Healthy skin. Healthy lives. A healthy life is our most basic aspiration. In dermatology we understand that healthy skin is a fundamental part of this goal. This is what those in dermatology have committed their careers to doing — helping patients achieve healthy skin, healthy lives. Dermatologists do this in many ways; the most obvious, perhaps, is providing quality care to patients every day. But there’s more to it. It’s a dedication to constant training and knowledge development to keep on top of a changing field. It also means making a positive impact on our communities. Those in dermatology are giving time, expertise, and financial support to help people around the world achieve healthy skin, healthy lives. Dermatology has a story to tell — many stories, in fact. They happen every day in dermatologists’ offices across the country. When we first met, Nancy shared the story of experiencing a terrible rash that made her self-conscious about her appearance. Within 48 hours of seeing a dermatologist and receiving treatment the rash was gone and Nancy felt healthier and more confident in her skin. Bill understood that experience immediately. It happens for patients in his office very day. Stories like this are one of the great pleasures of practicing dermatology — making a visible impact on patients’ well-being. I That’s what Aspire will do — share the stories about all of us in the dermatology community: individual dermatologists volunteering skin care to needy patients or supporting a cause or organization near to their hearts; corporations and their employees dedicating themselves professionally and personally to advancing skin health; the Academy, with it its wealth of programs and services; and recognize those who support our mission through donations and volunteerism. We will put faces and stories to those who help achieve the mission of healthy skin, healthy lives. You may have read some of these stories in our newsletters Today and Tomorrow, which went to members and other individual donors, and inPartnership, which was shared with our corporate partners. This new publication will consolidate and expand upon those efforts, tapping a wide and varied pool of voices and stories that truly demonstrates all the time and resources you give to enhance the specialty and deepen the impact of dermatology. Because when it comes to that core belief — that all people can and should aspire to and achieve healthy skin, healthy lives — we are one community reaching for a shared goal. We hope the joy that we have infused into these stories is infectious. We are proud of what they say about our specialty and think you’ll agree. How is the AAD’s vision of “Healthy skin, healthy lives” important to you? As a 30-year veteran of dermatology, I am passionate about the skin. The skin is our most visible organ and in many ways the interface by which we interact with the world. The skin is also vital to our perception of self. To me, these factors give dermatology a special magic that you do not get in other disciplines. Supporting the AAD helps the specialty to advance and to grow. Your website states that “dermatologists hold a distinctive position in improving human health.” What makes the company feel this way? The skin is not just important to physical health; it also plays a major role in psychological health and social interactions. This enables improvements in dermatology to have a positive effect on every aspect of patients’ lives. How important is it to Galderma to have a good working relationship with dermatologists? The “derma” in our name stands for skin. We are not just a general pharmaceutical company that makes effective and safe medical solutions for the skin. Galderma has remained exclusively dedicated to the skin and to the advancement of dermatology. Dermatology is the only thing we do, and the dermatologist is at the heart of that. How does partnering with the AAD help to put patients first? Dermatologists and other health care professionals have an empathetic understanding of the patient experience. Galderma is dedicated to improving that experience. Working closely with dermatologists thus helps us to determine how we can positively impact the lives of patients. Why is research such a priority? New treatments can improve the overall health and quality of people’s lives immeasurably. But there is no science and innovation without strong investments. The future of dermatology depends on the research we undertake now. You recently partnered with AAD in the research area to launch the AAD Translational Biotechnology Fellowship at Galderma. Tell us about the program. Galderma is hosting two one-year fellows, one beginning in 2013 and another starting in January 2014. These unique fellowships provide the opportunity for a physician to gain experience working in dermatological drug development and translational medical research. They are hosted at Galderma’s research and development facility in Sophia Antipolis, France. What