Laugh your way back up! There is definitely some truth to the proverb, “laughter is the best medicine.” Laughter can boost the immune system, improve one’s mood and increase energy levels. In addition, laughter is great for the heart because it can lower blood pressure. With heart disease being the No. 1 cause of death in women, it’s important to optimize our cardiac health. Therefore, we need laughter in addition to proper nutrition and exercise. Our annual women’s event: “Live Well, Laugh Often,” includes a special performance by comedian Michele Balan on October 25, at 5:30 p.m., at the Preakness Hills Country Club in Wayne. Michele was one of the finalists on NBC’s “Last Comic Standing,” and she was “The Last Woman Comic Standing.” She has appeared on the Joy Behar Show, Byron Allen’s “Comics Unleashed” and the “Outlaugh Festival” on MTV’s Logo network. Michele was voted one of the “Top 10 Comics” by Backstage magazine. Then again, it’s not every day that one of our patients experiences a truly remarkable recovery. As far as we’re concerned, special circumstances sometimes require a few special accommodations, like rolling out the red carpet for a favorite furry friend. The Chilton family was overjoyed to do just that, and more, for Christina Parente of Oak Ridge, who overcame tremendous challenges to survive a life-threatening illness. Parente’s traumatic ordeal began in April 2011, when she was rushed to Chilton’s Emergency Department after contracting a lung infection. What she thought was the flu turned out to be a severe case of pneumonia, putting her in critical condition and inducing a coma. Within hours Parente was admitted to Chilton’s Intensive Care Unit, where she was promptly evaluated by Dr. Timothy O’Donnell, who specializes in pulmonary disease medicine and critical care. While pneumonia is quite common, Dr. O’Donnell explains that it can be dangerous for individuals with certain risk factors, such as the elderly and those with weakened immune systems. Sometimes, as in Parente’s case, the infection can strike relatively healthy individuals and lead to serious complications, such as respiratory distress, infection, sepsis, even organ failure. “Chrissy faced all of those things, requiring surgery, respiratory support and other aggressive treatments, as well as the collaboration and expertise of many physicians,” says Dr. O'Donnell. Parente’s medical team at Chilton also included Dr. Richard Krieger (Infectious Diseases), Dr. Hemogenes L. Ciocon (Thoracic Cardiovasular Surgery) and Dr. Larisa K. Gershman (Internal Medicine) along with the critical care nurses who provided around-the-clock care. “The doctors and nurses were just amazing and never gave up hope,” asserts Carol-Lynn Parente, Chrissy’s sister. “They became our family, especially the nurses, who not only provided excellent care to Chrissy but also were nurturing and compassionate to us during this scary time.” Parente’s loved ones remained at her side throughout her extended stay, spending 12 to 14 hours at the hospital every day, enduring setback after setback, until she finally emerged from her coma and began to heal. Although Parente doesn’t remember much about her treatment, she vividly recalls the warmth and kindness she received when she awoke. “Everyone was so happy to see me,” One happy — and very grateful — family. Chrissy Parente with her father Rudy Parente and sister Carol-Lynn Parente. she says. “My family introduced me to the nurses, who were very comforting, supportive, patient… and always there for me.” Parente even received a bedside visit from Elmo, which was arranged by her sister, who works at Sesame Street. “Chrissy was a very sick young lady when she arrived,” recalls Dr. O'Donnell. “But despite all the odds against her, she survived and has gone on to do quite well.” Thanks to the extraordinary efforts of the entire Chilton team, Parente is enjoying a new lease on life — and doesn’t take a single day for granted — as she continues her journey toward a full recovery. In fact, she’s even considering a new career in health care and hopes to work at Chilton some day! “I would love the opportunity to give back,” she says, expressing a sincere desire to help others heal and persevere through challenging times. While she’s grateful to put the experience behind her, Parente and her family visit the hospital frequently, forever bonded to the caregivers with whom they shared birthdays, holidays, tears and ultimately triumph. However, the mere mention of any possibility of breast cancer can be quite a traumatic experience. Jennifer Orsillo, a 15-year veteran of the Breast Center, serves as a business partner. She offers personalized care at a level not found in other medical settings. Jennifer is the contact person for patients, whether they are Chilton patients or come from an outside physician referral. “I first contact the patient’s doctor to inform him or her that we recommend a biopsy,” Jennifer Jennifer Orsillo explained. “Then I’m there for the patient when she gets home and needs someone to talk to. Patients can call me any time with questions about the procedure or insurance. Many women have households to run and are under a lot of pressure to begin with, so I help take the burden off of them.” Jennifer is dedicated to patient care even in times of emergency. On one occasion, she came in on a Saturday during a snowstorm just to meet with an anxious patient and answer questions for her. According to Lead Mammography Technologist Lynn Case, Jennifer “goes above and beyond her duties” to help her patients. “She does whatever it takes to help put them at ease,” she said. “She is very knowledgeable about every type of biopsy we perform so she can answer any questions. Jenn is the perfect combination of kindness, knowledge and efficiency. We are very fortunate to have her.” Although Jennifer’s role of business partner is vital to enhancing the patient experience, she humbly declines full credit. “What I do at the Breast Center is a small portion of what everybody does here, but we all have the same goal: We are here for our patients,” she said. “We care and we want to make sure that everyone is taken care of. Breast cancer is the second leading cause of cancer-related deaths in women. Although there is no “magic bullet” prevention or cure, a healthier diet may help reduce the risk of getting the disease. Chilton Hospital’s Clinical Nutrition Manager Leslie Killeen, MS, RD, CDE, offers the following nutrition tips: Which vegetables are the most beneficial for avoiding breast cancer? “The best kinds of vegetables are ‘cruciferous.’ They contain phytochemicals, which may have a protective effect against some types of cancer, including breast cancer. Cruciferous vegetables include broccoli, cabbage, turnip, Brussels sprouts, cauliflower, kohlrabi, bok choy, watercress, collards, kale, mustard greens and rutabaga.” What other foods are beneficial? “Choose plant-based food, such as whole-grain breads and cereals, nuts, seeds, brown rice, whole wheat pasta and beans. These foods are high in fiber, which speeds the passage of food through the body. In turn, cells have less exposure to potentially carcinogenic waste. Fiber also helps remove excess estrogen from the digestive tract. In its early years, patients endured numerous days of recovery while confined to a bed, hooked up to many machines and on various pain medications. Today, recovery from TKR is much easier and less painful. A patient at Chilton Hospital is expected to begin physical therapy the next day and return home in three days. How is this possible? The solution is an advanced anesthesia method called a continuous femoral nerve block, which prevents pain signals from being transmitted from the thigh and knee to the brain. It’s similar to a dentist numbing an area of the mouth for a filling. Another advancement in TKR is the use of ultrasound technology to help physicians locate the femoral nerve for blocking much easier, faster and safer than ever before. “Once a femoral nerve block catheter is placed via ultrasound, a small, portable pump will deliver continuous local anesthesia before and after surgery, so pain is always minimized,” explained Chilton Anesthesiologist Andrew Topf, MD. “We leave the pump and catheter in for two days, and give the patient one pain relief pill every couple of hours, just in case. This is a huge improvement in patient comfort over the traditional method of intravenous morphine and limited mobility.” The femoral nerve block and portable pump provide a smoother transition to physical therapy, according to Dr. Topf. “In the past, both legs had to be numbed and physical therapy T could be quite painful,” he explained. “Now, only one leg is numbed and the pump is so unnoticeable that it doesn’t impede rehabilitation.” Another way that Chilton is increasing comfort for TKR patients is through personalized care. “I contact patients the night before surgery to explain the anesthesia process so they know what to expect and are familiar with me and anyone else managing their care,” Dr. Topf assured. “I coordinate with the nurse, physical therapist, social worker and surgeon so that the patient’s post-operative course is mapped out ahead of time. After the surgery, I see the patient every day to check on their progress.
