I
have been very impressed by the work of the Pakistan Dental Community and
the role it plays as a platform for the country’s dentists to meet, interact, collaborate
and share experience. As dentists, frequently working
alone or in a small group in dental practices, it is reassuring to know we share our goals with a wider community to whom we can have ready access when the
need arises.
Now, with the publication of the inaugural edition of
the PDC magazine, I can say that I am equally impressed. Not only has a great deal of attention been
given to its appearance and aesthetic, its contents are
entirely compatible with the sense of community that
is so much a part of the aims and objectives of PDC.
I note with some satisfaction that it is the Editor’s goal
to provide readers with relevant information not only from Pakistan but from the
wider world. In the international community of dentists, we can all learn from each
other and I am confident that this new magazine will significantly enrich the national intellectual landscape in the field of dentistry and oral health and the broader
field of health care.
As President of FDI, an organization that, through some 200 national member
bodies in over 130 countries, serves some 1 million dentists worldwide, I am particularly happy to see that PDC is also participating so enthusiastically through its
truly exceptional efforts to communicate the message of World Oral Health Day in
Pakistan.
I could not agree more with the words of PDC President, Dr Syed Muhammad
Faizan, who emphasized the need to initiate a voluntary self-sustaining and continuous process of education, prevention, and treatment especially for the deprived
segment of the society. It is in this spirit that we can achieve true improvements in
oral health throughout the population.
I would like to extend my warm personal regards to the officers and members
of PDC and congratulate you all on this new and worthy initiative in dental

T
he dental profession in Pakistan did not evolve in harmony with the medical
profession and hence lacked behind in its development in the overall medical profession of the country.As a result, the required level of
infrastructure, development and its healthy effects on
the society did not take place at par. Similarly it could
not attain its universally accepted higher status in Pakistan that it richly deserves.
Although still in its infancy, Pak Dental Community
(PDC) an organization with the mission of uplifting
the profession to international standards in Pakistan
and bringing back nobility into it, has achieved significantly in such a short period. It has provided a platform to the young professionals not only to collaborate
and learn from collective wisdom but also to instill a
sense of social responsibility among them so that they
may serve the true purpose of the white coat.
The philanthropic arm of PDC by the name of PDC Foundation, a program focused exclusively on the oral health care of the economically disadvantagedwas
formed in January 2013. With the help of generous donors and selfless & passionate volunteers, Outreach Programs have been conducted in Karachi, Hyderabad
and Lahore and are expanding its activities to more cities/towns very soon this year.
I am proud of my Team and thank all those who made this possible. What better
can you do than to start giving back as early as possible.
I firmly believe that we need to accelerate the culture of research & research training in Pakistan, that too of international standards. Thereforewe are committed
to play a leading role in the attainment of this objective. We arehappy to provide a
platform for professionals to not only undertake research but also create opportunities for them to play an active part at the international scene. Last year 6 researches
conducted by the PDC team were presented at FDI Istanbul Turkey.

n 1st January 2013, the philanthropic arm of Pak
Dental Community (PDC) was announced by the name
of Pak Dental Community Foundation. It is an independent non-profit program in Pakistan exclusively focused
on supporting delivery of comprehensive oral healthcare
for economically disadvantaged people.
Its partners include dental universities, hospitals as well
as private dental clinics & health care organizations.
Dental care is cost-prohibitive for low-income individuals and often, out of necessity, receives less priority compared to more pressing financial or medical concerns.
A huge number of people in Pakistan have no access to
the simplest dental relief, leaving thousands to face a
daily battle with pain and oral diseases.
Pak Dental Community is proud to present PDC Foundation, which will be the first & only independent non-profit program in Pakistan exclusively focused on supporting
delivery of comprehensive oral healthcare for economically disadvantaged people and making a difference in
people’s lives. As Pakistan’s premier dental philanthropic
and charitable organization, the PDC Foundation will be
a catalyst for uniting people and organizations to make a
difference through better oral health.
Our strategic ties with the Pak Dental Community, coupled with our strong volunteer leadership and our generous donors, give us a powerful yet flexible infrastructure
to anticipate and quickly respond to the most pressing
needs affecting dentistry and the public’s oral health.
Within a very short span of time we have expanded our
activities to multiple cities across Pakistan.
And it doesnt stop there. A big chunk of these poor patients end up at a local quack dentist for cheap treatment,
only to be infected with HIV/Hepatitis.

