CommunityReport 2015 Focused Letter to Our Community In 2015, WellSpan Good Samaritan Hospital experienced many exciting changes — including the new affiliation with WellSpan Health. As the new president of WellSpan Good Samaritan Hospital, I am enthusiastic about working alongside our many hardworking doctors, nurses, care professionals and support staff to build an even healthier Lebanon County. WellSpan Good Samaritan Hospital has a long history with generations of families in Lebanon County, and WellSpan is committed to building on this solid foundation. A highlight of recent successes include multiple awards recognizing the hospital’s quality care, the continuation of important community wellness programs and the opening of the Sechler Family Cancer Center, which brings leading-edge, integrated cancer care to our community. We celebrate Bob Longo and his many years of service as president of The Good Samaritan Hospital. His 32 years of dedication and leadership made it possible for Lebanon County residents to have access to key services such as open heart surgery, wound care and many specialty services that improve the lives of our friends and neighbors. This affiliation will help improve health care in Lebanon County and throughout the WellSpan Health service area by jointly recruiting physicians, sharing best practices, improving patient access to needed services and continuing efforts to deliver the highquality health care that patients deserve. The affiliation followed nearly a year of due diligence by both community boards of directors and senior leaders and approval by state and federal regulatory agencies. A shared focus WellSpan Health is a regional nonprofit health care organization working to support and enhance local systems of care 3 in Lebanon, Lancaster, York and Adams counties. WellSpan Health now comprises more than 15,000 employees, more than 900 physicians and advanced-practice clinicians in its multispecialty medical group and more than 140 patient care locations – including six respected hospitals. “We are delighted to have joined WellSpan Health,” said Robert J. Longo, recently retired president of WellSpan Good Samaritan Hospital. “Our board was very deliberate about working to select a partner that shared our mission, vision and values. Good Samaritan has an exemplary record for quality patient care and high patient satisfaction scores. In WellSpan, we have a partner who will help us continue to provide exceptional care in our community and also has the strength and expertise to navigate this era of change.” “Under health care reform, we are moving away from a system that primarily treats those who are sick and injured to one in which we actively work to manage the health of populations,” said Kevin Mosser, MD, president and CEO of WellSpan Health. “Partnering with Good Samaritan will help us in our efforts to achieve the scale and efficiencies needed to prepare our organization for the requirements of transforming health care in the future.” WellSpan Good Samaritan Hospital continues to be governed by a local board of directors, including members of the community and representatives from WellSpan Health. The local board has a formal relationship with the WellSpan Health board of directors. WellSpan’s organizational entities, which include the WellSpan Medical Group, WellSpan Specialty Services, WellSpan Ephrata Community Hospital, WellSpan Gettysburg Hospital, WellSpan York Hospital, Philhaven (which joined WellSpan in January 2016) and three philanthropic foundations, are governed pursuant to this structure. The Good Samaritan Hospital Foundation supports WellSpan’s health-related initiatives across Lebanon County. Community-based philanthropic foundations also exist in Lancaster, York and Adams counties. Committed to local communities In addition to the affiliation with Good Samaritan, WellSpan continues to grow through service expansion and partnerships with other organizations who share its commitment to improving the health of local communities. On Jan. 1, 2016, WellSpan and Philhaven – the 14th largest mental and behavioral health provider in the nation – finalized their affiliation, which merged the region’s most complete continuum of behavioral health services and central Pennsylvania’s largest, most comprehensive nonprofit health system. The merger will enhance access to behavioral health care across central Pennsylvania and improve care through the integration of mental and physical health. Through the affiliation, both WellSpan and Philhaven are committed to developing a behavioral health organization with diverse levels of care, support for all patients, innovative practices and positive outcomes. They also are committed to developing local education and training programs to attract future behavioral health providers to the region. 