In 2013 we introduced the concept of the Perioperative Orthopedic Surgical Home (POSH) model of care, a coordinated approach that is patient-centered, physicianled, and multidisciplinary. The Department of Anesthesiology, through many preoperative, intraoperative and postoperative initiatives, plays a vital role in this multidisciplinary model, with the primary goal of providing the highest quality surgical experience. Some of the Department’s perioperative initiatives, including the role of regional anesthetics, acute pain management and orthopedic critical care, are discussed at length in this report. We have also highlighted some of the less traditional but equally important initiatives that have taken place in our Department over the past year. The roles of focused transthoracic echocardiography, bioethics in anesthesia and perioperative medicine, and our global outreach initiatives set this Department apart from many others that simply provide basic clinical care. This differentiation is vital in maintaining HSS’s position as the global standard in orthopedics. In addition to clinical accomplishments, our physicians continue to publish ground-breaking research, lecture locally, nationally and internationally, and educate the next generation of regional and orthopedic anesthesiologists. ANESTHESIOLOGY AT HSS: SPECIALIZING IN REGIONAL TECHNIQUES The Department of Anesthesiology at Hospital for Special Surgery (HSS) prides itself on providing exceptional specialty care for an exceptional specialty hospital. Recognized as the premier physicians in the world for the practice of regional anesthesia and pain medicine for orthopedics, 85 percent of the 41,441 anesthetics performed by HSS anesthesiologists in 2013 involved the application of regional anesthesia (RA) techniques. Ideal for joint replacement procedures, RA is also a key contributor to the Hospital’s low rates of infection, complications and mortality. Its use significantly shortens surgical time, and patients benefit from decreased postoperative pain, less nausea and fewer side effects. Rates of deep vein thrombosis and pulmonary embolism are lower than with general anesthesia, and use of RA results in a markedly reduced risk of surgical infection. The Hospital’s low infection rates were acknowledged in New York State’s Hospital Acquired Infection 2012 Report, which was published in December 2013. The report confirmed that HSS’ risk-adjusted surgical site infection rate is less than half of the state average and the Hospital’s hip infection rate is less than half of what would be expected for our particular mix of patients. The Department continually supports research into how RA affects orthopedic outcomes. In 2013, Stavros Memtsoudis, MD, PhD, and a team of investigators studied the impact of anesthesia technique in patients undergoing total knee and hip replacement surgeries by analyzing national data from approximately 400,000 patients undergoing surgery at 400 hospitals. The goal of the study was to prove Dr. Memtsoudis’ theory that regional anesthesia favorably influences perioperative outcomes. Postoperative complications, 30-day mortality, length of stay, and patient cost were analyzed and compared based on anesthesia technique: regional, general, and a combination of the two. Dr. Memtsoudis published a landmark article after successfully proving that the utilization of regional anesthesia for primary joint arthroplasty is associated with superior perioperative outcomes. The study confirmed that patients who received regional anesthesia had statistically significant decreases in 30-day mortality, length of hospital stay and in-hospital complications, including fewer infections. department of anesthesiology 2013 Annual Report 3 highlights & strategic accomplishments AN INTEGRATED APPROACH TO PATIENT CARE “Dr. Kahn stood out amongst an excellent staff. A genuinely caring manner – one of the best I’ve ever experienced.” Anesthesiologists at HSS do much more than provide anesthesia in operating rooms; they are also responsible for many other patient care services integral to the entire perioperative experience. The Perioperative Surgical Home (PSH) model of care, a patient-centered, physicianled multidisciplinary system of coordinated care that guides a patient throughout the entire surgical experience, is currently being developed by the American Society of Anesthesiologists. The purpose of the integrated care provided via the PSH model is to create a better, more cohesive patient experience