Perhaps no other sector of our economy
has experienced more change this year than healthcare.
Our nation is moving to a system that rewards high quality
clinical and patient satisfaction outcomes; in this future-state,
patients are the greatest beneficiaries. VNA HealthCare has
remained ahead of this curve through its clinical innovation,
focus on patient-centered care and by implementing
nationally-recognized best practices before others. We have
committed our resources to being the region’s leader in
home health care, as well as being a leading advocate for
the industry by educating local and national policymakers on
the consequences that proposed cuts have on non-profit
VNAs who serve as the region’s health care safety net
through the provision of low-cost and highly effective
care to millions.
Innovation
VNA HealthCare has been a pioneer in
developing and implementing specialty programs aimed
at keeping patients out of the hospital. To that end, the
agency was an early adopter of telehealth technology
and was the first agency in the state to offer patients
a specialized pulmonary program led by a respiratory
therapist. We also augment our outstanding clinical care
with leading-edge wound, diabetes, and cardiac
nursing programs.

Quality
Last October, VNA HealthCare was named
to the 2009 HomeCare Elite™, a compilation of the
top-performing home health agencies in the United States.
This prestigious, annual review identifies the top 25 percent
of home care agencies as ranked by an analysis of quality
outcomes, quality improvement, and financial performance.
Thanks to the skill and expertise of our nurses, specialists,
therapists and support staff, the agency has consistently
exceeded many state and national home care quality
benchmarks throughout the year. We’ve also made
significant strides in reducing our patient re-hospitalization
rates and currently boast one of the lowest rates
among similar agencies in our service area.
Mission
Our focus on quality, innovation and leadership
poises our agency for success and, coupled with community
support, enables us to continue our 109-year-old mission of
caring for everyone in the towns we serve. We deeply
appreciate our community partners and friends whose

During the past year, the agency has successfully completed
its transition to electronic medical records. Achieving this

Restoring QuaLity
to Life
Sandra Werkheiser was granted a new lease on life.
Seven months ago, end-stage congestive heart
failure prevented the once-active 76-year-old
from participating in her favorite activities.
“I went from being an avid walker to being unable to
walk more than 20 feet. I didn’t feel well and couldn’t
socialize like I used to. I just wanted to enjoy life
again,” she explained.
“I have my life back,
and that means
the world to me.”
Deemed ineligible for a heart transplant due to age,
Sandra was left with few options. Concerned, she
turned to the experts at Hartford Hospital’s Henry Low
Heart Center where she learned about an innovative
procedure for transplant-ineligible patients: LVAD (Left
Ventricular Assistive Device) for Destination Therapy.
Sandra’s doctors explained how an LVAD implant would
help pump blood more efficiently through her body.
Eager to return to the activities she loved, Sandra
opted for the leading-edge procedure, becoming just
the second patient in the state to receive the treatment
that had been approved one month earlier.
Throughout Sandra’s four-week recovery in the hospital,
her team of cardiac specialists and case coordinators
worked hand-in-hand with Hartford Healthcare system
partner VNA HealthCare to ensure a seamless transition
home and to teach her visiting nurses about the highly
specialized care that she would require.
Twice weekly, Didi Prignano, RN, a visiting nurse with
extensive cardiac care experience, visited Sandra at
her home to perform a cardiac assessment, change
her wound dressings, and continue the education she
received in the hospital about her condition. The visiting
nurse continually updated Sandra’s doctors and nurses
at Hartford Hospital about her patient’s progress.

In addition to her regular nursing visits, Sandra’s vital
signs were checked daily by a home health telemonitor.
Often used for cardiac patients with complex conditions,
the device prompts them to take their own vital signs,
and securely relays the information via phone lines to
VNA HealthCare. A telemonitoring nurse with extensive
training in cardiac care reviews incoming results
and immediately intervenes at any indication
of an abnormality.
“With this technology,“ said Beka Apostolidis, MS, RN,
VNA HealthCare Cardiac Program Manager, “any
irregularities in patients’ vital signs can be discovered
and treated before they can exacerbate. We’re often
able to prevent patients from visiting the emergency
room or being admitted to the hospital.”
“The telemonitor definitely kept me compliant and in a
routine,” Sandra stated. “It was reassuring to know that
I was in good hands.”
Thanks to the collaborative efforts of Hartford Hospital
and VNA HealthCare, Sandra has regained her quality
of life and has achieved her goal of once again
attending her beloved bridge club with her friends.
“I have my life back,“ Sandra said “and that means
the world to me.”
OLd Guard of West Hartford:
Commitment to their Community
“When our