4 A Team Approach to Health Care Patient Centered Medical Homes shift focus from treating illness to lifetime wellness Health care in the United States is in the midst of a dramatic transformation – from a system focused primarily on treating individuals when they are sick or injured to one focused on keeping people healthy. As part of WellSpan Health, WellSpan Good Samaritan is introducing this approach to the Lebanon County community. The current health care system remains primarily a fee-for-service model, where hospitals, doctors and other health care providers are reimbursed by patients, their insurance companies or Medicare or Medicaid for the services provided to patients. WellSpan is moving toward a value-based environment, where providers will increasingly be reimbursed based on health care outcomes. The increased focus on preventive care, as well as health care quality and safety, not only benefits patients but also may help reduce unnecessary health care costs and improve overall community health. WellSpan’s approach to health care is centered on partnering with communities and residents to improve health by helping individuals to not only get better but also to stay healthy. At the heart of this approach is establishing Patient Centered Medical Homes (PCMH) through primary care practices. Nearly 30 WellSpan Medical Group primary care practices have achieved PCMH status in York and Adams counties, with several Lancaster County practices expected to join their ranks in the near future. Two of Good Samaritan’s primary care practices have PCMH status. The PCMH designation is awarded by the National Committee for Quality Assurance, and earning it is difficult. Practices must adopt cutting-edge techniques in pursuit of a primary care triple aim: better population health, better experience of care and lower cost. Through the PCMH, physicians and caregivers work together with patients to achieve optimal health. WellSpan’s success in improving local communities of care in Adams and Lancaster counties serves as examples of what Lebanon County residents can expect as a result of Good Samaritan’s affiliation with WellSpan. New and expanded services now available in Ephrata include: WellSpan has increased access to health care services and enhanced the quality of local health care in Adams County since affiliating with Gettysburg Hospital in 1999. Enhancements include: • Maternal fetal medicine/specialized care for high-risk pregnancies. • New cardiac treatment services. • Neurology. • Bariatrics/medical weight management. • Urogynecology & gynecologic oncology. • An expanded operating room at WellSpan Ephrata Community Hospital. • Several new ambulatory facilities, enhancing local access to primary care, outpatient surgery, rehabilitation, lab testing services, diagnostic imaging and retail pharmacy. • Cardiac catheterization services at WellSpan Gettysburg Hospital. • Expanded outpatient rehabilitation services. • Enhanced local access to subspecialty care, including neurology, neurosurgery, behavioral health, cardiology, perinatology, orthopedics, rheumatology, endocrinology and urology. • Enhanced access to radiation oncology at WellSpan’s Adams Cancer Center. • A new emergency department at WellSpan Gettysburg Hospital. 6 Integrated Cancer Care Comes to Lebanon Sechler Family Cancer Center opens its doors 7 Lebanon County’s first and only comprehensive, integrated cancer center opened its doors on Jan. 4. WellSpan Good Samaritan Hospital’s new $13 million Sechler Family Cancer Center offers chemotherapy, infusion therapy and radiation treatment, all under one roof. The 22,000-square-foot facility also includes offices for physicians, social workers and dieticians, a pharmacy and a laboratory. Future plans include participation in clinical trials through an affiliation with a major research hospital. The oncology program at WellSpan Good Samaritan provides about 6,000 treatments per year. It saw 700 new patients in 2014. The center’s goal is to make treatment more seamless and convenient for cancer patients, who have an average of 80 medical-related visits in the first year after their diagnosis. An integrated comprehensive cancer center such as Sechler can cut the number of those visits in half because its team of caregivers evaluate patients, plan their care and work to coordinate visits at the center. The center’s Lebanon location also means less transportation time and expense for patients and their families. The oncology program at WellSpan Good Samaritan is ranked in the top 1 percent of cancer care practices in the state and is certified by the Quality Oncology Practice Initiative®, an affiliate of the American Society of Clinical Oncology. Plans for establishing the cancer center have been in the works since 2007, starting with research. In 2013, the hospital received a $3 million Redevelopment Assistance Capital Program grant from the state. Other funding for the $13 million cancer center included a $2 million donation from Scott Sechler, owner of Bell & Evans, Fredericksburg. In addition to the Sechler Center in Lebanon, WellSpan’s regional network of cancer centers includes locations in Gettysburg, Hanover, Ephrata and York. Sechler Family Cancer Center Lebanon County Cancer Facts • Over age 50, the chances of being diagnosed with cancer double. • Approximately 500 Lebanon County adults are diagnosed with some form of cancer each year. • Nearly 44 percent of men and 38 percent of women will develop cancer in their lifetimes. • In the first year of diagnosis, cancer patients on average have up to 80 physician visits, treatments, lab and pharmacy stops and other associated visits. WellLebanon.org, an online resource launched in 2015, offers easy access to information about health screenings and educational events in the community. The site focuses on the most common health concerns in Lebanon County and is designed to help visitors take charge of their health to prevent and better manage chronic health conditions such as asthma, diabetes and heart disease. In addition to information on local wellness events, there are links to community resources and the WellSpan health information library. Insights into health risks Preventive health screenings can help identify hidden health risks and serve as a first step in early intervention to prevent potential health conditions later in life. Understanding their risks gives patients an important tool for working with their doctor to set the course toward a healthier future. 9 Each year, WellSpan Good Samaritan Hospital holds hundreds of health screenings throughout Lebanon County communities. In fact, in 2015 more than 385 screenings were held for glucose and cholesterol alone. Other frequently offered screenings include blood pressure, Body Mass Index (BMI), physical therapy and vein issues. In 2015, visitors to community fairs, expositions and health events, such as the Women’s Expo, Latino Health Fair and the 50+ Festival, have benefited from free screenings. For many people, the results are a first insight into the risks of health problems such as diabetes and heart disease. Caring for the heart WellSpan Good Samaritan is committed to raising awareness of the most common cardiac diseases and providing important tips on how to reduce, manage and even eliminate the risk of cardiovascular disease. Each February, “Love Your Heart” programs focus on the important topics of this killer disease, including recognizing the symptoms of a heart attack, controlling high blood pressure, heart-healthy cooking for the whole family, designing an exercise program and information on making appropriate lifestyle changes to lower the risk of cardiovascular disease. Support when you need it Knowing that you are not alone in dealing with an illness or emotional challenge can be empowering. A support group can provide emotional encouragement and an opportunity to share experiences, invaluable in fighting a feeling of isolation or being “different.” Let’s get well, Lebanon WellSpan Good Samaritan’s Diabetes SelfManagement Education Classes address topics such as healthy eating, increasing activity, low and high blood sugar, medicines, reducing risk of complications, coping skills and meter use and blood glucose testing. Partnerships with wellness-focused community organizations, such as the Lebanon Valley Family YMCA, help deliver programs like the Farm-to-Table healthy cooking demonstrations. The Healthy Steps program offered at the Lebanon Valley Mall offers a climatecontrolled indoor environment that allows residents to stay active regardless of the weather outside. For children coping with the death of someone important in their lives, the Rainbow Connection program helps diminish the feelings of being alone or misunderstood by offering opportunities to share with peers who are also experiencing grief. The free program helps them understand that what they are feeling is normal, in a comfortable, safe and fun atmosphere. Rainbow Connection is offered several times a year to children ages 5-10 who have experienced a significant loss. During National Prostate Health Month in September, prostate cancer screenings and information sessions help men, who are often reluctant to take preventive measures with their own health, understand the risks associated with the disease. In October, Breast Cancer Awareness Month events underscore the importance of regular mammograms and other preventive measures. In November, Lung Cancer Awareness Month programs raise awareness of lung health and assist with smoking cessation. Cancer Support groups are held throughout the year and are open to cancer survivors and their caregivers. These groups provide important emotional support and encouragement and include special education programs on nutrition and emotional health along with Q&A sessions. Those are just a few of the ways WellSpan Good Samaritan Hospital connects with the community outside of traditional health care settings. Programs are offered year round and have a common purpose – to help every resident reach their health goals and to make Lebanon County a healthier place for all. 10 A Year of Growth New practices, physicians enhance services to the Lebanon community In addition to finalizing its affiliation with WellSpan Health and opening the doors to the new Sechler Family Cancer Center, WellSpan Good Samaritan Hospital added new practices and physicians in 2015 to better meet the needs of Lebanon County residents. The physicians of Lebanon Orthopaedic Associates joined the WellSpan Medical Group in September. “WellSpan is a leading health care organization that is committed to developing our local system of care here in Lebanon County, and we share the vision that they have for our community,” said Fotis Mystakas, MD, a founding member of Lebanon Orthopaedic Associates, now called WellSpan Orthopedics. In addition to Mystakas, the team of orthopedic surgeons includes Victor Faralli, MD; David Overholt, MD; and Richard Slagle, MD. Zachary Geidel, MD, joined WellSpan Orthopedics in Fall 2015, expanding the practice’s specialties to include sports medicine. 