while making surgical care safer and more efficient. The Hospital’s pre-surgical screening process is tailored specifically to a patient’s history and complexity. Patients may receive pre-surgical consultation by a physician in the Department of Medicine, and depending on complexity of the surgery, medical history and past experience with anesthesia, the patient may also receive an anesthesiology consultation. During the anesthesia consultation the anesthesiologist evaluates past medical history and assesses the patient’s anesthetic options. Furthermore, patients with comorbidities or a complex medical history may be referred to the Complex Case Committee for review prior to surgery. Enhanced coordination in the management of surgical patients has been proven to increase quality, reduce complications and improve the patient’s perception of the surgical experience. department of anesthesiology 2013 Annual Report 5 STATISTICIANSPOTLIGHT SPOTLIGHT STATISTICIAN KARA FIELDS, MS In 2013, the Department’s Research Division expanded its formal collaboration with the HSS Epidemiology and Biostatistics Core with the addition of statistician Kara Fields, MS, to the research team. Ms. Fields obtained her Master’s degree in biostatistics at Columbia University. She interned at HSS while attending Columbia, and was subsequently hired as full-time biostatistician in September 2012. Under the direction of Stephen Lyman, PhD, and Yan Ma, PhD, Ms. Fields works on study design, statistical analysis planning, power calculations and data analysis. She evaluates research protocols using the Clinical Review Panel process and provides statistical support for the Department’s Journal Club meetings and Grand Rounds. In 2013, she co-authored two publications and gave an educational lecture to Department faculty about the published standards for reporting on randomized clinical trial methodology and results. When asked her thoughts on the collaboration, Ms. Fields remarked, “I am grateful to be working closely with a Department that is uniformly dedicated to conducting and publishing research of the highest quality. After assisting in numerous studies which explore methods to reduce postoperative pain and frequency of adverse events, I appreciate how the anesthesiologist’s responsibilities extend far beyond the operating room.” Stephen Haskins, MD, teaching HSS anesthesiologists Enrique Goytizolo, MD, Stephen Harris, MD, and Thomas Quinn, MD. FOCUS ASSESSED TRANSTHORACIC ECHOCARDIOGRAPHY FIRST COURSE IN NORTH AMERICA Transthoracic echocardiography (TTE) is a tool traditionally used by cardiologists to answer complex clinical questions about a patient’s cardiovascular health. However, few physicians without formal training in cardiology utilize a focused TTE exam for its other potential clinical benefits. Focused TTE is becoming an important tool for anesthesiologists and critical care physicians in the perioperative setting when used to supplement clinical evaluation and optimize cardiopulmonary resuscitation. There is growing awareness of the benefits of focused TTE among anesthesiologists and intensivists, but there are few opportunities available to adequately train physicians to incorporate this tool into their daily practice. The Focus Assessed Transthoracic Echocardiography (FATE) exam was established in 1989 and has been taught via training courses since 1996. The FATE protocol training course teaches physicians how to perform a focused TTE exam and then utilize the information to improve patient management and care. Prior to the course, participants complete an online e-learning module to prepare for hands-on training in the FATE exam. Although FATE originated in Denmark, the FATE training courses have expanded dramati- cally establishing a strong international presence throughout Europe, India, and South Africa. In March 2013, the Anesthesiology Department partnered with USabcd.org to host the first official FATE course in North America. Stephen Haskins, MD, coordinated the program and taught the course under the supervision of FATE protocol originator, Professor Erik Sloth, MD, PhD. Twenty HSS attending anesthesiologists participated in the course, learning to perform a focused cardiac echo exam to supplement standard clinical evaluation and to guide cardiopulmonary optimization. Subsequent to the March course, at the invitation of Professor Sloth, Dr. Haskins traveled to Denmark to receive further training and was designated as the first clinical instructor of the