Since the inception of “Pizza Night” over six years ago, the school has raised over $16,000 to support capital purchases. The program raises awareness about philanthropy. A Letter to the Community his year, there’s been no missing the fact that our campus along Beach Road in Oak Bluffs is a work in progress. It has been both profoundly satisfying and deeply humbling to watch as work progresses on the new Hospital that will serve this Island community for generations to come. Satisfying, because what we are building is a facility whose quality finally matches the excellent care being provided day in and day out by the dedicated and hard-working staff of Martha’s Vineyard Hospital and Windemere Nursing & Rehabilitation Center. Humbling, because this project has been made possible by this community’s unprecedented outpouring of support in response to the largest capital campaign in the history of Martha’s Vineyard. Construction of the new Martha’s Vineyard Hospital was well begun in 2007, and now as the summer of 2008 opens, the foundation is being poured and preparations are being made for framing to begin. We are happy to report that this construction project is on track, and still scheduled to meet its completion date of late 2009. But exciting as this new project may be, any hospital building is only a vessel for the caring work that goes on inside. And even as the bulldozers drone outside, all of us at Martha’s Vineyard Hospital are continuing to pursue excellence in the services we deliver to every patient in our care. T Meeting Community Needs In our report to you last year, we reiterated the Hospital’s commitment to providing an essential Island service by reopening the dental clinic which had been closed for several years. This year, we are pleased to be able to report that this promise to the community has been kept: the clinic opened in April, with Dr. Casey Pedro as its dentist and director, BOARD OF TRUSTEES John P. Ferguson, Chairman Timothy D. Sweet, Vice Chairman Earle A. Ray, Treasurer Mary R. Brown, Secretary Timothy J. Walsh, President & CEO and has been a busy operation ever since. Another successful initiative this year has been a new program for 10 members of the staff at Windemere, who are receiving professional training as Licensed Practical Nurses. This program, made possible by a $105,000 grant from the state and by contributions by the Hospital and Windemere valued at $250,000, is a win for everyone involved. For 10 certified nurses aides, it’s an opportunity for valuable career advancement. For Windemere, this training enriches the pool of skilled help so necessary in the work of an essential Island institution. We are also happy to report that the affiliation of Martha’s Vineyard Hospital with Massachusetts General Hospital and Partners HealthCare continues to advance and enrich the quality of services we provide. From professional education for our staff to better communication between caregivers here and at some of the world’s finest medical centers, the benefits of this affiliation continue to accrue. of the new Martha’s Vineyard Hospital – and we appreciate the patience and understanding of our staff and of the community for the occasional inconveniences this work has involved. But inside, we are making every effort to ensure that the quality of care delivered every day, to each and every patient, is of the highest caliber possible. When Martha’s Vineyard Hospital received a Summit Award for patient satisfaction last fall, the specific honor went to our emergency department, but the award spoke to a pursuit of excellence that permeates this institution. Safeguarding the health and welfare of all Vineyard residents with high-quality, accessible medical services is our mission at Martha’s Vineyard Hospital and at Windemere. It’s a job that is never finished. This work depends upon the dedication of a wonderful staff, and on the support of a wonderful community. For all those whose commitment and generosity make this caring work possible, we offer our deep appreciation and sincerest thanks. President and CEO Timothy J.Walsh Looking Ahead Last year we celebrated the conclusion of a historic capital campaign; next year we look forward to celebrating the completion of the new Hospital. But there’s an important sense in which our work never ends, because the work of caring begins with every person who walks through our doors. Reopening the dental clinic was a particular challenge, but the challenge of attracting primary care physicians remains. We understand the community’s need, and will continue our recruiting efforts and search for two more primary care physicians to join those already associated with Martha’s Vineyard Hospital. My wife, Dr. Karen Casper, and I can personally testify to the caliber of the providers at the Hospital following the safe delivery of our fourth child here last November. That we provide quality care is not just my opinion, but it is born out by the results on several independent patient surveys. Our volume of services and procedures continues to increase. In response to a significant community need we have added a dental clinic, staffed by Dr. Casey Pedro. We continue to expand other services such as dermatology and oncology. A radiation oncology team now holds office hours at the Hospital. In conjunction with national quality organizations we have introduced clinical pathways for heart failure and pneumonia. Compliance with national guidelines in these areas approaches 