Louisville is the 42nd largest metro in the United States with a population of
1.25 million and a labor force of over 643,000. It generated $54.4 billion in commerce in 2012 with a per capita real GDP of $43,500. National per capita real
GDP averages $45,604. Louisville metro’s economy lost more than 58,000 jobs
between June 2007 and February 2010 as a result of the Great Recession, but in the
four-plus years since has recovered all those losses and added another 37,000 jobs.
The metro’s largest employer is United Parcel Service, and Louisville is home to
international restaurant companies including Yum! Brands, Papa Johns, and Texas
Roadhouse. However, six of Louisville’s 10 largest individual employers are in of
the healthcare sector, whose leading member is Humana. Norton Healthcare and
KentuckyOne Health are top fi e employers also.
Manufacturing is the second largest industry by employment in the region with
nearly 12 percent. Growth is driven by the auto industry, specifically Ford and
Toyota vehicle assembly plants, as well as appliance manufacturing expansion by
General Electric.
Louisville metro has nearly 3,900 health-related establishments, making up
almost 15 percent of all businesses. Collectively, they employ over 100,000
persons with an annual payroll topping $3.8 billion. All major components of
Louisville’s healthcare industry have posted growth since 2006.
n (Figure 1) Average annual pay in the Health Care and Social Assistance
category locally is $41,295. Construction ($46,321) and Real Estate ($39,454)
provide similar average compensation.

n (Figures 4, 5) The University of Louisville had 123 grants from the National
Institutes of Health totaling over $47 million in 2013. Grant numbers were steady,
but UofL dropped in nationwide NIH rankings from 76 in 2004 to 83 in 2013. In
comparison, the University of Kentucky has 231 grants totaling over $77 million
and ranks 53rd.
n (Figures 2, 3) Healthcare accounts for 16.3 percent of the Louisville area’s jobs. Th
sector increased 19.3 percent between 2003 and 2012. Louisville-based Humana
and Kindred are two of the 23 Fortune 1000 headquarters of health-related
companies in the 12 comparison metros. Nashville hosts six major headquarters,
while Cincinnati and Columbus each contain four. Six publicly traded healthcare
companies reside in Louisville with a total market value of over $36 billion.
n (Figure 6) Private sector research and development in Louisville ranked last
among similar metros in private research and development in 2006 and has experienced only marginal increases. Louisville has seen growth in the previously nonexistent pharmaceutical industry with four new establishments and has maintained
the same number of medical device companies. Louisville lags in small business
technology grants (13) totaling $5.8 million, ranking in the middle of comparison
metros and behind Lexington’s $8.1 million.

per capita are $2,406, an increase of 2.16 percent from 2004 to 2012, suggesting
hospital margins have decreased over time.
• Hospital and Ambulatory Care revenues increased by 9.3-10.2 percent from
2002 to 2007. Nursing & Residential Care Facilities revenues experienced a surge
of 24.9 percent.

• Louisville’s healthcare sector was immune to the most recent recession, according to the Brookings Institution, experiencing 4 percent job growth both during
the recession and current recovery. Only the leisure and hospital industry grew
faster with a net 10.7 percent employment growth.
• All health sector job categories are experiencing growth with the leaders being
Home Health Aides (64 percent), Lab Technicians/Medical Diagnostics (42 percent),
and Physical Therapists (35 pe cent)/Physical Therapist Assistants (40 pe cent).
• Health Care and Social Assistance Industry per capita payroll in metro Louisville is $2,803, an increase of 8.6 percent from 2002 to 2012 but placing the
Louisville metro in the lower half of similar metros.
• Relative to similar metros, the Louisville metro has fewer internists (-75 percent), massage therapists (-62 percent), and home health aides (-61 percent). However, metro Louisville has a surplus of therapists and medical assistants – in excess
of 90 percent more than similar metros.
• Chiropractors (-26 percent), optometrists (-17 percent) and dentists (-14 percent) are under compensated relative to the average in similar metro areas. However,
therapists receive the greatest location premiums with the top three being physical
therapists (21 percent), podiatrists (20 percent), and other therapists (19 percent).

metro areas for healthcare research and development. Louisville contains 37
scientific research & development establishments while similar metros have from 31
to 176 establishments with an average of 70. Louisville has 2.96 scientific research
establishments per 100,000 residents compared to a national average of 5.55 per
100,000 residents.
• However, the metro area increased the number of pharmaceutical & medical
manufacturing establishments from zero in 2004 to four in 2012. Similar metro
areas have an average of seven.
• Themedical equipment and supplies industry employs 1,036 at 35 establishments,
which is near the median of similar metro areas. Average annual pay in this sector is
$55,927, situating Louisville above the $51,413 average among similar metros.

