2013 ICAHN COMMEMORATIVE PROGRAM
CRITICAL
ACCESS HOSPITAL
Celebrating 10 YEARS
OF STRENGTH THROUGH COLLABORATION
From Vision to REALITY:
A Timeline of Strength through Collaboration
The Medicare Rural Hospital Flexibility Program was established by the U.S. Department of Health and Human Services
to strengthen and help sustain rural hospitals. Funding for the program was authorized by the Balanced Budget Act
of 1997 and states were required to develop and implement plans for the certification of the newly designated critical
access hospitals.
The Illinois Rural Health Plan was developed by the Illinois Department of Public Health’s Center for Rural Health.
The Illinois plan, approved in March 1999 by the Centers for Medicare and Medicaid Services, was the second plan
approved nationwide. The Illinois Department of Public Health also received the Medicare Rural Hospital Flexibility Grant
(Flex Grant), which was administered by the Center for Rural Health and was used to implement the new critical access
hospital program in Illinois.
The Illinois Rural Health Plan was prepared with the leadership and vision
of Mary Ring who was chief of the Center for Rural Health, and Barbara
Dallas, Director of Small and Rural Hospital Constituency at the Illinois
Hospital Association, with input from executives of the small rural hospitals
statewide. With acceptance of the state plan by CMS, the Center for Rural
Health assisted two rural hospitals – Thomas H. Boyd Memorial Hospital
and John and Mary E. Kirby Hospital – to become the first critical access
hospitals in Illinois. The Center then contracted with Pat Schou to become
statewide coordinator of the Flex Program.
The members of the ICAHN Board of Directors are proud of the history
of the network’s development and support of ICAHN members, and look
forward to the future as ICAHN celebrates 10 years of dedicated service
to our critical access hospital membership. We hope you enjoy this
commemorative walk through history and we thank you for your efforts
in ICAHN’s past and into its future.

We have been a member of ICAHN since its very
beginning. ICAHN has been instrumental in
guidance and education. With the use of grants,
we have been able to utilize the services for
education, IT services, and the purchase of
essential equipment needed to meet the
needs of current technology.

“
Thomas H. Boyd Memorial Hospital was completed in September of 1941, with the help of a memorial and community support. The
hospital’s 73-year history is one of change. The hospital started as a full service hospital with acute care, emergency room services,
obstetrics, and surgery and in 1970, a 40-bed nursing home was added. The hospital has continued to adapt to the changes in health care.
The obstetric unit was closed in the late 1970s, the surgery unit was closed in the late 1990s, and the nursing home was closed in the late
2000s. In the early 1990s, the hospital took over the management of Greene County’s only ambulance service and began to focus on
primary care with the start of the first of four Medicare-certified rural health clinics. All these changes led the administration of Thomas
H. Boyd Memorial Hospital to realize that becoming a critical access hospital was vital to its community, and in 1999 Thomas H. Boyd
Memorial Hospital was the first critical access hospital certified in Illinois. In 2003, because primary care, pre-hospital care, and acute care
were provided for Greene and the surrounding counties, the hospital board identified all entities as Boyd Healthcare Services. Today, Boyd
Healthcare Services continues to adapt and change with the health care needs of its service area residents.

Kirby Medical Center has benefited greatly from
ICAHN ... from the network’s leadership and
from the development of the ‘community’
relationship we feel with all our other critical
access hospital partners in Illinois. The networking,
grant opportunities, and best practices that
are shared among us are invaluable.
– Steven Tenhouse, CEO
The original John and Mary E. Kirby Hospital was located in one of the majestic
mansions of Monticello and opened for business in 1941. In the early 1970s, a new
Kirby Hospital was built on that same location and served the community for over 30
years. Following much planning, development, and construction, a new facility with
a new name – Kirby Medical Center – opened September 29, 2011. Kirby Medical
Center continues to provide primary care and surgery services but with advanced
medical technology, enhanced patient privacy, and adequate space for all departments and services in the new facility. Health care services have been expanded
to include a full complement of rehabilitative specialty care; improved efficiency
opportunities for staff in the delivery of care; food service for inpatients as well as
the public; and expanded space for life-saving
emergency medicine and convenient
care. The new location also
allows appropriate
space for future
expansion.