does the future hold for dermatology? The world’s population is both growing and aging. These changes will increase the demand for dermatological care and also present new medical challenges. AMERICAN ACADEMY OF DERMATOLOGY 3/7/14 6:51 PM AAD and Me The Leadership Institute: Supporting the advancement of growth among dermatologists By Amit Garg, MD A s dermatologists, we have opportunities to lead in a multitude of arenas including community based practice, academia, policy making and advocacy, and organized medicine. Few of us are naturally equipped with the ability to lead most effectively, and many of us overlook the key moments of introspection that inform us of our opportunity to make self-improvements. Now maybe I slept through my medical school or residency lectures on time management, getting tasks done, giving effective feedback, managing my hot buttons, resolving conflict, building a practice culture, leading change, delivering a great media message… Or wait, is it, like it is for most of us, that I never received content around leadership that facilitated self-reflection on my proficiency with these skills and the ability to augment them? Even after training, I found there was a gap in faculty development and leadership training programs, at least at academic medical centers. The available programming was fairly limited, and to me, always seemed rather generic with respect to content, phase of career, and specialty. My search led me to the American Academy of Dermatology’s Leadership Institute (LI). I found the institute to be comprised of experienced and thoughtful individuals who worked with content experts to put together programs to fit the needs of physicians in all three major career phases and in most settings. One of my most valuable experiences from the LI has been getting and giving the most out of mentorship. I’ve spent a lot of time in mentored relationships, on both sides of the experience. Come to think of it, I’ve even had both roles in the same relationship, and that’s a testament to the bidirectionality of truly great mentoring relationships. The mentoring skills I have developed and enhanced through the LI have helped me become more effective in my various roles in academia, which have involved working closely with trainees as well as About the Leadership Institute The American Academy of Dermatology’s Leadership Institute provides training and mentoring opportunities to help dermatologists develop skills to become leaders in their careers, their communities, and organized medicine. Programs include courses at the Academy’s Annual and Summer Meetings, a weekend Leadership Forum for early career dermatologists, an Academic Dermatology Leadership Program, and a Mastery Course for those with advanced leadership skills. The program also provides networking and mentoring opportunities. new faculty. Specifically, I have a better appreciation of my roles and responsibilities as a mentor, and I also know what a good mentee needs to bring to the table. My mentoring sessions are more structured, which optimizes the experience and the outcomes. I realize the necessity of using active listening skills, and I have become more effective at offering meaningful and sometimes difficult feedback. It’s an exciting time for me personally, and I am looking forward to the challenges and the rewards that come with a new leadership position as chair of a dermatology department. I’ve only been on the job a couple of months, though already I have deep appreciation for the skill sets required as well as recognition of the contributions from the LI that will allow me to be most effective in this opportunity. I don’t think I would have been as well prepared to take on new leadership experiences without this breadth of training, and it wasn’t something that was readily available to me elsewhere. I am recommending the LI to anyone who wishes to be more effective in their immediate sphere of influence and also to those who are looking to expand this sphere with the intention of improving society and profession. From my perspective, not engaging the LI in some capacity, either as a leader or one in training, is a missed opportunity. Of course, dermatologists know that sunshine comes with a price — so what better venue to build awareness about sun safety and recognizing the signs of skin cancer than at Major League Baseball games. By Jeff De La Rosa This cause is especially close to me, and I am proud that we continue to work with the American Academy of Dermatology to educate as many people as possible about skin cancer prevention and detection. Bud Selig, MLB Commissioner 6 141750.indd 6 For 15 years, the American Academy of Dermatology has been doing just that through the Play Sun Smart™ program, a partnership with Major League Baseball (MLB) and the MLB Players Association. The program elevates awareness of skin cancer detection and prevention among baseball players and fans and provides screenings to baseball