Summary
The Louisville metro healthcare service sector continues to grow. Established
hospital networks such as Norton Healthcare Inc. and Baptist Health have made
capital improvements reaching the suburban areas, and the nursing industry ranks
second among similar metropolitan areas in establishments, revenue and pay.
Louisville still lags in research and development.
However, there is a growing
pharmaceutical sector that

experienced the largest increases in job growth and wages. Compared to similar metros, Louisville’s healthsector generates similar levels of revenue but carries payrolls in the lower quartile.
To improve educational attainment of the labor force and increase payrolls and
productivity in its healthcare sector, Louisville Metro should continue supporting the
55,000 Degree Initiative (55000degrees.org/). UofL has new undergraduate degrees
in public health and an MBA with a health concentration at the graduate level.
The Louisville Metro should build on its strong Nursing Facilities sector, which
ranks second among similar metropolitan areas.
Additional tax incentives for medical research and manufacturing establishments
could bolster the underdeveloped research and development sector. Increased grant
writing training is likely needed.
Louisville should identify the reasons its new pharmaceutical companies decided
to locate in the metro to encourage growth in private research.

Integrative medicine brings patient and practitioner together in a dynamic partnership dedicated to
optimizing health and healing. This approach focuses
on the whole person, recognizing that the subtle interactions of mind, body, spirit and community have
a direct impact on vitality and well-being.
Rather than managing disease, integrative medicine works to optimize health and treat the whole
person instead of treating symptoms. Integrative, or
complementary, therapies are used alongside mainstream medicine, not as replacements or alternatives,
and may include nutrition, acupuncture, stress reduction, therapeutic touch, mindfulness training, guided
imagery, yoga and tai chi.
Integrative therapies also will be a focus for migraine suffe ers needing inpatient care. The hospital
will have six beds dedicated for patients experiencing
headache pain that is not responding to outpatient
treatment. The rooms will be designed to minimize
light and sound, and alternative therapies will be used
to help with chronic pain.

we’d already taken extraordinary steps in the past
few years in terms of combating cancer incidence

support and approval of the NCI has already made
a huge impact in terms of both research and our
clinical care.”

Markey’s already robust roster of specialists now

Cancer Research
Markey landed a major established research team
in metabolomics last year, who brought with them
more than $18 million dollars in funding. One of
the major focuses of the team’s work is to develop
early diagnostic approaches for lung cancer based on
metabolism markers.
Over the past two years, Markey has increased its
funding from the NCI by 27 percent and from other
National Institutes of Health divisions by 16 percent.
Since the end of calendar year 2012, Markey’s total
research funding from both peer-reviewed and nonpeer-reviewed sources has increased by $7.3 million.
Moving forward, Evers notes that his team will
continue to seek out new clinician-scientists who
have experience in clinical trials and early phase drug
development, with the goal of significantly increasing
the number of patients who participate in trials.
Another emerging field of research for Markey is

uses gentle, healing touch to help relieve symptoms associated with cancer treatment. Markey
offers several services, including a dietitian, financial counselors and social workers to make
sure all of a patient’s needs are met.

state and beyond through the Markey Cancer Center
Affiliat Network. The Affiliat Network is a group
of healthcare facilities that provide high-quality
cancer services and programs in their communities
with the support and guidance of the UK Markey
Cancer Center, allowing patients to receive their care
closer to home. The network currently comprises 12
hospitals across the state of Kentucky.
The Future
of Cancer Care
NCI designations are renewable every fi e years,
and Evers hopes that Markey’s next application will
be for comprehensive status. Currently, 41 of the 68
total NCI-designated cancer centers in the country
hold a comprehensive cancer center status.
To reach that level, Markey has a long to-do list,
including increasing cancer-related funding, accruing more patients into clinical trials (including
pushing these trials out into the state via the affiliat
network), and maintaining and increasing focus on
Appalachian Kentucky.
“Our progress in the past year has been
spectacular, but we can – and should – do more,”
Evers said. “As the only NCI-designated cancer
center in Kentucky, it’s our responsibility to be the
leader in cancer care and to always seek out new
ways to improve rates of cancer incidence and death
in the state. Earning a comprehensive cancer center
designation from the NCI will be another big step
in that direction.”

