Hello,
This membership packet will help you understand your Smilebuilderz Dental Plan (SDP) as well as
includes important information about Smilebuilderz and the services we offer in all locations.
SDP is not dental insurance. It is a discount dental program. If you have signed up for a family membership,
all immediate family members are included in the dental plan. (Children under 18 do not require cards). Your
SDP card is immediately active. Remember, with SDP your exams and x-rays are always free. If you need a new
patient exam or want a second opinion, the service is free.
Individuals who are not currently patients with us can expect the following when they schedule their first
appointment and free comprehensive exam. We will spend about 80 minutes with you. A thorough review
of your dental history will be completed at that time and we will take a complete set of x-rays. You will then
receive a comprehensive oral exam from the dentist. Your dentist will review your x-rays and high resolution
color images of your teeth with you so you are completely educated about the status of your oral health. Your
dentist will also show you how to properly care for your teeth and gums. Again, there is no charge for this visit.

Welcome to the Smilebuilderz Dental Plan (SDP). SDP is an innovative dental plan developed and offered
by Smilebuilderz that allows you (and your immediate family) to receive comprehensive dental care at
significant savings.
What is Smilebuilderz?
Smilebuilderz is unlike any dental practice you’ve ever seen. We have general dentists, cosmetic dentists,
orthodontists, oral surgeons, pediatric dentists, anesthetists, periodontists and root canal specialists
in one location. This means if you need wisdom teeth extracted or braces, you don’t need an outside
referral or have to locate a specialist yourself. We have one right here for you.
We cater to busy families with convenient evening and weekend hours. You don’t need to miss work and
the kids don’t have to miss school for a dentist appointment. And, if you have a dental emergency, we
now offer dental care 7 days a week at our Urgent Dental Care locations. No appointment needed.
Smilebuilderz also offers convenient monthly payment options for more extensive services through
CareCredit®.
What is SDP?
Smilebuilderz Dental Plan (SDP) is a discount dental plan that gives you and your immediate family access
to the full range of the services offered at Smilebuilderz, at discounts that make excellent dental care
affordable. With SDP, there’s no limit and no maximums to the services available. You’re eligible for
fillings, oral surgery, dentures, implants, braces or other services at discounted rates. And best of all, your
exams and x-rays are always FREE! Regular checkups are a good way to avoid more serious problems, and
with SDP, you and your immediate family can have checkups twice a year at no charge.
Smilebuilderz Urgent Dental Care
If you have a dental emergency, you can use your Smilebuilderz Dental Plan savings at our Urgent Dental
Care locations. Urgent Dental Care is open 7 days a week, no appointment is necessary. Walk-ins are
welcome and we take care of children and adults, treating every dental emergency, from toothaches to
teeth that are knocked out!
What services are included in SDP?
Everything! You’re eligible for any service that’s offered at Smilebuilderz, including advanced cosmetic
dentistry, such as veneers, tooth whitening and bonding. The following is a list of some of the services

A. Yes, depending on plan selection, your immediate family members can be included in
the plan. Call to inquire. Our contact information is below.
Q. Do I ever have to pay for x-rays or exams?
A. No, Smilebuilderz will provide these services free of charge for the lifetime of the
membership.
Q. Are there waiting periods for enrollment or procedures?
A. No, your SDP card is immediately active.
Q. Can I use my Smilebuilderz Dental Plan card just for my children’s
braces or my spouse’s root canal?
A. Yes, you can use any of the specialists at Smilebuilderz including Orthodontists (braces),
Endodontists (root canal), Periodontists (gum and implant), Pediatric Dentists (children’s
care), and Oral Surgeons (wisdom teeth extraction).
Q: Do you accept traditional insurances?
A: Yes, Smilebuilderz accepts most insurances.