100%, far above national averages. We have purchased a simulation mannequin that allows us to train and test our clinical teams in the continuously evolving protocols for resuscitation and are very proud to be one of only a few small community hospitals that have access to this piece of equipment. We will continue to add pathways and quality initiatives as we strive to perfect the rural community hospital model. In the last year we have started to realize more benefits from our affiliation with Massachusetts General Hospital. Our providers now have full access to the records of Vineyard patients at Mass General. We continue to work on streamlining the referral process, facilitating access for Island patients to Boston specialists. Taken together these initiatives ensure not only optimal care but also excellent follow up back here on the Vineyard. We are energized by the daily progress now being made on our new hospital building and realize that without your support this would not have been possible. To say that the construction of the new 90,000-square-foot facility begins from the ground up is to understate the case. Work began last fall about 30 feet below the surface, with a process called fibro-compaction to densify the ground material and create a solid base to support the Hospital. Cornelius (Connie) Bulman, an affable 68–year old, first–generation Irishman who is serving as the Hospital’s Clerk of the Works on this project, describes the work leading up to this phase more colorfully: “We have been rolling, vibrating and pounding sand.” The results of the compaction process can be measured in the 11,000 tons of material that was trucked to the site in order to build the grade up to the original elevation. Testing by independent engineers retained by the Hospital found the results of the fibro-compaction process met A and in most case exceeded specifications. Mr. Bulman is a 43-year veteran of hospital work that includes a great deal of experience in health care construction management. Most recently he served as the Chief Operating Officer of Carney Hospital in Dorchester. He worked closely for 17 years with Tim Walsh, the President and CEO of other members of the project team as one of “healthy tension” that allows the project to proceed without creating hardship for the patients who will continue to use the Hospital throughout the project. “It’s my responsibility to ensure our patients have convenient access to the Hospital and the staff has the proper environment to administer quality care,” says Mr. Bulman. “The very nature of construction is disruptive, but I have to advocate for the patients by keeping them safe while they are on the Hospital campus and helping them get inside the doors as easily as possible during the project.” Excavation for a basement mechanical equipment room began in April and will be followed by trenching for foundations and underground plumbing. The foundation walls and slab are scheduled to be poured in June, a major project that involves hauling approximately 100 truckloads of concrete between Goodale’s gravel pit at the blinker light and the Hospital site. Work has already begun on fabrication of the steel and the logistics of transporting the steel to the Vineyard site. This phase of the project will require a great deal of coordination between the contractor, suppliers, truckers and barge operators. “If we get the first stick up in September we should complete the steel framing by mid-November,” Mr. Bulman says. Summing up, Mr. Bulman says the construction project is advancing on time and he continues to be optimistic that the new Hospital will be completed by December of 2009. There have been some small hitches along the way thus far but no major ones. “In a construction environment and location like ours you’re always going to have unforeseen things happen,” he says. “There’s a degrees of difficulty you wouldn’t have if you were doing this project on the other side of the pond. But so far this project is going well.” And as the work progresses, Mr. Bulman promises that he’ll remain mindful of his role as the advocate for Martha’s Vineyard Hospital and its patients through the building process. “There’s a degree of difficulty you wouldn’t have if you were doing this project on the other side of the pond. Page 14 — MARTHA’S VINEYARD HOSPITAL 2008 ANNUAL REPORT Hospital Reopens Dental Center time factor, and if you’re already in a low-income situation, it’s even more difficult.” Dr. Pedro says that in his months of work preparing to open the new practice, he heard from scores of Vineyarders who have not received any dental care since the former clinic at the Hospital closed in 2004. “We understand that to have good overall health,” he says, “it’s essential to have good oral health. I think that from this perspective, it’s so important to start bringing dental services to this under-served population.” The day the new clinic opened, it already had hundreds of people on its waiting list. Says Dr. Pedro, who is fluent in Spanish and Brazilian Portuguese, “Obviously, the things we’ll need to address first are the most urgent needs.” Mr. Ferriter says that because this new center is being integrated into the outpatient services of the Hospital, getting it started has been complicated. But Martha’s Vineyard Hospital has made this commitment realizing that for some people, dentistry can serve as a point of entry to a broader range of health care. “There are people who may never go to see a