Weighty Issues
In 2012, more than 29 million people in the
United States suffe ed from diabetes, and that number continues to grow. In Kentucky, 1 in 10 residents
have been diagnosed with diabetes, while more go
undiagnosed. Since its creation in 2008, UofL’s Diabetes and Obesity Center has sought ways to address
the profound effect these diseases have on people’s
health and quality of life.
“We approach diabetes and obesity not as individual disease states, but as pieces of a larger, more
comprehensive puzzle,” said Aruni Bhatnagar, MD,
center director and the Smith and Lucille Gibson
Chair in Medicine. “Thus, our researchers are working to understand diabetes and obesity as the outcomes of a more comprehensive dysfunction that
profoundly affects all major organs and increases the
risk of developing heart disease and cancer.”
Diabetes and Obesity Center scientists have “made
significant gains in our understanding of diabetes
and obesity,” said David L. Dunn, MD, PhD, executive vice president for health affairs. “They have revealed an entirely new mechanism to regulate glucose
sensitivity and a new avenue for preventing obesity.
They also have increased understanding of how en-
zymes that regulate glucose metabolism contribute to
secondary diabetic complications, such as heart
failure and the narrowing of blood vessels.”
The center’s ultimate goal, Bhatnagar said, is to
discover new means for preventing and treating
diabetes and obesity. To this end, the center recently received a fi e-year $11.25 million Center of
Biomedical Research Excellence (COBRE) grant
from the National Institute of General Medical
Sciences, its second since the center’s inception.
The need for new prevention methods and treatments grows daily, Bhatnagar said. “Diabetes and
obesity are the most significant health threats of our
age. Over 90 million people in the United States are
obese, while 18 million adults are living with type 2
diabetes. These epidemics are rapidly eroding recent
gains in longevity.
“Diabetes doubles the risk of cardiovascular
disease. In fact, an astonishing 60 ‒ 70 percent of
diabetic patients die of heart disease,” Bhatnagar
said. “Our team is making discoveries every day that
provide better understanding of the link between
cardiovascular disease and diabetes and obesity. The
more comprehensive our understanding of these
conditions, the closer we come to developing an
effecti e strategy for treatment and prevention.”

“Thebest way to get a doctor to a rural area is to get a
medical student from a rural area.” Theprogram funnels students from small towns who want to study
family medicine back to their small towns.
Jessup says the program was fast-paced but
manageable. In fact, Jessup was able to find the time
to get married during her time in the RMAT program.
Jessup rejects the label of a pioneer and gives the
credit to others. “I would consider all the faculty and
staff who envisioned the program as the trailblazers,”
she said. She views herself as just another kid who
dreamed of becoming a doctor, and hopes that more
medical students will follow in her footsteps at UofL.
Two students are taking part in the RMAT program,
and Crump says several future students expressed an
interest in the program.
The RMAT program is the newest tool in UofL’s
effo t to bring health care to rural communities. In
1974, UofL helped found the Kentucky Area Health
Education Centers. A national program begun
in 1971, the aim is to bring health care education
services, providers and students to medically underserved areas of the Commonwealth. The program
gives people of those areas access to health care they
might otherwise not have, while students receive
medical training they might not otherwise gain.
Judging by the UofL School of Medicine Class of
2013, the program is working: 31 percent of class
members said they plan to practice in an underserved area, and 83 percent will volunteer in free
clinics for the underserved.
Implementation of the Affo dable Care Act and
the establishment of the Kentucky health care exchange have resulted in more than a quarter million
people in the Commonwealth acquiring insurance.
This significant reduction in the uninsured does not
guarantee access to a physician. In fact, Kentucky is
predicted to have a shortage of some 3,000 physicians by 2020. This shortage is especially prevalent
in rural areas.
In 2011, the University of Louisville School
of Medicine began the Rural Medical Accelerated
Track (RMAT) to address these concerns. In May,
Ashley Jessup became the first graduate of the
program, which enables students to finish medical
school in three years, reducing cost and time commitments for rural students who plan to open practices in small towns in Kentucky. Proponents of the
RMAT hope that successes like Jessup’s will place
additional doctors where they are needed most.
William Crump, MD, associate dean of the UofL
School of Medicine, says that outcome is likely. He
stressed that “placing one more physician in a county
may move it from being an underserved to an adequately served county.” Crump says the RMAT program will definitely help reduce Kentucky’s shortage
of healthcare providers in rural areas. In his opinion,

