Members conduct their practices in the following locations: Illinois, Indiana, Iowa, Kansas, Kentucky, Michigan, Minnesota, Missouri, Nebraska, North Dakota, Ohio, South Dakota, Wisconsin & Toronto, Ontario. MEET AGING LIFE CARE - BRANDING UPDATE By Jan Welsh, MS, LPC, CMC, CRC In my opinion our new name more clearly identifies our industry and our market. Our new identity line “The experts in aging well” is simple and descriptive. And it is not offensive to Baby Boomers, who don’t want to be labeled as old, seniors or elders. What a fantastic Branding Week in early December! Thank you to everyone who attended the presentation. If you missed the one-hour branding video, you can view it at this link: www.caremanager. org/membersonly/member-news/ the-branding-project-2014/ The presentation covers the Branding Project Research, Branding Solutions, and Implementation à la Jane Cavalier of BrightMark Consulting, the branding expert who is helping NAPGCM with this effort. It is natural to be apprehensive about how one might incorporate a new organizational name, possibly a new job title, a new acronym, or new identity. The new materials will be released at the National Conference in Denver and will include examples, options and templates. I am already trying out this new language when I meet with prospective clients or give presentations. Here are a few the examples that I have used to strengthen my presentation. If you have branding questions or comments, please contact: Callie Daters or Kaaren Boothroyd at the national office or me Jan Welsch at: 513.242-3587 [email protected] This year the MW Annual Fall Conference was held at the historic and haunted ELMS Hotel and Spa just outside of Kansas City, Missouri. The beautiful late October weather provided the final touch to a conference that was not only educational but also relaxing and a little spooky! The conference committee worked hard to provide a weekend designed as promised with education, collaboration and relaxation. The conference began with a presentation on the innovative ways seniors can age at home. Mark Notle, Don Peterson and Dr. K. Grandy discussed the future of patient care in the smart medical home. Kelly Jones, LMSW followed with excellent information and tips on assessing the needs of individual’s with dementia. The afternoon concluded with a live / virtual presentation by the well known hoarding expert and TV personality Matt Paxton. Matt walked attendees through the hoarder and family mindset before, during and after the clean up. The challenges of clean up and strategies for effective communication were included. Our day ended with a poolside reception and opportunities to network. Saturday morning started early with exhibitors available to share information and services. Nancy Luther, from the Johnson County Mental Health Center discussed the many losses experienced in later life, some that are also under-acknowledged as losses. She discussed grieving styles, complications of grief and how grief can become depression. Dr. L. Leaf, a neuropsychologist, talked about behavioral issues and interventions in geriatric settings. Michala Gibson followed with a review of the many issues around polypharmacy in the elderly. Everyone enjoyed the annual business luncheon held at the Elms Hotel Carriage House. The afternoon offered attendees much more to think about during this exciting time of growth and change in our field. Dr. D.C. Hayley gave a very personal and educational talk on dementia in the older adult and how dementia affects other health issues. Mitzi McFatrich presented thought provoking information on public policy initiatives which promote access to quality long term care in the home and facility settings. To end the day Attorney Jeff Easterday reviewed incapacity and planning for death. The final day of the conference started with NAPGCM own Ruth Cohen giving an excellent presentation on the spirituality of care management. She shared her wisdom and heart with the group. We then shared a good laugh with Novella Perrin who shared the importance of humor in workplace. A personal favorite at the close of the conference was a session lead by Natalie Schneider on best practices. Attendees had the opportunity to share and learn from one another. I always leave a conference feeling re-energized with new information, new friends and new ways to improve my skills and practice. I urge everyone to mark your calendars now to attend the 2015 MW conference ”The Aging Well Tapestry . . Weaving the Care Together” October 23-24, 2015 in Chicago. I look forward to seeing you there! PRESIDENTIAL MOMENTS Debbie Feldman Chapter President 2013-2016 by I hope that everyone had a wonderful Holiday Season and I wish you all a very happy and healthy 2015! As we move into the New Year, I wanted to share an article I read recently in the Chicago Tribune “The #1 Health-Booster in 2015 is Meditation”. The article states that this is one of the simplest of New Year’s resolutions and that “taking the time to meditate can make all the difference in your health and well-being”. The author is Virginia Sole-Smith if you are interested in reading the article. On behalf of the Midwest Chapter and Midwest Chapter Board I would like to take this opportunity to thank Marianne Ewig and Gail Lee for all of their hard work during the time they have served on the board. They have given so much to our Chapter and although they are currently retiring from the Midwest Chapter Board I know that they will continue to be vital Midwest Chapter members. I would also like to thank Chris Bangtson for all of her hard work over the past three years. She has been a strong Co-Chair of our Unit Committee and a strong leader. I regret to say that due to personal reasons she has resigned from the board and will not be completing the second year of her term. She will be missed. I would like to congratulate and introduce our new board members for the 2015-2016 Term: Anne SuttonBurke from Cincinnati, OH; Dorian Maples from Fort Wayne, IN; Sara Moore from State Line, IN. Chris Bangtson’s seat. We have a lot to do during the coming year and with a full Board, we should be able to accomplish all that we set out to do. One of our biggest projects in 2015 will be promoting and implementing our new brand. Please check out NAPGCM’s website and search “branding project” to learn all about our name change and the implementation of our new brand. And keep a watch for e-blasts from both NAPGCM and our MW Chapter promoting our new brand. I am in the process of planning our Chapter Board Retreat to be