Yampa Valley Medical Center continues to adapt to the changing health care environment. In 2013, the rollout of the Affordable Care Act (ACA) was an important milestone. Our business office partnered with the Northwest Colorado Visiting Nurse Association to help members of the community enroll in the Connect for Colorado plans. We also partnered with the Steamboat Springs Chamber Resort Association and Yampa Valley Bank to present three educational programs to local businesses and the community about the ACA. We are very proud of the fact that our Cancer Care Program achieved accreditation with a “Gold” commendation from the Commission on Cancer. The journey to accreditation began more than four years ago and could not have happened without the tremendous support of Karl Gills, our former CEO, and our Board of Trustees, Cancer Committee, physicians and staff. This accreditation recognizes the depth of our program and our talented care team. A full report on our Cancer Program starts on page 18. And in recent news, we just announced our collaboration with the Mayo Clinic Care Network. This is an exciting collaboration that will offer many benefits to physicians and patients. For more on this see the report from our Chief of Staff and Medical Director on page 5. Some of the other highlights in this report include the move of the Doak Walker Care Center to Casey’s Pond, ongoing staff education and news from the Healthcare Foundation. We are proud to be the regional medical center for northwest Colorado and thankful for the ongoing support of the community. Participation by the Board of Trustees, administrative leadership, hospital staff, physicians and community members is essential in order to accurately represent the needs of the community and build on the foundation of excellence that is our heritage. At YVMC we serve patients and their families from Routt, Moffat, Grand and Jackson counties along with residents of Baggs, Wyoming. We understand that our “community” is beyond our immediate area and see it as our mission to serve the regional needs of northwest Colorado. Our awards, including the Healthgrades Patient Safety Award, Guardian of Excellence Award and our collaboration with the Mayo Clinic Care Network demonstrate the high-quality care we deliver. The strategic plan is designed to meet the chal- lenges of the future while continuing to meet the needs of our regional community. There are four key areas of the plan: patient experience, people, finance and growth. We seek to: • Achieve optimal patient healthcare experiences and outcomes • Create excellence through the development of physicians and employees • Maintain a financial position that will allow future self-determination • Establish an integrated regional healthcare delivery system As we look to the future, we are excited about the possibilities in growth and collaboration. We will continue to work with national and local providers to offer the very best in health care to the residents of northwest Colorado. The New Mobility Program at Yampa Valley Medical Center is the first program in Colorado to be designated as a Center for Advanced Recovery. The program is part of a national best-practice model for hip and knee replacements. YVMC’s New Mobility team of physicians, therapists and nurses are committed to continuous improvement through the ongoing measurement of outcomes. As part of the nationwide network, the Center for Advanced Recovery offers a proven approach. According to patient surveys, 94% of New Mobility patients would definitely recommend the program to a friend or family member. Additionally, 94% of New Mobility patients leave the hospital sooner and go home, vs. to a rehabilitation facility. Because of our size and remote location in northwest Colorado, this collaboration is especially significant. Moving forward, YVMC physicians will be able to benefit from Mayo’s leading research and experDr. Larry Bookman tise as they treat patients here at home. As they consider the care needs of their patients, YVMC physicians will have the ability to reach out to Mayo Clinic specialists. It is noteworthy that prior to joining the network, all members undergo a rigorous review process to ensure their commitment to the delivery of health care through high-quality, data-driven, evidence-based medical care and treatment. YVMC is proud to have been invited to join this special group. As members of the Mayo Clinic Care Network, a number of tools will be available to YVMC physicians including: • AskMayoExpert, an online medical resource created to provide the latest Mayo-vetted information on disease management, care guide- lines, treatment recommendations and reference materials for a wide