The essence of PDF’s work over this period may be encapsulated into the following theme: People Advancing Science/ Science Informing People. In these phrases rests the heart of PDF’s strategy: linking our dreams for tomorrow with the necessary tasks of today. Let us explain. People Advancing Science. In this dimension of our work, PDF harnesses the energies of committed doctors, gifted scientists and generous donors to push the science forward in the quest for new Parkinson’s treatments and — ultimately — a cure for the disease. This commitment has been at the core of PDF’s research strategy since the organization was created more than a half-century ago. This research — investigator-initiated and rigorously reviewed by peers and people with Parkinson’s — takes many forms. One could be the individual scientist in her laboratory, studying potential disease-modifying treatments for Parkinson’s. A second could be the work of a diverse panel of experts exploring the debilitating impulse control disorders that affect so many people with Parkinson’s. A third could be the long-term collaborations of a university-based research team exploring the role of genetics in Parkinson’s disease. In addition to the communities of doctors, scientists and donors who have traditionally led this work, PDF includes a fourth community: lay advocates in the scientific process, trained and deployed by PDF though in-person trainings and an online course. Through this program, known as Parkinson’s Advocates in Research (PAIR), people touched by Parkinson’s can join our network of more than 230 Research Advocates from 42 states who are working to bring about better treatments at a faster pace. These individuals are taking on a variety of challenges, including recruiting individuals for drug trials, providing advice to industry and federal regulatory agencies, and educating support groups. 3 | Letter from Leadership Letter from Leadership Science Informing People. As much a philosophical precept as it is a program strategy, placing information and education on the firm basis of science is a core principle at PDF. Whatever the medium — our publications and newsletters, our popular website, our online educational seminars, or our online programs of professional education for nurses and physical therapists — PDF serves as an entrepot of relevant, timely and authoritative knowledge and information on Parkinson’s. Our goal is simple: to identify the questions that are uppermost on people’s minds and then to give them answers they can rely on, based on the best science available. Many of you who read this report will be the people who already support PDF’s work financially. It is your generosity that powers our work — on the one hand, advancing, through research of the highest quality, our understanding of Parkinson’s disease, and on the other, through science-based education, improving the lives of people who are touched by the disease. Our comprehensive strategy is focused on three results: solving the mystery of its causes, developing new treatments for those who live with the disease and ending Parkinson’s disease forever. 5 | People Advancing Science People Advancing Science David Sulzer, Ph.D.: Finding Potential Targets for Preventing or Treating Parkinson’s Although the vast majority of cases of Parkinson’s disease (PD) have no known genetic cause, among those that do run in families, the most common mutations occur in the LRRK2 gene (leucinerich repeat kinase-2). Now, researchers led by Ana Maria Cuervo, M.D., Ph.D., at Albert Einstein College of Medicine and David Sulzer, Ph.D., at PDF’s Research Center at Columbia University Medical Center, funded in part by PDF, have shown for the first time, why the faulty LRRK2 protein is harmful, and how it can block the normal cell recycling process. This research was published in the March 3, 2013, online edition of Nature Neuroscience and builds upon earlier work also funded by PDF. Normal cells, including those in the brain, use a recycling and waste disposal system to stay healthy. The process — known as chaperone-mediated autophagy — begins when molecules called chaperones patrol for damaged proteins and then transport them to a recycling center (the lysosome). After arriving there, the damaged proteins travel to the interior region of the lysosome, where they are broken down into components that can be reused. Drs. Cuervo and Sulzer showed that chaperone-mediated autophagy was capable of recycling healthy LRRK2 proteins, but it was not as successful in breaking down those LRRK2 proteins that had the mutation found in people with Parkinson’s disease. In addition, these faulty LRRK2 proteins then prevented other proteins, such as alpha-synuclein (a protein that causes PD), from getting broken down and recycled. The studies of Drs. Cuervo and Sulzer over the years have spotlighted the need for developing therapies that can help restart or speed up cellular recycling — therapies that could potentially benefit people whose PD has no known genetic cause as well as those who have LRRK2 mutations. Daniel Segal, Ph.D.: Learning from Other Diseases to Find New Treatments In Parkinson’s disease, the protein that clumps together and causes the disease is called alphasynuclein. If scientists could stop alpha-synuclein from clumping in the brain, this could potentially provide a new treatment for the disease. With funding from PDF, Daniel Segal, Ph.D., of Tel Aviv University, has developed a new molecule, based on quinone, a compound that has been shown to inhibit certain cellular pathways, that may be able to do this. His molecule is small enough to fit into the brain, and is similar to another molecule shown to stop clumping in the protein that causes Alzheimer’s disease. Dr. Segal found that in a petri dish, this molecule was able to effectively prevent alpha-synuclein