ANXIETY LEVELS IN DENTAL PATIENTS

INTRODUCTION:
The behavioral sciences have become an increasingly
important component of dental education and research.
Fear of dentists and dentistry is a common and potentially distressing problem, both for the public and for
community dental practitioners. Anxiety can be defined
as a state of unpleasantness with an associated fear of
danger from within or a learned process of one’s own environment Anxiety related to dental treatment is a fairly
common phenomenon. It is a problem in dentistry, and
a notable factor in the avoidance of dental care.
Symptoms of dental pain or discomfort, fear of the dentist’s disapproval, negative expectations concerning the
outcome of treatment, a history of low-quality dental
treatment, complications experienced in previous dental
care, and negative dentist-patient interactions, have all
been associated with high dental anxiety. Patient anxiety
makes it harder to provide treatment, causes lost time,
and can potentially decrease the quality of dental work.
An understanding of how anxious patients are when they
undergo dental treatment would aid dentists’ efforts to
improve patient care. The intensity of dental anxiety is
different among certain groups in the population. Several studies have shown that younger people, people with
low income or low socioeconomic status, and people with
lower levels of education tend to have more severe dental anxiety than people who are elderly, more affluent, or
better educated.
OBJECTIVE:
Measurement of anxiety levels of patients coming to a
dental OPD/Teaching Hospital by using Norman Corah’s
Anxiety Scale.

Sampling Technique: Convenience
Total no of 166 patients within age group 5 –to- 75 yrs were
randomly selected from all pat ients visiting dental hospital
OPD for dental treatment, between Jan 2012 – Mar 2012.
Dental anxiety was estimated using Corah’s Dental Anxiety Scale (DAS) which was based on a questionnaire regarding anxiety levels in patients.

9-12 = moderate anxiety but have specific stressors

15-20 = sever anxiety (or Phobia)
Results:
A total of 166 patients completed the questionnaire regarding their anxiety level and were included as a part of
this study.
34.5% Males and 28.2% Females felt a little un easy, 20%
Males and 25.5% Females were tense and 5.5% Males and
8.2% Females were very anxious.
Population undergoing invasive procedures including
extraction, impaction removal, fillings and root canal
treatment comprised of 83 individuals (approx. 50%
population).
In educated strata 10 Males and 26 Females had Mild
anxiety levels followed by 5 Males and 10 Females with
Moderate anxiety levels, 2 Males and 5 Females with
High anxiety levels and 1 Male and 4 Females with Severe anxiety levels.
In uneducated strata 1 Male and 3 Females with Mild
anxiety levels followed by 4 Males and 11 Females with
Moderate anxiety levels and 1 Female with Sever anxiety
levels.
Population undergoing non-invasive procedures include
routine consultation, scaling and orthodontice treatment.
In educated strata, 18 Males and 21 Females had Mild
anxiety levels followed by 5 Males and 14 Females with
Moderate anxiety levels, 3 Males and 5 Females with
High anxiety levels and 2 Males and 6 Females with Severe anxiety levels.
41% of the patients belonged to age group of 5-24 years
37% of the patients belonged to age group of 25-39 years
In un educated strata, 2 Males had Mild anxiety levels followed by 2 Males and 2 Females with Moderate anxiety
levels and 2 Females with Severe anxiety levels.
22% of the patients belonged to age group of 40-75 years
According to Corah’s questionnaire:
Relating to a visit for dental check up 40 % Males and
25.5% Females were looking forward to it as an enjoyable
experience.
Question relating to waiting in a dental office, approx.
49.1% Males and 47.3% Females were relaxed.
Question regarding the noise of a drill 36.4% Males and
30.9% Females were relaxed while 36.4% Males and
27.3% Females felt a little uneasy and approx.3.6% Males
and 8.2% Females felt very anxious.
Question relating to tooth cleaning and instrument preparation 27.3% Males and 26.4% Females were relaxed,

