For Modification on NICOP: Original NICOP card (required detail police report if lost) with additional
following documents are required:
For change marital status:
- Notarized copy of marriage/divorce certificate
For name change:
- Notarized copy of court order for name change
For correction in Date of Birth (Fees is $85.00* for correction in DOB):
Notarized copy of any of these documents:
- Birth certificate issued from NADRA only (manual or hand written birth certificate is not
acceptable for correction in DOB).
- Valid Pakistani passport with correct date of birth.
- Court order for change in date of birth.
For change permanent address (Pakistani address):
- Notarized copy of land registry/local domicile/allotment letter of property against applicant /
father / mother/spouse name is required for change in permanent address.

Note: Bring original documents when submit by person or we may ask original documents any
time if sending application by mail. Application will not be accepted if it is without signature
and thumb impression in field 28 (except minors below age of 5 years) or photocopies of
documents are not clear.
