SHOP ORDER TERMS & CONDITIONS
-- REVIEW each line item and corresponding Shop Notes for completeness and
accuracy. This Shop Order is an interpretation of the original contract and, in
correlation with the Engineering Drawings included herein, is the only document
used for manufacturing of this order.
-- Some Part Numbers may have an empty line in the Shop Note that requires
further information to complete the Shop Order. This order can not be released to
Manufacturing upon such time as all pertinent information is provided. Failure to
provide this information will result in a delay in final delivery of the apparatus and
void any contractual delivery times (see original Contract agreement).
-- CORRECTIONS are be clearly marked next to the corresponding part number
and any corresponding location on the Approval Drawing. Major corrections may
require a second approval process, as determined by Engineering.
-- APPROVAL of this order is required by the signature of a duly authorized
representative of the purchaser (see last page of Shop Order). It can be approved
"As Is With No Changes" or "As Is With Changes Noted". Approvals with
"Changes Noted" will be corrected and a copy of the corrected Shop Order will be
returned to the purchaser for verification. No further approvals are required. This
also applies to the corrsponding Approval Drawing.
-- REQUIRED RETURN TIME - This Shop Order is required to be returned not
later than 10 days after receipt to Hackney with signatures. Failure to return in

CONDITIONS OF ACCEPTANCE
This Shop Order and any preceding chassis related specifications supersedes
the customers issued specifications and reflect the apparatus proposed to be
manufactured and delivered by Hackney. All applicable exceptions were recorded
separately and titled "Exceptions or Clarifications" in the submitted Bid/Proposal.
Final inspection of the completed apparatus will be conducted using the Proposal
Specifications and Engineering Proposal Drawing(s).

Changes made to this order after receipt of approvals are subject to a minimum
of $250.00 engineering surcharges, plus cost of components. No changes will be
permitted after production has started.
To facilitate an accurate inspection environment, Hackney will, upon the request
of the purchaser, schedule specified inspections during regular working hours
Monday through Friday. Said inspections will be at the discretion of the
purchaser. Hackney will make the following factory personnel available during the
inspection process: sales administrator responsible for the contract; engineering
representative familiar with the contract; production foreman or equivalent
assignment responsible for the manufacturing process of the contracted
apparatus and a representative of the upper management team authorized to
make decisions for the contractor.

DESCRIPTION
TEST DRIVE & INSURANCE REQUIREMENTS
VEHICLE LICENSE PLATES/TAGS REQUIREMENTS: VT Hackney (VTH) may
issue temporary tags to a customer picking up a completed emergency vehicle
only for an out-of-state delivery. NC DMV laws do not permit VTH to issue
temporary tags for vehicles delivered within the State of North Carolina. If the
chassis is customer supplied or dealer supplied driveaway tags must be provided
by the customer/dealer. In the event VTH provides a temporary tag proof of
insurance must be provided prior to leaving the VTH premises. Temporary tags
are for the purpose of transporting the vehicle from the factory to the end users
location only and cannot be used once the vehicle is placed into service.
MANUFACTURER'S STATEMENT OF ORIGIN (MSO): VTH corporate risk
policy does not permit the release a MSO until such time as full payment is
received for the completed and accepted vehicle.
DRIVERS LICENSE: All drivers will be required to submit a valid driver's license,
a copy of which will be inserted into the Customer File. For vehicles with GVWR
of 26,001 or more the driver will be required, by North Carolina DMV Law, to
present a valid CDL (Commercial Driver's License).
TEST DRIVES: VTH's underwriter will not permit a non-employee to drive any
vehicle off its premises unless a copy of proof of insurance for the vehicle is
provided prior. A non-employee cannot operate a vehicle using a VTH
Manufacturer's license plate. Either a temporary tag or the customers tag must
be attached prior to the test drive, unless such is conducted within the VTH
campus. In the event any of the above conditions cannot be met by the customer,
VTH will provide a qualified VTH employee to drive the vehicle for the test with a
customer representative(s) on board.