important,”
The term “Old Guard” is often used to describe any
group of distinguished, respected and trustworthy
individuals - and it’s a fitting name for the Old Guard
of West Hartford.
Founded in 1966 by the West Hartford YMCA, Old
Guard members gather for weekly meetings replete
with songs and guest speakers. More than a social club,
the group of 300 retired professionals is staunchly
committed to giving back to their community and has
been one of VNA HealthCare’s most loyal cadres of
Meals on Wheels volunteers for more than 34 years.
“Everyone experiences a transition during retirement,”
explained member Dick Harris. “As you leave something
behind, you also want to look forward to something.
Volunteering keeps our members linked with the
community and their colleagues, and keeps us
all younger.”
Harris, along with members Tom Montgomery, George
McGauley and Brad Blanchard, coordinates 50 Old
Guard members to deliver thousands of meals each
year to elderly and disabled West Hartford residents.

“Every so often, we’ll notice that something is amiss
with a client,” Harris said. “When that happens, we
call VNA HealthCare immediately so that their staff
can check on them.”
“Sometimes, we’re the only individuals that these people
see all day. We always stop to say hello and provide
that human touch,” Harris explained.
“When our volunteers finish delivering meals on
a route, they have a real sense of having accomplished
something important,” he said. “Every day, our
customers thank our volunteers for being there.
That’s reward in itself!”

“Our volunteers know that Meals on Wheels is often the
one component that keeps individuals living in their own
home and out of a nursing home. A lot of people on our
route are too sick or disabled to cook or shop. Some
receive meals on an interim basis after an operation or
serious illness; it takes a huge burden off their shoulders
as they transition back to health,” said Harris.

“Receiving these meals is certainly a big help,” said
Frances, a West Hartford resident whose Meals on
Wheels are delivered three times a week by the
Old Guard. “These gentlemen are excellent,
and so is the program,” she stated.

Stacey DiPiazza:
Pursuing Her Passion
Stacey DiPiazza wears many hats. She’s a wife and a
busy mom of three young girls. She’s the owner of
a booming business. And she’s a caregiver to her
96-year-old grandmother.
When Stacey’s family determined that her beloved
96-year-old grandmother, Margaret, could no longer
remain by herself during the day, the solution, Stacey
said, was clear: VNA HealthCare.
But despite her hectic schedule, Stacey always finds
time to pursue her passion: helping to raise funds and
awareness for VNA HealthCare, an organization close
to her heart.
Although the family wasn’t sure what services their
grandmother would need, an intake nurse specialist at
VNA HealthCare helped them determine that Margaret
would benefit from a Personal Care Attendant. Shortly
after the phone call, Elcira Navarro arrived at the
front door.
“I truly believe that everything happens for a reason,”
Stacey said, alluding to her initial involvement with
VNA HealthCare.
“Seven years ago, I received a call from the agency’s
Vice President of Development asking if I would join
their fund board. He said that he just read my honoree
profile in the Hartford Business Journal’s ’40 Under
Forty’ section and thought that my experience
would complement their goals,” she explained.
After discussing the opportunity with her husband, Jack,
Stacey was determined to not only give her time and
talent, but also in-kind shredding services from her
company, InfoShred.

“We talked about VNA HealthCare’s reputation in the
community and how helpful it was to Jack’s grandmother
when she was ill. We felt that the agency could use the
money saved on shredding services and put it back
into their other vital programs.” she said.
Stacey enthusiastically plunged into her new role on the
fund development committee. Since 2003, she has been
an eager leader, generous philanthropist, and corporate
sponsor, donating nearly $100,000 in in-kind shredding
services to the agency. She also volunteers with the
agency’s Nurturing Families program and has been a
catalyst and champion for the annual “A Taste for Hope”
event, which to-date has raised nearly $75,000 for
VNA HealthCare’s hospice program.
To the family, Elcira represents much more than daily
help with personal care and housekeeping. She’s a
much-loved companion and best friend to Margaret.
And to Stacey and her parents, the Personal Care
Attendant is “peace of mind personified.”
Stacey’s experience with helping to coordinate her
grandmother’s care has only strengthened her resolve to
ensure that others have access to the same exceptional
care. She admits, however, that her enthusiasm for
volunteering has a deeper meaning.
“I want to set an example for my daughters on how
important it is to give back to others,” she stated.
“My hope is that by watching my actions, they will
choose that path for their own lives.”
“If I am able to inspire
my girls or anyone
else to get involved,”
she explained, “then
everything I have
done has been worth
it. I truly believe that
what you give out to
the world, you get back.

