The meetings are open to the public and the Board welcomes citizens to attend and voice their opinions. The Parks and Recreation Department will only notify you if a program is full or cancelled. If you would like to receive an email confirmation, please make note on the registration form. *Registrations will not be processed without payment. A $30 fee will be assessed for any checks returned. Refunds for unsuccessful registrations will be issued within 21 days of the start of a program. The Parks and Recreation Department does not provide medical insurance coverage for injuries suffered while participating in Department sponsored activities and events. Each participant must assume responsibility for all medical bills incurred from personal injury while participating in a Department sponsored activity or event. CLASS SIZE Residents are persons who pay their property taxes directly to the Village of Grafton. Resident fee status applies to Village of Grafton residents for all programs and facility usage. Town of Grafton residents are considered non-residents when signing up for all Parks and Recreation Department programs. Programs are designed according to an instructor/student ratio and at an age level that will benefit each student. Certain programs have class size limits. The decision to exceed these limits is at the discretion of the Program Director and the Parks and Recreation Director. We will strive to accommodate all requests. If a chosen program has reached capacity, your name will be placed on a waiting list. You will be notified by phone or email of any openings that may become available. PHOTOGRAPHY POLICY AGE GUIDELINES RESIDENCY During programs and events, we will periodically take pictures for publicity. By participating in events/programs you are giving us permission to use your photo in media publications. If you do not want your picture taken or used in publications or advertisements, please let us know immediately. WAITING LIST A waiting list will be started after a program has reached the maximum number of participants. Payment is not accepted and participation is not guaranteed at this time. If a vacancy occurs, participants will be called in the order of registration and full payment will need to be made prior to attending the program. Please do not request to have children placed in a class they do not belong in. All age requirements are set to benefit children by providing a safe and uniform learning environment. Age=As of the last day of the program Grade=Level of the student as of September 2, 2014 4 REGISTRATION INFORMATION ONLINE REGISTRATION BEGINS APRIL 1, 2014 For Village of Grafton Residents and Non-Residents *Beginning April 7, Mail-In and Drop Box registrations will be processed in the order they are received. All participants who register online in the month of April will be entered into a drawing to win either a 2014 Family Aquatic Center Pool Pass or a certificate for a park rental. The winners will be notified in early May. 3 EASY WAYS TO REGISTER ONLINE REGISTRATION You can view activities prior to registration, review program details, check available openings, and register online from the comfort of your home, 24 hours a day, 7 days a week. Benefits of online registration: Quick: Avoid waiting for mail to be processed. Timely: Receive email confirmations. Private: Access your secure account details. Secure: Safe online payments with Visa, MasterCard or Discover. A username and password are needed to register online. If you have previously registered in our programs, sign in using your information. REGISTRATION INFORMATION 5 24 HOUR DROP BOX A 24 hour drop box is located just outside the main entrance of the Municipal Services Facility, 675 N. Green Bay Road. Drop box registrations must include registration form and payment (Visa, MasterCard, Discover, or a check made payable to Village of Grafton.) MAIL Mail in registrations will be processed starting April 7, for Residents and Non-residents. Please mail your completed registration form (page 38) with payment to: Village of Grafton, Parks and Recreation Department 675 North Green Bay Road Grafton, WI 53024 *All mail and drop box registrations will be processed in the order they are received. PROGRAM REFUND POLICY 1. Unconditional refunds will be allowed 2 days prior to the start of a program. An administrative fee of $10.00 will be assessed for refunds meeting these criteria. Registrations paid by credit card will be credited back to the credit card less any applicable administrative fee. To avoid the administrative fee, you may apply the refund as a credit to your account and use the funds toward a future registration. 