11 Sreenath Joshi, MD Olamide Odubogun, MD Good Samaritan Pediatrics on Cornwall Road in Lebanon welcomed Sreenath Joshi, MD, and Olamide Odubogun, MD. M. Scott Beltz, MD David J. Cozzi, MD at Yale New Haven Children’s Hospital, New Haven, Conn. Joshi completed a pediatric residency at the Children’s Hospital of Cleveland Clinic Cleveland, Ohio, as well as residencies in Neonatal-Perinatal Medicine, Pediatrics and Neonatology and in Pediatric Gastroenterology in the United Kingdom. M. Scott Beltz, MD, a well-known urologist in the Lebanon area, joined Good Samaritan Urology Associates. Beltz earned his medical degree from Indiana University School of Medicine and completed his residency at The Penn State Milton S. Hershey Medical Center. Odubogun earned his medical degree at the Penn State College of Medicine in Hershey, and completed a residency The addition of David J. Cozzi, DO, to the Good Samaritan Digestive Health Specialists physician team expanded Qammar Abbas, MD access to gastroenterology services in Lebanon. Before joining Good Samaritan Digestive Health Specialists, Cozzi was a staff gastroenterologist at St. John’s Hospital/Mercy Health System in Springfield, Mo. Qammar Abbas, MD, joined the staff of Good Samaritan Pulmonary Associates following the retirement of Dr. Wilson Morris. Abbas comes to the practice from Memorial Sloan-Kettering Cancer Center in New York, where he was a critical care medicine fellow. MedExpress In November 2015, WellSpan Health and MedExpress Urgent Care announced a collaboration to enhance the coordination of high-quality urgent care with expert orthopedics, primary care and other services for Lebanon County residents. As part of the initiative, MedExpress coordinates care with WellSpan Good Samaritan Hospital, the locally based orthopedic surgeons of WellSpan Orthopedics and other local physicians should patients require additional services, while continuing to respect patient choice. For example, patients who experience an orthopedic injury will be able to receive immediate attention at the MedExpress Lebanon location with seamless, timely follow-up care by one of five WellSpan orthopedic surgeons, if necessary. 12 Recognition for High-Quality Health Care WellSpan Good Samaritan earns awards and certifications Awards and certifications underscore WellSpan Good Samaritan’s commitment to providing high-quality health care to the communities it serves. Areas recognized in 2015 included cardiac, stroke, wound care and maternity. The American Heart Association recognizes this hospital for achieving 85% or higher compliance with all Get With The Guidelines® Heart Failure Achievement Measures and 75% or higher compliance with five or more Get With The Guidelines® Stroke Quality Measures for two or more consecutive years to improve quality of patient care and outcomes. 2015 Get With The GuidelinesHeart Failure Gold Plus with Target: Heart Failure WellSpan Good Samaritan Hospital received the Get With The Guidelines®–Heart Failure Gold-Plus Quality Achievement Award for implementing specific quality-improvement measures outlined by the American Heart Association/American College of Cardiology Foundation’s secondary prevention guidelines for patients with heart failure. The Get With The Guidelines program aims to speed recovery and reduce hospital readmissions for heart failure patients through educational tools, prevention programs and treatment guidelines. Hospitals are required to meet criteria that improves medication adherence, provides early follow-up care and coordination and enhances patient education. WellSpan Good Samaritan also received the association’s Target: Heart Failure Honor Roll. 13 The American Heart Association and American Stroke Association recognize this hospital for achieving 85% or higher compliance with all Get With The Guidelines® Stroke Achievement Measures and 75% or higher compliance with five or more Get With The Guidelines® Stroke Quality Measures for 12 consecutive months to improve quality of patient care and outcomes. 2015 Get With The GuidelinesStroke Silver Plus Quality Achievement Award WellSpan Good Samaritan Hospital received the American Heart Association/American Stroke Association’s Get With The Guidelines®-Stroke Silver Plus Quality Achievement Award. To earn the award, hospitals must achieve 85 percent or higher adherence to all Get With The Guidelines-Stroke achievement indicators for at least 12 consecutive months and during the same period achieve 75 percent or higher compliance with five of eight Get With The Guidelines-Stroke Quality measures. The measures are designed with the goal of speeding recovery and reducing death and disability for stroke patients. They focus on appropriate use of guideline-based care for stroke patients, including aggressive use of medications such as clot-busting and anti-clotting drugs, blood thinners and cholesterolreducing drugs, preventive action for deep vein thrombosis and smoking-cessation counseling. Highmark Blue Shield Blue Distinction Center+ Cardiac Care Blue Distinction® is a designation given by the Blue Cross and Blue Shield companies to hospitals that deliver superior results for high-risk, high-cost procedures. In order to become a Blue Distinction Center, a hospital must employ dedicated medical teams that focus on particular areas of specialty care and demonstrate high clinical quality through use of evidence-based care, proactive patient follow-up and quality improvement initiatives. The designation identifies hospitals that are proven to provide superior results — such as fewer complications and readmissions, shorter inpatient stays and better overall health outcomes — for common but complicated (and costly) procedures. Blue Distinction Centers+ represent hospitals that are 20 percent more cost efficient than non-designated hospitals. WellSpan Good Samaritan was among the first hospitals in the nation to receive the new designation level, which recognizes hospitals that deliver both quality and cost-efficient specialty care. Blue Distinction Centers+ for Cardiac Care provide a full range of cardiac care services, including inpatient cardiac care, cardiac rehabilitation, cardiac catheterization and cardiac surgery (including coronary artery bypass graft surgery). 