Conclusion:
Patients who visited the OPD for the first time for certain
non-invasive procedures (such as orthodontic treatment,
scaling, filling or routine dental checkup) scored Mild
Anxiety Levels on Norman Corah Scale.
Patients who had visited the OPD before and had gone
through an invasive procedure (such as extractions, RCT)
scored Moderate to Severe Anxiety Levels on Norman
Corah Scale
Female patients had higher anxiety level scoring as compared to male patients.
Uneducated patients had higher anxiety levels than educated patients

factor for tooth loss. The aim of this study was to estimate the
risk of tooth loss in smokers.

data was collected prospectively during previous 3 years
(2010 – 2012). Total 15,289 subjects visited the oral surgery departments in both centers (Fatima Jinnah dental
college hospital, Azam Town Karachi & Abbasi Shaheed
Hospital, Nazimabad Karachi) in a period of 3 years for
extraction. 6277 cases were selected for the study (having age10 years and over) and matched with 2639 controls for same gender and age group. The subjects having
hematological disorders, diabetes mellitus and neoplastic disorders were excluded from the study. The study
sample was divided into 6 age groups (10 – 20 years, 20
– 30 years, 30 – 40 years, 40 – 50 years, 50 – 60 years
and above 60 years). Cross tabulations were performed
gender wise and age group wise, and relative risk was
calculated for each group.

RESULT:
The mean age of the sample was (36.9) with male:female
ratio of 1:1, In male population 26.5% of case gave a history of smoking while 18.9% of control group presented
with a history of smoking. In female population 3.8% of
cases were smokers while 0.8% in control group were
smokers. The association between smoking and tooth
loss was positive with significant results in age groups
10 – 20 years (0.006 Sig. value) and 40 – 50 years (0.001
Sig. value) respectively.
CONCLUSION:
The authors found that women who smoke were at increased risk of experiencing tooth loss than men. The relative risk of tooth loss due to smoking among Pakistani
population is higher than what has been reported among
western population. In accord with other studies[i] smokers had evidence of more severe periodontal disease than
non smokers.

E-CADHERIN AS A MARKER FOR NODAL
METASTASIS IN HEAD AND NECK
SQUAMOUS CELL CARCINOMA

Objective:
To identify E-cadherin as a marker for prediction of lymph node
metastasis in head and neck squamous cell carcinoma.
BACKGROUND:
METHOD AND MATERIALS:
Head and neck squamous cell carcinoma (HNSCC) is the
sixth most common malignant tumor worldwide, characterized by locoregional disease with a propensity for
metastasis to the cervical lymph nodes. The pre-operative
detection of lymph node metastasis is crucial to the effective treatment of patients with head and neck squamous
cell carcinoma. Cervical lymph node metastasis cannot
always be predicted from the size and the extent of invasion of the primary cancer and also the simple fact that
metastases need to reach a certain size before they are
detected(>3mm). Although the most recent techniques
of CT scan, MRI, ultrasonography, PET scan and ultrasound guided FNA biopsy have reached a sensitivity of
about 80% in detecting occult metastasis, the detection
of occult, microscopic metastastic deposits will continue
to evade recognition and the true lymph node status of
neck will remain uncertain. As a result of these limitations many head and neck surgeons perform radical or
selective neck resections and as many as 80% of patients
with N0 neck disease will receive unnecessary treatment,
along with its concomitant morbidity.
Cross-sectional analysis of 54 subjects with head and
neck squamous cell carcinoma, who underwent neck
dissections, was carried out. Expression of E-cadherin
was evaluated using immunohistochemical analysis and
traditional histological parameters, and the correlation
of E-Cadherin with the histologically verified presence of
regional metastases was determined. The data was subjected to descriptive statistics and chi square using Spss
v.16.0.