Recently, Sheila Guelda, MD, founder of
Springhurst Kids’ Doc on North Hurstbourne Lane
in Louisville, joined the pediatric faculty. Guelda
graduated from the UofL School of Medicine in 1980
and completed a pediatric residency at UofL in 1983.
“We think this partnership will be mutually beneficial to the university and to Dr. Guelda and her
patients,” said Gilbert Liu, MD, UofL Pediatrics division chief. “Thiswill improve patient access to UofL
pediatric specialists’ expertise and creates learning opportunities for our trainees. Dr. Guelda benefits from
the university’s latest innovations in medical care,
teaching and research.”
In addition to the typical care that comes with
taking a child to a pediatrician, UofL Pediatrics’ services also include social work, psychology, dental care,
home health, speech therapy, WIC nutrition services,
and legal counseling. “We think an arrangement that
offers ‘one-stop shopping’ for multiple health care
providers will be good for all our patients,” Liu said.
Historically, trainees spent time in community
pediatric practices, but these practices may struggle
to host students because of changes in the healthcare
landscape. With the expansion of sites, the uncertainty regarding training sites should be eliminated. “It is
part of our educational mission to expand primary
care opportunities,” Rabalais said.

Telemedicine’s priorities are increased
provider-patient contact and better outcomes

telemedicine products is
Anywhere Care, which
KentuckyOne Health launched
Nov. 1. It gives users aroundthe-clock access to doctors
and nurse practitioners for
medical diagnosis, treatment
planning from any Internetconnected computer with a
web-cam and microphone.
It costs $35 per use.
In 1956, the movie “Forbidden Planet” introduced audiences to Robby the Robot. A triumph of
special effects for that time period, the self-ambulating
robot gave the world some of the most enduring popculture images of how technology may one day be
applied to serve people’s needs.
Perhaps, though, a more prophetic vision of the
use of robotic technology in contemporary healthcare
may be seen in a 1976 movie, “Logan’s Run.” One of
its critical scenes depicts a plastic surgeon operating

a machine equipped with robotic arms and surgical
instruments designed to perform various types of
enhancement surgeries.
The machinery was huge and clunky, but the idea
of applying imaging and robotic technology as an
extension and enhancement of a medical provider’s
skills was visionary for its time.
In a very real sense, the real world application of
“robot” technology in contemporary healthcare has
matched or even exceeded the vision of science fi -
tion. When it works best, the ever newer technology
allows more frequent and better contact between providers and the patients who need their expertise – with
participants quickly and naturally focusing on their
human interaction rather than the high-touch process.
Visitors to the Kentucky Clinic in Lexington are, by
now, so familiar with the self-propelled courier robots,
Bot-1 and Bot-2, traveling their pre-programmed paths
between the medical lab and the old Chandler Medical
Center that they rarely draw much notice.

Medical Robots and
Cart-Based Telemedicine
In 2007, the then-Owensboro Medical Health
System – now simply Owensboro Health – in collaboration with Dr. Kerri Remmel, chief of the University of Louisville’s Neurology Department, was one of
the first hospitals in Kentucky to experiment with an
exciting new application for robot technology.
The hospital was facing a crisis, said Dr. Robert
Knight, director of the OH Emergency Services
Department. One of its two staff neurologists had
accepted a new position, which left the hospital
short-handed in a critical specialty. Meanwhile, a new
interventional treatment called thrombolytics was
being introduced for acute stroke patients.
“The e are plenty of good outcomes that arise from
this treatment, but there are also plenty of potentially
bad outcomes because it involves increased bleeding
during the procedure,” Knight said. As ED director,
he preferred to have access to a specialist for a consult
before performing thrombolysis. “It was impossible
for our one remaining staff neurologist to be on call
24-7 to advise us.”