14 Blue Distinction Centers (BDC) met overall quality measures for patient safety and outcomes, developed with input from the medical community. A Local Blue Plan may require additional criteria for facilities located in its own service area; for details, contact your Local Blue Plan. Blue Distinction Centers+ (BDC+) also met cost measures that address consumers’ need for affordable healthcare. Each facility’s cost of care is evaluated using data from its Local Blue Plan. Facilities in CA, ID, NY, PA, and WA may lie in two Local Blue Plans’ areas, resulting in two evaluations for cost of care; and their own Local Blue Plans decide whether one or both cost of care evaluation(s) must meet BDC+ national criteria. National criteria for BDC and BDC+ are displayed on www.bcbs.com. Individual outcomes may vary. Highmark Blue Shield Blue Distinction Center+ Maternity Care Highmark Blue Shield also recognized WellSpan Good Samaritan as a Blue Distinction Center+ for Maternity Care. Health care facilities that earn the designation demonstrate expertise and a commitment to quality care during the delivery episode of care, which includes both vaginal and Cesarean section delivery. NCQA Patient Centered Medical Home Level 2 Certification Good Samaritan Pediatrics and Good Samaritan West Cornwall Family Practice were awarded the National Committee for Quality Assurance (NCQA) Patient Centered Medical Home Level 2 Certification. The Patient Centered Medical Home (PCMH) model of primary care combines teamwork and information technology to improve care, improve patients’ experience and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinician-led care teams that coordinate treatment across the health care system. To receive recognition, practices must demonstrate the ability to meet the program’s key elements embodying characteristics of the medical home. Pennsylvania Hospital Engagement Network Safety Across the Board Excellence Award 15 WellSpan Good Samaritan Hospital earned a “Safety Across the Board” Excellence Award from the Pennsylvania Hospital Engagement Network. The award recognizes achievement of exceptional performance with Partnership for Patients Program goals of reducing preventable harm and readmissions. WellSpan Good Samaritan was recognized in the areas of action planning and data submission for VTE (blood clot) prevention and treatment measures. Good Samaritan Hospital President Robert Longo Retires Celebrating 32 years of service to the hospital and the community “After 32 years, I felt the time was right,” Longo said. “The work was both challenging and rewarding. Good Samaritan has been blessed with many wonderful employees, physician partners and volunteers. I will miss my daily interactions with so many wonderful people who work hard every day to improve the health of our patients and our community.” Good Samaritan transformed in many ways to deliver advanced services and to add new services that were needed by the community. The hospital placed diagnostic services at various locations throughout the county to enable patients to gain convenient access to tests close to home. As demand for physicians increased, Good Samaritan created a multidisciplinary physician network. In addition, a medical residency program was established to help train the next generation of physicians. Good Samaritan also built an outpatient surgery center for patient convenience. When research indicated that a wound care center was needed, Good Samaritan stepped in to create a center so patients could receive care within the county. Similarly, the hospital opened the Sechler Family Cancer Center in January 2016. The Good Samaritan Hospital experienced many changes over the course of Longo’s career. Most recently, the hospital finalized an affiliation with WellSpan Health. During Longo’s time with the hospital, the former Lebanon Valley General Hospital (now the WellSpan Hyman S. Caplan Pavilion) was acquired by Good Samaritan. In addition to his roles at Good Samaritan, Longo has been an active leader in the community. He served on a variety of boards, including the United Way of Lebanon County, Lebanon Valley Chamber of Commerce and the Lebanon Division of Fulton Bank. He also was instrumental in the efforts to create the Community Health Council of Lebanon County in the 1990s. Longo managed the major expansion of the hospital in 2004. The $46 million project doubled the size of the hospital. The expansion enabled the hospital to dramatically expand emergency services by building a new 34-bed emergency room that manages care for more than 50,000 patients annually. The hospital also built the facilities needed to create a state-of-the-art cardiac and vascular program that is ranked as one of the nation’s best. Before joining Good Samaritan, Longo worked for a Philadelphia-based health care consulting firm. He received a master’s degree from Northwestern University, Kellogg School of Business and a bachelor’s degree from Drexel University. Longo is also a Fellow in the American College of Healthcare Executives. Robert J. Longo retired from his position as senior vice president of WellSpan Health and president of The Good Samaritan Hospital on Dec. 31, 2015, after serving The Good Samaritan Hospital and the Lebanon community for 32 years. Prior to assuming the role of president, which he held for 22 years, Longo served as Good Samaritan’s chief financial officer for seven years and chief operating officer for three years. 16 Leading your Community Hospital Medical Staff Executive Committee 2015-2016 Bryan G.