players, team staff, and fans. “The reality is our game is one that’s played out in the elements and often in the sun. To be responsible for our kids and ourselves, we’ve got to protect ourselves,” observed Cleveland Indians President Mark Shapiro. “Although being outside can be one of the greatest things in the world — watching baseball, playing baseball — it can also be something where a lot of damage is incurred, potentially lifethreatening, if you don’t take the right precautions.” The program started with a just a few teams in 1999 and has expanded significantly over the years. During this time dermatologists have given thousands of skin cancer screenings to MLB players and staff and raised awareness among hundreds of thousands of fans watching the games. “It’s a one-two punch,” said dermatologist Darrell Rigel, MD, who helped to launch the program and has been an active volunteer since. “Our screenings have identified cancers and precancers on players and coaches as well staff and family members. And at the same time, we’re getting the word out to fans about prevention and early detection.” Play Sun Smart has a special place in the heart of MLB Commissioner Bud Selig, a melanoma survivor. “This cause is especially close to me, and I am proud that we continue to work with the American Academy of Dermatology to educate as many people as possible about skin cancer prevention and detection,” he said. Play Sun Smart hit hard with its message of prevention and early detection during the anniversary year, with high profile activities at ballparks across the league. Public address announcements reminded fans to reapply sunscreen during the 7th inning stretch and crowds watched an animated Play Sun Smart race on the Jumbotron featuring tips for staying safe in the sun. The Oakland A’s and Philadelphia Phillies stenciled the Play Sun Smart logo onto the field for games. In Seattle, two-time All-Star and former Mariner John Olerud made a special Play Sun Smart appearance with his dad, dermatologist John Olerud, MD, during a Mariner’s game. Skin cancer patients lent faces, voices, and stories of survival at many games, where they were honored during pregame ceremonies; several threw out the first pitch. In Miami, Fox Sports interviewed T. J. Sharp, who has blogged about his battle with melanoma under the moniker Patient #1. Kids played a prominent role in the anniversary activities. At many parks baseball’s youngest fans pitched in and delivered sunscreen to dugouts before the game. The Arizona Diamondbacks honored the winner of a kids’ sun-safety poster contest conducted in partnership with the state’s Department of Health Services. At Chicago’s Wrigley Field, the AAD and the Pediatric Sun Protection Foundation distributed 4,000 youthsized sun-safety T-shirts to young fans attending the game. Hundreds of thousands of baseball fans attended the various festivities, and potentially millions more saw them on television or heard about them in the media, proving that Play Sun Smart is a powerful vehicle for the AAD’s SPOT Skin Cancer™ messages. AMERICAN ACADEMY OF DERMATOLOGY 3/7/14 6:52 PM Shading a nation People in parks and playgrounds across the U.S. have it made in the shade thanks to donations from AAD SPOT Skin Cancer’sTM Shade Structure Grant Program. In 2012 and 2013 the program funded 28 structures, which will shade as many as 23,000 people every day. The program allows public schools and nonprofit groups to submit a dermatologist-sponsored application for funding to erect shade structures in outdoor locations. Three additional shade structures were supported by direct donations from individual dermatologists and dermatologic organizations. Learn more about the program at www.spotskincancer.org. MASSACHUSETTS A shade structure was erected at Wheelock College in Mattapan, Mass., thanks to a donation from La RochePosay Laboratoire Pharmaceutique. WASHINGTON, D.C. CALIFORNIA A donation from the Murad Family Foundation supported a shade structure at Tuvia School in Redondo Beach, Calif. Suzanne Olbricht, MD, donated funds to support a shade structure at the National Zoo in Washington, DC. The structure will be unveiled later this year. 238 600 Each structure protects as many as 600 people per day. But the reality is that they are out there in the community saving lives. Linda Pelligra, 13-year melanoma skin cancer survivor By Jeff De La Rosa Annie Appleby was enjoying a sun-drenched jog on California’s Santa Monica Beach when a stranger demanded her attention. “He practically grabbed me off the path,” recalls the fitness-conscious Appleby, founder and creative director of YogaForce. “Informing me that he was a dermatologist, he pointed out a large freckle on my back, just below the strap of my bikini top and told me ‘Get it looked at immediately’.” Florida news reporter Kate Eckman was home for Thanksgiving when her brother, a physician, noticed a dry scaly patch on her forehead. “The spot had been developing for years,” remembers