from clumping together, and that higher doses were more effective than lower doses. Now he is testing the molecule in animal models. His early tests in fruit flies found that treatment with this molecule resulted in improved motility (measured by climbing behavior) in PD fly models containing excess alphasynuclein in their brains. He now will extend his testing to PD mice models. If he finds this molecule to be successful, this could mean a promising new therapy for Parkinson’s disease. Of course, clinical testing would have to be done in humans to ensure that the molecule is safe and works the same way in humans as it does in flies and mice. Impact: Biennial Report 2012-2013 | 6 People Advancing Science Sheng-Han Kuo, M.D.: Using Genetics to Find New Treatments Genes, in ways still unknown, contribute significantly to the risk of Parkinson’s disease. Understanding the genetics of those who live with Parkinson’s disease may hold the key to finding a cure. To further our understanding of genetics, PDF established in 2012 the Lucien Côté Early Investigator Award in Clinical Genetics, which supports postdoctoral researchers and junior faculty in the New York City metropolitan area who are studying the genetics of Parkinson’s. One of the first recipients of this award was Sheng-Han Kuo, M.D., of Columbia University Medical Center, who is using induced pluripotent stem cells (iPS cells) to study how changes, or mutations, to certain genes may cause PD. iPS cells are skin cells from people with Parkinson’s disease that have been chemically induced to develop into dopamine neurons, which are the cells that are lost in the brains of people with PD. Dr. Kuo is studying dopamine neurons derived from skin cells that have a specific change to a gene called GBA (the full name is glucocerebrosidase). He wants to determine how changes to this gene affect normal function in the brain, especially how they affect the clumping of alphasynuclein (a protein that causes PD). Past studies in rats have shown that changes to the GBA gene do cause clumping of alpha-synuclein in the brain. Now the goal is to see if this clumping also occurs in human brain cells containing the GBA mutation, and the mechanisms by which this happens. This knowledge may potentially lead to the development of new PD treatments. Since 2000, PDF has provided over $69 million to more than 400 scientists as part of a comprehensive and multifaceted approach that supports both research and training. 7 | People Advancing Science “THE PARKINSON’S DISEASE FOUNDATION HAS PROVIDED ME WITH CRITICAL SUPPORT TO CONTINUE MY PARKINSON’S RESEARCH EARLY PHASE IN THE OF MY ” CAREER DEVELOPMENT. Sheng-Han Kuo, M.D. Columbia University Medical Center Antonio Strafella, M.D., Ph.D., and Mark Stacy, M.D.: Understanding Impulse Control Disorders in Parkinson’s Disease The PDF Conference Award program was launched in 2012 to support gatherings of experts in the field to address emerging clinical or basic science questions about Parkinson’s. Two of the initial recipients were Antonio Strafella, M.D., Ph.D., of Toronto Western Hospital and Mark Stacy, M.D., of Duke University. Together, they used the PDF award in conjunction with other funding to gather clinical and preclinical experts from various research backgrounds to develop a strategic plan for studying the debilitating impulse control disorders (ICDs) that affect so many people who have Parkinson’s. ICDs can be a side effect of dopamine agonist therapy and include binge eating, compulsive gambling or shopping and increased sexual behavior. The conference, “Impulse Control Disorders in Parkinson’s Disease,” was held in October 2012 and featured experts from fields including neurology, psychiatry, neurobiology, pharmacology, addiction, neurologic imaging and epidemiology. This multidisciplinary meeting addressed the spectrum of symptoms associated with ICDs, and generated new ideas for clinical, experimental and translational approaches to better understand ICDs. It also facilitated interaction and collaboration among experts from diverse fields, and developed a network of interested scientists and clinicians to further study the disorder. The proceedings of the conference will be published in Movement Disorders. Impact: Biennial Report 2012-2013 | 8 People Advancing Science Benjamin George: Measuring the Benefit of “Virtual” Office Visits Could we be returning to the tradition of seeing our doctors at home? This is the concept of telehealth. But how does it compare to conventional in-person medical care? Investigators led by E. Ray Dorsey, M.D., M.B.A., at Johns Hopkins Medicine, and including PDF Summer Fellow Benjamin George, recently found that people with Parkinson’s may benefit from a consultation with a specialist via online video conferencing from home, providing an alternative to traveling to an in-person appointment. These findings appeared in a study published on March 11, 2013, in JAMA Neurology. This study is one of the first to investigate whether telehealth appointments are feasible and beneficial to people with Parkinson’s disease. Researchers measured the participants’ quality of life and PD symptoms, and found the same results for individuals, regardless of whether they saw a doctor in-person or via video conference. In addition, compared with in-person visits, each "virtual" visit saved participants, on average, 100 miles of travel and three hours of time. Although the number of participants in this study was small, the results provide encouragement that video conferencing with a specialist can provide similar benefits to people with PD as in-person visits, while reducing the time and expense of seeing the doctor. However, before virtual visits become a reality, researchers must conduct studies with larger numbers of participants, and with participants of more varied socioeconomic backgrounds. In addition, issues surrounding technology and insurance reimbursement and rules in the United States that require doctors and the person being treated to be in the same state, will need to be resolved. 