variety of medical conditions. • eConsults, a service that allows physicians to connect electronically with Mayo Clinic specialists to get input on specific patient care questions. • Health Care Consulting, a service that allows members of the network to learn more about how Mayo Clinic operates, and how its clinical care and practice models are designed and implemented. YVMC and Mayo Clinic share the commitment that health care should be provided close to home whenever possible. We are working with Mayo Clinic so our patients can benefit from leading medical expertise and physician collaboration that is focused on keeping care close to home. ~ Dr. Larry Bookman Message from the Chief of Staff Our patients are constantly giving us positive feedback about the care they receive at YVMC so we were not surprised when this past year, YVMC received two significant awards. In July, YVMC was recognized by Healthgrades as being in the top 5% of the nation in patient safety. Healthgrades Inc. is an organization that develops quality and safety ratings of health care providers, including hospitals, nursing homes, physicians and dentists. The ratings are derived from publicly available patient safety data and analyzed with proprietary technology developed by Healthgrades. A Healthgrades rating is an important indicator of the quality of care that patients receive. YVMC’s ranking in the top 5% of all hospitals in the United States means that our physicians, nurses and staff deliver care that results in exceptionally safe and consistent results. In November, YVMC’s Emergency Department received a 2013 Press Ganey Guardian of Excellence Award. This is especially significant since Press Ganey surveys patients from more than half of all U.S. hospitals. The ongoing “Taking Care of Me” series of free educational presentations provided community members with the latest information on a wide range of topics including bladder cancer, prostate cancer, diabetes, women and heart health, seasonal affective disorder, back, knee and joint pain, holistic medicine, meditation and the prevention of bullying and suicide. This past year saw the launch of the “Real Food” series. Dr. Charlie Petersen of Yampa Valley Medical Associates and registered dietitians Laura Stout and Cara Marrs of YVMC’s Integrated Health created this program to address the ongoing confusion about food and nutrition. The monthly programs have developed a loyal following and have covered a wide variety of topics including genetically modified foods, tips for healthy grilling and “superfoods.” In March, YVMC’s Integrated Health presented the “Finding Balance Forum,” an all-day event at Colorado Mountain College. The event featured programs on nutrition, mindfulness meditation, stress management, acupuncture, fitness training, yoga and Qigong. More than 70 community members enjoyed the introductory remarks by Dr. Will Baker, Cardiologist, Dr. Charlie Petersen, Internist, and Angela Melzer, LCSW, OTR/L, Director of Integrated Health. YVMC Annual Report 2014 • yvmc.org 7 Doak Walker Care Center Farewell to the Doak With heartfelt hugs and more than a few tears, Yampa Valley Medical Center bid farewell to the residents and care team of the Doak Walker Care Center on October 29, 2013. Thirty families assisted in the well-coordinated move. Within five hours all 58 residents were safely settled into their new home at the Casey’s Pond senior living campus. For 34 years, the skilled nursing home was an important part of the hospital’s spectrum of services. Our connection began in 1979, when the troubled Alpine Meadows Nursing Home nearly closed. At community request, the hospital took on the long-term care operation, renamed the building Routt Memorial Extended Care Center and established an award-winning standard of care. In 1981, GrandKids Child Care Center opened in a wing of the ECC. The bonds that formed between children and elders proved that love is ageless and generations can grow together. Though GrandKids remains on the YVMC campus, the intergenerational program continues as preschoolers and toddlers regularly visit their friends at the new Doak Walker House for activities. 