“This was a no brainer
for us. UofL was buying the
equipment and giving it to
us with no risk. What we
got out of it was round-theclock access to a specialist at a
time when we needed them,”
Knight said.

still being marketed, is outfitted with a high-definitio
video screen that projects an
image of the doctor’s face and
a moveable video camera on
the top of the video screen.
Remmel can control its
camera and move the robot down halls and into rooms
using a laptop or tablet computer in her Louisville office, Knight said. It has an electronic stethoscope and
attachments on the machine that allowed for two-way
conversations – in privacy if necessary – between Remmel and ED doctors or the patient in Owensboro.
“In the first few years we had it, we used that robot
a lot,” Knight said. “It ‘rounded’ on patients admitted
by the neurologist when our on-staff neurologist was
off. We used it for consults with Dr. Remmel in the
emergency department, and the robot was also used to
monitor premature newborns in our neonatal unit.”
Patient response was uniformly positive. Usually
after a few moments, patients and doctors reported
that the technology “disappeared.”
“We got no patient push back whatsoever about
the unit,” he said. In fact, one unexpected benefit to
the patient was in cases when they would be transferred to Louisville. Patients had already established
a familiar relationship with Remmel through the machine. “They had seen and talked with her on screen;
they knew her. In all the time that we really used the
robot, it worked well for us.
Owensboro Health’s use of the robot declined
after its successful recruitment of new neurologists,
decreasing the need for direct access to the UofL Neurology department.
“Our new facility has allowed us to increase our
medical staffin and brought with it new specialists we
didn’t have before,” Knight said, referring to the $385
million, 477-bed medical center that opened in June.

In many hospital pharmacies, robots perform
service like an elaborate vending machine. They store,
organize and track all the medications that a hospital
keeps in stock, use bar-code readers to fill prescriptions,
and even prepare the night cart for inpatient distribution. When not engaged in an order, the robots are also
programmed to monitor expiration dates and properly
dispense with out-of-date pharmaceuticals.
On billboards in most of Kentucky’s larger cities,
hospitals frequently buy advertising space to promote
their acquisition of da Vinci Surgical Robot Systems.
Since 1999, when Intuitive Surgical Inc. of Sunnyvale,
Calif., first introduced the da Vinci surgical robot, its
use has exploded across the country. The stationary
system was designed to enhance minimally invasive
cardiac surgery by combining high-definition imaging and specialized microsurgical tools that can reach
places within the body a surgeon’s hands can’t.
Since its introduction, da Vinci surgical systems
have been adapted to perform a wide array of minimally invasive procedures, decreasing disruptions that
complicate treatments and healing for kidney and
bladder disorders, cancer treatments and more.
The technology is now almost a decade old and the
machines themselves very similar to that classical conception of science-fiction-type robots. Their functions
are more refined and precise, but they are still only elaborate devices designed to relieve medical professionals
of tedious functions or to extend a surgeon’s skills.
Healthcare reform effo ts have fueled the
development of what can best be described as medical
outreach technology.

most of the tasks at its medical center
inpatient medications pharmacy such as
storage, dispensing, returning, restocking
and crediting of bar-coded, unit-doses.
However, he said, this type of robot still could be
useful in a smaller community hospital that has less
access to specialists – or even primary care doctors.
Ultimately, the robot proved useful for the outreach function it could perform in the absence of a
local specialist much more so than for the fact that
it moved on its own and acted very similar to robots
seen in science fiction
A diminished use of the robot in recent years
reflects new developments in how medical programs
are approaching the advances in broadband connections and shared information in communication and
imaging technology.
“Robots are very sexy technology,” said Rob
Sprang, director of the University of Kentucky’s
Telehealth Program. “However, when you consider
the time it takes to guide a robot from one patient
room to another, it doesn’t seem to be a very productive use of a physician’s time.”
While less suggestive of the human form, newer
cart-based telemedicine devices perform the same
function as a robot. The diffe ence is its components
are wheeled around on site manually rather than
driven remotely by a motor.