9 | People Advancing Science “THANK YOU FOR PROVIDING THE FUNDING TO ALLOW ME TO CONTINUE TO CONDUCT RESEARCH TO FURTHER UNDERSTAND AND IMPROVE ACCESS TO DIVIDUALS CARE WITH FOR IN- PARKIN- ” SON’S DISEASE. Benjamin George Johns Hopkins Medicine PDF Research Advocates: PAIRing Up to Advance Parkinson’s Research PDF believes that better treatments can be found at a faster pace when people with Parkinson’s are primary partners in research alongside scientists, industry and government. Since its founding in 2008, PDF’s Parkinson’s Advocates in Research (PAIR) program is making this partnership a reality. Here are some of the ways in which these 230 volunteer advocates from 42 states are actively changing how research is done: • Rich Hoffman, Pharm.D.: Speeding Drug Development "As a Consumer Representative with the US Food and Drug Administration, I represent the urgency of people with Parkinson’s, while helping to ensure that potential new drugs are effective and safe." • Steve DeWitte: Increasing Trial Participation "When I ask people with Parkinson’s why they are not involved in research studies, many reply, ‘Because I have no transportation.’ My solution was launching the Clinical Trial Transportation Program (CTTP). By partnering with Beth Israel Deaconess Medical Center in Boston, MA, and Columbia University Medical Center in New York, NY, CTTP has helped 37 volunteers to participate in studies by driving them to these two medical centers.” • Jane Denmark: Informing the PD Community “This is a hard disease. Subjects for the one hour online seminars are selected based on surveys of people with Parkinson’s, and the faculty are experts from around the US. More than 15,000 individuals from 43 countries participated in at least one PD ExpertBriefing. Elise C., Ramsey, NJ Impact: Biennial Report 2012-2013 | 12 Science Informing People Improving Resources for NonEnglish Speaking Communities As the number of non-English speaking individuals increases in the United States, the need for resources for these communities continues to grow. In fact, by 2050, the Hispanic share of the elderly population is expected to almost triple to 17 percent from six percent in 2005. Since age remains the biggest risk factor for Parkinson’s, the prevalence of the disease in this community is expected to increase. PDF is committed to ensuring that all people — regardless of who they are, where they live, what language they speak or how much money they have — receive needed Parkinson’s information and resources. In 2012, PDF expanded our HelpLine to include interpreters who are available to speak with callers in Spanish and more than 200 other languages. Our HelpLine is staffed by a team of information specialists who can answer questions about Parkinson’s treatment and care, identify legal and financial resources and provide referrals to doctors and support groups where available. PDF also expanded our line of educational materials available in Spanish to include fact sheets on topics such as deep brain stimulation (DBS) as a treatment of Parkinson’s, care partner concerns, the role of hospice in Parkinson’s care, managing Parkinson’s medications and understanding pain in Parkinson’s. In 2012 and 2013, more than 5,000 individuals requested information in Spanish. Since 2007, PDF's HelpLine has responded to inquiries from almost 50,000 individuals around the world as part of its commitment to ensuring that everyone has access to Parkinson's information and resources. 13 | Science Informing People “I AM EVER SO APPRECIA- TIVE OF YOUR THOROUGH RESPONSE. Sylvia G., Yuma, WA Educating Physical Therapists About Parkinson’s While no treatment can prevent or reverse Parkinson's, research suggests that physical therapy can help improve strength, fitness level, walking speed and balance, leading to reduced disability and greater independence. Unfortunately, there is a lack of trained physical therapists to care for the growing Parkinson’s community. To address this need, PDF launched in April 2012 a free online course, “Parkinson’s Disease: A Practical Approach to Evaluation and Treatment for the Physical Therapist.” The course was designed by expert physical therapists in the field of Parkinson’s to help other physical therapists provide better care to people with the disease. PDF led the development of this course in collaboration with the American Parkinson Disease Association (APDA), the American Physical Therapy Association (APTA) and the Visiting Nurse Service of New York (VNSNY). The course includes ten educational sessions and two interactive panel discussions. Since its launch, more than 6,000 physical therapists have participated in this program. This program was made possible with the support of Kenneth M. and Susan Granat Weil in memory of John and Joan Haine; the Edmond J. Safra Philanthropic Foundation and the American Physical Therapy Association. It also received kind support from Rusk Rehabilitation at NYU Langone Medical Center. Karen Smith Peggy Willocks Page Morton Black: 1916-2013 In 2013, the Parkinson's Disease Foundation (PDF) lost a great, muchloved, deeply admired ambassador for all those who live with Parkinson’s disease — our Chairman Emeritus Page Morton Black. It was William Black, Mrs. Black’s late husband and founder of the Chock full o’Nuts coffee and restaurant chain, who created PDF in 1957. Mrs. Black served as Secretary of PDF until assuming the role of Chairman after her husband’s death in 1983. She resigned from the Board in 2012, serving as Chairman Emeritus until her death. In 2005, PDF presented Mrs. Black with its Lifetime Achievement Award.