VNA HealthCare honored seven local doctors,
nurses and community members for their
commitment to providing quality end-of-life care to
the Greater Waterbury community at its November
19th “Angel of Hospice” awards reception.
“The program is based on recognizing those
whose professionalism and compassion for the
terminally ill have made a profound difference in
the lives of hospice patients and their families,”
said Tara Green, Director of Hospice.
Harold Leever Cancer Center oncologists Dr. Anamika Katoch and Dr. Kert Sabbath were
recognized with an Angel of Hospice Award for their support of terminally ill patients and
symptom management. Accepting on their behalf were staff Nan Gano, Sharon Ignatavich,
Kelly Danforth, Holly Bemonte, and Karen Bisson.

by the agency’s H.O.P.E. (Hospice, Oncology, and

11th Annual VNA HealthCare Golf Classic
The Annual Golf Classic was held on May 24, 2010,
at the Shuttle Meadow Country Club in Kensington
and raised more than $64,000 in support of Meals
on Wheels. More than 13,000 meals were made
available to community neighbors through
this tournament.
The Meals on Wheels program is a vital service
for many homebound individuals. For many, the
home-delivered meals they receive, coupled with
the personal interaction received with each
delivery, make the difference between being able
to remain living independently in their own home
or moving to an institutional healthcare facility.

5th Annual Taste for Hope
A Taste for Hope, VNA HealthCare’s annual wine
tasting event, was held at Cavey’s Restaurant in
Manchester. The sold-out event raised more than
$32,000 for the Hospice program.

On May 6th, 2010, more than 500 people gathered at the
Hartford Marriott Downtown to celebrate Connecticut's
unsung healthcare heroes: nurses.
Hosted by VNA HealthCare, the Eighth Annual Hartford
Regional Nightingale Awards for Excellence in Nursing Gala
honored 102 nurses from 30 local health care institutions.
The goals of the gala – Connecticut’s largest nursing awards
event – are to focus public attention and recognize the
breadth and scope of nursing practice at the local level,
to encourage nursing retention, and to inspire the next
generation of nurses. Since 2003, the Greater Hartford
Nightingale Gala has provided more than $46,000 in
scholarships to local high school and college students
who are interested in pursuing a career in nursing.
VNA HealthCare is proud to continue to host the
Nightingale Awards for Excellence in Nursing Gala and is
pleased that so many of our colleagues and regional health
care institutions have joined in support of this effort.

What role does home health
care play in the health
system of the future?
What is the biggest challenge
facing health systems
and home care?
JIM BLAZAR: Throughout our country’s health care
system, a major shift is taking place towards creating
incentives that reward patient outcomes and patient
satisfaction. Forward-thinking health systems are
building continuums of care to improve transitions
from one health care setting to the next, to enhance
communication between providers, and to ensure
that patients receive the right care in the right
setting to maximize outcomes.
ELLEN ROTHBERG: Certainly the impact of legislation
is critical to home care’s future. Health care reform
is needed, but must be undertaken with a full
understanding of the role and value for the components
of the health care system. For example, I continue to
advocate on behalf of home care in Washington and in
Hartford. The balance of Medicare and Medicaid is
delicate and together allows home health care to treat a
wide population of all those in need. Cuts in one impact
the other, and I strongly urge legislators to take that into
account as they evaluate healthcare reform proposals.

resources by achieving strong clinical outcomes, reducing
patients’ need for re-hospitalization and achieving high
levels of satisfaction in the process due to being treated
in the comfort of the home environment with highly
skilled, compassionate professionals.

along with emotional and spiritual care for both patients and their loved ones.
Meals on Wheels: 13.3% With the goal of ensuring health and independence, these dollars are
directed to supplying prepared meals to individuals who lack funding or resources for this service.
Nurturing Families: 0.4% Financial contributions support a landmark home visiting program that
helps first-time parents meet the challenges of parenthood, create a safe environment for their
child and learn positive parenting skills.
Subsidized Patient Care and Specialty Nursing Services: 78.6% Donations provide basic patient
needs through care to individuals who lack the financial resources or adequate insurance to cover
the full scope of care needed to meet their health care needs. These funds also enable us to support

