Our skilled clinicians and home health aides provided more than 130,000 visits this year. VNHSC is one of the largest employers in the town of Vernon with a staff of more than 430 individuals who provide services to 26 towns in north central Connecticut. In 2010, VNHSC was chosen to receive the 2010 Tolland County Chamber of Commerce Award in the category of “Economic Development”. The home health care industry is truly at a crossroads as we face numerous challenges including increased regulations, decreasing reimbursement, severe clinical staffing shortages and increasing complexity of patient care. Consistent with our past, VNHSC considers these challenges a part of our operations and the manner in which we overcome them an evolving process. In retrospect, we can appreciate how far we have come and the efforts of those who brought us here; however, in today’s uncertain environment we cannot rest on past success. In order to fulfill our mission to provide home and community-based health care to patients and families, we must continue to find innovative solutions to complex problems. We here at VNHSC have always welcomed and embraced the challenges we have faced. We understand that with difficult challenges come great opportunities. Even facing these significant hurdles, we recognize and are energized by the future opportunities evident for the home health care industry and VNHSC. We are heartened by provisions in the health care reform bill focused on strengthening and promoting the use of home health and community-based care to manage the increasing population of patients with long-term chronic illnesses. Together with our valued partner ECHN, we are collaborating to reduce the occurrence of re-hospitalization for three main diagnoses: heart attack, pneumonia and congestive heart failure. Starting in 2012, provisions of the healthcare reform bill call for hospitals to reduce re-admission rates for these three diagnosis or face significant reimbursement reductions. VNHSC has demonstrated excellence by once again posting the lowest re-hospitalization rate of any home health care agency serving north central Connecticut. In fact, VNHSC’s long-standing commitment to outstanding care has been proven year after year, and for the third year in a row we have been recognized as an Elite Top 500 performer by a nationally recognized organization, Outcome Concepts Systems. Together with the progressive leadership at ECHN, VNHSC will be a large part of the solution to this new problem that our hospitals now face. By partnering with one another, we all benefit from shared resources and cross access to specialty areas, helping to deliver a continuum of care that will undoubtedly impact patient outcomes. As we move forward, there is no question that we will continue to encounter challenges along the way. We are confident that by applying what we have learned and maintaining our focus we will continue to overcome the hurdles we encounter and celebrate the goals we achieve. We are an institution on which hundreds of patients and their families rely every day. It is an awesome responsibility, and we will continue to be there where and when we are needed. Kimberly Quigley Chairperson of the Board of Directors Todd Rose President/CEO 1 2 Message from Rosemary Harding I VP Clinical Services Our Telehealth home monitoring program, begun in 2003 with 15 video units, is one of our most successful programs. Today the program has 22 video units, 120 remote monitor units and 15 Zoe monitors. Our clinicians find the units to be effective teaching tools because patients must take their vital signs on a daily basis and answer questions on the monitor that are pertinent to their disease. Because of the sophistication of the equipment, we have been able to provide care to patients with very complex and acute illnesses. This year the Telehealth Program has provided service to patients using a Left Ventricular Assist Device or LVAD. Tufts Medical Center has chosen our agency as their preferred home care agency in the State of Connecticut to coordinate care for patients waiting for a heart transplant. A mechanical circulatory device, the LVAD is used to partially or completely replace the function of a failing heart. Known as a “bridge to transplantation”, long-term LVADs are used to keep patients alive with a good quality of life while they wait for a heart transplant. LVADs are sometimes used as destination therapy and sometimes as a bridge to recovery. Close monitoring by a Telehealth unit is essential in order for patients to remain safely in their homes. Patients using this equipment are also visited by a home care nurse who assesses their status and monitors blood work. The success of the program has allowed us to reach out to other area hospitals using LVAD programs. Statistics In 2009 VNHSC achieved Top 500 Home Care Elite status for the third year. Winners are ranked by an analysis of performance measures in quality outcomes, quality improvement and financial performance. Quality Benchmarks provided by Centers for Medicare & Medicaid Services This specialized program has contributed to our patient satisfaction. Ninety percent of users report that they felt an increased sense of security with the equipment in place and were better able to manage their illness. The careful daily monitoring of blood pressure, pulse, oxygen level and weight by our nurses, has