To receive your DANNON coupons by mail, please do the following:
1. Fill out this form completely, including marking which Level you have earned through purchase (see below).
2. Circle the participating products and their corresponding prices on your original dated cash register receipt
3. Put both the completed form and original cash register receipts in a stamped envelope and mail to:
