You’ll be prompted to enter a one-time password that you’ll receive by text message, automated voice call or by using a Remote Access SafeWord/Mobile Pass card. Be sure your home and cell phone numbers are up to date on Marketplace to receive the MFA password. This ensures that your personal data as an employee has the same level of security that applies to our banking customers. Medical plan changes We’re offering the same medical plans* in 2015, but we’re making some changes to those plans. These changes include higher contributions for the High Deductible Health Plan (HDHP) and higher coinsurance (your portion of the cost of care) for both the ChoicePlan 500 (CP500) and HDHP. What isn’t changing Most of our benefits aren’t changing significantly for 2015. And Citi’s commitment to offering employees access to quality health care coverage isn’t changing either. Health care remains a significant operating expense for Citi. Each of us must help manage this expense in order to continue to make health care benefits possible. PRINT HOME Medical plan changes Offering medical plans that are affordable for both you and Citi — plans that encourage a healthy lifestyle — is more important than ever. For 2015, we’re making a few changes to our medical plans. Paycheck contributions If you’re currently enrolled in the High Deductible Health Plan (HDHP), you may notice a greater increase in your contributions again this year compared to the other medical plans. As we mentioned last year, this is part of our effort to contribute more equally to the cost of the medical plan options. Find out how you can reduce your paycheck contributions by earning Live Well Rewards and avoiding the tobacco penalty. Also, consider how to avoid paying for too much coverage by selecting the most appropriate plan for your needs. Changes to the ChoicePlan 500 (CP500) and Oxford PPO Plans These plans will have higher coinsurance — your portion of the cost of care — in 2015. If you use an in-network provider or facility, Citi will now pay 80% of the cost, compared with 90% in 2014; if you go outside the network, Citi will pay 60%, compared with 70% in 2014. Keep in mind that coinsurance applies after you’ve met the annual deductible and is capped by an out-of-pocket maximum. In addition, there will be separate out-of-pocket maximums for medical and prescription drug expenses. Currently, you have an out-of-pocket maximum that includes medical costs, but not prescription costs. Should your out-of-pocket expenses exceed the out-of-pocket maximum, the plan will pay 100% of covered medical expenses for the remainder of the plan year. In 2015, your out-of-pocket prescription drug costs will be capped at a maximum of $1,500/Individual and $3,000/Family. However, if you are enrolled in an HMO plan, we encourage you to contact your HMO plan administrator for more information. Prescription drug changes for certain medications Changes to the Express Scripts Step Therapy, Prior Authorization, and Drug Quantity Management pharmacy programs will help provide better outcomes through closer collaboration with patients and their health care providers to ensure they are receiving the right drugs, in the right quantities, at the right time. If you are personally impacted by any of these changes, you will receive a letter from Express Scripts with more information. 3 2015 Interactive guide to enrollment Ready to enroll? > PRINT HOME Medical plan changes (continued) TotalComp@Citi > Other changes to know about Live Well at Citi > n The HDHP will have higher out-of-network coinsurance in 2015. If you use an in-network provider, Citi will pay 80% of the cost. If you use a provider or facility outside the plan’s network, Citi will pay 60% of the cost after you meet the annual deductible, up to the out-of-pocket maximum. Resources at a glance > n For both the HDHP and CP500, keep in mind that networks change from time to time. It’s always important to check whether your doctors are in-network before receiving care. n Physical and occupational therapy will be limited to 60 visits combined. There will also be a 90-visit limit for speech therapy. Contact your medical plan carrier for more information. n As of May 1, 2014, Aetna modified acupuncture coverage to include nausea related to pregnancy and post-operative and chemotherapy-induced nausea and vomiting. In addition, some formerly covered services are now excluded from coverage. n Oxford is making some changes to its covered services: — Newborn facility services: Coverage will now be under the child, not the mother, and is subject to the plan’s deductible and coinsurance. — Coinsurance for labs is changing. Citi will now pay 80% in-network or 60% out-of-network for lab services after you meet the deductible. n Some HMO benefits are changing. The CP500 and HDHP will have the same coinsurance in 2015, making them more similar than ever before. However, with the HDHP, you have the option of opening and contributing to a Health Savings Account (HSA) to pay for eligible health care expenses now or in the future. Citi will even contribute to this account on your behalf. Take time to learn about the HDHP to see if this is the right plan for you. Resources at a glance > Dental plan changes Vision plan changes We’re decreasing dental contributions for both the MetLife PDP and Cigna DHMO plans and increasing copays for some services in the Cigna DHMO, such as sealants, space maintainers, inlays, onlays, and crowns. We’re making some changes to the vision plan, including changes in what you pay for premium progressive lenses and premium lenses with antireflective coating. In addition, the amount you pay out of your paycheck for vision coverage will increase in 2015. For more information on the vision plan changes, refer to the uniform summary of benefits and coverage (SBC) provided to you by Citi. Chat with a doctor online or by phone Anthem and Aetna will offer the option to connect live with a doctor online or by phone through a telemedicine service in 2015. You’ll have 24/7 on-demand access to a national network of boardcertified doctors and pediatricians who can video chat, talk by phone and even fill prescriptions by email, saving you time and money when you need it most. As the spouse/domestic partner of a Citi employee, you can also evaluate your current coverage and help select Citi benefits for 2015. This year’s annual enrollment period will take place from October 6 to October 31, 2014. The Citi enrollment period is based on the state in which your Citi spouse/domestic partner works. Note to Citi employees: Know your options The information in this section is intended for your spouse/domestic partner. Please ask your spouse/domestic partner to review this material so you can both evaluate which coverage — Citi coverage or non-Citi coverage — provides the most value to you and your family. You and your Citi spouse/domestic partner should assess your collective medical plan options to determine how well they meet your individual and family needs. Some suggestions are: “Spouse/domestic partner” includes legal spouse (same or opposite sex), domestic partner and civil union partner. n Learn about Citi’s ChoicePlan 500 (CP500) and the High Deductible Health Plan (HDHP) to ensure you aren’t paying for more coverage than you need. n If you’re currently covered under a Citi medical plan but you have access to another plan, consider which plan provides the most value. Compare the cost of enrolling separately in “Employee Only” coverage through your own employer’s plan with the cost of spouse/family coverage through Citi. n If you have children, evaluate the best way to cover them. Tobacco penalty applies to covered spouses and domestic partners Employees and their spouses/domestic partners who are covered under one of Citi’s medical plans must attest to their tobacco user status during annual enrollment. If your spouse/domestic partner is enrolling in a Citi medical plan – even if it’s the same medical plan as 2014 – he or she will pay a $600 tobacco penalty if either of you uses tobacco products ($1,200 total if both of you use tobacco products), or if he or she doesn’t attest to both of your tobacco-user statuses before the end of his or her enrollment period. 7 2015 Interactive guide to enrollment PRINT TotalComp@Citi > A guide to annual enrollment for spouses and domestic partners (continued) Live Well at Citi > Live Well at Citi Ready to enroll? > Resources at a glance > Note to Citi employees: The information in this section is intended for your spouse/domestic partner. Available to all employees and their families, the Live Well at Citi Program offers resources and tools, including telephonic health coaching and online programs, to help you manage your health care and achieve your health goals, such as losing weight or managing chronic pain. The programs and resources are free and designed to be easy to use and secure. If you’re enrolled in Citi health coverage and you complete the Live Well Health Assessment (offered through RedBrick), you can earn a Live Well Reward of $150 toward the cost of your health coverage ($300 total if your Citi spouse/domestic partner also earns the Reward) between October 1 and October 31, 2014. If your Citi spouse/domestic partner declines to enroll in Citi’s health coverage, you can still complete the Live Well Health Assessment and receive rewards through a Health Care Spending Account (HCSA) set up under your Citi spouse/domestic partner’s name. Visit the RedBrick Portal to take the Live Well Health Assessment. You’ll be prompted to create a user ID and password if you don’t already have one. The Health Assessment is your gateway to all the health improvement programs offered by RedBrick, and the first step to earning Live Well Rewards! Note: If you’ll be covered under your spouse’s/domestic partner’s Citi benefits for the first time in 2015, you can earn a Live Well Reward of $150 by completing the Health Assessment between November 10 and November 21, 2014. What is needed to show proof of a domestic partner of the opposite sex, to have him/her covered under my health plan? Citi offers benefit coverage to your certified or registered unmarried domestic partner of the same or opposite sex. You must submit a domestic partner coverage application or your registration, as applicable, before you can enroll a domestic partner or a domestic partner’s child(ren) under your Citi coverage. My spouse’s benefits enrollment isn’t until mid-November; how can I make the best decision for my family without having all the information from both companies? Use the information available to you now to make the best decision for your family’s needs. When your spouse becomes eligible to enroll in benefits in November, you can compare your options and make a change at that time if you want. Since this is a qualifying change in status, you will have 31 days from your spouse’s enrollment period to change your Citi benefits. No, you are not required to complete the Tobacco Free Attestation or Health Assessment. However, the Health Assessment is available to all benefits eligible employees, and we encourage you to take it to better understand your health status and risks. Taking the Health Assessment will also allow you to access the free Live Well Healthy Lifestyle Programs. PRINT HOME Answers to the top 10 questions employees ask during enrollment 7 I’m currently in the HDHP with an HSA. What happens to the balance in my HSA if I switch to the CP500? You will not lose any accumulated funds in your account, since you own the HSA. However, if you switch to the CP500, you will no longer be able to make contributions to your HSA. You can use the funds that remain in your HSA to pay for qualified medical expenses incurred after you enroll in another plan. 8 What happens if I have an emergency early in the year and don’t have enough money in my HSA to cover the out-of-pocket costs? If the amount of your medical expense exceeds the amount in your HSA you would be responsible for paying that expense out of pocket. You may reimburse yourself from your HSA as additional funds become available. 9 Can I change the amount I contribute to an HSA at any time during the year? Yes, contributions (subject to annual limits) can be changed at any time as long as you continue to be enrolled in a qualified HDHP. 10 My child is 26 years old and covered under my plan from Citi. Her eligibility ends in December 2014. Should she enroll herself in health coverage from the public Marketplace through an exchange? As a reminder, under Health Care Reform, everyone is required to have health care coverage or pay a penalty. There are many options available through private insurers as well as state and federal marketplace options. Your child should look at his or her options carefully to determine which might best meet his or her needs before making a decision. Did you know that many employees enrolled in the ChoicePlan 500 (CP500), could have saved money by enrolling in the High Deductible Health Plan (HDHP), such as the 25% of participants who had less than $500 in claims last year? Citi wants to help you choose the best medical plan for you and your family. Find out how you can avoid paying for too much medical coverage. n Think about how much you’ve spent on health care in 2014, and for what. Do you think you’ll use the same medical services in 2015 that you used in 2014? n Carefully consider your and your family’s health care needs for 2015. Only you can decide which plan is best for you. You owe it to yourself and your family to think about how you will use benefits in the coming year (e.g., annual physicals, medications) and determine which benefits will help you maximize your budget while meeting your health care needs. n Think about the whole cost. The deductible amount shouldn’t be the only factor you consider when comparing plan costs. The same goes for premium contributions. It’s important that you think about the total cost you’ll spend out of your paycheck and out of your pocket. n Consider both the HDHP and the CP500. WHAT’S THE SAME? Both plans cover the same services and are designed to help you take control of your health and the dollars you spend on your health care. Each plan covers 100% of the cost for certain recommended preventive care services with no out-of-pocket cost to you when services are provided in-network. And, both plans offer specialized programs to help manage chronic conditions. WHAT’S DIFFERENT? A key difference between the plans is when you pay. So both plans offer you the option to choose your own doctors. If you choose an innetwork provider, you typically pay less for your care. Annual enrollment is your opportunity to make informed decisions for 2015. If you don’t take action by the deadline: 1 2 4 5 You’ll pay the tobacco penalty: If you’re currently enrolled in a Citi medical plan and don’t take action during your annual enrollment period, you’ll pay the tobacco penalty for 2015, regardless of whether or not you use tobacco products.