2. Illness and/or injury refunds will be allowed after the start and before completion of 50 percent of a program. Fifty percent of the program will be assessed as an administrative charge for refunds meeting this criteria. 3. Refunds will not be allowed after completion of 50 percent of a program. 4. Full refunds will be given for Department cancelled programs such as insufficient registration for a particular swim session. Prorated credit toward future registrations will be given for Department cancelled program dates. FACILITY USAGE REFUND POLICY 1. Unconditional refunds will be allowed 30 days prior to a park/facility usage date. A $10.00 fee will be retained as an administrative charge. 2. For park cancellations less than 30 days prior to a park/facility usage date, 50 percent of the park fee will be retained as an administrative charge. 3. Refunds will not be granted on or after a park facility usage date. 4. Full refunds will be given if the Village of Grafton cancels a park reservation date. (i.e., closed due to repair or maintenance.) 6 REGISTRATION/PROGRAM INFORMATION STUDENT VOLUNTEER OPPORTUNITIES Are you looking for community service work for your college applications? Call the Parks and Recreation Department at 262-375-5310 up to 30 minutes prior to the start time *Mid-Ozaukee Softball will be called at the fields. CONCUSSION FORMS The Grafton Parks and Recreation Department requires all participants to review the Concussion Awareness Information on page 39 prior to registration. Certain programs will require signed concussion forms prior to participation. If registering online, please be sure to read and check the waiver box. If registering by mail or drop box, please check the concussion form box to confirm you have read the information sheet and sign. HEAT INDEX POLICY The Parks and Recreation Department may require that outdoor activities be cancelled when excess heat and/or humidity are dangerous to program participants, staff and spectators. When the National Weather Service (www.nws.noaa.gov) issues a Heat Advisory or Warning for our area, the following guidelines will be implemented: Heat Index of 105 or above All outdoor youth and adult activities will be cancelled! It is the responsibility of the parent/guardian to make the ultimate decision as to the participation of their child when heat may be a factor, taking into consideration the age and physical condition of their child. Please provide proper sunscreen prior to programs with limited shade protection. The Parks and Recreation Department will make every effort to reschedule classes cancelled due to inclement weather, provided the instructor is available. These are only some of the activities this playground program will offer boys and girls. The program will prepare participants who are interested in playing high school or recreational volleyball. Campers will have fun learning dance routines choreographed to popular songs. This is an excellent opportunity to prepare for the High School Dance Team. The last day of the program, participants will have an opportunity to show what they have learned. Parents are welcome. The purpose of the American Red Cross Babysitter’s Training Course is to provide boys and girls, ages 11 and up with the information and skills necessary to provide safe and responsible care for children in the absence of parents or adult guardians. This training will help participants develop skills in five critical areas: care for children and infants; to be good leaders and role models; solve problems and stay safe; handle emergencies such as injuries and accidents; write resumes, and interview for jobs. Participant book, Emergency Reference guide, and a CD-Rom loaded with interactive tools to help participants run their own babysitting business is included in the program fee. Instructed by a certified American Red Cross Babysitter’s Training Instructor. Certificates will be issued to participants who successfully complete all skills and knowledge tests. The program is designed to encourage kids to have fun with the soccer ball and to help develop basic skills. Children are exposed to fundamentals, techniques and games. Micro Soccer will provide your child with exposure to the learning process, social interaction and teamwork. Parents are welcome to actively participate with their child. Bring cleats, shin guards, a water bottle and ball (if your child owns one) with their name on it. Please Indicate T-shirt size on Registration Form. In partnership with the Parks and Recreation Department, this 1 week camp provides players of all ages and abilities the rare opportunity to receive high-level soccer coaching from a team of international experts. Each day includes individual foot skills, technical drills, tactical practices, small-sided games, coached