8 YVMC Annual Report 2014 • yvmc.org While it was difficult to end this chapter of YVMC history, we are excited about the new experiences that Casey’s Pond residents will enjoy. YVMC Chief Executive Officer Frank May summed up our feelings: “We celebrate the addition of a new community asset that enhances the lives of people in our region and also feel sadness for the departure of the residents and staff who have been an integral part of our organization for the last 34 years,” he said. “The Doak Walker Care Center was one of the best long-term care and skilled nursing homes in the country. To the staff involved in the care of the Doak residents, we say thank you. Our organization will not be the same without you.” Former Administrator Lee Dickey, now Chief Quality and Innovation Officer at Casey’s Pond, praised the team effort. “As part of the Routt Memorial Hospital and Yampa Valley Medical Center organization, our nursing staff cared for hundreds of amazing residents,” Dickey said. “Many people have supported our efforts, working long and hard to earn rightful recognition as one of the outstanding long-term care homes in the nation.” Patient Thank-Yous Patient Thank-Yous I am a family physician who has worked in two different hospitals. The care I received at YVMC was more than excellent. The physicians, PA, nurses, radiology techs, lab techs, OTs and PTs were so kind and knowledgeable. I was impressed in how well I was taken care of. I have been disappointed in the quality of care my mom received recently in two different hospitals, and I wondered if this was the way hospitals were run these days. But YVMC changed my thinking. I had a wonderful experience (despite my broken hip) at YVMC. I was given top notch care & everyone was kind and courteous. Thank you so much. Every single one of you was friendly, helpful, efficient and a competent professional who knew his or her job very well. My care was awesome! Hypothermia. Acute coronary syndrome. What do all of these disease states have in common? They have all been topics for training in the HBB Simulation Lab at YVMC. In April 2012 the lab opened for training of YVMC staff and students. Funds for construction of the lab were generously donated by the HBB Foundation. Since that date, over 125 clinical nurses have been trained in scenarios ranging from pediatric pneumonia to hypothermia to acute coronary syndrome. These trainings take place three times a year. “The SimLab mannequins, fondly named ‘Marge’ and ‘Homer,’ offer the opportunity to learn new skills or refresh training for both new and experienced clinical staff,” Staff Development Coordinator Debby Harris said. “We are also excited about a new mannequin for training in obstetrics. “The training is especially important for low-volume, high-risk cases because it allows clinical staff to practice procedures they are not often called on to perform,” Harris said. “Often we are able to give the nurses an introduction to complicated diagnosis along with the actions and tools for their use.” In the past year, the SimLab has trained staff on the following diagnoses or conditions: acute coronary syndrome, stroke, hypoglycemia, seizures, sedation and sepsis for children and adults. In the fall of 2013, the SimLab partnered with Colorado Mountain College in Steamboat Springs to train the Emergency Medical Technician class. “The SimLab provides a great environment for students to treat medical emergencies in safe, yet realistic scenarios that mimic as real-to-life situations as possible” said Tim Baldwin, EMS Coordinator, Colorado Mountain College-Alpine campus. “The feedback we had from our students about the SimLab training was overwhelmingly positive. Working together, the students were placed in stressful situations and even had the opportunity to communicate patient reports to the Emergency Department physicians,” Baldwin added. New Clinical Education Council The Clinical Education Council at YVMC is a team of nurses representing a variety of clinical departments including Obstetrics, Patient Care Unit, Intensive Care Unit, Special Care Nursery, the PostAnesthesia Care Unit, Emergency Department, and the Operating Rooms. The purpose of the group is to address ongoing education needs from all nursing areas of YVMC. The nurses collaborate with senior leaders to design hands-on educational programs. Nurses participating in the Clinical Education Council include Susan Engle, Katie Noble, Kristin Kotkas, Julie McFadden, Sarah Kerrigan and Priscilla Schultz. Laboratory Laboratory Has New Leadership Up to 70 percent of all decisions regarding a hospital patient’s diagnosis and care are based on tests that are processed and interpreted behind the scenes in medical and pathology laboratories. Yampa Valley Medical Center’s Laboratory provides a vital link between our care providers and the people we serve. Pathologist Dr. James Hopfenbeck has good reason to be excited about strengthening that link. “We recently installed a new tracking system for our laboratory specimens that puts us on par with larger labs in metropolitan areas,” Dr. Hopfenbeck said. The equipment allows for positive identification of each specimen from the time it enters the laboratory through slide preparation and distribution