We understand that dental care is an important part of health and wellness. That’s why we offer
the AARP MedicareComplete Optional Supplemental Plans — the Dental Silver and Dental Gold
Plans — with a network of contracting dentists to serve you.
How the optional dental plans work
The optional dental plans offer discounted fees for more than 100 different dental procedures.
Some procedures, such as office visits, oral exams and routine cleanings, are provided for just an
office visit copayment. Each dentist within the contracting network has agreed to a discounted
fee schedule for AARP MedicareComplete members. Keep in mind that whenever you make an
appointment with your assigned contracting dentist, you may be responsible for the office visit
copayment in addition to the fee for the procedures performed.
For additional information, please refer to the Dental Benefits sheets included in the Pre-enrollment
Kit or request a Dental Benefits booklet from Customer Service. The AARP MedicareComplete Dental
Silver and Dental Gold Plans and contracting network are administered by Pacificare of Arizona, A
UnitedHealth Group Company.
Choice and Convenience
This directory lists a wide choice in contracting dental providers — some right in your community.
PacifiCare of Arizona inspects each office and conducts periodic audits to monitor that quality
standards are maintained.
How to choose a dental provider in our contracting network
1. Choose the office location that best suits you.
2. Enter the name and facility number of your desired contracting dental provider on the line
provided on your AARP MedicareComplete Plan enrollment form.
It’s that simple!
Note: If you have chosen the Deluxe Rider plan, there is a different dental provider network available.
Please call Customer Service at 1-800-347-8600 (for the hearing impaired, 1-888-685-8480), 8 a.m.
to 8 p.m., 7 days a week, to request a dental directory be sent to you. You may only choose one dental
plan, including the Deluxe Rider plan.

The Dental Care Provider Network
Making dental provider changes in our contracting network
If you wish to change assigned dental offices, you may do so by contacting Customer Service at
the number below. If your change request is received by the 15th of the month, the change will
be effective the 1st of the following month. For example, if we receive your change request on the
15th of July, your change is effective on August 1st. If we receive your change request on the 16th
of July, your change is effective on September 1st.
Who to call for help and assistance
If you’re not yet a member, but would like assistance or recommendations before selecting
your contracting dentist, simply call: an AARP MedicareComplete Sales Representative at
1-800-547-5514 (for the hearing impaired, 1-866-832-8671).
If you’re currently an AARP MedicareComplete member and would like more information regarding
the Dental Silver or Dental Gold Plans, simply call: Customer Service at 1-800-445-9090 (for the
hearing impaired, 1-888-685-8480), 8 a.m. to 8 p.m., 7 days a week.
Note: Members who are covered through their employer groups (MedicareComplete Retiree Plan
Members) may not see their county of residence listed in this directory. If your county is not listed and
you are a MedicareComplete Retiree Plan Member, please call Customer Service at 1-800-347-8600
(for the hearing impaired, 1-888-685-8480), 8 a.m. to 8 p.m., 7 days a week, for the name of a
contracting dentist near you.
All contracting dental offices are subject to availability. Should the contracting office you select
be unavailable, PacifiCare of Arizona reserves the right to assign you to another contracting
dental office. Some offices may have been added since this list was last printed. You may phone
Customer Service for additional locations.
