About this guide
Welcome to Eldercare, a guide to senior services in Ventura County. Here at the Ventura County Star
we are proud to work with the Ventura County Area Agency on Aging (VCAAA) to provide a comprehensive directory of services for our community of seniors.
The articles and listings in this guide are meant to be just that—a guide to help seniors and their
families navigate needs that come with aging. The landscape of senior care has changed since the Star
first published the Eldercare senior resource guide seven years ago. Since then, the Affordable Care
Act has become law, and more boomers have reached retirement age. It is our hope that every older
adult in the county will use this guide to find the information they need to make informed decisions
about their futures. Whether you are looking for an attorney to help settle an estate, a counselor to
comb through insurance forms or some low-impact fitness activities, Eldercare is the place to find it.

A: The different parts of
Medicare cover your specific
needs. There are four parts,
all of which work in tandem
to deliver healthcare services.
Part A (hospital insurance):
Hospital insurance helps pay
for inpatient care in a hospital
or skilled nursing facility (following a hospital stay), some
home health care and hospice
care.
Part B (medical insurance):
Medical insurance helps pay
for doctors' services and many
other medical services and
supplies that hospital insurance doesn't cover.
Part C (Medicare Advantage plans): If you have Medi-
care Parts A and B, you can
join a Medicare Advantage
plan. Private companies offer
Medicare Advantage plans,
which are approved by Medicare. These plans generally
help you pay the medical costs
not covered by Medicare Part
A and B.
Part D (prescription drug
coverage): Prescription drug
coverage helps pay for medi-
cations doctors prescribe for
treatment.

Healing in the
comfort of home
has its advantages
Today, medical home care
is a much-needed service as
patients are realizing the advantages of healing in their
homes.
Hospitals strive to decrease a patient’s length of
stay by treating them and
sending them home quickly.
Accredited medical homecare agencies work closely
with hospitals to transition
the care and provide nursing,
physical therapy, social services and home-health aides
in the privacy of the patient’s
home under the supervision
of a licensed medical doctor.
With an increase in the elderly population due to aging
baby boomers, it is important
to understand services available through insurance and
Medicare. The trend in medical care is to decrease unnecessary readmission rates to
hospitals.
“Medical home-care agencies provide community outreach programs to include
free services to seniors, advocate on behalf of loved ones
and communicate with the
patient’s primary physician,
such as medication reconciliation issues, which is among
the leading causes for patient
readmission to hospitals,” according to Dr. Suzanne Fussell
with Healthwise Home Care
Solutions.
There is a Medicare certification process for medical
home care providers. The certification requires that strict
standards be met in providing
the medical services needed
by the elderly, disabled or
those who qualify for Medicare services.
The Joint Commission of
Healthwise Home Care Solutions
provides licensed clinicians to provide
free community outreach programs like
blood pressure and glucose screenings.
Hospitals Organization reports that high standards in a
hospital setting can be maintained in the patient’s home
while taking advantage of
medical home-care services.
The industry began in the
U.S. in the 1880’s and about 12
million individuals currently
receive care from more than
33,000 providers according to
the National Association for
Home Care & Hospice.
Accredited medical homecare agencies provide education for family members and
friends while maintaining
the recipient’s dignity and
independence, qualities that
can be lost even in the best institutions. Medical home care
also allows patients to take an
active role in their own care.

providers with home seekers
HomeShare is a free program that addresses the
housing needs of seniors.
The program matches home
providers with home seekers
who are willing to help with
household tasks, transportation, companionship, financial
support or a combination of
these in exchange for housing.
All parties involved in the
housing arrangements go
through a detailed screening
process conducted by HomeShare staff.
This program provides
housing opportunities for
seniors, often where they do
not otherwise exist, although
the program is not limited
to seniors. This can include
matching seniors with college students or with a family.
Housing arrangements are
beneficial to both parties because the rental agreements
are constructed based on
needs specific to both parties.
In Ventura County, there
are limited options for affordable housing for the growing
senior population. This program allows seniors to maintain their independence by
remaining in their homes or
in the community they know.
Participation in a HomeShare arrangement also provides peace of mind, knowing
there is someone in the home
to check on them or aid them
if they have an emergency.
Lastly, for the home seeker,
a HomeShare arrangement
provides an affordable alternative to long waiting lists for
subsidized housing.

Aging in place: maintaining
independence by reducing fall risk
The desire to stay at home when growing into older adulthood is referred to as
aging in place. Aging in place is a priority
for many older adults who wish to remain
as independent as possible. Unfortunately, older adults who are aging in place are
also at risk for falls and fall-related injuries. Oftentimes falls go unreported to
family, caregivers or healthcare providers for fear of losing one’s independence
in the home.
Although most reported falls occur in
the home, they also occur outside. Some
of the risk factors for falls include: being
over the age of 65, having poor balance
and strength, taking five or more prescription medications, having any form
of vision impairment and environmental
hazards. The following tips can help prevent and reduce falls while maintaining
independence while aging in place.
Stay active
As we age we may experience a natural decline in areas of balance and mobility, which may increase the risk for
falls. Physical activity is the best way to
help slow down and even prevent loss of
balance and movement. Not only does
staying physically active reward you
with many other benefits, such as an increased quality of life, it is proven to help
reduce and even prevent falls. Get into an
evidence-based physical activity class in
your community. Contact the Fall Prevention Program at 805-477-7343 to get a list
of classes in your area. Keep some form
Get your vision checked regularly and
get your eyeglass prescriptions updated
regularly.
Talk to somebody
Keep your family, friends, caregivers
and healthcare providers informed. Ask
for help when you need it. Look for resources offered in the community to help
educate you on fall prevention.
The Fall Prevention
Program can help

The Fall Prevention Program was created by the Ventura County Fall Prevention Coalition in an effort to help reduce
falls in adults 65 years old and older,
who are aging in place, and who have
experienced a recent fall. The program
partners with local hospitals, Ventura
County Emergency Medical Services,
home- health providers, public health providers and VCAAA to service its clients.
The Fall Prevention Program is a pilot
program currently serving west county,
and is expected to expand to serve east
county. Our goal is to provide appropriate
resources in order to help facilitate in rehabilitation, mitigate fall risk and prevent
future falls.

Make your home safer
Remove anything on the floors that you
may trip over, consider home modifications if you haven’t already (i.e. grab bars
in shower or near the toilet), use non-slip
rubber mats in the shower, wear socks
with no-slip grips on the bottom, move
furniture around to create more space
and clear pathways and increase lighting throughout the home.
Review your medications with your
doctor. Talk with him or her about
your fall history and any concerns
you may have about being at risk for
falls.
Ask your doctor to prescribe an exercise program or physical therapy.
