A Win-Win Situation
An outreach project brings great rewards

N
ot surprisingly, submitting their nomination essay was
a group effort for the 2011 Best Nursing Team in the
Mid-Atlantic & Lower Great Lakes. The administrator
and nurse leaders knew they had an experience worth sharing
with their team’s outreach efforts. The nurses and support staff
at Paoli Surgery Center in Paoli, PA, part of Surgical Care Affiliates, have come together in a unique way to bring the spirit of nursing into the
community. They have challenged themselves to take their teamwork outside
the workplace to bring much-needed supplies to an elementary school.
While Paoli Surgery Center received the top honor from our judges, four
other stand-out teams earned runner-up recognition.
The Day Medicine Unit at Nemours/Alfred I. DuPont Hospital for Children
in Wilmington, DE, developed and implemented a nurse-driven initiative to
replace existing sedation protocols with nitrous oxide for minor procedures.
This has decreased patient/family anxiety; alleviated pain and discomfort;
increased staff efficiency; and increased overall satisfaction.
Women’s Health at Sentara Norfolk General Hospital in Norfolk, VA, has
made it their mission to provide crucial education about safe sleeping for
infants. Nurses share SIDS risk-reduction strategies by serving as role models
and educators.
When they realized their outpatient clinics would be short-staffed, the
Digestive Health Center at the University of Virginia Medical Center,
Charlottesville, took a proactive approach. Every member of the team did
whatever was needed to make the day go smoothly, and their patients were
the beneficiaries of their efforts.
Members of the Heart and Kidney Unit at Children’s National Medical
Center, Washington, DC, lived on their unit during the massive snowstorm of
February 2010. They delivered excellent care for their patients, their families
and each other. After this storm, they knew could accomplish anything.

Tops Emergency Medicine
SJH Cardiac ICU Wins Second Beacon Award

HealthGrades’ 2011 list of the Top 10 Cities for
Emergency Medicine.
HealthGrades included the ranking in its recent
Emergency Medicine in American Hospitals
study, which investigated overall mortality rates
for Medicare patients admitted to the hospital
through the emergency department for common
and life-threatening conditions such as heart
attack, stroke and pneumonia.
The top 10 list for emergency medicine also
included Dayton, OH (4), Cleveland (5), Baltimore (8) and Detroit (10).

Price Elected ASPAN President

was recently elected president of the American Society of PeriAnesthesia Nurses
(ASPAN). From 2011-12,
she will serve as president;

For the past 8 months, Washington has served as
assistant vice president of nursing and interim chief
nursing officer. She joined the hospital center’s nursing team in 1997 as a clinical manager in pulmonary
medicine and has steadily moved up the ranks.

(Translating Unique Learning for Incontinence Prevention) Project. The study,

urinary incontinence in women older than 55 who
attend a bladder health class to those who view
a DVD on the same topic. Newman is scheduled
to earn a DNP from Thomas Jefferson University

Kruta Named Clinical Supervisor
▶ Chesapeake, VA Eleanor Kruta, RN, CEN,
HealthGrades designated hospitals performing in the top 5 percent in the
nation as Emergency Medicine Excellence Award hospitals. According to
HealthGrades, Medicare patients treated at Emergency Medicine Excellence
Award hospitals had a 40 percent lower risk of death compared to patients
treated at hospitals without the award.
In top-ranking Cincinnati, six hospitals earned the award: Bethesda North
Hospital, Christ Hospital, Good Samaritan Hospital, Mercy Franciscan
Hospital, The Jewish Hospital and University Hospital.
In Dayton, OH, four hospitals were Emergency Medicine Excellence

hen you think of nursing, backpacks, jump rope and sidewalk
chalk aren’t the first things that
come to mind, but these were the very tools
that brought a group of clinicians together
to become ADVANCE for Nurses’ 2011 Best
Nursing Team.