Hospital services in Galena began in 1962 with the opening of Northwest Illinois Community Hospital and its 31 short-term acute care beds.
In 1971, the hospital expanded to include a 34-bed skilled nursing care facility, and by 1978, the hospital was renamed Galena-Stauss
Hospital & Nursing Care Facility in recognition of the financial contribution from William Clifford Stauss, a Galena merchant who assisted with
the original organization of the hospital. Designated the first replacement critical access hospital in the state of Illinois in December of 2007,
the hospital name was changed to Midwest Medical Center and the long-term care facility was named Galena Stauss Senior Care Community. Today, Midwest Medical Center is a 25-bed critical access hospital featuring a state-of-the-art emergency department, surgical suites,
diagnostics and laboratory services, physical therapy, fitness center, a medical office building, Vista Café, and the Whispering Willow gift
shop.
Dr. John Warner
Hospital
Dr. John Warner arrived in DeWitt County in 1842 and practiced medicine until 1856.
A few months before his death on December 21, 1905, Dr. John Warner announced
he would erect a structure for hospital services and donate it to the community. His
will provided for the construction of the hospital. The building was completed in 1906
but stood idle for four years as there was no money for maintenance and furnishings.
The city of Clinton took ownership of the hospital in 1909 and remains city-owned
today. Public support for the hospital has been a longstanding part of its heritage.
The hospital currently has more than 150 employees, making it the third largest
employer in DeWitt County.

As a relatively new CEO of a critical access
hospital, ICAHN quickly became my ‘go-to’
resource for accurate and current information as
well as advice and support from my more
experienced peers. I cannot begin to put a value
on how ICAHN has helped me to grow and
develop in my role. It is the single best
resource that critical access
hospitals have in Illinois.

Local coal miners witnessed first-hand the numerous injuries to their colleagues and
knew the nearest hospital was miles away. They had a vision for local care that would
become Community Memorial Hospital. From its beginning in 1951, including a
significant addition to the structure in 1963, Community Memorial Hospital now offers
acute inpatient care, rehabilitation services, physical therapy, cardiac rehabilitation,
a 24/7 physician-staffed emergency room, and numerous specialty clinics. An
extensive renovation and addition will begin soon and will allow Community Memorial
to continue its mission of keeping health care close to home for many years to come.

provides helps us stay on top of key issues.
ICAHN also brings both federal and state issues
to our attention, which allows us to be proactive in

so it is a great value.

Hancock County faced an exciting set of circumstances in the post-World War II 1940s. Unparalleled optimism swept the war-weary United
States and that confidence was reflected in the attitudes and actions of west central Illinois residents. Efforts were launched in 1945 to
advance plans for a new hospital and by 1949, enough money was raised to open the doors of Memorial Hospital, so named for the local
heroes who brought the country to victory. Today, nearly six decades later, Memorial Hospital has written another chapter of service to the
Hancock County community, which celebrated the grand opening of a new facility in July 2009. The new Memorial Hospital includes 21st
century technology so critical to patient care while reflecting the values and dedication to community that helped the original hospital open
its doors in 1949. Services available at Memorial Hospital include a 24/7 emergency room with over 4,000 visits per year, surgical
procedures, medical imaging technology, OB deliveries, and several community clinics.

Throughout my 44 years in health care, I’ve
never been associated with an organization quite
like ICAHN. As a member hospital, ICAHN
provides us multiple learning opportunities by
bringing together leadership, resources, and
expertise in the health care field. This helps to
strengthen the services each of us provides to
our communities. Personally and professionally,
I am very thankful for the relationship ICAHN
has with Pinckneyville Community Hospital.
– Tom Hudgins, CEO
Like many rural hospitals, Pinckneyville Community Hospital’s roots go back to a
physician-owned facility that served its community for many years. Over time, the
ownership transitioned to a community hospital and subsequently to a public facility
now known as Pinckneyville Community Hospital District. The hospital offers an
acute care unit, emergency services, a family medical center, imaging department,
laboratory services, an oncology clinic, pharmacy, a respiratory therapy and
cardio-pulmonary department, specialty clinic services, and a full range of outpatient
and inpatient physical therapy services. Presently, Pinckneyville Community Hospital
is planning a new facility that will serve its community both as a source of health care
services and as an employer for decades to come.

Washington County Hospital was established in 1962 as a district hospital, largely due to a bequest from a local family that challenged the
community to obtain donated land and to construct a building within 24 months. Obviously the residents of Washington County met the
challenge, and this year Washington County Hospital is celebrating 50 years of service to the county’s 15,000 residents. Washington County
Hospital has 22 acute/swing beds and 28 private pay long-term care beds. There is also a provider-based rural health clinic on campus that
employs three full-time physicians and two midlevel health professionals.