desperate need of secure communication solutions.”
The Health Information and Patient Privacy Act
that required stringent security protocols to protect
patient information didn’t bother the company. It had
been producing secure communication in top-secret
circles for nearly two decades, Jennings said.
“The e are so many areas in broadband communications that are not secure. We encountered situations

of telemedicine since its inception in the mid-1990s.
In those early days, components were as large, clunky
and fitfully reliable as their early science fiction counterparts. The technology held telemedicine systems
back. In the last seven years, the technology has not
only improved dramatically, it has gotten cheaper and
more reliable.
“Where we used to be proud to handle the transfer
of megabytes of information, the technology is handling gigabytes and terabytes today. Also, when you
turn something on, you know it’s going to work,”
Sprang said.
Companies with a background in secure videoconferencing are finding new markets with telemedicine.
One of the new businesses to explore this frontier is
Trinity Audiovisual Systems, based in Louisville.
Trinity began life as a secure video-communications company with lucrative federal government and
law enforcement contracts. In an age in which it is
becoming increasingly difficult to maintain top-se et
communications, Trinity emerged as a market leader
in the field
John Jennings, chief technology office and assistant vice president of Trinity’s Midwest operations,
said the company recognized three years ago that it
needed to diversify and found the healthcare industry
was a natural fit
“Healthcare is becoming a shared technology industry,” Jennings said, “but it needs a secure environment
for such things as electronic health records and private
communications among healthcare providers, their patients and in consult with other doctors. It was logical
for Trinity, a company specializing in secure commu-

patients using Skype. Internet social media platforms
are not even remotely secure platforms,” he said.
Two years ago, Trinity introduced its own
portable telemedicine platform. Its engineers and
designers conferred with a client concerning their
needs and created a portable platform that a hospital
staff person can move easily from one hospital bed
to another.
“These days, we’re not being asked so much for
devices as much as we are being asked for secure software systems that can be uploaded to a laptop, tablet
or even a smart phone,” Jennings said. “We can work
with any kind of endpoint device.”
“I have seen fully automated robots that do much
the same thing as a telemedicine platform, but I have
also seen price tags of nearly $100,000. Portable units
tend to run much cheaper,” Jennings said.
Even In-Touch – the leader in robot technology –
has developed a manual version of its RP-7 telemedicine robot.
Trinity is finding that the market for their products is more with long-term-care facilities and homecare companies than hospitals. The pilot project that
led Trinity to more fully develop its telemedicine
service came with a contract between it and Signature HealthCARE, the progressive Louisville-based
company that runs several long-term-care facilities.
Buyers are leaning toward telemedicine carts with a
mounted video screen, high-resolution cameras and
microphones, and a fi ed tablet or laptop device and
other related acute-care peripherals. Systems are getting to the point where patients and doctors can have
direct remote encounters at the workplace, in the
home, at a long-term-care center, or even in another
doctor’s offic
Regional and community hospitals are looking
at portable telemedicine units as a way to connect
patients with specialists without forcing either party
to travel to see each other.

Care service this month.