The relationships forged among the staff were
evident when ADVANCE visited the surgery center
to present their award April 26. One by one, the
team members appeared from the operating room,
PACU and prep areas to pose under a green and
blue balloon arch celebrating their achievement.
But what you won’t see in the resulting pictures
is the banter that went on between shots or the
good-natured jibes from a surgeon who waited
patiently, just out of the shot, for their moment
of glory to be done so they could all get back to
caring for the patients of the day.
In addition to surgery center staff, Lynnette Harris-Scott, principal of the lower school at Belmont,
endured a traffic jam on the Schuylkill Expressway
to be there for the award presentation. After chatting with ADVANCE, she excused herself to snap
a picture of the growing mountain of playground
equipment intended for her students.
From Project to Partnership
The cross-departmental committees set to work
last autumn to gather 500 books and more than
100 backpacks they distributed to the students.
That was one of the best parts of the project for
Janet Owens, BSN, RN, CRNA, CNOR, operating
room nurse manager.
“I enjoyed seeing the good we did,” she said. In
addition to the current collection of playground
equipment, the team also has assisted children
at Belmont with winter coats, ample supplies of
tissues and hand sanitizer, snacks for the school
day and a recent eighth-grade trip to Washington,
DC. “A long-range goal is to help create a more
permanent playground for the school [instead of
just blacktop]," Owens added.
Pamela Hudson, PhD, administrator, couldn’t
be happier with the results of the outreach efforts
for the school and for the surgery center. They
may perform 100 surgeries a week, but helping
repair carpal tunnel and rotator cuff injuries
hadn’t brought them together the way this outreach effort has.
“The project has become more of a partnership
now,” Hudson said. “We have a cohesive team
that has demonstrated incredible teamwork that
spilled over into our work.” n

The surgery center’s locale is mostly comprised
of middle- to upper-class housing developments
with well-funded schools. At Belmont, many of
the students were lacking the basics for a successful school year, such as backpacks, a library
and playground equipment. In addition, all of the

Heart and
Kidney Unit

During the massive snowstorm that hit the
East Coast in February 2010, staff on the
heart and kidney unit at Children’s National
Medical Center in Washington, DC, prepared
to live at work for a few days as the snow was
cleared. The team delivered excellent care for
patients and their families, and to each other.
Staff realized that, after weathering this storm,
they could accomplish anything.
Defining that moment when you realize
just how incredible the team is that you work
with is not easy when every day gives evidence
to a high-performing team. However, during
“Snowmageddon 2010,” it became quite clear we
could adapt to anything that comes our way.
For many of us, facing such a large-scale hospital emergency was a first-time experience and
we knew that, although patient care needed to
remain seamless, we had to adapt to the urgency
of providing safe staffing. To say the least, we
did it! We were more than ready, each member
of our team prepared to make the hospital our
home, for as many days as we needed, four to be
exact. The Heart and Kidney team adapted to this
situation with great attitudes and perseverance.
For a unit that prides itself on its cohesiveness
and ability to work together, these 4 days even
surprised us.
We adapted to a circumstance far beyond our
control and we stepped up. We gave safe patient
care; we pulled together and cared for each other.
We are very proud of ourselves for being the kind
of team that could weather any storm. n

When they realized their outpatient clinics
would be short-staffed, this team took a proactive
approach. Regardless of title or job description,
every member of University of Virginia Medical
Center’s Digestive Health Center did whatever
was needed to make the day go smoothly — their
patients were the beneficiaries of their efforts.
While preparing for the day’s clinics, I realized
we were only going to be two RNs together with
the doctors’ RN coordinators and two PCAs with
eight outpatient clinics scheduled that day. It was
definitely going to be one of the most difficult days
in the clinic that year.
Navigating through the day, I suddenly realized what adaptability truly meant. … Everyone
worked together as a one big organism, where all
parts and levels interact in perfect harmony, even
in the absence of any written guidelines or premanufactured instructions. Patient assessments,
admissions, vaccinations, blood drawing, vitals,
medicine reconciliation, after-visit discharge
summaries, procedural scheduling and educa-
tion — everything simply went flawlessly.
At 5 p.m. that Friday, we looked at each other
and we could not believe we made it. We had
almost 65 patients that day. … The care provided
to our patients was as good as always.
Moreover, we could help our colleagues to
be off, as they needed [one to take her NCLEX
and another to be with her pregnant daughter].

