We’ll have coffee and breakfast pastries, and someone
will win a sweet new pair of Hi-5 gloves, courtesy of
Helimot!
Bring your donation, your signed waiver (print this
thing and fill it out!) and a functional motorcycle
with a tank full of gas.

WAIVER & RELEASE OF LIABILITY AGREEMENT
I AM AWARE THAT MOTORCYCLE RIDING IS A DANGEROUS ACTIVITY. I VOLUNTARILY AGREE TO
ACCEPT ANY AND ALL RISKS OF INJURY OR DEATH WHICH MAY OCCUR WHILE I AM RIDING, OR
PARTICIPATING IN THE “RIDE TO THE QUAIL WITH CITYBIKE” CHARITY EVENT, EVEN IF SUCH
INJURY OR DEATH IS CAUSED BY THE ACTIVE, PASSIVE OR EVEN GROSS OR WANTON
NEGLIGENCE OF OTHERS.

I HEREBY AGREE for myself, my personal representatives, heirs, and next of kin, TO RELEASE AND
WAIVE LIABILITY AND COVENANT NOT TO SUE OR PROSECUTE ANY LEGAL ACTION
AGAINST CityBike magazine, CityBike Magazine Inc., Surj Gish and all members of his family, as well as any
of their agents, employees, heirs, friends, personal representatives and next of kin (hereinafter the
“Releasees”).
I agree that this release and waiver of liability agreement is intended to be as broad and inclusive as permitted
by the law of and that if any portion thereof is held invalid, it is further agreed that the balance shall
notwithstanding continue in full legal force and effect. It is further understood that this agreement may be
used as evidence of my intent to release and waive liability in a court of law.
I UNDERSTAND THIS MEANS I CAN'T SUE ANYONE IF I AM INJURED OR KILLED, EVEN IF MY INJURY
OR DEATH IS CAUSED BY THE ACT OR OMISSION OF SOMEONE ELSE, OR BY ANY PERSON OR
ENTITY'S NEGLIGENCE, EVEN GROSS NEGLIGENCE OR RECKLESS DISREGARD FOR SAFETY.
I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENT. I AM AWARE
THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND THE RELEASEES.
I VOLUNTARILY SIGN AND FURTHER AGREE THAT NO ORAL REPRESENTATIONS, STATEMENTS OR
INDUCEMENTS APART FROM THE FOREGOING WRITTEN AGREEMENT HAVE BEEN MADE.