to the pathologist. This technology helps reduce the risk of labeling errors and inconsistency. “The tracking system enables us to put a bar code on every specimen we handle,” Dr. Hopfenbeck said. “Not only does this ensure patient safety, the advanced technology enables us to view slides remotely. We can provide a faster diagnosis and improved outcomes for our patients.” According to Dr. Hopfenbeck, the technology is only a few years old, and YVMC is one of the few hospitals in the intermountain region that has installed the system. A native of Denver, Dr. Hopfenbeck joined YVMC as the pathologist in September 2013 after 20 years in practice at Lutheran Medical Center and Rose Medical Center in Denver. He earned his medical degree at Northwestern University and completed his residency and fellowship at the University of Utah affiliated hospitals. Dr. Hopfenbeck replaces Drs. William Cox and Greg Achenbach following their retirement. With 14 years of experience in the YVMC lab, new Director James Wirta is dedicated to providing accurate, timely test results to enable physicians to make correct diagnoses. “We can run thousands of tests, but we are very aware of being cost-effective and medically appropriate,” Wirta said. “That’s why it is so important for the lab team to work with the doctors to determine which tests will be most likely to provide answers to their medical questions.” Dr. James Hopfenbeck and James Wirta Brad Noble, MT (ASCP), has been with the Laboratory for 21 years. Our Emergency Department received the prestigious Guardian of Excellence Award from Press Ganey for consistently being in the top 5% of hospitals in patient satisfaction in 2013. YVMC was a recipient of the 2013 Healthgrades Patient Safety Excellence Award which also placed us in the top 5% of all hospitals for excellence in safeguarding patients from serious, potentially preventable complications during their hospital stays. The “Partnership for Patients” campaign is a national patient safety initiative that aims to reduce inpatient harm by 40 percent and avoidable readmissions by 20 percent in U.S. hospitals over a three-year period. With YVMC’s ongoing participation in this important initiative through the Hospital Engagement Network, we were successful in achieving ZERO patient harm incidents in 2013 in the following categories: ■ ■ Catheter associated urinary tract infections Central line associated blood stream infections ■ ■ ■ Early elective deliveries Pressure ulcers Surgical site infections due to MethicillinSensitive Staphylococcus Aureus (MSSA) ■ Ventilator associated pneumonia ■ Venous thromboembolism In 2013, YVMC instituted a quarterly Summit of Excellence award that recognizes hospital departments and multidisciplinary teams that have demonstrated excellence in improving overall quality and/or safety of care. Reward recipients for 2013 included: ■ The Respiratory Therapy department for their Arterial Blood Gas project. This project increased efficiency, patient satisfaction and quality by providing a mobile point-of-care testing process that helps ensure accurate patient identification, and delivers immediate test results to the physician. ■ The Bedside Medication Verification (BMV) team which included nursing, pharmacy and health information technology professionals. This project improved patient safety by automating the process of Quality Report Patient Comments (Emergency Department) verifying that the correct medication is being given to the correct patient. ■ The GrandKids team for their “Bottle Double Check” project designed to reduce the risk of infection that could occur from children receiving the wrong breast milk from improperly labeled bottles. We Are Listening The Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPs) is a national survey that asks patients about their experiences during a recent hospital stay. It covers topics such as how well nurses and doctors communicated, how responsive hospital staff were to patient needs, how well the hospital managed patients’ pain, and the cleanliness and quietness of the hospital environment. Based on data available on the Hospital Compare website, our patients rated YVMC higher than the Colorado average and national average on ALL ten important hospital quality topics surveyed in 2013. The following graphs show how we compared on patients’ overall rating of YVMC, and on how likely our patients are to recommend YVMC to friends and family. We feel very blessed and proud to live in Steamboat where every experience at YVMC is excellent and we love to recommend it! I have been in the ER numerous times in my life… different hospitals. The “Spending per Hospital Patient with Medicare” graph shows whether Medicare