sedation and infusion services, and our nurses
were accustomed to administering midazolam to
help with anxiolysis and amnesia prior to procedures. But we knew the oral formulation tastes bad
and nasal administration burns. We became aware

on the development of a nitrous oxide program
as a team initiative.
To date, nitrous oxide has proved to be an
invaluable asset to the sedation team and the
families we serve, and we are very proud of our
nurse-driven efforts to implement and improve
upon the program. n
The Women’s Health team at Sentara Norfolk
General Hospital in Virginia has made it their
mission to provide crucial education about safe
sleeping for infants to eliminate the heartbreak
of SIDS. The nurses and staff of women’s health
spread the word about SIDS risk-reduction strategies with their patients and the greater community by serving as role models and educators.
All babies from birth should be ensured a
safe sleep environment, and it was the mission
of Nursery Services to put our babies in the
“perfect” position, so to speak. As a Women’s
Health team, we used our expertise to raise
awareness among healthcare providers, parents
and the outside community regarding SIDS risk
reduction.
Our goal is to ensure a safe sleep environment
for all newborns while hospitalized and upon
discharge home. We do this with the hope that
guidelines recommended by the American Academy of Pediatrics will be embraced and delivered
to all parents, grandparents and child providers
in our community.
As a team, we celebrate our expertise as not
only newborn nurses and special care nurses,
but as a unit that employs the disciplines of labor
and delivery, ante- and postpartum, respiratory
therapy and lactation consultants. It truly was a
labor of love from creation to implementation,
and from the birth of ideas based on national
guidelines to the delivery at time of discharge of
educational materials. We modeled, molded and
fashioned [it] to ultimately prepare our parents
for the day they left our community and stepped
knowingly and securely into theirs. n

effective clinical team

t is 7 a.m. and day shift staff members are
gathering to listen to report. As you expected,
the regular charge nurse is away and you will
be in charge today.
You take a look at the staffing sheet. You
recognize most of the names and are relieved to
see a few of your strongest nurses and nursing
assistants — people on whom you can always
rely — will be working with you today. There
also are two people you know well, but are
not looking forward to working with. One is
a nurse who frequently complains when other
people and other departments do not do what
is expected of them; the other is a nursing
assistant who lacks initiative and often has to
be told what to do.

You see two people you do not know are
assigned to your unit. One is a nurse from a
staffing agency; the other is a per diem nurse
who usually works on another unit. As you think
about what your unit will need to accomplish
today, you are conscious your first task is to
organize your team.
Nurses Work in Teams
Whether the work setting is an inpatient unit
or an outpatient clinic, nurses generally work
as members of multidisciplinary teams. This is
true whether the unit uses a team nursing or
primary nursing practice model. These teams
often include both licensed and unlicensed clinical staff, as well as non-clinical staff, such as unit
secretaries and housekeepers. Some staff, such
as discharge planners, respiratory therapists,
physical therapists and others, may spend part
of their day working alongside staff assigned to
the patient care unit.
Team building is a daily activity in nursing. On
any given day, the patient care team may include
per diem staff, staff who float from other units
or other shifts, and staff from outside agencies.
These individuals may or may not be familiar
with the work setting, and they may or may
not be known to other members of the team.
In practice, it is frequently necessary to form a
new team at the start of each shift.
Because clinical teams work within a larger
organizational context, much of the work of
team building has been done before the team
members even assemble. Healthcare organizations have regulations, policies and procedures
that explicitly define formal aspects of organizational culture, including the roles and functions
of specific types of teams, and the roles, qualifications and competencies of team members.
For example, the number and responsibilities
of nurses, nursing assistants and housekeepers
are defined before any individuals are assigned
to fill those roles. Team members may be new
to this shift or this unit, but most of them will
have worked many times on other shifts or
other units. Because the team is embedded in
the organizational culture, teams know they
can rely on the organization’s logistical and
moral support.
Equally important, the organizational culture provides a shared frame of reference that
enables everyone to understand the purpose of
the team and the roles of its members before
the team is even assembled. Individuals with a
shared frame of reference approach a particular
situation with a common orientation and set
of values. They have a shared understanding
of the actions that must be taken, and of the
role played by each person who takes action
in that situation.