Our relationship with ICAHN has been very
beneficial throughout the years. Pat Schou was
integral in assisting hospital leadership through the
process to become a critical access hospital. She
prepared us well for the initial IDPH survey. The
information shared through email, conference calls,
conferences, and the group Listservs has been
invaluable to our success and survival. Through the
SHIP and other CAH grants, we have increased
both staff and community education, improved our
quality reporting, patient satisfaction and
information technology resources.
– Anna Laible, Administrator
Eureka Hospital began in 1901 in a large two-story home with a wrap-around front
porch. Dr. J.T. Wyatt’s family lived in the residence where he also had an office and
infirmary. It is the first and only hospital in Woodford County. A new building was
completed in 1960 at a cost of $190,000. Following several expansions and modernizations, Eureka Hospital became a not-for-profit charitable organization in 1978
when it was acquired by the Mennonite Hospital Foundation. In 1985, the hospital
became part of BroMenn Healthcare when Mennonite Hospital and Brokaw Hospital
merged, and in 2001, Eureka Community Hospital received its federal designation
as a critical access hospital. By 2010, ECH became part of faith-based
Advocate Health Care, the largest fully integrated health
care delivery system in Illinois, and officially
changed its name to

the families we serve within our community. ICAHN

– Lynn Klein, CEO
“
Mendota Community Hospital opened its doors at 1315 Memorial Drive on June 10, 1951. The hospital was named as a memorial to all
who made the supreme sacrifice in World Wars I and II. A bronze plaque located at the entrance to the building simply states, “A memorial
to the heroes of our community we must not forget.” The laying of the cornerstone occurred on October 2, 1949. Residents, former
residents, business people, and corporations of Mendota and the surrounding communities donated all the funds required to build and
equip the original hospital. The hospital was, in every sense of the word, a community project. On December 7, 2011, MCH relocated to its
new state-of-the-art facility at 1401 East 12th Street in Mendota. Today, Mendota Community Hospital is one of the largest employers in the
community, with more than 350 employees on staff. Directed by a volunteer board, the critical access hospital is an acute care not-for-profit
corporation.

Fairfield Memorial Hospital served the health care needs of the surrounding area
since 1950. In 1998, Fairfield Memorial Hospital completed a $4.1 million expansion
project that included a new surgical and emergency department and expansion of
the laboratory and imaging departments. Additionally, the entire second floor was
converted to all-private rooms. In August 2008, FMH opened Horizon Healthcare, a
certified hospital-based rural health clinic, in a temporary building until the construction
of the new FMH Medical Arts Complex was complete. In May 2011, FMH opened
the Medical Arts Complex, a 25,468 square foot facility, which includes Horizon
Healthcare, specialist suites, board room, and community/education rooms. Today,
Fairfield Memorial Hospital is a fully accredited, not-for-profit critical

Pike County residents raised $60,000 to help build Illini Community Hospital in 1942 and again gave thousands of dollars to keep it open in
1972. Multiple renovations, including the addition of a critical/coronary unit, heliport for emergency air transportation service, laboratory and
surgery departments, operation of a 24/7 emergency room, and coordination of the ambulance service ensued. Illini was only one of three
hospitals in or around Illinois performing cholecystectomy by laparascope in 1990. Illini Community Hospital Rehabilitation Center opened
in 1994, and new telemetry monitoring equipment for critical care was added in 1998. Blessing Corporate Services began overseeing
operation of Illini Community Hospital in May 2000. Blessing Corporate Services purchased Blessing Care Corporation d/b/a/ Illini

in 1995, the same year Country Terrace – a 30-apartment senior independent living
community – was opened. In 2000, the hospital opened its first family practice clinic
and since has opened four more in adjacent communities. In 2010, an official affiliation agreement was signed with Carle Foundation Hospital giving patients access

within that first year, the hospital employed 32 people, which is a skeleton crew compared to the over 500 people employed by GAHHS
today. After years of expansion and growth, GAHHS now has seven family health clinics in its surrounding communities and is in the process
of completing a new state of-the-art obstetrics unit, 60 percent larger than the current OB unit. With the increased size, the new OB unit will
have five private labor, delivery, recovery, and postpartum rooms with top-of-the-line amenities. This expansion will also increase the size of

Franklin Hospital District opened its doors in April 1955 and provided a medical and surgical unit, intensive care unit, obstetrics, and an
emergency department. In Summer of 2002, the hospital was reorganized by its board of directors. With closure imminent in May 2002
the board, along with local elected officials and the community, came together to keep the doors open in the face of mounting debt.
Through the generosity of county residents, over $40,000 was raised and used as interim operating cash, while the United States
Department of Agriculture processed a $4 million loan request. Inherent in the loan request was the requirement that the hospital attain
critical access hospital status. This designation, as well as support and services provided by ICAHN, were necessary for Franklin Hospital
to be able to continue providing health care services in its rural communities. Today, Franklin Hospital offers inpatient medical-surgical
services, an emergency department, and a wide array of outpatient clinical services. Additionally, plans for remodeling are currently being
evaluated that further enhance the quality of service to the residents of Franklin County.

