregiver Experience:
Prior to opening her personal care home, Ms. Usher was an Emergency Room Technician at local hospitals. She has provided direct
care to individuals in her personal care home since 2005. Ms. Wanda Tubbs is a Certified Nursing Assistant and has been a caregiver
at the home for more than seven years.
Home Description:
Usher Residential & Community Services is a six-bed home with two private and two shared bedrooms, three full bathrooms, living
area and exercise/activity area. The home has two ramps.
Neighborhood & Recreational Activities:
The home is located in DeKalb County near activities such as church, library, movies, restaurants, shopping, and the senior center
and park. At home, the residents enjoy Bingo, chess, exercise ball and cards.
Meals & Transportation:
Usher Residential & Community Services offers three home cooked meals and two snacks daily. Meals are tailored to special diets.
Special arrangements may be made for transportation to medical appointments.

The home is not owned by Altrus or any of its principals. Altrus is
an enrolled provider of certain state Medicaid waivers and in this
capacity under contract with the state of Georgia provides supervision of care for the CCSP, SOURCE, ICWP and COMP waivers.