scrimmages, and a daily tournament. Equally important, the Challenger coaching staff provides your child with lessons in respect, responsibility, integrity, sportsmanship and leadership. Camp includes a free ball and shirt and a free British Soccer jersey for online registrations 45 days prior to camp! Hitting, throwing, catching, and base running will be stressed along with organized games. Picture Day! July 15 at 9:00 a.m. Wear your team t-shirt. Individual pictures and purchase of picture package is voluntary. SESSION Tuesday and Thursday Centennial Park July 8 - 31 FEE: Resident Kindergarten (5 years old) 9:00 - 10:00 a.m. $25.00 Non-resident $41.00 FUTURE STARS BASEBALL CAMP This four-day camp, designed specifically for 7-8 and 9-10 year olds, will include instruction in infield, outfield, hitting and throwing mechanics. Campers will play an instructional game each day. Also included are fun competitions/games for the kids to use what they have learned all week. Winners of each competition will be awarded a prize at the end of each day. Campers learn from experienced High School players. Campers should bring baseball glove, baseball shoes, and appropriate clothing for weather changes. Camp size is limited, camper to instructor ratio 10:1. In-swing fundamentals include: club path, timing, tempo and balance. Short game skills like chipping, pitching and putting will be introduced. Learn how to play the game using a state-of-theart practice range and finish your week using a regulation golf course to test your new skills! Class size is limited. Students must bring their own golf clubs. Each student will receive a participation gift! Classes will be taught by PGA Certified Instructor, Andy Runkel. Participants should bring their own racquet. Due to repairs being made to the High school Tennis Courts, this program is being relocated to the Broad Street tennis courts. Class sizes will be limited. The courts are open to the public, when not being utilized by the Ozaukee Tennis Recreation League. 13 HERSHEY TRACK MEET Wednesday, May 14 Learning, participation, enjoyment and physical exercise are the main elements of this program. This FREE program is for boys and girls ages 9 - 14 years of age. Participants are limited to 3 events No spiked or turf shoes allowed Age Groups: 9-10, 11-12, 13-14 Open to boys and girls (age as of December 31, 2014) PRE-REGISTRATION IS REQUIRED! Swim Team should not be used as an alternative to Swim Lessons. Each child must be accompanied by an adult, and wear a “swim diaper” in the pool. BUBBLES - (Beginner) Students must be age 3 to enter program. This is the child’s first water experience without a parent. Children are introduced to the water through games and activities. Bubbles is a water exploration class designed for children to become comfortable in the water. UNDERWATER BUBBLES - Students must be age 3 to enter program. Children must be able to put their face/head in the water. Must also be willing to start dependent floating and kicking. PRE-LEVEL RED - MUST BE ages 3 - 5 years and passed Underwater Bubbles. This new class is being offered to children who have passed Underwater Bubbles but are too young for Level Red. Your child’s ability to follow directions will open up unique challenges. RED - MUST BE ages 5 years and older, no exceptions. Prerequisites: put entire head in the water, have a general security with water, and be familiar with front and back floats. The Parks and Recreation Department now accepts Late Swim Lesson Registrations will not be accepted on the 1st day of lessons. If openings exist after the 1st day, you may register on the 2nd day at the Parks and Recreation Department. NOTE: The following information is designed to help participants place students in an appropriate level. The skills listed below are prerequisites that must be attained BEFORE the child is placed in the following levels. Ages are approximate guidelines. ORANGE - Ages 6 and older. Prerequisites: float on back, roll over onto front independently, and unsupported kicking with a kickboard. YELLOW - Ages 7 and older. Prerequisites: front/back crawl and front/back glides, and sculling. GREEN - Ages 8 and older. Prerequisites: coordination (breathing and arms) front crawl, at least 12 yards of front/back crawl (will be expected to swim 25 yards by the end of the session) elementary backstroke and be able to jump off diving board. BLUE - Ages 9 and older. Prerequisites: swim 25 yards of the front/back crawl (50 yards at end of session), elementary backstroke, sidestroke kick, butterfly kick, breaststroke kick, and standing dive, tread water for 1 minute. INDIGO - Ages 10 and older. Prerequisites: swim 50 yards of front/back crawl (100 yards at end of session), 25 