Team Development
We can think of the process of team building as
happening in stages: individuals assemble, the
individuals join together as a team and the team
engages in the task at hand.
It begins with a collection of individuals who
are assembled, ready to begin a new shift. They
are available, but in a sense, they are not yet
committed to the team, and they are not yet
engaged in the task at hand.
For team building to be successful, two transitions have to be achieved. First, the individuals
must join together to become members of a
team. This requires agreement on shared goals
and a process to achieve those goals. When
this happens, they become committed to one
another and to the team effort. Second, the team
members must become engaged in the task at
hand (see Figure).
In a classic work on team development, Bruce
Tuckman described this process as forming,
storming, norming and performing.1 In Tuckman’s model, forming occurs when individuals
assemble and become oriented to the task at
hand. Storming describes the initial resistance
to group influence and task requirements that
often occurs as the group transitions to norming, the development of group cohesiveness, the
adoption of group standards and the assumption
of individual roles. This process enables the individuals to join together as a team that is ready to
engage in the task at hand. Individual concerns
have been resolved and the group is ready to
work together by performing constructively.
Effective Team Building
The team leader has primary responsibility for
team development. Teams are most effective
when one person is designated as the formal
leader to whom everyone on the team is accountable. Qualified leadership requires definition of
the leader’s role and assignment of a particular
individual to fill that role. The team leader may
be designated by a job title, such as charge
nurse. Alternatively, the leadership role may
be defined generically. For example, more than
one individual may be called upon at times to
function in the role of charge nurse. By means
of policies and procedures, the organizational
culture provides a mechanism for defining the
leadership role and filling it.
Being an effective team leader is not the
same as being a competent nurse. As nurses,
we are educated to provide clinical care within
our scope of practice. Through experience, we
develop our skills and competence. For most

Individuals assemble
Individuals join
together as a team
nurses, being able to provide excellent care to
our patients is a source of great personal and
professional satisfaction. Although team leaders
often find opportunities to apply their clinical
expertise to direct patient care, their primary
responsibility is to facilitate the patient care
delivered by members of their team.
Competent team leaders are able to look at
the big picture. Nurses who assume a leadership
role must be able to understand and prioritize
the work of the unit as a whole. They must
understand the flow of patients from and to
other departments, such as the emergency
department or the operating room, and how
the clinical unit interacts with ancillary departments, such as imaging or discharge planning.
To make assignments and plan effectively, team
leaders must understand the work and scope of
practice of each member of the team.
Team leaders must be able and willing to lead.
In a very influential article, “Leadership: Good,
Better, Best,” Bernard M. Bass suggested there
are three major leadership styles: laissez-faire,
transactional and transformational. Laissez-

faire leaders are passive. They show little interest
in the work of the team and tend to act only
when there is a problem. Transactional leaders
actively set goals, monitor progress and reward
successful performance. Transformational leaders go beyond that approach. They seek to inspire
excellence and address the individual needs of
team members to improve the performance of
the team as a whole.2
These leadership styles have a real and significant effect on team performance. In a study of
staff and patient satisfaction, and quality-of-life
outcomes in mental health treatment programs,
Patrick Corrigan and colleagues demonstrated
that patients’ “… satisfaction and quality of
life were inversely associated with laissez-faire
approaches to leadership and positively associated with both transformational and transactional leadership.” They found staff “… who
viewed their leaders as charismatic, inspirational
and considerate of individuals worked in programs with [patients] who reported a relatively
higher quality of life.” The authors’ conclusion is
relevant for clinical teams in any setting: “Active