USabcd. org FATE Course in the United States. In July, Dr. Haskins, assisted by Jemiel Nejim, MD, Sean Garvin, MD, and Stephen Harris, MD, conducted a second FATE Course at HSS. In January 2014, HSS held a FATE protocol course open to non-HSS physicians to share this important skill with doctors outside the HSS community. This was the first open FATE course in North America. highlights & strategic accomplishments ORTHOPEDIC SPECIAL CARE UNIT In October 2013 the Orthopedic Special Care Unit (OSCU), a oneof-a-kind, dedicated intensive care unit, opened on the fifth floor of the Hospital. This four-bed unit was designed to specifically care for high acuity patients with complex medical needs. Although anesthesiology and ethics are not always readily associated, as facilitators of perioperative surgical services, anesthesiologists are confronted with difficult patient care decisions that go beyond perioperative evaluation, selection of anesthetic technique and postoperative care. The Department of Anesthesiology addresses these ethical challenges through multiple clinical, educational and research initiatives. In 2013, Douglas Green, MD, Michael Nurok, MB ChB, PhD, and Gregory Liguori, MD, were part of a team that revised the Hospital’s Do Not Resuscitate (DNR) policies, ensuring alignment with New York State Law and national society guidelines on patient options for DNR in the operating room. In addition, they are currently developing a policy for situations where requested care may not be clinically appropriate. HSS anesthesiologists serve on a variety of committees to promote ethics in medicine: Mary Chisholm, MD, serves as a member of the HSS Institutional Review Board. The Board oversees the treatment of human subjects and brings a focus on ethics to the committee’s activities. Dr. Nurok serves as an alternate member. Dr. Chisholm and Dr. Nurok are members of the HSS Ethics Committee, and Dr. Green serves as Co-Chair. Committee members foster an interdisciplinary approach to clinical ethics and convene regularly to review current research and approaches. The committee supports clinicians, patients and family members who are facing ethical challenges, and members provide consultations to assist in the resolution of ethical issues. D r. Nurok serves on several national society committees that manage ethics and related topics, including the American Society of Anesthesiologists Committee on Ethics and the Society of Critical Care Medicine’s Quality and Safety Committee. Lecture topics in the field of ethics. eight-week course include the rise TEACHING SPOTLIGHT ANNUAL INTERDISCIPLINARY PROFESSOR IN ANESTHESIOLOGY In December, Christopher Wu, MD, Chief of the Division of Obstetrics & Regional Anesthesia and Acute Pain at Johns Hopkins Medicine, presented Grand Rounds to the HSS Medical Staff as the third annual Distinguished Visiting Professor in Anesthesiology. Dr. Wu also moderated a Visiting Professor Journal Club for the anesthesiology staff, visited HSS clinical settings to share best practice information and joined members of the Hospital’s leadership team for continued discussion of his lecture topic. Dr. Wu’s Grand Rounds lecture: “Dying for the Weekend: Does the Weekend Effect Exist?” detailed his research on the weekend effect, a possible increase in morbidity and mortality when patients are admitted or cared for over the weekend. His research and literature review showed that the existence of a weekend effect varies; for example, hospitals with staffing rates that remained the same showed no weekend effect, while those with reduced staffing on weekends did report a significant effect. While the existence of this effect is not yet fully proven, if the weekend effect is confirmed it may lead to changes in healthcare policy and delivery. GLOBAL INTERNATIONAL OUTREACH Anesthesiologists at HSS, as leaders in the field of orthopedic anesthesia, are committed to reducing disparities in healthcare and narrowing the burden of surgical and anesthetic disease in low and middle-income countries. Most of these countries have a critical shortage of qualified surgical and anesthesia providers, causing a healthcare workforce crisis. To help alleviate this, HSS anesthesiologists take an active role volunteering in surgical missions around the world, including Dominican Republic, Ecuador, Ghana, India and Haiti. They are making strides within global international outreach through their participation in these missions, which provide surgery to those who would otherwise be