Remembering a beloved friend
Cosmo loved coming to work. He loved everybody in the office, especially at lunchtime.
But when it came time to visit patients he was all business. All Joan had to say was, “Time to
go to work,” and he stood alert and ready to go. At the nursing home doors he would shake off
his puppy prance and put on his work face. He always seemed to know which resident he was
visiting and would walk right into the room without needing encouragement. And he seemed to
know who wanted kisses and who didn’t. Who wanted him on their lap and who didn’t. Joan
Lilly, Cosmo’s “owner” and High Peaks’ social worker at the time sends these stories:
His first patient was a 90-something tiny French lady who had advanced dementia when she
came on hospice, but had spoken three languages at least, was artistic, and very cultured in her
day. Cosmo would greet her and then sit looking very dignified in her bay window where she
could admire him. Often, she would start a story and then losing her drift, she would brighten
and say, “I think Cosmo knows what I mean.” And if he didn’t , he wasn’t saying.
And then there was the patient in the nursing home who was a very unhappy fellow.
Estranged from his long-suffering wife, he would not speak to me or his nurse, and barely
acknowledged us. I started bringing Cosmo to visit because his wife said he loved dogs.
Cosmo loved this man as he did everyone. With Cosmo this grumpy man was all smiles
and affection, which of course Cosmo returned. He even began to speak to me since
I was Cosmo’s friend. We ended up using photos of this patient with Cosmo in some
hospice literature, and from his expression petting Cosmo, you’d never know what an
unhappy man he was most of the time.
In fact, Cosmo seems to have used his charms most effectively with men who were
struggling. A lovely gentleman in Ticonderoga who was quite depressed about his health
but also would not acknowledge that he was dying – even to his wife – didn’t really want to talk to me about his situation, but he
would welcome visits, which his wife liked, because it gave him an opportunity to see Cosmo and feel a bit like himself. He couldn’t
be too physical but he could still roughhouse with a 22-pound dog.
That was Cosmo. He brought out the best in all of us and made us feel strong. After twelve years of service, our beloved Cosmo
passed away from congestive heart failure on January 5TH. We may have lost his company but never his love.

A message from Executive Director Meg Wood
This report clarifies our financial position and acknowledges the support of our
many generous donors; our volunteers; members of the Board of Directors; and our
agency staff. The services we provide are successful because of the team approach in
each of the hospice disciplines. It is important to acknowledge this teamwork.
Our administrative staff keeps HPH abreast of federal and state regulations. They
manage communications among our clinical offices, the Board of Directors, and the
communities we serve. They balance patient and staff needs against the check book’s
bottom line.
Our support staff provides services in conjunction with clinical care for patients
and their families. Our social workers solve practical problems. They bring a holistic
view to the entire family’s needs and solutions that enhance clinical care. Our pastoral
care staff also gently assess and fill any spiritual needs, working within a family’s own
framework of support. A bereavement program tailored to individual needs not only
follows our families for thirteen months after a patient’s death, but provides services to
any community member regardless of hospice participation. Bereavement coordinators utilize the expertise of other staff disciplines as well as professional services in our
communities. Our volunteer program channels the good will in our communities into
assistance for every aspect of the service we provide to our patients and their families,
as well for our office workers. Our development staff organizes fund-raising, informational events, and media releases. In addition, they work with our professional liaisons
to coordinate continuing education and collaboration for community groups, other
medical care providers, and our own staff.
Our clinical staff works with a patient’s primary physician. Our medical directors,
also physicians, provide general guidance and specific assistance when needed. Our
patient care coordinators oversee each of our clinical offices with the assistance of our
office managers. This team makes certain that immediate clinical and regulatory
requirements are followed. Clinical and social services are provided directly by our
case manager RNs, NPs, LPNs and hospice aides, as well as volunteers and our
support staff. Because we are an agency that operates around the clock, our nurses
manage on-call hours with the help of our triage RNs.
Although the day to day services provided by HPH are managed by our staff,
oversight is provided by the Board of Directors. These generous volunteers bring their
expertise into the evaluation of our business model against our strenuous mission.
Their perspective and recommendations ensure implementation of policies that meet
our current needs and proper planning for the future of hospice as an integral part of
the health care system in the North Country.
Our hospice faces the challenge of a sparsely populated rural 5,400 square mile
region having an uncertain economy. The on-going federal and state changes in our
health care delivery system will drive our agency into new directions as we provide
hospice care. Our task will be to accept the expanded challenge while we live our
mission to provide dignity and care to those facing the end to life’s journey.

