Brokenhead Ojibway Nation Build it Right Conference 2013 Program Alignment CHAOS Model Case Management Model-Edmonton CHAOS Model Client intakes many different times with a duplication of services and processes Programs work in isolation and do not attempt to communicate or, Communicates and minimal Coordination on a case-by-case basis (informal referral process, cross-training and programming) Staff burn-out, high turn-over and increased frustration Limited opportunities to access partnerships Client’s needs are not fully addressed and may result in falling back into the same cycles Clients continue to feel that they cannot achieve their goals to improve their quality of life and may stop trying. Government cuts backs due to low statistical success rates… Goals The goals of the Brokenhead Ojibway Nation Transitional Support Program are to promote: Skilled laborers with willingness to work; Long-term employment opportunities; Sustainable economic future. Phase 1- Evaluation & Partnerships Identified key programs to partners and recruit to the working group… Met every 3 weeks Evaluate what has been done in the past and determined what was effective and what was not working Gather community input by a community-based committee and the participation of a Youth and Elder’s SWOT Analysis and surveys Develop a vision, identify goals and steps to reach the goals Obtained Leadership support of the initiative in January of 2013 Location, location, location! The Income Assistance and Entrepreneurship, Employment and Training Office relocated from the Governance and Administration Office (Band Office) into their own building in order to deliver Active Measures programming in the community.