unable to afford them. Over the course of 10 one week, teams are able to perform an average of 50 operations, and many of these surgeries prove to be lifechanging for patients. orthopedic hospital, serves as a major teaching institution and referral center for orthopedics. As a government hospital, HTO treats patients who cannot afford private healthcare, a majority of whom are orthopedic trauma patients who require surgery. Through this global collaboration, HSS anesthesiologists visit HTO twice each year to teach regional anesthesia techniques to their anesthesia providers, helping to improve outcomes, safety and postoperative pain analgesia while decreasing the need for expensive equipment. Ultimately, the goal of the collaboration is to strengthen the anesthesia workforce in Vietnam and provide an improved standard of anesthetic care. Sustainable development through teaching is the key to empowering local communities and providing lasting change. HSS anesthesiologists take an active role in volunteering in surgical missions around the world, including Ghana, India and Haiti. While short-term surgical missions are impactful, the effect is often finite. As a result, the Department is fostering a new focus on long-term collaborations that provide a greater impact by teaching anesthesia providers modern and safe practices, with a particular focus on regional anesthesia for orthopedic surgery. In 2013, the Department began a collaboration with the Hospital for Traumatology and Orthopedics (HTO) in Ho Chi Min City, Vietnam. This 440 bed highlights & strategic accomplishments department of anesthesiology 2013 Annual Report 11 5 department events 1 1–5: In June 2013, the Department again proudly hosted HSS participation in the JPMorgan Corporate Challenge, a 3.5 mile race benefiting the Central Park Conservancy. More than 400 HSS staff members participated in a night filled with fun, camaraderie and team spirit. 2 5 3 4 7 7: At the Department’s bi-annual retreat, 50 staff members and physicians participated in an afternoon of teambuilding, and gained insight about themselves and their coworkers by completing the MeyersBriggs personality assessment. 8–9: Many of the Department’s research and administrative staff members attended the American Society of Anesthesiologists’ Annual Meeting, held in San Francisco in October 2013. Six physicians and staff members participated in a pre-conference 5K race, and Research Assistant Marcus DiLallo came in second place with a time of 17.17. Marcus also finished the 2013 New York City Marathon with a time of 2:55:37. 12 8 9 department growth The Department of Anesthesiology proudly announced the addition of five outstanding physicians to our team in 2013. STEPHEN C. HASKINS, MD Assistant Attending Anesthesiologist Dr. Haskins is a graduate of New York University School of Medicine. He completed residency training at NewYork Presbyterian-Weill Cornell Medical College, where he served as Chief Resident and received the Ronald A. Leff Award for Excellence in Clinical Teaching by a Resident. He is a graduate of HSS’ regional anesthesia and acute pain medicine fellowship, where he received the Attending Anesthesiologists’ Award for Clinical Excellence. Dr. Haskins is boardcertified in anesthesiology with additional certifications in Basic Perioperative Transesophageal Echocardiography and Focus Assessed Transthoracic Echocardiography (FATE). MEGHAN KIRKSEY, MD, PHD Assistant Attending Anesthesiologist residency at NewYork-Presbyterian Hospital-Weill Cornell Medical College and a critical care anesthesiology fellowship at NYPH-Columbia University. She received the Resident Research Award from Weill Cornell Medical College and the Award for Clinical Excellence during her HSS residency rotation. Dr. Kirksey is double board certified in anesthesiology and critical care. KANUPRIYA KUMAR, MD Assistant Attending Anesthesiologist Prior to joining the HSS medical staff, Dr. Kumar served as Director of Regional Anesthesia at Johns Hopkins Bayview Medical Center. She received her medical degree at Albert Einstein College of Medicine and completed residency training in anesthesiology and critical care medicine at Johns Hopkins University. Board certified in anesthesiology, Dr. Kumar is a graduate of HSS’ Fellowship in Regional Anesthesia and Pain Medicine. Dr. Yang worked as a staff anesthesiologist at North Shore University Hospital prior to joining the HSS medical staff. She is double board-certified in anesthesiology and critical care.