Guidance. Support. Patience. These are just a few words that describe the necessary skills and abilities needed to build an
effective fundraising team.
Compassion. Dedication. Perseverance. These are three powerful words that describe a good Development Director. All
of us at HPH&PC would like to wish Darsie Townsend, Development Director a happy retirement!
Darsie Townsend joined HPH&PC in 2002 as a Development Coordinator; organizing events, soliciting gifts for mail campaigns and strategizing ways to reach new donors. Her tenure with HPH&PC has seen many changes, including office moves,
staffing changes and an ever-changing economic climate.
Her ability to motivate staff, leadership and the Board of Directors is indicative of her professionalism. Darsie’s belief in the
mission of High Peaks Hospice & Palliative Care is due to her first-hand experience. Her mother was diagnosed with a terminal illness in 1998 and given 2-3 months to live. ‘Lady’ as she was known to her grandchildren, decided then and there that
she wanted hospice so that she could maintain a quality of life and live the largest life possible
in the time she had left. And live she did – for eighteen months. Her mother was served by
the very nurses and clinical team Darsie worked with every day.
Stories such as this, as told by Darsie, are what will remain with me. I’m grateful to her for
her wisdom, her selflessness, and her diplomacy. What I’ve learned from Darsie, as a mentor,
will stay with me for the rest of my career. Thank you.

A Special
Greetings. I am the son of the late Makeisha Hoffay
that you cared for in late December. I am writing today on
behalf of the Hoffay family. I greatly appreciate Tabitha,
Michelle and Dr. Tedesco for their non-stop care while my
mother was home. With the help of Hospice, my mother
was able to fulfill her final living wish, which was to be
home with family for Christmas. This is held dear to my
family and I, as this was the last holiday we were able to
spend with my mother. This was made possible by the tireless efforts of High Peaks Hospice.
We appreciate the continued around the clock care for
my mother. As we know it was a very difficult process.
The professionalism and understanding of the nurses and
doctors was so fantastic, they fit into part of the family.
The thing that hit home the most with us, was seeing the
Hospice nurse, doctor and therapist at the funeral service
held for my mother. That showed us that Hospice really
does not only care about the patient, but the wellbeing of
the family as well.
Again, I thank ALL of you who work for this incredible
organization.

2013 Giving
2013 Fund-raising Highlights
Doris Forcier golf Tournament – The 2013 tournament had a record
breaking attendance of 62 registered players and raised $4,640 for
hospice.
Radio Day – The 2013 Radio Day was another fun and, as always, exciting
day of stories, education, challenges and fundraising! The students
at LP Quinn Elementary School were as enthusiastic and unpredict
able as ever. By the end of the day we had raised over $48,000
in pledges.
Keene Party – The Marshals generously hosted the annual cocktail party

5th Annual Claire Sweet Golf – The Sweet family and Top of the
World Golf hosted the annual tournament, raising $16,628.
Dining for a Cause – The clinical staff and 99 Restaurant at Aviation
Mall raised $687.03 during National Hospice Month.
Gerber – Dr. Jonathan Gerber and Gerber Family Chiropractic &
Wellness/LiveWellADK joined forces with HPH&PC to donate
funds contributed by their patients.
Essex Party – The McCormicks opened their Skenewood home on Lake
Champlain to host a party, raising $3,816.

Jessica Roemischer in Concert – The musical performance hosted by the

Blue Clothing Donation Boxes – The Mineville box was placed in early
March and brought in $1,531.75 from donated clothing, fabric,