yards of sidestroke and breaststroke, perform an open turn, and dive from side. VIOLET - Ages 10 and older. Prerequisites: 75 yards of front/back crawl using flip turns, 25 yards of sidestroke, breaststroke, and butterfly. Shallow dive from the side. Tread water for 2 minutes. Each park and open space serves it own special purpose within the community. The following guidelines are followed in determining reserved usage: A permit must be filled out in person at the Parks and Recreation Department. Reservations will not be accepted before January 1st. Exception: Park reservations for the Robert P. Zaun Pavilion will be accepted starting September 1 (or first business day following September 1) for the following calendar year. The fee is due at the time a permit is filled out. Permits are issued on a first come, first served basis. Town residents are considered non-residents for park usage. Thank you! 37 The Grafton Parks and Recreation and Cedarburg Parks, Recreation, and Forestry are scheduled to host the NFL’s Punt, Pass, and Kick competition. The NFL PUNT, PASS, AND KICK (PPK) program is a FREE national skills competition for boys and girls between the ages of 6 and 15 to compete separately against their peers. Girls and boys in five separate age divisions (6-7, 8-9, 10-11, 12-13, 14-15) compete against each other in punting, passing and place kicking in a fun and engaging forum. A concussion is caused by a bump, blow, or jolt to the head or body that causes the head and brain to move rapidly back and forth. Even a “ding,” “getting your bell rung,” or what seems to be a mild bump or blow to the head, can be serious. WHAT ARE THE SYMPTOMS OF CONCUSSION? Signs and symptoms of concussion can show up right after the injury or may not appear or be noticed until days or weeks after the injury. If an athlete reports one or more symptoms of concussion listed below after a bump, blow, or jolt to the head or body, s/he should be kept out of play the day of the injury and until a health care professional, experienced in evaluating for concussion, says s/he is symptom-free and it’s ok to return to play. SIGNS OBSERVED BY COACHING STAFF Appears dazed or stunned Is confused about assignment or position Forgets an instruction Is unsure of game, score, or opponent Moves clumsily Answers questions slowly Loses consciousness (even briefly) Shows mood, behavior, or personality changes Can’t recall events prior to hit or fall Can’t recall events after hit or fall SYMPTOMS REPORTED BY ATHLETES Headache or “pressure” in head Nausea or vomiting Balance problems or dizziness Double or blurry vision Sensitivity to light Sensitivity to noise Feeling sluggish, hazy, foggy, or groggy Concentration or memory problems Confusion Not “feeling right” or “feeling down” CONCUSION DANGER SIGNS In rare cases, a dangerous blood clot may form on the brain in a person with a concussion and crowd the brain against the skull. An athlete should receive immediate medical attention if after a bump, blow, or jolt to the head or body s/he exhibits any of the following danger signs: One pupil larger than the other Is drowsy or cannot be awakened A headache that gets worse Weakness, numbness, or decreased coordination Repeated vomiting or nausea Convulsions, seizures, or slurred speech Cannot recognize people or places Becomes increasingly confused or agitated Has unusual behavior Loses consciousness for any amount of time WHY SHOULD AN ATHLETE REPORT THEIR SYMPTOMS? If an athlete has a concussion, his/her brain needs time to heal. While an athlete’s brain is still healing s/he is much more likely to have another concussion. Repeat concussions can increase the time it takes to recover. In rare cases, repeat concussions in young athletes can result in brain swelling or permanent damage to their brain. They can even be fatal. WHAT SHOULD YOU DO IF YOU THINK YOUR ATHLETE HAS A CONCUSSION? If you suspect that an athlete has a concussion, remove the athlete from play and seek medical attention. Do not try to judge the severity of the injury yourself. Keep the athlete out of play the day of the injury and until a health care professional, experienced in evaluating for concussion, says s/he is symptom-free and is ok to return to play. Rest is key to helping an athlete recover from a concussion. Exercising or activities that involve a lot of concentration, such as studying, working on the computer, or playing video games, may cause concussion symptoms to reappear or get worse. After a concussion, returning to sports and school is a gradual process that should be carefully managed and monitored by a health care professional. As a parent and as an athlete it is important to recognize the signs, symptoms, and behaviors of concussions.