Eckman. “I thought it was from touching my face, adjusting my hair. I had kept it hidden under makeup.” Eckman’s brother told her he thought it could be cancer. Linda Pilkington Pelligra noticed a sudden change in an otherwise normal looking mole. The businesswoman and mother got to a physician as soon as possible. “I never thought I could have cancer.” These women are not just three of the millions of people who are diagnosed with skin cancer each year. Rather, they are survivors in the battle against a deadly disease. Not an invisible killer but a visible one, betrayed by telltale marks if one knows what to look for. Eckman was diagnosed with basal cell carcinoma. The disease has a 99 percent survival rate, but can result in significant morbidity, including disfigurement and scarring. Appleby and Pelligra were diagnosed with melanoma, which claims thousands of lives every year. She was caring and compassionate, everything I needed her to be at that time. outcomes, and, in the case of Abbleby and Pelligra, had their lives saved thanks to the intervention of dermatologists on the frontline of the fight against this visible killer. “So many people think dermatologists focus mainly on cosmetics,” Pelligra observes. “But the reality is that they are out there in the community saving lives.” “The personal connection between dermatologist and patient is especially important with skin cancer,” says Hensin Tsao, MD, melanoma researcher and chairman of the American Academy of Dermatology (AAD) Melanoma/Skin Cancer Committee. “Cancer is an incredibly charged diagnosis. There is a lot of fear about the future.” Appleby puts it more directly. “Cancer is a very scary word when you hear it applied to you.” Pelligra recalls sitting at a softball game wondering if it would be the last time she got to see her daughter play. options.” Pelligra had the worst prognosis of the three. Her cancer had spread to the lymph nodes, which had to be removed surgically. There followed a year of treatment with interferon. She likens the medicine’s side effects to the symptoms of a severe flu that lasted all year. “But I’m here, 13 years later,” she observes gratefully. “I’ve been seeing the dermatologist who diagnosed me ever since. She was caring and compassionate, everything I needed her to be at that time.” Dermatologist tells skin cancer ‘Take a hike’ Dermatologist Ellen S. Marmur, MD, has one thing to say to skin cancer: Take. A. Hike. The outdoor enthusiast, hiker, and skin cancer survivor is backing up the strong words with stronger actions. In July 2014 she will take a 16,732 foot hike up the face of Tanzania’s Mt. Kilimanjaro to raise awareness and funds for AAD’s SPOT Skin CancerTM. “As one who knows about the dangers of high-altitude exposure to UV, I have committed to climbing Mt. Kilimanjaro with a group of likeminded women as a visible way of educating the public about skin cancer prevention and detection,” said Dr. Marmur. All three survivors were saved through timely medical intervention by vigilant and skilled clinicians, including dermatologists. But treatment is just one front in the battle against skin cancer. Millions more lives can be saved through advances in education and awareness. “We view skin cancer as an important problem and a purely preventable one,” says Rear Admiral Boris Lushniak, MD, a dermatologist and acting United States Surgeon General. “We know there is a small subset of people predisposed to melanoma. But for the majority of people, we think the problem could be solved with prevention.” Dr. Lushniak compares the growing public awareness of skin cancer to the drastic change in attitudes toward smoking, which began 50 years ago with the Surgeon General’s first report on the subject. “In the world that I grew up in, it seemed that everyone was smoking. Likewise, people were intentionally exposing themselves to sunlight without protecting their skin.” At diagnosis, Annie Appleby’s melanoma had already spread deep into the skin, and there was some fear the cancer had metastasized. People must learn to protect their skin, to wear sunscreen, and to avoid artificial tanning. He notes signs of positive change, such as seeing children playing on the beach with covered skin and wide-brimmed hats. “We’re not there yet, but the tide is turning,” he remarks. “People, if nothing else, realize that sunburn is dangerous, and that is a great first step.” Survivors rank among dermatologists’ greatest allies in this battle. Annie Appleby’s fitness company, YogaForce, has supported skin cancer awareness through ongoing advocacy, public speaking, and donating yoga mats autographed by celebrities like Jessica Biel Timberlake and Tobey Maguire, whose signed mat raised funds for the AAD’s SPOT Skin Cancer™ program last year. for a special event designed for wine connoisseurs. The AAD has arranged a privately curated weekend tour of some of Napa Valley’s most renowned vineyards this fall. Space is extremely limited. Kate Eckman shared her story, including graphic depictions of her surgery, with television news viewers. “I wanted people to see the unsexy side of sunbathing, the ugly side of not caring for your skin. I would rather be someone’s lesson than to have them go through it themselves.” CONDUCT A FREE SCREENING THIS MAY! Linda Pelligra left her job in the pharmaceutical industry to join the Melanoma Research Foundation. She remains active in the battle against skin cancer as a patient advocate. For 13 years she has been cancer free but still undergoes yearly screenings. Looking back on her struggle with skin cancer, she says, “I hate to think of all the ‘what if’s’.” 