VNA HealthCare has completed a major upgrade of its clinical application program to
a state-of-the-art electronic medical record system with the support of a $350,000 grant
from the Hartford Foundation for Public Giving, the community foundation for the 29-town
Greater Hartford region. This gift, combined with significant capital investment on the
part of VNA HealthCare, has enabled the Agency to implement a new Electronic Medical
Record (EMR) system that supports high-quality patient care decision making, medication
management, financial information and increased staff efficiency.
The new realities of complex medical care and health care funding challenges have made
EMR implementation a necessity. VNA HealthCare has also been able to complete the
purchase of laptop computer “tablets,” with wireless capabilities, so that all field staff
can input patient data while on patient visits. To make this possible, the Agency has
used Allscripts computer software to create an EMR system customized to meet the
needs of individual patients, medical professionals and funding sources throughout
the continuum of care.
To complete this EMR system, VNA HealthCare will install scanners and related software.
This technology enhancement will enable staff to scan hard copies of the necessary
documents and images into patient files in the EMR system.
The Agency is a pioneer in the full adoption of this EMR technology. At a recent
New England-wide Allscripts meeting, VNA HealthCare was the only home
health care agency to use this EMR system as intended in a fully paperless
mode. Already the new technology has enabled
the Agency to document high quality outcomes
that demonstrate how home health care
can enhance patient care and reduce
re-hospitalization rates.

the Hartford Foundation for Public Giving.

nonprofit organizations increase their use of technology
to better serve residents of Greater Hartford.“
Commenting on the impact of this technology grant,
Ellen Rothberg, VNA HealthCare President, said,
“We are fortunate to have the support of the Hartford
Foundation for Public Giving in an endeavor that is
a critical factor in our ability to meet the needs of
our patients. High quality patient care management
is only possible through the innovations inherent
in new electronic medical record technologies.”

VNA HealthCare is one of Connecticut’s largest providers of
comprehensive home health care, hospice and communitybased health and wellness services. VNA HealthCare is
dedicated to its founding mission to improve the health and
wellbeing of individuals throughout the community, from
the neighbor next door to the family across town.
The agency was founded in 1901 in Hartford and over time
merged with other local VNAs to become one of the largest
VNAs in the state, serving over 60 communities in the Greater
Hartford, Central Connecticut and Greater Waterbury regions.
The scope of services has broadened through a partnership
with the Hartford Healthcare system to create a continuum
of care between hospital and home that is essential to
community health.

way as to improve residents’ access to health care services,
enhance the health of the community, advance medical
or health knowledge or relieve/reduce the burden on

55-year-old Alex*, a highly respected and articulate
educator, was diagnosed with an aggressive form
of cancer. Deemed terminal by his doctor, Alex
wished to spend his final days at home with his
family. Private insurance covered the cost of his
hospice care, but refused to pay for a home care
aide to provide the personal care he could no
longer perform himself. The insurance company
justified their ruling, claiming that Alex’s family
was large enough to sufficiently care for him
on their own.
But Alex did not want to subject his children, all
young adults, to bathing him and tending to his
other personal care needs. He wanted to die with
dignity, he said, and didn’t want his condition
to alter his relationship with his children in his
final days.

to still be children...”
Using charitable funds, VNA HealthCare was able
to send a home health aide to Alex’s house daily
to provide the personal care he urgently needed
and assistance with housekeeping. With a support
system in place, Alex and his family were free
to focus on the most important thing: spending
quality time together.

My View

When any one of us encounters a need for nursing
and home care for a family member or close
friends, it becomes a tremendously lonely period
in one’s life. We all need to have some place to
turn to help us navigate through those difficult
times. And sooner or later, we will all face those
decisions, and we need to know which institutions
have both compassion and state-of-the-art care.
Fortunately, there are institutions like VNA
HealthCare that have the passion and expertise
to help us get through those hard times. That is
why I have chosen to financially support VNA
HealthCare and to volunteer as a board member.
As we face the challenges of health care reform
and rightsizing reimbursements and system
partnerships, I can think of no better institution
to support than VNA HealthCare.

Planned Giving
Gifts in Kind
Many gifts are cash contributions that are
made by check or credit card. These gifts
are tax deductible as allowed by law and
provide VNA HealthCare with immediate
funding for ongoing operations.
Gifts made through estate planning
provide for the future growth of the
institution. The John N. Lewis Society
honors those who have made provisions
for VNA HealthCare in their estate plans.
VNA HealthCare welcomes gifts in kind
including donations of goods and services
that meet the programmatic needs of
VNA HealthCare.
Securities and Property
To make a bequest gift to VNA HealthCare,
please use the following wording:
Gifts of stock, other securities or property
benefit the institution and provide the
donor with a tax deduction for the fair
market value of the gift when it is made.
Additionally, in most cases, the donor does
not have to pay capital gains tax on the
appreciated value of the stock or property.
“I give, devise and bequeath to
VNA HealthCare, Inc. the sum of
_____________ (or otherwise describe
the gift or specify a percentage of
the estate).”
Special and Restricted Gifts
VNA HealthCare has a number of funds
that have been established by donors for
special purposes such as programs or
client/patient needs.

Many companies provide their employees
with the benefit of increasing their gifts to
certain organizations by matching those gifts
either dollar for dollar or by a percentage.
