Objective Based Evaluation:
A. Performance based subjects were selected from the NFPA 472 standard and IFSI SWMD Hazmat classes
B. Each subject area will be evaluated on a1-10 point system. With 1being the lowest score and KbeS a
Benchmark" performance. The rating system is listed below
g
C. The total score from all the objectives shall be tallied and each team shall be evaluated according to their
Rating System: Numerical Grading 1 thru 10
Means unsafe and/or unsatisfactory practice, process, decision made, activity, event and/or
tactic that requires or required immediate corrective action(s) for adesirable outcome Alack
of knowledge of the task at hand.
Means practice, process, decision made, activity, event, and/or tactic that was condurted~o7~
earned out in such amanner that its outcome with regard to expectation was marginal When
the safety officer must frequently make safety reminders, especially about PPE product
identification, entry procedures, or decontamination this becomes less than marginal In these
cases, the evaluator should be able to identify behaviors that could be improved and
document example of the expected performance improvement
Means practice, process, decision made, activity, event, and/or tactic which was conducted o7
earned out in such amanner that its outcome met or exceeded the requirement, expectation '
and/or objective in an outstanding, excellent manner.
10
Me*nsthat.in the evaluatcr's opinion, the practice, process, decision made, activiiTe^nT
and/or tactic was outstanding, exceptional and/or excellent and may be amodel for the '
program.
