We provide care for the homeless, behavioral health and social service referrals. We teach patients about nutrition, chronic illness management, breastfeeding and healthy lifestyle choices. We reach more than 26,000 patients annually through five health center locations in Chicago. South Chicago Roseland 9119 S. Exchange Ave. 120 W. 111th St. Pullman Chicago Lawn 556 E. 115th St. 3223 W. 63rd St. East Side 10536 S. Ewing Ave. We grow more and more each year. In 2010 we opened in Chicago Lawn, marking the first time the center expanded north and west. In 2013 we will complete the Pullman Replacement Project replacing the original site first opened in 1987. It is our second-largest facility where we treat more than 6,000 patients annually in 13 exam rooms and three dental operatories, in 3,500 square feet of patient care space. The replacement facility will feature 23,000 square feet of space, 23 medical exam rooms, nine dental operatories and two behavioral health consultation rooms to more than double the number of patients seen there. The Pullman Replacement Project was made possible by a $6.2 million U.S. Federal Affordable Care Act/Capital Development Grant. We need you online at www.chicagofamilyhealth.org.