Anywhere Care allows Kentuckians to get in contact with a healthcare provider by phone or by web
camera 24 hours a day, seven days a week, including
when they are away from home.
Thoughin its earliest stages of implementation, the
Anywhere Care service takes telemedicine to the next
logical evolutionary step. People experiencing a sudden illness no longer need to go to a clinic, emergency
room or physician’s offic to consult a physician. Via
a toll-free number, they can schedule a phone call or
video chat on their computer or tablet.
Anywhere Care participant patients will be explaining
their symptoms with a primary care physician or nurse
practitioner within 30 minutes of their call, according
to KentuckyOne Health, the state’s largest care system
with more than 200 locations and 15,000 employees.
Furthermore, the practitioner being consulted can prescribe medications (with the exception of controlled
substances) or recommend over-the-counter treatments.
Patients pay directly for the telemedicine consult,
which is $35. Health insurance is not billed for Anywhere Care services, but the simple flat rate is cheaper
for the patient than an ER visit or a visit to an urgent
care clinic. If symptoms warrant, the practitioner will
refer patients for a follow-up visit to a clinic.
KentuckyOne’s Anywhere Care service is the result
of a partnership between the Kentucky healthcare fir
and Carena Inc., a Seattle-based telemedicine company.
KentuckyOne Health also recently launched
special telehealth clinics in Wolfe and Powell counties equipped to let patients there have videoconference visits with remote doctors and practitioners.
Other Future Directions
If the direction of medical reform continues along
the path blazed for it in the Affo dable Care Act, then
Sprang said remote patient monitoring may be one
of the bright futures for applied medical technology.
“We are moving to a place in a reformed system of
healthcare where we no longer wait until a patient gets
sick and then care for them, but to actually try and
keep patients from getting sick,” he said.
The vast improvements in Bluetooth and Internetbased communication that have fueled the rapid development of telemedicine are enabling hospitals to
introduce advanced patient monitoring systems in
their intensive care units.
Owensboro Health announced in October that its
32 ICU beds would receive 24/7 remote monitoring
from the Christ Hospital Critical Connections program, which is based in Cincinnati. The service does
not replace ICU staffing but it does use a powerful
in-room video monitoring system to detect changes
in skin color or eye movement or other warning signs
of a potentially fatal event.
Another example of applied patient monitoring systems involves a study conducted in North Carolina with
some 700 patients who, in a 90-day period, accounted
for an inordinate number of inpatient stays at a hospital.
Thesepatients, Sprang said, had a tendency to resist
altering their behaviors to improve their health. Th
hospital conducting the study outfitted participants’
homes with a cellular or Bluetooth-based communication hub that linked to a patient-monitoring station at
the hospital. The home hub connected to an ordinary
bathroom scale, a blood pressure cuff and a pulse oximetry device to monitor a patient’s progress after an
inpatient hospital stay.

Each day, participants would use the connected
devices and staff at the remote monitoring station
would review the results. If there were warning signs –
an abnormal weight increase, a rise in blood pressure
– a provider called the home and to ask the person if
they had been engaging in behaviors that contributed
to their poor health.
The hub remained with the patients 90 days, and
their hospital re-admissions rate dropped 65 percent
below the norm. Thiswas only one study, but application of basic telemedicine technology “seemed to have
a positive result with people,” Sprang said.
It suggests one future direction for medical
technology could be a “virtual” return to the ageold home visit doctors conducted in the days
before there were even telephone lines strung across
the landscape. Technology has made an incredible
impact on the quality and efficienc of care, but
at its heart, the circuitry is all about creating more
effecti e human connections.

What is Glycemic Index?

Theglycemic index, simply put, is a measure of how
quickly a food causes our blood sugar levels to rise.
The measure ranks food on a scale of 0 to 100.
Foods with a high glycemic index (GI) are quickly
digested and absorbed, causing a rapid rise in blood
sugar. These foods that rank high on the GI scale are

Diets centered on mostly low-GI foods can make it
easier to achieve and maintain a healthy weight, since
these foods keep us feeling fuller, longer. Low-GI diets
have also been shown to improve insulin resistance,
and lower glucose, cholesterol and triglyceride levels
in people with type 2 diabetes.
One exception to the recommendation of a mostly
low-GI diet is after intense or prolonged exercise.

can actually be more beneficia

rapidly digested.
Glycemic Index: An

but Useful Tool
often — but not always — high in processed carbohydrates and sugars. Pretzels, for example, have a glycemic index of 83; and a baked potato without the
skin clocks in at 98.
Meanwhile, foods with a low GI are digested and
absorbed at a slower rate, and, subsequently, cause a
slower rise in blood sugar levels. Theseare typically rich
in fibe , protein and/or fat. Examples of these include
apples with a glycemic index of 28, Greek-style
yogurt at 11, and peanuts at 7. Keep in mind
that a low GI doesn’t mean a food is
high in nutrients.