leadership skills that inspire and intellectually
stimulate appear to be an important element in
programs that are satisfactory to consumers.”3
Building Your Team
Instead of rushing to hear report and make
assignments, take a few minutes to build your
team. An article in Harvard Management Update
describes specific techniques leaders can use
to help a group of individuals join together as
members of a team, quickly and effectively:
1. Share personal histories. Personal stories
reveal competencies, generate respect and foster
cooperation.
2. Ask: “What has worked for you in the past?”
This signals that past experiences are valued as
potential contributions.
3. Describe how the team will work together.
Clearly state the vision, purpose and plan, and
describe each person’s role within the team.
4. Optimize individual team member’s
strengths. Make realistic assignments that take
advantage of each team member’s strengths.
5. Establish norms for making decisions. Let
team members know what types of decisions
they are expected to make on their own and
what types of decisions will be made by the
team leader.
6. Establish a process for giving and receiving
feedback. This allows information to be exchanged
quickly, easily and in all directions.4
At the start of a shift, it may seem as if there is
not enough time to do all this. In practice, however, it takes only a few minutes, and it sets up the
team for a productive and successful shift.
Putting It Together
Everyone is seated waiting for report. You greet
them, introduce yourself and tell them a bit about
your professional background.
You ask the team members to introduce
themselves. As they give their names, you ask
questions about prior experience on this and
other units, and types of work each person has
done. You casually elicit information about
the nurse from the staffing agency and the per
diem nurse who usually works on another unit.
You acknowledge prior acquaintance with your
regular team members and tell a few anecdotes
of experiences you have shared in the past.
You give the team a brief overview of the plan
for the day and describe any issues you anticipate,
such as special procedures or difficult clinical
problems. You ask if they have any preferences
regarding procedures and routines. After each
person has been heard from, you make the
assignments and outline your expectations of the
team. You conclude by saying how much you are
looking forward to working with this team.
The whole process has taken less than 10
minutes. What you have done is create in your
team members’ minds the thought, “I want to
be part of this team. This team is likely to be
successful, and these are people with whom I
will enjoy working.” You also find that you feel
the same way.

Tips for Team Leaders
• Clinical teams have three goals: 1) provide
good clinical care; 2) provide good service to
patients and families; and 3) use resources wisely.
Your most valuable resource is your team.
• Patient care is provided by teams. The quality of care provided by a clinical team depends
on how well the team members work together.
Team leaders can make it happen.
• Everyone who works on your unit is part of
the patient care team. By including the nursing
assistants, the housekeepers, the unit secretary and
others in your planning, decision-making and team
recognition, you increase the capacity of your team
to achieve successful patient care outcomes.
• Work with the team you have. Ignoring
new team members or complaining about being
stuck with particular individuals is insulting,
rarely changes anything and does not lead to
successful results.
• Diversity is not only a policy; it is a fact of
life. If you find ways to use people’s strengths,
you will spend less time trying to cope with
their weaknesses.
• Address the needs and concerns of your
team members. If you do this, they will be
more interested in addressing your needs and
concerns. n