RESULT:
54 patients included 33 males (61.1%) and 21 females
(38.9%) aged from 18 to 73 (mean 44.8±12.7). Predominantly the male population of the study was most commonly found within the age group 39-48. Whereas the
most frequent age group among females was 49-58.
On histological examination moderately differentiated
squamous cell carcinomas occurred most frequently
(34/54). A statistically significant relationship between
the Downregulation of E-cadherin and histologically
verified presence of nodal metastasis was established. (

CONCLUSION:
Conclusion: This study shows that low E-cadherin expression is useful for predicting lymph node metastases
in cases of head and neck squamous cell carcinoma.

A five-hour hands on workshop that provides participants with both theoretical and practical exposure to minor, dento-alveolar procedures at the level of the general
dentist was conducted on the 5th of Jan, 2012.
Aims of the workshop was to to impart basic surgical
principles, including case assessment, sound surgical
techniques, post-operative care, and the management
of problems arising from surgical treatment, and to
provide participants with the opportunity to apply these
principles through the treatment of patients in the public
health setting.

COURSE OUTLINE:
Patient evaluation and treatment planning.
Assessing surgical difficulty and risk on radiographs.
Preparation for surgery (infection control, draping,
scrubbing).
Suturing Techniques and Wound Closure.
Applied Anatomy of local anaesthesia.
Raise and suture a mucoperiosteal flap to maximise access and visualisation.
Tooth sectioning and bone guttering.
Safely use elevators to remove teeth/roots.
Manage broken roots during extraction.
Adjunct procedures – troubleshooting the failed local
anaesthetic.
Indications for antibiotics/analgesia and management
of surgically induced pain.
Postoperative care, complications of oral surgery.

Karachi, December 8, a hands-onworkshop on handling
medical emergencies in a dental setting was organized by
Pak Dental Community at the Central Mess, N.O.R.E 1.
The workshop led by Dr. Hasan Mehdi, Dr. Jaffer Abbass
and Dr. Saif Ullah Khan,was designed to benefit the participants in gaining knowledge, skills, and confidence to
handle the variety of possible medical emergencies that
can occur pre-operatively, during the procedure or postoperatively.
The instructors highlighted the importance of being prepared in order to manage an emergency, reviewed common office emergencies and their management including video demonstrations, identified the basic emergency
equipment which should be present in the dental office
and a discussion regarding the basic emergency medications, their dosage and administration.
Participants practiced Basic Life Support protocol for
Cardiopulmonary Resuscitation on AHA’s recommended mannikin (resus annie) manikin and volunteered for
a simulation intended to learn how they would react to a
medical emergency in their dental office.
PDC’s workshop was a successful effort to benefit the
participants in learning and gaining confidence to deal
with emergencies and well emp hasized the need to be
prepared for an emergency, as Benjamin Franklin has
very well quoted “By failing to prepare, you are preparing
to fail.”

arachi,January 12. A seminar was organized on NBDE
USA as part of PDC’s Ace The Examination Series, and
was conducted by Dr. Syed Muhammad Faizan and Dr.
Sadaf Razzak at the Central Mess N.O.R.E 1, Karachi.
The aim of the seminar was to guide the students on
how to prepare for the National Board Dental Examination USA and guide them through the registration process. Taking it one step ahead the instructors enlightened
the students regarding the procedure to apply in various
American dental universities and the license requirements to practice in USA as a dentist. An interesting
question and answer session concluded the seminar.
Pak Dental Community’s Ace The Examination series
is based on the thought that students need to be properly guided and at the most appropriate time on how to
prepare for wide variety of both, Pakistani and Foreign
examinationsso that they may develop their careers on a
more well defined path.
PDC plans to hold more seminars to guide the students
and professionals seeking to study/work bothin Pakistan
and abroad.

