FAMILY REUNON REGISTRATION FORM
To register for the 2014 Family Reunion, please do one (1) of the following:
 Website: - Purchase T-shirts, Park Parking Ticket, Registration & Banquet Ticket:
If you have not registered with the GilesMitchell Connection please do the following first:
• Go to www.gilesmitchellconnection.org
• Click on the Settings button (top right corner) to find the Register option
• Submit the Registration form to receive user name and password for the site in an email
• Once you get your temporary password, log in (you will be prompt to change your password
• Click on the Store Tab (use the temporary password to log in)
• You can change the password in the store to match the password that you just changed
If you have already registered with the GilesMitchell Connection and have a user name and password,
please do the following

Login to the Store using your User Name and Password
Purchase using PayPal (There will be a minor handling fee charged by PayPal)

Agreement
1. RESERVATIONS: Reservations will need to be made by: 06.25.14
After that date, the group block will be released and the rooms will go back into inventory and will
be sold, first come first serve at the published rate.
2. CANCELLATION AND PERFORMANCE: Cancellations must be done by 05.31.14, or the
individual whose name is on the reservation, their credit card will be charged one night room
and tax.
3. OUTSIDE FOOD AND BEVERAGE: Due to state law, you may not bring to the Hotel alcoholic
beverages. You must obtain prior approval from us before you bring in any food or non-alcoholic
beverages from outside sources.
4. SECURITY: Hotel is not responsible for any loss or damage to your property. For the safety of
persons and property, no fireworks or incendiary devices may be used indoors at the hotel.
Group agrees to comply with all applicable federal, state and local laws including health and
safety codes and federal anti-terrorism laws and regulations. Group agrees to cooperate with
Hotel and any relevant governmental authority to ensure compliance with such laws.
5. INDEMNIFICATION & INSURANCE: To the extent permitted by law, each individual having a
reservation at this hotel, will agree to protect, indemnify, defend and hold harmless the Hotel, The
Staybridge Suites, and the owner of the Hotel, and their respective employees and agents against all
claims, losses or damages to persons or property during their stay.
