PURPOSE CRITERIA The Citizens of Mill Valley take this opportunity to honor those among us who, through their accomplishments in the Arts, have brought honor to our community. They have done this through their talents and actions in the state, national or international arenas. We wish to recognize their achievement in the Arts professions and their contributions to the creative life of the Mill Valley community. We honor those living artists and artist-teachers who were born in Mill Valley, or went to school here, or work here, or have made their home here, and who have enriched our lives. CATEGORIES We are accepting new nominations in: • Visual Arts & Design • Literary Arts • Musical Arts • Performing & Film Arts • Contributions to the Creative Life of the Mill Valley Community (Individuals whose vision, initiative and leadership played a major role in establishing and nurturing Mill Valley’s cultural treasures.) Selection of Mill Valley Creative Achievement Award winners (“Milleys”) is based upon one or more of these criteria: • Outstanding achievement in creating, performing or teaching in the Arts. • Demonstration of a significant body of work. • Service to the Arts community. RECIPIENTS Recipients will be selected from the listed awards categories. Formal presentation of up to five Milley Awards will be made at a gala event on Sunday, October 18, 2015. All award winners will be acknowledged by the Art Commission and the Mayor of Mill Valley. An appropriate permanent record will be kept by the Art Commission of all honorees. • Past nomination packets are available in the History Room of the Mill Valley Library to give you an idea of what needs to be included. • All materials must arrive at Mill Valley City Hall by 5 p.m. on April 17, 2015. You may drop off or mail materials to 26 Corte Madera Avenue, Mill Valley, CA 94941. NOMINATORS All nominators will receive notification of the Commission’s decision at the time of announcement of winners. Nominations from the community, due Friday, April 17, 2015, will be screened by the Creative Achievement Award Selection Committee to be sure they meet the criteria and are complete. All nominations will remain current for a total of three years; nominators may update and renominate their nominations annually.