doctor,” he says. “But when they have a toothache, and we see them, that’s another access point for them with the whole range of services.” Mr. Ferriter says he is deeply satisfied to see patients coming into the new clinic, and grateful to Dr. Pedro for stepping up to lead it. “The reality,” he says, “is that Casey could probably make a lot more money in a private practice on the Island. And Martha’s Vineyard Hospital won’t be making any money on this service – we’ll try to manage it as efficiently as we can, but there will undoubtedly be some subsidy involved.” Looking back on the Hospital’s four-year quest, Mr. Ferriter says, “It’s taken us awhile to get here. But it’s never been a question of our commitment to meeting this community’s need.” example, we’ve been looking very carefully at maternity cases. We already plan to have highrisk mothers deliver their babies off-Island, because we don’t have the neonatal capabilities here that a tertiary hospital would have. “But another thing you have to look at here, is when you have a mother who may not be high-risk, but there are things happening during labor that could lead to high risk – uniquely at this hospital, you have to think about the weather. You have to think, if the need arises, can you get this mother and baby off-Island? Is there a storm coming, or is fog rolling in? That’s something you don’t learn about in med school.” Another unique challenge for Martha’s Vineyard Hospital is the intense seasonality of the resort community it serves. But this challenge actually concerns Ms. Wieler less than you might expect. She explains, “This hospital has learned, over the years, how to augment its staffing each summer to meet its needs so that quality doesn’t suffer. That’s something this hospital is very good at.” She adds, “Another thing this hospital is very good at, compared to the mainland, is handling disasters – ramping up for something like a bus accident on the Island. People have had to mo- bilize on the Island, quickly, for a long time. Surgeons and anesthesiologists have had to rush in. The Vineyard is very good at that.” Ms. Wieler has worked hard to build a process that encourages hospital staff to join a continuing conversation about ways to improve the quality of care. There was a time, she admits, when a visit from the quality officer wasn’t the happiest event in a hospital department: “It used to be that people would see the quality people coming and say oh, here we go, they’re going to tell us how to do our jobs, because somebody made a mistake. But that’s a part of quality improvement that has really changed over the years. We’ve spent a lot of time in this field, trying to get people to think of quality in terms of a process, as opposed to being about a specific individual who has performed in a negative way.” Ms. Wieler says that even the reporting forms she hands out to employees reflect this new attitude. You can file a report now without signing your name. And there’s a new category for reporting a potential error that an employee might have noticed, even when nothing bad actually happened. She says, “You could just be going through your daily routine and realize, oh my God, this could have happened. Now, you can tell me about it.” The idea, says Dedie Wieler, is not to miss any opportunity to raise the quality of care at Martha’s Vineyard Hospital. “We’re trying to make it so people are as comfortable as they can be about reporting their concerns.” It’s a conversation”, she says, “that can’t be too broad or too deep. Eventually, almost anything in a hospital can become a quality issue.” groups. “I’ve been a marketing machine lately,” she says, “because I feel so passionately about this.” Slowly but steadily, she says, the number of referrals to this new program has been growing. Ms. Belcher works with all ages, from the young to the old, helping them cope with the after-effects of their treatment for a wide range of cancers, from lung cancer to breast cancer, from leukemia to prostate cancer. So it’s hard to say what a typical course of therapy looks like, except that education is always central. Ms. Belcher’s goal is to put people on a track toward self-sufficiency, with a program they can carry forward on their own. “When someone first sees me, our evaluation starts with a self-report questionnaire on quality of life issues. It starts with a complete medication list. And then we move to a person’s issues – perhaps you have range of motion problems after surgery, as well as some pain and balance issues. “My key is education.” I’ll say, “here are some exercises you can start doing at home. And as she helps them, one by one, this new program at Martha’s Vineyard Hospital is sure to grow. That’s exactly the way she wants it to be. She’s confident that the cancer rehab service is making a positive difference in Islanders’ lives. Accreditation signifies that Martha’s Vineyard Hospital has met the standards of the nation’s most rigorous health care accrediting W organization. Before granting this highest level of certification, the Joint Commission conducted a site visit to the Hospital, measuring its quality of care against dozens of standards, from the way caregivers communicate to how medication errors are prevented, from strategies that cut the risk of infections to practices that reduce the risk of patient injury due to falls. But as pleased as she is with this high honor, Ms. Wieler is quick to explain that the credit goes ultimately to every