prevented patients from being hospitalized. VNHSC staff has been working hard to keep our rehospitalization numbers below the state benchmark (the lower percentage, the better). The initial rate of 31% has been reduced to 24% for VNHSC, while the state average is 32%. This improvement was achieved through our innovative specialty programs and the hard work and dedication of our staff. LVAD A Left Ventricular Assist Device (LVAD) is a battery-operated, mechanical pump-type device that’s surgically implanted. The internal device exits through the abdomen wall and attaches externally to a monitoring system. It helps maintain the pumping ability of a heart that can’t effectively work on its own. Message from Dianne Welch I Director of Hospice 3 As we reflect on our journey of helping others at the end of life, we want to take a moment to reflect on the past year of change–change influenced not only by a competitive market, but also by our desire to give our full attention toward what North Central Hospice & Palliative Care is most passionate about and what we’re best at–providing quality care to our terminally ill patients. The most recent data received from the National Hospice and Palliative Care Organization Family Evaluation of Hospice Care (NHPCO FEHC) survey indicates that our families are very satisfied with our services. We scored above the top national benchmark in the following areas: ® Treatment for breathing (100%) ® Clear explanation of plan of care (96.7%) ® Providing needed supplies and equipment (100%) The Hospice team continues to provide compassionate care to our patients and families with the emphasis on quality, comfort, respect and dignity. We perform a comprehensive assessment of the entire family to address not only the physical needs, but the spiritual and psychosocial needs to help the family through the end-of-life journey. We are extremely proud to have become a trusted resource for our patients and families when and where they need us. Our culture of caring is carried in the hearts and minds and individual talents of each member of the hospice team. It is our firm belief that Hospice care is not a privilege, it is a right, and to this end we will continue to work diligently to educate our physicians, our community, our referral sources and others about the value of hospice care to the entire family. When asked if they would recommend Hospice to others, 100% of our families surveyed responded “yes”. Sincerely, The Cable/Ambeau Families 4 Message from Janet Gallugi I Vice President, Private Duty Services A Caring Hand (ACH), an affiliate of Visiting Nurse & Health Services of Connecticut, is dedicated to providing private duty services you can trust. The goal of ACH is to keep your elderly loved ones at home for as long as safely possible. Choosing the right home care and private duty care services may seem difficult. Those seeking employment through A Caring Hand go through a rigorous interview and orientation process to ensure that all field staff caring for clients reflect the philosophy of genuine compassion and dedication to quality care. A Caring Hand offers four levels of care to meet individual needs: Companions, Homemakers, Personal Care Attendants, and Live-in Caregivers. All caregivers provide socialization and activities to keep clients active and safe. Personal care attendants provide assistance with personal care which may be difficult for the client or clients’ family. A Caring Hand also offers live-in services by certified nurses aides and home health aides to assist clients with all needs 24 hours a day, 7 days a week. With more than half of our client base having some form of dementia, we have added ongoing training for our staff in that area. After completing training, all employees have an in-depth understanding of the disease process and behaviors associated with the illness. Our senior population is one of the fastest growing demographic in the United States. Most own their residence and have no plans to move. A Caring Hand gives seniors a sense of empowerment and allows them to remain at home and retain close friendships with neighbors, friends and family. After Walter Swolka’s mom died, his dad faced hip surgery and a long period of recovery at home. She treats me with respect and like a person, never talking down to me like some others have. She treats me the way I like to treat other people. Before A Caring Hand came in, my daughter would come to the house and find I had forgotten to take my medication for a few days. Now, she helps me make sure I take them each day. We get along great. It’s wonderful to have someone to talk to when you live alone. To work with so many generous, selfless donors is an honor, and it is important for you to know that your gifts are appreciated not only today, but also by generations to come. 6 The Care Award This year’s Care Award recipient John Comeau, RPh was born in Hartford and grew up in East Granby. John received his degree in Pharmacy from the University of Connecticut School of Pharmacy in 1975. The owner of Beacon Prescriptions in downtown Rockville, John and his staff are highly respected by families living in the area. Always pleasant and caring, he and his staff know all their customers by name and regard them as part of their extended family. with “grand puppies.” Recently married, daughter Amy, is a nurse at Hartford Hospital and lives