Ferrell Hospital was founded in 1925 by Dr. J.V. Ferrell and was located on the second floor of a downtown Eldorado business building.
In 1928, a one-story structure was added to the former hospital with a 12-bed capacity and five employees. It remained a one-physician
hospital until 1937 when the late Dr. Robert V. Ferrell (son of the founder) joined his father in practice. The first addition was completed
in 1941 where the second story was added, increasing capacity of the facility to 23 beds. A north wing was added in 1948, and the final
addition to the old facility was completed in 1958. On February 1, 1975, the new facility was open and ready for occupancy. The hospital
remained open and went through much growth and many changes over the years. In the Summer of 2012, a rural health clinic, Ferrell
Hospital Family Practice, opened on the hospital grounds. There are now two physicians and five midlevel providers that provide care
at the rural health clinic. Ferrell Hospital continues to serve as a major economic factor in its community as one of its largest employers,

1919. Kewanee Hospital continued to grow and expand as health care changed.
In 1922, a training school for nurses was opened, and in the late 1950s, the hospital
further expanded to include physical therapy, a new pharmacy, and a school of x-ray

Morrison Community Hospital. In 1974, the hospital embarked on a $3 million expansion that increased capacity to 84 beds. Currently,
there are 60 beds, of which 25 are a mix of acute care/skilled nursing and 35 are long-term care. Today, Morrison Community Hospital

can always count on ICAHN support.
– Peggy Sebastian, President/CEO
St. Joseph’s Hospital was established in January 1878 in Highland and the first
official hospital facility open to the public on August 21, 1879. Tragically, fire
destroyed St. Joseph’s Hospital on January 24,1892. New additions and renovations
were completed in 1897, 1926, and 1937, but in June 1950, the cornerstone for the
four-story facility addition was laid and construction completed. The next chapter in
the history of St. Joseph’s Hospital is written on a site approximately one mile north
of the previous facility located on Main Street. On January 7, 2011, St. Joseph’s
Hospital announced that it purchased 60 acres located in the southeast corner of
the intersection at North Poplar Street and Troxler Avenue. The land owner, Kathryn
Winet Grotefendt, also agreed to donate 12.4 additional acres. This combined 72.4
acre site is where the new St. Joseph’s Hospital campus stands. Construction on
the new St. Joseph’s Hospital began April 1, 2012, on a green-field site about a mile
north of the previous hospital and was substantially
completed May 31, 2013. The new
facility officially opened to
the public August 22.

Enduring a tumultuous start, Princeton’s first hospital opened in 1903, only to close its doors after one year. Reopening in 1905, it
again closed in 1910 after a small pox epidemic. Despite these setbacks, Julia Rackley Perry donated $25,000 to reopen the
Princeton hospital as a municipal hospital in 1913. Named Perry Memorial Hospital in her honor in 1920, the hospital offered 28 beds
and five bassinets. Perry Memorial Hospital became a critical access hospital in 2004 and underwent three major renovation phases
in 2009 to create the modern facility that stands at 530 Park Avenue East in Princeton today. In 2013, Perry Memorial Hospital
continues to offer a proud tradition of high quality care and the continuation of excellent customer service.

– Kevin Poorten, KishHealth System CEO
The hospital’s history began in the early 1900s when a donation of property and
money by a local physician began Woodward Hospital. That building was used
until mid-century when Sandwich Community Hospital, at the corner of Main Street
and Pleasant Avenue, was constructed and opened in April 1960. That hospital still
stands and changed names, becoming Valley West Hospital when it joined the KishHealth System in 1998. An $11.2 million year-long building project ended in Fall 2013
and a new patient wing opened. The new space supports key strategic initiatives of
the health system, such as patient safety and comfort, care delivery improvement,
and facility and campus development. The improvements also enhance the health
system’s ability to continue to attract new physicians,
specialists, and other clinical staff.

Pana Community Hospital was the vision of Dr. Jacob Huber, a physician in Pana in the late 1800s. Dr. Huber’s will provided money
from his estate to be used to build a modern hospital in the city of Pana, originally known as Huber Memorial Hospital. After his death, a
Catholic order of nuns was contacted to own and operate the hospital. The hospital construction was completed and the building readied
for occupancy on May 10, 1914. The Sisters of Misericorde of Montreal, Quebec, Canada, owned and operated the hospital until January
20, 1966. With great surprise to the residents of Pana, the Sisters announced that they would be selling the six-acre hospital complex.
This immediately set the community into action to attempt a purchase of the facility. Through an entire community effort, the hospital was
purchased and documents were signed on June 20, 1967. The community had worked together and successfully raised over $500,000
to support this purchase. The hospital was then renamed and since has been known as Pana Community Hospital. In 2014, the Pana
Community Hospital will celebrate 100 years of providing quality health care to its community.

