a team who knows how to make every moment count.
nurses are dedicated to providing the best skilled nursing care for our
patients. They manage the care of each patient and at Our Hospice we call them
Case Managers because they report changes to the medical staff as needed.
They are experts in pain management and symptom control. Our nurses are all
Registered Nurses (RN’s) and are on call 24 hours per day, 7 days per week to
support patients wherever they reside. In addition to direct patient care, they
offer assistance and education to family members and caregivers about how to
meet the patient’s needs, as well as coordinate all medications and medical
supplies as needed.

Our expert Medical Care Team of physicians and
nurse practitioners oversees the medical care of the
patient, provides expertise in pain and symptom
control and works closely with the hospice team.
Physicians support the 24 hour service and meet
weekly with the team to discuss each patients’
progress. Patients may select to remain with their
primary physician or select a hospice physician as
primary.
The care team meets weekly to discuss patients and the plans of care.
Our Home Health Aides (HHA) assist with personal care and other activities of
daily living. This support is available according to the patient’s needs. They work
under the direction of the Nurse Case Managers. For individuals residing in
extended care facilities, hospice home health aides will supplement the care
provided by the nursing home staff.

Our masters-level Social Workers support the social and emotional needs of
the patient and the family. They help coordinate community resources as well
as provide personal counseling when needed.
Our Chaplains provide spiritual care and support for patients and family
members. They meet individually with families and have a strong network of
support in the community if the families wish to be connected to a particular
faith network.

Our Bereavement staff provide counseling support by talking with patients
or their family about concerns and understanding grief. Families receive
bereavement support for 13 months following the loss of their loved one.

Speech, physical and occupational therapy is available to patients when
needed, as well as a dietician and volunteer pet therapy teams.

Friends of Our Hospice Advisory Group
Volunteers are people who have a special interest in helping
and providing support in a variety of ways, ranging from
running errands, offering companionship or providing brief
caregiver relief. In addition to the volunteers who work
directly with patients, other volunteers help with office tasks
and assist with fundraising events.
Our Veteran volunteers support us through our “Vet-to-Vet” patient visit
volunteers and our Veteran volunteers who lead our pinning ceremonies to
honor the military service of our patients.

“Every
step is an adventure,” is how Jim explains what it’s like in the final chapter of his
life. Jim became an Our Hospice patient in the summer of 2014 and can’t say enough good
things about his hospice experience. He enjoys the care and attention given to him by his
care team and says, “They keep an eye on me; I’m not worried a bit.” He said the team
has been so helpful, he’s been amazed. “They let me do what I can do; we have a good
routine worked out,” he continued. “I’m so glad Our Hospice was there for me, or I’d be
in a nursing home, because I don’t want to be a burden on my family.”
Jim (94) is at his home and is cared for by his children, supported by the hospice team. He
enjoys spending time with his family and seeing the natural views of his wooded property
from his picture window. Jim’s advice to others facing a life-limiting illness is, “Have no
fear; if you need help that’s the proper way to go, it beats anything I’ve ever seen.”

Hospice care provides an increased benefit to persons
with a life-limiting condition. The goal of hospice care is
to improve the quality of their life for as long as we
can. Families often tell us they wish they had come to
hospice sooner. In 2014, our patients were cared for
by Our Hospice staff for an average of 67 days.
We care for patients in 15 South Central
Indiana counties from four locations. Our
patient population is largest in Bartholomew
County where we were founded; however,
over the past few years we have seen
significant growth in our other 14 counties. In
2014 we cared for 450 patients as compared
to 2013 when we cared for 488.
People come to hospice when they have decided not
to seek further curative treatment. While anyone can
request an evaluation to determine a patient’s
eligibility, there is a need for a referral as a part of the
process. In 2014, 88% of our referrals came from
physicians, hospitals and nursing homes.
In the early years, cancer was the primary diagnosis for
patients electing hospice care. Today, we still care for
many cancer patients, but have seen an increase in
patients experiencing heart disease and Alzheimer's.

The largest percent of our budget is due to operating
expenses or costs associated with our staff. In 2014,
our uncompensated care (costs not covered and
patients with no ability to pay) increased to almost
$200,000.
Hospice is a Medicare benefit available to anyone who is
eligible. Over 90% of our reimbursement comes from
Medicare, Medicaid or private insurance. This is not enough
to cover all our patient expenses, so we rely on donations,
memorials and fundraising to fill the gap and allow us to
provide excellent patient care to anyone who needs us.