228,000 The Surgeon General’s office has set a 10-year goal to reduce melanoma deaths by 10 percent. But in order to succeed in such goals, Dr. Lushniak explains, “we need dermatologists to take up such goals and to broadcast them. We need buy-in from the dermatological community and the public.” The Surgeon General’s goal is one that dermatologists, through the American Academy of Dermatology and its SPOT Skin Cancer program, have long been committed to. Through various programs, including skin cancer screening, public awareness advertising, advocating for indoor tanning restrictions, online educational resources, and much more, the AAD and its SPOT program are working to raise awareness of detection and prevention. Rachel Gormley, MD: A teledermatology milestone achieved F access to a dermatologist’s expert eye was lifesaving. Rachel or one young, uninsured woman in inner-city Philadelphia, free GLOBAL COMMUNITY In 2013, 15 dermatology residents received the AAD’s Resident International Grant. Grant winners received funds to travel to Gaborone, Botswana, where they rotate between the Princess Marina Hospital and the Baylor International Pediatric AIDS Initiative (BIPAI). The program allows residents to learn about the care of tropical and HIV-related dermatologic conditions and gain experience providing care with finite resources. Grants were awarded to Shane Clark, MD, Marcelyn Coley, MD, Bethany Cook, MD, Jeremy Davis, MD, Kanya Furguson, MD, Shilpa Gattu, MD, Rachel Gormley, MD, Tamia Harris, MD, Julia Kasprzak, MD, Jo Martin, MD, Onaopemipo Ofodile, MD, Swapna Reddy, MD, Caitriona Ryan, MD, Michelle Scott, MD, and Robert Stavert, MD. On-site set up of the clinic in 2013 was provided by dermatologist Adam D. Lipworth, MD, who also supervised the first week of the program. As part of an effort by the Global Health Service Partnership, which seeks to expand the base of medical educators in countries with 12 141750.indd 12 critical provider shortages, dermatologist Scott Norton, MD, and dermatology resident Dominique Pichard, MD, volunteered to train Peace Corps volunteers to better recognize and treat dermatologic conditions. As part of their effort they distributed the AAD’s laminated teaching cards. The partnership is a joint effort of the Peace Corps, the U.S. Government, and Seed Global Health. The AAD’s SkinCare in Developing Countries Grant Program provides grant funding to assist volunteer and humanitarian projects that serve beneficiaries in underserved regions of the world. In 2013 the AAD awarded three grants. Isabel Maria del Pilar Casa, MD, received funding to support a community dermatology program in Argentina. Efforts supporting teledermatology in Botswana, Guatemala, and South Africa were supported by a grant to Carrie L. Kovarik, MD. A grant was also awarded to Mohamed Al-Kamel, MD, to help with efforts at the Regional Leishmaniasis Control Center in Yemen. • Gormley, MD (left in photo), a third-year dermatology resident at the University of Pennsylvania, caught an atypical lesion while volunteering for the AAD’s AccessDerm program, which provides free outpatient teledermatology consultations to primary care providers seeing underserved patients across the U.S. Residents like Dr. Gormley can make initial assessments and give recommendations, which are then reviewed by an attending dermatologist. After reviewing an image submitted to AccessDerm, Dr. Gormley leapt into action. Believing it was a potential melanoma, she shared it with attending physician Rudolph Roth, MD, who agreed. And, with Dr. Gormley’s help, the patient was seen promptly in UPenn’s dermatology clinic, where an excisional biopsy was performed. Dr. Gormley also assisted the surgeon during the full therapeutic excision. Dr. Gormley shares credit for this positive patient outcome — the first melanoma caught and cured thanks to AccessDerm — with her mentors and colleagues at UPenn, including Carrie Kovarik, MD, and Bill James, MD, who tirelessly championed the launch of the program in 2010. “AccessDerm at Penn really is a team sport with engaged, compassionate primary care doctors on the front lines of community health in Philly, a great group of volunteer dermatologists, and of course, the wonderful group of patients we have the opportunity to serve. We are all united by the belief that