spends more, less, or about the same per Medicare patient treated in a specific hospital, compared to how much Medicare spends per patient nationally. In 2013, YVMC performed well on measures that show how often we provide care that research shows gets the best results for patients with certain conditions. The table on page 17 highlights some of these comparative scores (higher scores are better): When you or someone you love experiences an illness or injury that needs immediate attention, you can take comfort in the fact that we have an Emergency Department that is staffed by board-certified physicians and highly trained nurses who deliver lifesaving care with a combination of compassion and efficiency. Timely and effective care in the ED is essential for good patient outcomes. Delays can reduce the quality of care and increase risks and discomfort for our patients with serious illnesses or injuries. Our patients spent an average of 194 minutes being treated in the ED before they were admitted to the hospital, compared to 248 minutes for Colorado hospitals and 275 minutes for hospitals nationwide (lower is better). Patients at YVMC spent an average of 105 minutes in the ED before being discharged home, compared to 137 minutes for both Colorado hospitals and hospitals nationwide (lower is better). Childs, M.D. 1 6 YVMC Annual Report 2014 • yvmc.org As you may have heard by now, YVMC’s Cancer Care program earned accreditation with “Gold-Level” commendation from The Commission on Cancer of the American College of Surgeons in November 2013. The journey to accreditation was a long one. To earn Commission on Cancer accreditation, a cancer program must meet or exceed 46 CoC quality care standards, be evaluated through a survey process and maintain levels of excellence in the delivery of comprehensive, patient-centered care. Only 30 percent of all hospitals in the United States are accredited by the CoC. YVMC’s accreditation came with “commendation,” the result of exceptional teamwork and commitment by numerous physicians, care providers and leaders at YVMC. This accomplishment could not have happened without the support and guidance from our former CEO Karl Gills, our current CEO Frank May, the YVMC Board of Trustees, and the dedicated physicians and staff who have served on the Cancer Committee for the past four years. The CoC Accreditation Program provides the framework for YVMC to improve its quality of patient care through cancer-related programs that include prevention, early diagnosis, cancer staging, rehabilitation, lifelong follow-up for recurrent disease, and end-of-life care. As a CoC facility, our treatment protocols are measured against national standards using the National Cancer Data Base. The kits were then processed at no charge. Out of the 12 kits that were picked up, seven were processed with one positive finding. Prostate Cancer As part of the annual community health fair in September, PSA blood tests were made available to community members at the low cost of $30. Over 210 PSA tests were evaluated. Of these, 16 patients had an elevated PSA over 4.0. Breast Cancer In October, a special pink insert was created for two local newspapers, Steamboat Today and the Craig Daily Press with the message for women to get screened. A link was created on the Cancer Care webpage so that individuals could forward a reminder message to their friends. This link was accessed by over 1,170 individuals. During the month of October and early November, extended hours were offered during evenings and on Saturdays for mammography screenings. During this time period, 267 patients received mammograms. Of those, 2 were positive. In 2013, YVMC performed a total of 2,774 screening and diagnostic mammograms and ultrasounds with 22 positive findings. For those positive cases, each individual was notified within 24 hours and follow-up took place. 2013 Community Outreach Prevention Programs To inform our community about the range of our Cancer Services, a comprehensive 12-page brochure outlining all aspects of our cancer program was mailed to over 10,000 residents of Routt, Moffat, Jackson and Grand counties in December 2013. During this past year, the cancer website has been enhanced with more information. More than 500 individuals have accessed the website. The following programs were presented to community members during the calendar year 2013: Whole Person Wellness Programs A monthly series of talks titled “Whole Person Wellness Programs” was offered at Yampa Valley Medical Park. Each month an expert in the field discussed a single topic. Subjects that were addressed included exercise, yoga, nutrition, acupuncture, massage and Qigong, communication with health care