held in Chicago in late February. We will be establishing our strategy for accomplishing tasks on our 2015 Strategic Plan. One recap for 2014 I’d like to mention was our successful and very educational Chapter Conference in Excelsior, Kansas, October, 24-26, 2014. Because we had a more intimate group of attendees, there was a lot of opportunity to network and develop some good relationships. I would like to thank not only the Conference Committee for all of their hard work, but the vendors and attendees as well. I would also like to thank Amy Abrams from San Diego, California, one of our National Board Members, for attending the conference. We enjoyed sharing our Midwest hospitality with her and in turn she shared with us some of National’s accomplishments over the past year. And congratulations also to our two re-elected board members Judy Mange from St. Louis, MO and Cheryl Hendrixson from Indianapolis, IN and to Mary Pitsch from Sheboygan, WI for her re-election as our Treasurer. And, just a reminder to mark your calendars for the NAPGCM Annual Conference in Denver, Colorado April 29, 2015 through May 2, 2015, as well as our next Midwest Conference in Chicago, Illinois October 23-24, 2015. Please note that we are changing the conference format a little for 2015. We will be holding the conference all day Friday and Saturday ending around 4:00 pm. Finally, to round out our elections and open board seats, I would like to thank Natalie Schneider from Overland Park, KS for accepting a one year term, filling All the best, Debbie Units On The Go! 4TH QUARTER UNIT REPORTS The MW Chapter currently has 12 active units. Eight units held a total of 15 meetings in the fourth quarter; 4 units did not report meetings in this quarter. A total of 122 members and 22 guests attend these unit meetings. Chicago Programs and special projects include Legal Planning with Dementia on September 17, 2014 and Alzheimer’s Disease: Diagnosis, Treatment and Support on November 19, 2014. The unit is also busy planning the 2015 Midwest NAPGCM Conference. Cincinnati PR activities include multiple contacts with referral sources such at attorneys, home health and durable medical equipment providers. MW Chapter funds were received for post cards to send end of the year greetings. Ann Sutton Burke was elected to the Midwest Board and just completed (2014) her position as President of the Association of Professionals in Aging – Cincinnati. Unit members are very positive about the branding project and looking forward to the roll out next year. Detroit We had a Family Law Attorney speak on DPOA, Guardianship and HIPPA. Erica Saum co – chair has resigned her position as co-chair of the Detroit unit, due to a change in her job responsibilities. Illiana The Illiana Unit, which is spread across rural Indiana and Illinois, is now meeting at various facilities across Indiana and Illinois. This is a win-win for the facilities which provide education about their services, tours and refreshments and the local GCM’s who can reinforce the value of their services to the facilities and their clients. Future plans include educational programs for the public and professionals hosted with the facilities. Linda Brown of Family Care Management is having much success keeping chronically ill clients from returning to the hospital. Unit members Linda and Jackie Witges of Seimer Law Firm, are enjoying partnering with the Humana Cares Program. One of the exciting aspects about our Illiana Unit is that we have diversity. We are blessed with a group of social workers, nurses, counselors, organizers, clergy, and one working within a law firm. This makes for very interesting unit meetings and cases studies. Indiana Scott Severns an elder law attorney spoke with us about recent changes in Indiana’s Medicaid program. Association. Several members will attend an ACMA program in December on Observation Status. The Happy Hour in November was enjoyed by all who attended. Our December meeting was a CEU presentation on Life Beyond Retirement- the benefits of volunteer programs. We also had a presentation on a new Access and Functional Needs Registry that was recently started in the St. Louis area. The discussion on Observation Status and the 2-Midnight Rule continued. The unit has begun working on a PR program/project for 2015 to present a movie screening of “Alive Inside” For PR three press releases were sent out to the media- there has been no response. Congratulations to Dorian Maples & Cheryl Hendrixson for being elected to serve on the NAPGCM’s Midwest Chapter Board! We know we will be well represented. All members now have instructions on how to do a Google Alert- to continue to send press releases and keep care management in the forefront for the media. Congratulations also to Gail Fifer, a proud new first time grandmother to Madeline Ann. Member, Kate Moore, received her master’s degree in Gerontology and will begin working more in the field of care management. Kansas The Kansas City Unit hosted the MW Annual Conference. Unit member Michala Gibson presented “Poly Pharmacy with Elders” at the Midwest Conference. Future goals include growing the unit membership and offering educational programs. St. Louis Our October meeting was a CEU presentation on DME Options for the home and was held in the DME showroom. In November we reviewed the Chapter meeting for those present. Discussion of the Observation Status continues with a task force working with the American Case Management SE Wisconsin PR activities include a reprint of our member ad in our regional Senior Resources Directories for 2015. This is partially funded by the MW Chapter and unit members. We are developing our Advanced Clinical Topics for 2015 and await the rollout of Aging Life Care Services. The committee’s goal is to offer the membership a place to discuss/brainstorm any ethical or business practice dilemma which may be of concern to their practice. Committee’s responses are based on the NAPGCM Code of Ethics and Standards of Practice. These are available on the web site (www.caremanager.org ) under the “About Care Management” tab. From there you go to “Certification and Professional Conduct” where you will find the Code of Ethics and Standards of Practice. These are great to refer to when you have a question. Most recently, a member contacted the committee for guidance concerning an issue where the care being provided to the client by the facility was felt to be unethical. If you have any questions where you think that the committee can help, please contact someone on the Chapter Ethics Committee.