in South Windsor with her husband Matthew. Son, Darin is a student at the University of Arizona, pursuing a degree in Pharmacy. John has two stepchildren; Laura resides in Southern California while Michael in Utah. Looking forward to becoming grandparents, John and wife, Linda are currently happy John’s interests and hobbies include tennis, “traveling anywhere”, wine-tasting, beermaking, photography, swimming, kayaking, gardening, playing pool, and, of course, watching the UCONN Huskies! An integral member of the hospice interdisciplinary team for a number of years, John can be counted on to be present at weekly meetings where staff review patient plans of care and John is available to offer recommendations on medication and pain control. When asked why he supports VNHSC and Hospice and Pallative Care, John’s response is quick and to the point. He states “It because it is the right thing to do. Paula Paradise first experienced hospice in 2002 with the passing of her father after he lost his battle with cancer. Two years later, in November of 2004, Paula lost her husband of 21 years to the same disease. Following the death of her husband Jimmy, Paula became a strong advocate for hospice care and is committed to telling others about the peace and dignity that allows every person and family to participate fully in this stage of life. Swiss Cleaners Swiss Cleaners and Lite 100.5 FM WRCH partner on an annual basis to host a fund drive benefiting North Central Hospice & Palliative Care. Swiss Cleaners, family owned and operated since 1913, donates a portion of the proceeds of incoming orders from all 8 locations. Through this campaign, Swiss Cleaners has contributed over $35,000 to support specialty services of North Central Hospice & Palliative Care. Foundations SBM Charitable Foundation Visiting Nurse & Health Services of Connecticut has been extremely fortunate to receive funding from the SBM Charitable Foundation since our partnership began in 2001. This funding has allowed us to continue providing quality home care to the communities we serve. The Foundation has provided us with over $825,000, enabling us to carry out our mission of dedication to care within the homecare arena. The SBM Charitable Foundation, established in 2000 by the parent company of the former Savings Bank of Manchester, perpetuates the vision of the former Savings Bank of Manchester. The Foundation is committed to bettering the lives of those who live and work predominantly east of the river in Hartford, Tolland, and Windham Counties. United Way of Central and Northeastern Connecticut The United Way of Central and Northeastern Connecticut continues to support VNHSC with monies that allow us to continue our Meals on Wheels program, allowing us to deliver over 66,000 meals in 2009. The United Way has funded Meals on Wheels as well as other programs of VNHSC for over 20 years. United Way of Central and Northeastern Connecticut is a nonprofit organization that has been meeting human care needs in 40 towns across central and northeastern Connecticut since 1924. Their mission is to engage people to improve lives and change community conditions. United Way advances the common good by creating opportunities for a better life for all. They focus on the building blocks of a good life: education, income and health. Proceeds from the Jimmy Paradise Memorial Golf Outing tournament are directed to the Patient Care Fund to help make hospice care available to those without insurance or whose insurance limits the care they can receive. The event in its 5th year has raised over $60,000 for Hospice & Palliative Care. LIFE INSURANCE: One of the simplest ways that you can make a significant gift is to name VNHSC, Inc. to receive all, or a portion of, the proceeds of a current policy which is no longer needed for family protection. BEQUEST BY WILL: In addition to being one of the simplest ways to distribute your estate, your will can also be a creative vehicle through which to make thoughtful gifts. ANNUITY TRUST: A Charitable Remainder Annuity Trust will allow you to ensure a future gift while retaining a fixed income from your trust for life. UNITRUST: Like the Annuity Trust, the Charitable Remainder Unitrust also provides you with lifetime income and immediate and future tax advantages. But, unlike the Annuity Trust, the income from the Unitrust may increase or decrease based upon the performance and value of assets in the trust. You determine the payout percentage when you create the trust. CHARITABLE LEAD TRUST: This unique arrangement allows people to create income that becomes a contribution to VNHSC, Inc. for a fixed period of years after which the property in trust passes to loved ones. PERSONAL PROPERTY COLLECTIONS: Gifts of tangible personal property, such as cars, boats, art, antiques, jewelry, etc. are another way of giving. You may designate how the funds from the sale of your contribution are to be used. RETIREMENT ACCOUNT: Whether you are in a company pension plan or other private accounts, such as an IRA, you may accumulate funds beyond your actual needs. When this occurs, you may consider making a gift of these accounts to VNHSC, Inc. upon your death, to realize significant tax advantages. TRIBUTE GIFT: For many, a contribution in memory of or to honor a loved one or friend brings satisfaction. A tax-deductible gift may be given at any time and for any occasion.