The first hospital in Vandalia was called Mark Greer Hospital and was opened in 1925. Despite an addition in 1936, Mark Greer Hospital
was deemed to be too small to serve the community of Fayette County by 1949. The hospital district board planned and built Fayette
County Hospital between the years of 1949-1955. Fayette County Hospital was named “Outstanding Hospital of the Year” by the American
Hospital Association in 1955 for design, beauty, and services. It was the first totally air-conditioned hospital in the United States. In 1987,
Fayette County Hospital and Long-Term Care began its affiliation with Christian Health Services in St. Louis, Mo. At this time, work began
on a master site plan. A plan was developed and implemented including site development, ER, OR, and radiology. Later, they merged to
form BJC Health System and is now called BJC Health Care. Site development included an emergency room addition and progressive rehab
renovation in 1997, a surgery unit addition in 1998, and a radiology renovation in 1999. On January 1, 2005, Fayette County became
affiliated with Heartland Health System and Alliant Management Services.

surgery suites to the emergency room, radiology to laboratory, and physical therapy
to specialty clinics. Currently, Salem Township Hospital is embarking on a multi-phase

moved to its new replacement facility at 20733 North Broad Street on July 31, 2010. The new facility offers 25 private patient rooms,
centrally located nursing support stations, and highly inter-related departments adjacent to one another. The new site also offers space

Regional Hospital
St. Clement’s Hospital was built in 1900 as a stop-over for Sisters (nuns) who arrived
in Red Bud by train, en route to the Motherhouse in Ruma. The building was also
used for Sisters to recuperate from illness. Local doctors realized the potential of
the building and requested permission to use it as a hospital. The first patient was
admitted in August 1900. By 1965, additional health care facilities were needed in
Red Bud and a new hospital was built in 1971. In 1979, a nursing home was built
next to the hospital along with a tunnel connecting the two facilities. In 2001, the
hospital changed ownership and was renamed to Red Bud Regional Hospital.
Today, it operates as a 25 bed critical access hospital with a 115-bed nursing home

The roots of Sparta Community Hospital can be traced back to the 1930s when J.C. Sutherland, M.D., expanded his local medical
practice into a 15-bed hospital in a large, two-story brick house. In 1953, Sutherland Hospital was purchased from Dr. Sutherland
by the newly formed Sparta Community Hospital District. In 1955, the first patients were admitted to Sparta Community
Hospital after an extensive remodeling effort and in 1971, a Medical Arts Building was built directly behind the hospital. In 2005,
a 16,500 square foot addition to house surgery, the emergency department, laboratory, Auxiliary gift shop, and customer service was
constructed. Although much has changed through the years, one thing has remained constant for this community hospital, and that

Energy
Supply & Management
ICAHN
Preferred
Partner
Welcome to Midwest Energy - an
Preferred Business Partner
Midwest Energy is Illinois’ premier energy supply management resource for healthcare energy
buyers. Midwest Energy manages energy throughout Illinois and the U.S. for more than 600
industrial, commercial and institutional sectors covering more than 2,300 locations.
Today, energy supply costs can be controlled and managed by leveraging the right resources and
expertise. This is the primary reason a growing number of ICAHN hospitals turn to Midwest Energy
for their energy management needs.
Since 2006, Midwest Energy has worked with 20 ICAHN hospitals to help them save from 5-20%
compared to their regulated electric and natural gas utilities. Our 98% customer retention rate
shows our commitment to excellence and superior service levels.
Energy Consulting
Our unbiased and objective analysis clearly illustrates the financial and contractual differences
between retail suppliers and their products. This enables your hospital to make a highly informed
and educated decision on which supplier and supply option best fits your requirements. Our energy
procurement consultants are experts at plan implementation. Their experience and knowledge
enable our clients to reduce energy costs and improve their returns on energy investments.
Natural Gas
Having been involved in Natural Gas deregulation in Illinois since its inception in the late 1980s,
Midwest Energy professionals have extensive experience managing and customizing natural gas
solutions for all industries and unique business requirements.
Electricity
In Illinois, the electric utility market has been deregulated since 1999. This means your hospital has
the choice to buy lower cost energy from an alternative supplier. At the same time, you continue to
receive the same dependable service from their local utility, resulting in the best overall value for
your operations.