World Oral Health Day – 20th
March 2014
Karachi 20th March, Ziauddin College of dentistry in collaboration with Pak Dental Community PDC celebrated
World Oral health day 2014 themed “Celebrating Healthy
Smiles”.
The seminar was inaugurated in the esteemed presence
of Prof Dr Pirzada Qasim Raza Siddiqui, Vice chancellor
Ziauddin University, Dr Mahmood Shah, President PDA
Sindh and Chief Guest, Dr Saqib Rashid, President Pakistan Dental Association (PDA) in the presence of whopping audience of more than 300 young dentists and dental students.
Three prominent speakers including Dr Ambreena
Qureshi, Dr Saif Ullah Khan and Dr Syed Muhammad
Faizan addressed the audience on the importance of oral
health education and the need to spread awareness at every
level.
President PDC, Dr Syed Muhammad Faizan speaking on
the occasion expressed “Maintaining oral health and inculcating hygiene consciousness among general public is a
challenge due to lack of basic education. Pakistan has one
of the highest rates of oral cancer, while there are about 11
thousand registered dentists, which means that for 16,500
people there is just one dentist.” Dr. Faizan emphasized the
need to initiate a voluntary self-sustaining and continuous
process of education, prevention, and treatment especially
for the deprived segment of the society.
Senior Registrar, Oral Medicine, Ziauddin University, Dr.
Saif Ullah Khan explained “Oral health in different stages
of a woman’s life”. He discussed that women make half
of world’s population, and are more prone to oral diseases
due to hormonal changes at different stages of their life
like puberty, pregnancy, using oral contraceptives and
menopause , and experience dental problems due to lack
of education and care of the oral manifestations. Prompt
medical attention and education is required ti maintain a
healthy life style.
DrAmbrinaQureshi described the importance of World
Oral Health Day. “It is an international event about the

benefits of a healthy mouth and promotes worldwide
awareness of issues around oral health and its importance for both old and young” she expressed.
Speaking on the occasion President PDA Dr Saqib
Rashid, President PDA emphasized the need to educate
our population to prevent oral diseases as the statistics
show that only 20 percent of Pakistan’s population has
access to treatment of oral health.

PDC makes Pakistan proud at
FDI AWDC 2013 Istanbul
Istanbul 29 August, Turkish Dental Association hosted
the 101st FDI Annual World Dental Congress. The World
Dental Federation FDI is the principal representative
body that represents 1 million dentists worldwide. The
event gathered dentists from around 120 countries and
addressed the major themes and concerns of dental practitioners today, presented by some of the world’s leading
clinicians and experts in the field which aimed to promote
professional development among dentists by acting as a
mean for communication and by encouraging discussion,
networking and research in all areas of dental practice.
PDC made its mark on the global dental community as,
amazingly, six researches were shortlisted to be presented
in the 2013 101st FDI conference held at the Istanbul
Congress Centre, Istanbul, Turkey.

24 Members of Pak Dental Community (PDC), accompanied by the President of PDC, Dr. Syed Muhammad
Faizan, attended the conference& made Pakistan proud
by presenting the researches.
Research topics spanned a wide range of topics in the
areas of Epidemiology/Public health, Orthodontics, Operative dentistry, Oral Pathology and Surgery. After each
presentation presenters were fielded questions from the
audience comprised of Professors, Dentists and students.
Presenters included Dr. Syed Muhammad Faizan, Dr.
Saima Butt, Dr. AfsheenRehman , Dr. Mubashir Saleem,
Dr. ArifPyarali and Hamza Syed whose work was received
with much appreciation.

DENTAL SCHOOLS FIRST BOWLING
CHAMPIONSHIP
On the 12th of November 2012, the much-awaited Dental
schools first bowling championship organized by PDC
youth was held at Arena Club Karachi. It was the untiring effort and dedication of PDC Youth’s team that such a
social event at such a large scale was successfully planned
and executed. Students from all dental schools participated in great numbers along with senior names like Dr
Saqib Rashid, Dr Hassan Mehdi, Dr YawarAbidi and Dr
IrfanQureshi.
The bowling championship was based on knock out
rounds in which players with the highest scores qualified
for the next rounds. The championship got more interesting as the semi final and final rounds reached where

Shaista Zafar among girls and Ammar Hussain Pabaney
among boys, both studentsof Fatima Jinnah Dental
College,won the championship following an intense finale. The winners were awarded HTC cellphones and was
presented to them by Dr Hasan Mehdi.

dental studentson one platform where they interacted
with students as well as teachers of other colleges and
participated in a healthy activity as one large dental community.