professionals, journaling and mindfulness meditation. Cancer Program Striving for Excellence In collaboration with Rocky Mountain Cancer Centers of Denver, YVMC Cancer Care Program offers each patient and their families a full array of services to overcome the barriers and negative effects of cancer. In 2013, 80 newly diagnosed cancer cases were treated at YVMC. All of these individuals were navigated by a licensed clinical social worker and the cancer service program director. These individuals were offered patient care notebooks, counseling, genetic and nutritional counseling, rehabilitation therapies, and education on the availability of clinical trials. YVMC is fortunate to have a talented group of Medical Care Breast Imaging Clinical Trials Chemotherapy and Infusion Genetic Counseling Oncology Clinic Patient Navigation Surgical Services physicians and caregivers who actively participate on the Cancer Committee and Cancer Conference. This multidisciplinary group meets on a regular basis to discuss and identify best treatment protocols for individual cases. In 2013, Cancer Conference presented and discussed over 50 cases. The most common cancers presented were breast, prostate, colorectal, melanoma, and lung. Additionally, each year the cancer committee identifies annual goals for the Cancer Care Program. YVMC provides a wide spectrum of sophisticated healthcare services, delivered with the personalized care one would expect of a small town. Our medical facilities are the envy of communities throughout this country that rival us in size and complexity. YVMC continues to make improvements to meet the demands of a rapidly growing population and fast-evolving healthcare industry. These enhancements are often more costly than the hospital can support through operations. The Foundation seeks to meet funding needs so YVMC may continue to deliver top patient care. In its infancy, led by my longtime friend and fraternity brother Steve Dawes, HFYV raised funds as tasked by the YVMC’s Board of Directors. The HFYV became a feisty adolescent under the leadership of Chair Diane Tarnoff, acting where it also saw needs, along with responding successfully to other fund-raising challenges as tasked by the YVMC Board of Trustees. Two community events, “The Penguin Plunge” and “The Doc Willett Awards,” came into being during this time. HFYV’s focus now, in its adulthood, has intensified. Led by former Chair Wade Gebhardt who passed the reins to me last July, we amended our By-Laws to state that our sole function is to raise funds for YVMC for the benefit of the patients it serves. We will continue our activities to address and fulfill the specific needs of YVMC as directed by its Board of Trustees. We have also increased work on Major Gifts and Planned Giving programs with our very capable staff led by Executive Director Liz Finegan. The 2013 awards celebrate two leaders who each provided more than 30 years of health care to Yampa Valley. Diane Moore, executive director of Advocates Building Peaceful Communities, received the Health Care Community Advocate Award and Dr. David Wilkinson, Medical Director of the YVMC Emergency Department, was presented the Health Care Professional Award. This event raised $144,000 to benefit the most-needed programs at YVMV. The refreshing plunge raised more than $101,000 to benefit YVMC’s Cancer Care program. Gloria Gossard Supports Breast Health Center Gloria Gossard is fondly remembered for her many contributions to a variety of causes in Steamboat Springs, including the Tread of Pioneers Museum and the Bud Werner Memorial Library. However, her generosity did not stop there. In her lifetime Gloria Gossard contributed more than 20 gifts to Yampa Valley Medical Center. Her legacy of giving includes a major gift to the Healthcare Foundation for Yampa Valley Medical Center, making the construction of the new Gloria Gossard Breast Health Center a reality. Gloria had a history of supporting the special needs of women. She recognized the importance of a well-equipped hospital and compassionate people and believed care from friends and neighbors makes a difference. One of her more significant gifts provided funds for the purchase of digital mammography equipment. This more recent gift brings a new level of technology and tranquility to the special care given to women at YVMC. The state-of-the-art equipment includes new technology for breast MRI. Dr. Terese I. Kaske, a renowned radiologist, has joined YVMC staff as the Medical Director. “Gloria has given with grace. She encouraged everyone to be part of the giving to our hospital so it can continue to provide the best services to people here,” stated Geneva Taylor, one of her closest friends.
