2015-2016* Pizza Credit Order Form

Pizza Fridays!!
Beginning October 9th, St. Thomas Parents Club will once again offer the option of a catered pizza lunch from Cinelli’s
Pizzeria of Franklin Square on Fridays through May 13th.
**NEW THIS YEAR**
No more Friday morning panic for coupons and envelopes!
We are moving to a pizza credit system.
***Pre-purchasing is required and no payments will be accepted on pizza days.***
You may order your pizza credits using the order form on the reverse side, or you may download the order form from the St.
Thomas website under “Parents Club”. One or two slice options are available and both include choice of a snack and drink.
Pizza credits may be purchased as full year, half year, monthly or by the Wednesday prior to a Pizza Friday. Pricing can be
found on the attached order form. Children who are not paid by Wednesday of a pizza week will not receive a pizza lunch
that Friday.
NOTE:
 Remaining coupons from last year can be credited toward this year’s order. Simply enclose your remaining coupons
with your order and payment envelope, and deduct the dollar amount from your total. Old coupons will only be
accepted for credit through November 4, 2015. Old coupons cannot be sent in as payment on a Pizza Friday.

Attendance will be tracked; therefore, at the end of the year, if your child missed any paid pizza days, you will be
issued a voucher to resubmit with next year's order. **These vouchers will be necessary in order to receive
credit the following year. Lost vouchers will not be honored.**

WE NEED YOUR HELP!! Volunteers are needed Fridays anytime between 11:15am and 1:15pm to help distribute pizza,
snacks, and drinks. Volunteers must be Virtus trained.