single employee at the Hospital, “In the end, you can’t look at this as my job. Quality is really everybody’s job here.” And with her 30 years of experience in hospital quality of care, she also knows that there never comes a time for resting on one’s laurels. “The idea,” she says simply, “is to improve care. It’s a process, and you’re never done.” Ms. Wieler works closely with the Hospital’s many performance improvement teams, which focus on issues from medication safety to stroke response, and she keeps an eye on quality indicators tracked in twelve different hospital departments. Her job is to keep up with the constantly advancing standards of excellence in patient care, and to see that these standards are applied to improve the quality of service at Martha’s Vineyard Hospital. The mandates of the outside world – from government regulators to third-party insurance payors – demand constant attention. But Ms. Wieler also tries to focus, whenever she can, on quality of care issues unique to this hospital and its community. Martha’s Vineyard Hospital is quite small by modern standards, and its Island location raises unique issues for quality of care, she says. “For Cancer Rehabilitation Service Making a Positive Difference ccupational therapist Jill Belcher, OTR/L, has helped launch a new service at Martha’s Vineyard Hospital, and she’s on a mission to spread the word and reach more people. The new service, now in place for a year and a half, is cancer rehabilitation, and already Ms. Belcher has worked with 15 people who otherwise would have had to travel to off-Island medical centers for the help they’re now receiving here. Says Ms. Belcher, “People don’t often think of what this therapy can do for someone who has a diagnosis of cancer. But range-of-motion issues often can come up after surgery. There can be cancer-related fatigue after chemotherapy or ra- O diation. There can be soft-tissue limitations that lead to range-of-motion problems, and pain that can cause body disturbances involving balance. There can be numbness after chemotherapy or when a nerve is disturbed during surgery.” For many Islanders with cancer, treatment involves off-Island surgery, radiation or chemotherapy. “In my practice here,” Ms. Belcher says, “I noticed that sometimes when these people come back, they have these difficulties and they don’t know whom to turn to. I thought we could offer this service here, because these patients have to do so much traveling already.” She has reached out to Hospital doctors, and meets regularly with Island cancer support “In the end, you can’t look at this as my job. Quality is really everybody’s job here.” MARTHA’S VINEYARD HOSPITAL 2008 ANNUAL REPORT — Page 15 In Obstetrics: A Focus on Quality oyce Capobianco has spent her nursing career caring for people across the whole span of her life. A former director of nurses at Windemere, she’d also been involved with the Visiting Nurse Service before being asked by chief nursing officer Carol Bardwell, two years ago, to take her current job as nurse manager of Martha’s Vineyard Hospital obstetrical unit. It had been more than a dozen years since Ms. Capobianco last worked in an obstetrical department. “But I think the need in obstetrics was really for a manager,” she says. “Carol was willing to take me on and orient me and get me comfortable, clinically, with the emphasis on working with this group here. And I have to say, it’s been like a dream come true – because here was this group of women just waiting to take this department to a new level, and they’ve made all this so easy.” Of her experience in geriatric care, Ms. Capobianco says, “Working with the elderly is something that makes me feel really good. In this new experience with the obstetrics department, I’d have to say that what’s been most fulfilling for me is the people I’ve been working with professionally. They’re such a wonderful team.” With a small Island community to serve, the Martha’s Vineyard Hospital obstetrics department has a working rhythm that’s not unlike a fire department – long stretches of waiting punctuated by hours of excitement. In 2007, the Hospital saw a record 180 births, but even that would be less than a week of activity for a large urban medical center. In fact, when you consider the frequent downtime, this department at Martha’s Vineyard Hospital is hardly a profit center. The obstetrics staff can focus on quality of service, not quantity, and the families they serve are the beneficiaries. “One of the things we pride ourselves on,” says Ms. Capobianco, “is that delivering a baby here is such a unique experience. This hospital has really made a commitment to the community, for mums J to be able to deliver here. What that ends up meaning is that we have a unit that’s constantly staffed, 24/7, with adequate nursing staff, adequate medical coverage, and a nurse-midwife.” As Martha’s Vineyard Hospital increasingly adopts the Planetree model of patient-centered care, Ms. Capobianco finds that most of the Planetree precepts are already in line with the practices and priorities of her obstetrics department. She embraces, for example, the Planetree commitment to improving medical care from the patient’s perspective. The obstetrics team meets with each mother a month before her due date and prepares a detailed document called a birth plan, outlining the mother’s wishes. To make mix-ups less likely, the Hospital works to separate these medicines at every step of the path from pharmacy to