Equipped to Lead
Networking and sharing best practices highlight the annual
AONE meeting By Candy Goulette
▶ There was no “resort casual” apparel or attitude in evidence at the 44th annual Meeting and
Exposition of the American Organization of Nurse
Executives (AONE). Instead, more than 2,500
nurse leaders from across the country and around
the world came to San Diego on April 13-16
dressed to work, spending scheduled time in preconference, plenary and concurrent sessions, and
unscripted time networking with colleagues.
While much of the activity focused on issues
they face daily — care delivery and sustainability,
patient safety and quality, leveraging technology
and the future of the profession — nurse leaders
spent time sharing best practices and catching up
with friends and colleagues.
Both proved valuable, said 2011 AONE President Cheryl Hoying, PhD, RN, NEA-BC, FACHE.
“The time set aside to network and reconnect is
one of the strengths of the AONE annual meeting,” Hoying told ADVANCE. “Equally important
as hearing the outstanding plenary speakers is
spending time with nurse colleagues to find out
how they’re handling similar issues and learning
from them. They’ve tried things and piloted things
and can share what worked and what didn’t. I
know I always value the advice I get.”
Hoying, also senior vice president of patient
services at Cincinnati Children’s Hospital Medical Center, recalled the story of one new chief
nursing officer who came into the role with some
uncertainty. Through her membership in AONE,
she connected with other local AONE members
and found support through national contacts
made at the 2010 annual meeting. This year, she
presented a paper on the development of her role,
made smoother because of AONE.
Shifting Sands
Saying the job of a nurse leader is both rewarding and taxing, Hoying told the group it is an
invigorating time to be a nurse leader.
“A lot of the work ahead of us as nurse leaders
is preparing nurses to change and adapt to the
future,” she said. “With each passing year, new
advances in research are discovered. We must be
on the cutting edge to leverage this new knowledge. Part of the nurse leaders’ responsibility is
to bring those efforts forward and improve our

care that we have for patients. We must have zero
defects and errors. At the same time, we must be
effective and help reduce healthcare costs. That
means our nurses must be confident in what
they do. They also must be flexible and able to
adjust readily.”
Hoying noted it is the responsibility of nurse
leaders to help chart the course of healthcare on
local, regional and national levels.
“We have a golden opportunity to make positive

other organizations would want to emulate it, learning and transplanting that knowledge into their
own situation. But unless you put those lessons into
practice, there is no guarantee of success.
“The trouble is, speaking metaphorically, a
musician could play an instrument flawlessly and
still mess up things for the orchestra,” he said.
“The fact that he plays his instrument well is no
guarantee he will be effective in the organization.
It takes the total interconnectivity of the orchestra
to produce a successful product.”
That includes the conductor or, for healthcare
organizations, the nursing leader.
“It’s not only the musicians who listen, gather
data and react; it’s also the leader,” Nierenberg

changes in healthcare,” she said. “AONE is a great
avenue to assist nurse leaders in their organizations
with healthcare reform by keeping its members
abreast of proposed legislation. It’s important for
nurse leaders to know we can a have a broader
impact on a larger scale. Our nation is in great need
of our leadership. As nurses, we promote health.
As the voice for nurse leaders, AONE’s more than
8,000 members promote nurses as one of the main
change factors for healthcare reform.”
Music Paradigm
During the AONE meeting, plenary sessions
allowed nurse leaders to view their roles through
the lens of music, writing, social connections
and innovation. Parallels were drawn to leadership, staff development and communication,
essential skills for healthcare professionals and
those aspiring to be.
The keynote address, presented by acclaimed
conductor Roger Nierenberg, became an interactive session for attendees in the large ballroom
interspersed throughout with musicians from
the San Diego Symphony. Nierenberg began
by saying the orchestra is a metaphor for any
other organization, i.e., it fundamentally comes
down to individuals carrying out tasks for which
they are hired.
explained. “Tuning into subtle things: it’s an
important part of the leader’s role. Critical listening, knowing exactly what’s going on, is critical
for success. If the leader doesn’t step up to that,
there’s no way for the people of the organization
to compensate for it.”
Nierenberg said a good conductor — or nurse
leader — must learn to listen three ways, then act
on what she’s heard.
“You must listen for problems that have to be
fixed, for things that are good that can be built on
and for what has not yet happened,” he said. “The
orchestra — or the nursing staff — responds to
the power of the conductor’s strategic visionary
leadership.
“When the leader comes with a possibility of
what could be and then puts that forward to the
workforce, it has a completely different power than
just fixing the things that are wrong or supporting
the things that are right,” he continued. “That is
what really engages people, especially when they
sense the leader is partnering with them. You want
to show your people that success comes from total
collaboration.”

surgery who may be at risk for infection.
“These studies affirm that the 3M Skin and