everyone deserves care,” Dr. Gormley said. This patient — who according to the referring physician lacked insurance and would not have otherwise received care — perfectly exemplifies the mission of AccessDerm and its volunteers at UPenn and nationwide. AccessDerm has provided consultations to nearly 1,000 underserved patients to date. AMERICAN ACADEMY OF DERMATOLOGY 3/12/14 12:18 PM Robert Kalb, MD: AAD member honored for volunteerism obert Kalb, MD, knows firsthand how education can shape the path Rof nine of a person’s life in potent and positive ways. The first of his family children to attend college, he moved on from there to medical school, residency training, and a fulfilling career in dermatology. His gratitude for that opportunity has fueled a lifelong commitment to the educational aspirations of others. Since 2006 Dr. Kalb and his family have volunteered in Cambodia with Health Volunteers Overseas, an organization built on the philosophy that if you teach a man to fish you feed him for a lifetime. Dr. Kalb is now the program director for HVO’s site in Phnom Penh. His days there are busy, training medical students and supervising them in a dermatology clinic treating diseases such as scurvy, pellagra, and Hansen’s, diseases that have been all but wiped out in the U.S. Familiar diseases are often more challenging and severe due to lack of treatment, delayed care, or insufficient medication. Dr. Kalb embraces those challenges as an opportunity to learn and grow himself. “It’s very worthwhile to see what dermatology is like in another country, what the access to health care is like, and what people struggle with. It helped us appreciate what we have,” he said. The American Academy of Dermatology is a sponsor of HVO, through which volunteer physicians provide care, but also train health care providers in developing countries around the world, leaving behind critical knowledge and skills. In Cambodia, a country of 15 million with only one board certified dermatologist, the expertise imparted by dermatologists like Dr. Kalb, has a profound and lasting impact. In 2013 Dr. Kalb’s dedication was honored by Health Volunteers Overseas who awarded him their Golden Apple Award for his extraordinary contributions to dermatologic education in Cambodia. He is the first dermatologist to receive this award. “I’ve long been a believer in the power of education to lift someone out of poverty,” said Dr. Kalb of his commitment to HVO and dermatologic care in Cambodia. • LOCAL COMMUNITY In 2013, four dermatology residents were recipients of the AAD’s Native American Health Services Resident Rotation Grant and completed rotations providing dermatologic care to the Navajo Nation population in Chinle, Ariz. Tracy Donahue, MD, Jeffery Tiger, MD, Thuzar Shin, MD, and Michelle Scott, MD, worked with primary health care providers to assist with diagnosis and management of dermatologic diseases under the supervision of the Indian Health Service. Dermatologists across the U.S. volunteer to provide expert consultative care to underserved patients through the Academy’s AccessDerm telemedicine program. Through the program, primary care physicians in community clinics can submit images for review and diagnosis by a dermatologist. Caitriona Ryan, MD, a hat would it be like to practice dermatology PGY4-dermatology resident at Baylor University in Dallas can tell you first hand. It is an effort of innovation, creativity, and diligence. In 2013, Dr. Ryan went to Gaborone, Botswana, as the recipient of the Academy’s Resident International Grant. There, because of limited resources, she learned how to employ innovative practice models to treat patients with tropical and HIV-related dermatologic conditions, “I am quite certain that the month I spent in Botswana will have a profound effect on my dermatology career and my life in general,” she said. “The opportunity afforded to me by The American Academy of Dermatology and the University of Pennsylvania, under the leadership of Carrie Kovarik, MD, has helped me learn how to be resourceful and effective. They have also succeeded in creating an incredible symbiotic health care partnership in Botswana.” Dr. Ryan’s passion for dermatology and her patients is reflective in how she spent her free time in Botswana. After long, busy days treating patients, she made time to work on a recently published paper, Research gaps in psoriasis: 141750.indd 20 Opportunities for future studies, which appeared in the January 2014 issue of JAAD. The paper, of which she is a co-author, outlines recommendations to advance understanding and treatment of this chronic disease. Despite electricity cuts for energy conservation and occasional issues with WiFi, Dr. Ryan, completed the article while in Botswana. Not one to dwell on the difficulties, Dr. Ryan, describes the opportunity as “a great privilege.” She will finish her residency training this year and will join the faculty at Baylor University Medical Center. • 3/7/14 6:52 PM