patient. You won’t find sound-alike drugs shelved side by side in the pharmacy; they’re stored apart, which makes it that much harder for a technician to reach for the wrong one. Incorrect dosages are another problem the pharmacy works proactively to prevent. The Pyxis machine is organized so when a nurse asks, for example, for the blood-thinning drug Lovenox®, the drawer that opens will have only one available dosage inside. And in the national campaign to eradicate medication errors, doctors’ handwriting – the butt of so many old jokes – continues to be a big issue. It’s all about eliminating ambiguities, Mr. Caron says. The Hospital’s medication safety task force has a list of dangerous abbreviations, and encourages physicians to use alternatives that are less open to misreading. Where physicians once wrote “5.0 milligrams” on a prescription, they’re now encouraged to write “5 milligrams,” because this leaves no chance for missing the decimal and mistakenly administering 50. Similarly, the old shorthand “q.d.” for daily dosage is now discouraged, because it can be confused with the abbreviation “q.i.d.” – which means four times per day. As technology advances, the issue of doctors’ scribbles will someday be taken out of the safety equation entirely. Mr. Caron says the next wave – called CPOE, or computerized physician order entry – is arriving now at the nation’s biggest medical centers. Medication Safety is Constant Focus artha’s Vineyard Hospital has some 1,600 medications in its formulary, and administers more than 100,000 doses to patients each year. If you’re inclined to worry about medical errors, that’s an awful lot of opportunities. And the bar is dauntingly high. Ask Dave Caron, director of the Hospital pharmacy, how many medication errors is an acceptable number, and his answer is an emphatic and unhesitating “Zero.” It’s a tall order, but that’s the goal the Hospital’s medication safety task force is constantly chasing. Mr. Caron and his task force teammates, chief quality officer Dedie Wieler, chief nurse executive Carol Bardwell and emergency room physician Mark Shapiro, meet together every two weeks to advance this dogged pursuit of perfection. In the field of medication safety, as in so many aspects of health care, Mr. Caron says the most important work is often not fixing a problem after it’s come up, but a matter of prevention before the fact. “It’s the responsibility of the health care organization,” he says, “to be proactive.” M One way Martha’s Vineyard Hospital is proactive in preventing medication errors is through the use of technology. The Hospital pharmacy uses a computerized dispensing machine called the Pyxis – Mr. Caron calls it “a glorified vending machine, if you will.” The pharmacy staff creates a profile of medications for each patient and enters that data into the Pyxis equipment. When a nurse logs in and enters a patient’s name, Mr. Caron explains, “You pick a medication from the touch screen and a particular drawer opens up, giving you that specific order – from the physician, to the pharmacist, to the nurse.” The Hospital task force closely follows the work of national agencies that track problems and solutions in the field of medication safety. When the Institute for Safe Medication Practices issues its quarterly report on incidents and issues from around the country, Mr. Caron and his Barbara Murphy of Chilmark holds daughter Mary Murphy (Boyd) born at the Hospital on April 24, 1978. the entire Martha’s Vineyard Hospital stroke team, which works closely with Mass General to coordinate emergency treatment protocols. The stroke team is truly interdisciplinary, involving all departments of the Hospital. Its members include team leader Mary Patrice Ruocco, Jill Belcher, Dave Caron, Karen Dumont, Margaret Ekholm, Mary Gross, GageDavid Hansen, Claudia Macedo, Lynn Mercer, Margaret Pattison, Gayle Poggi, Donna Rhoades, Timothy Tsai and Elizabeth VanLandingham. This was the first Partners in Excellence awards program at Martha’s Vineyard Hospital since it joined in an affiliation agreement last year with Partners HealthCare, a system of hospitals in Boston which includes Mass General and Brigham and Women’s Hospital. The awards, created in 1996, recognize individuals and teams at all levels who have contributed to the success of the partnership and its member institutions. Recognizing Excellence distinguished visitor from Massachusetts General Hospital made his first visit to Martha’s Vineyard Hospital on Feb. 1 for a ceremony honoring two individuals and one Hospital team for their commitment to excellence. Jeff Davis, senior vice president for human resources at Mass General, made his first presentation to Denise Brouillette, lead housekeeper at Martha’s Vineyard Hospital. Tim Walsh, Hospital President and CEO, said nominating Ms. Brouillette for the Partners in Excellence Award was an easy choice. “She’s a dream to work with. She’ll do anything, anytime, for anybody.” Another individual award went to Sue Canha, a Windemere employee since 1975. In the same room. You won’t have to be moved from room to room until it’s time for you to go home.” Summing up, Ms. Capobianco returns to the quality of her colleagues on the maternity staff. “We have a group here who are highly skilled in maternity and those areas around obstetrics, and they’re feeling energized and motivated. I think the biggest thing we’re able to express to mums right now is that we offer very individualized, family-centered care.