who’s ever eaten food knows, this isn’t always how we
eat. Sure, a bag of pretzels may be a stand-alone snack,
but how often do we eat just a plain potato with
nothing else?
Add a lean steak or a piece of salmon, a side of
broccoli and a salad with vinaigrette, and the protein,
fiber and fat will all serve to lower the glycemic index.
In addition, the glycemic index doesn’t take into
account how much we’re actually consuming. Th
GI value of a food is determined by giving people a
serving of the food that contains 50 grams of carbohydrate minus the fibe , then measuring the effect on
their blood glucose levels over the next two hours.
A serving of 50 grams of carbohydrate in one sitting is reasonable for a food like rice, which has 53
grams of carbs per cup. But for beets, a GI ranking
of 64 is a little misleading. Since beets have just 13
grams of carbs per cup, we would need to consume
nearly 4 cups of beets in order to cause that spike in
blood sugar levels.
An Alternative to Glycemic Index
Glycemic load (GL) is a formula that corrects for potentially misleading GI
by combining portion size and
GI into one number. Th
carbohydrate content
of the actual serving
is multiplied by the
food’s GI, then that
number is divided by 100.
So for a cup of beets, the GL
would be: 13 times 64 = 832
divided by 100 = a GL of 8.3.
As a frame of reference, a GL
higher than 20 is considered high, between 11 and 19 is considered
moderate, and 10 or less is
considered low.
The bottom line: Even
though the glycemic index
isn’t a perfect system, it can be a
useful tool to identify lower-glycemic foods that are
often more nutrient-dense, as well as what foods are
higher in refined carboh drates.

Inflammation is a protective process you are probably more familiar with than you think. It’s the body’s
method of healing itself in response to an injury or
exposure to a harmful substance. This is useful when,
for example, skin is healing from a cut; however,
inflammation is not always beneficia
Chronic (or ongoing) inflammation occurs when
the immune system attacks the body’s healthy cells
leading to autoimmune diseases such as rheumatoid arthritis, states of immune deficiency including
Crohn’s disease or skin conditions including psoriasis.
Underlying chronic inflammation also may play a role
in heart disease, diabetes and Alzheimer’s disease.
Evidence supporting the impact of specific foods
on inflammation in the body is limited. We know
that some foods have the capacity to suppress infla mation, but it’s unclear how often and how much
is needed for this benefit. Though there’s promising
research for the impact of foods such as fatty fish
berries and tart cherry juice, but beware of anything
touted as an anti-inflammato y miracle.
Eating to Reduce Inflammation
Luckily, eating with reduced inflammation in mind
may be easier than you think.
n Let fruits and vegetables make up at least half
your plate at meals.
n Opt for plant-based sources of protein including
beans, nuts and seeds.
n Choose whole grains instead of refined ones.
Give up white rice and replace it with brown, black or
wild rice; whole oats or barley for cream of wheat; and
whole-wheat bread instead of white.
n Pick heart-healthy fats as opposed to not so
healthy ones. Olive oil, avocados, nuts and seeds are a
few delicious choices.
n Choose fatty fish such as salmon, sardines and anchovies to get a heart-healthy dose of omega-3 fatty acids.
n Season your meals with fresh herbs and spices.

Though diet is important, it’s not the only factor.

Do you know if you’re eating the
proper portion size? Get out the measuring cups and see how close your
portions are to the recommended serving size. Using smaller plates, bowls
and glasses can help you keep portions
under control. Use half of your plate
for vegetables, a quarter for grains and
a quarter for lean meat or protein.
To complete the meal, add a glass of
fat-free or low-fat milk and a serving
of fruit for dessert.

of whole-wheat toast. Find more quick breakfast ideas
for when you’ve hit the snooze button too many times.
n Drink More Water
Our bodies depend on water to regulate
temperature, transport nutrients and oxygen to cells,
carry away waste products and more. For generally healthy people who live in temperate climates,
the Dietary Reference Intakes from the Institute
of Medicine recommend a total daily beverage
intake of 13 cups for men and 9 cups for women.
n Be Active
Regular physical activity lowers blood pressure and helps your body control
stress and weight. Start by doing what exercise you
can for at least 10 minutes at a time. Children and
teens should get 60 or more minutes of physical activity per day, and adults should get two hours and
30 minutes per week. You don’t have to hit the gym
— take a walk after dinner or play a game of catch
or basketball.

Healthy snacks can sustain your energy levels
between meals. Whenever possible, make your
