INSTRUCTIONS FOR CASHIER TABLE 1. FORM NEEDS TO HAVE BEEN REVIEWED BY CHECKER BEFORE COMING TO CASHIER—IF NOT CHECKED, SEND THE PERSON BACK. 2. COST IS $75 FOR 1ST PLAYER, $55 FOR EACH ADDITIONAL PLAYER. 3. ANYONE WHO NEEDS ASSISTANCE IN PAYING SHOULD BE SENT TO REGIONAL COMMISSIONER, ASSISTANT COMMISSIONERS OR REGISTRAR FOR APPROVAL. 4. CODE STAYS BLANK WE ARE REGION 220 MEDICAL INFORMATION MUST BE FILLED OUT EMERGENCY CONTACT AND PARENT INFORMATION MUST BE FILLED OUT AT LEAST ONE PARENT’S OR GUARDIAN’S INFORMATION MUST BE FILLED OUT—BOTH IS BETTER 2. THE QUESTION “DOES THIS CHILD HAVE ANY DISABILITIES………” MUST BE CHECKED YES OR NO 3. UNIFORM SIZE MUST BE ON THE FORM 4. THE FORM MUST BE SIGNED 5. FOR NEW PLAYERS (BLANK FORM) THE “DOB VERIFIED” BOX MUST BE INITIALLED. 6. THE CHECK NUMBER, FEE AND AMOUNT PAID BOXES WILL BE FILLED IN BY CASHIER. 7. VOLUNTEER FLYER MUST BE FILLED OUT AND RETURNED BEFORE REGISTRATION CAN BE COMPLETED. GENTLY URGE ALL PARENTS TO SIGN UP FOR SOMETHING. 8. SEND THEM ON TO THE REGISTRATION FORM CHECK TABLE. Having several stamps available at Registration is very useful as we immediately endorse the registration checks. We purchased them at Office Depot at a cost of approximately $35 each. I provided a link to their website with a similar stamp. The American Youth Soccer Organization (AYSO) is a national, non-profit volunteer organization that provides soccer programs to youth worldwide. Local AYSO Region 969 serves approximately 650 players every year, ranging from ages 4 to 18, in the Del City, Midwest City, and Tinker Air Force Base community. AYSO follows SIX PHILOSOPHIES that separate us from all other youth sports. Everyone Plays Our goal is for kids to play soccer. We mandate that every player on every team must play at least half of every game. Balanced Teams We set up teams as evenly balanced as possible— it is more fun when teams of equal ability play. Positive Coaching The way to make winning kids is by building them up—not knocking them down. We Don't "Stack" Teams. We start each season by setting up new BALANCED TEAMS as evenly as possible. When some players have more developed skills than others, those with less experience can learn from those with more. It's more fun when teams are of equal ability! (Not only soccer skills are learned...many learn leadership, role modeling, and social skills, too. This builds self-esteem and self-confidence in every player!) I’ve registered! Now what happens? What’s the Board doing? Immediately after the last registration date, your Board begins putting teams together for the upcoming season. When this is completed, the uniforms will be ordered and the coaches will be notified. When do the coaches get their teams? Coaches should expect to receive their team rosters in (insert date) When do the players get called? Teams may begin practice on or after (insert date). The coaches will call the players usually from mid to late July. They will give you all the information you will need in regards to practice times and location. Each coach sets his/her own practice days, time and location. The practice fields are located throughout our (insert location) It’s after August 1 and my child has not been called. What do I do? Please do not panic if you see teams practicing and your child has not been called. Not all teams begin practice at the same time. The very young teams, in particular, do not always begin practice on (insert date). Also some coaches may be out of town. If your child has not been called by (insert date), you may leave a message at (insert phone number). What else does my child need? The registration fee includes: complete uniform (jersey, shorts, socks) supplemental accident insurance and team and individual pictures. In addition, you may need to obtain shoes, shin guards, a soccer ball (size depends on age) and a water bottle. How often are practices and games? Practices are held twice a week (field and times to be determined by your hild’s coach). Games will be on Saturdays with the first game on (insert date). Games and practices will be held at a field in (insert location). Game fields for some in the older divisions may be (insert location). Refund? Players withdrawing prior to (insert date) will be issued a full refund. After (insert date & Region Refund Policy). All requests for a refund must be submitted in writing and accompanied by a selfaddressed stamped envelope. Mail to (insert address). Late registration? Late registration begins after our last registration on (insert date & Region Late Registration Policy). Those interested may call (insert phone number & email). Please Remember….. AYSO is a parent-based volunteer organization established to provide a positive experience for our children. Go to the games. Learn the rules.
