disabilities. We are all so
proud to be apart of such an
amazing organization.
What Can I Do?

Learn about different disabilities and how they affect people differently: read, ask
questions, and respect differences.

Support the efforts of people with disabilities to lead full lives, work at rewarding
jobs, and participate in all aspects of life.

Correct and challenge stereotypes and slurs in media, advertising, and everyday
conversation.

Support local groups working for diverse multicultural institutions.
Horizon Housing Development Company was
founded in 2004 as a 501(c)3 nonprofit
organization. We provide permanent housing
to St. Louis City individuals with developmental
disabilities who are homeless. Although
permanent housing is our main goal, we also
contribute to the fight to end homelessness.
Housing is a key stabilization factor for
success in future endeavors including but not
limited to, employment and scheduling daily
activities. HHDC is helping individuals with
developmental disabilities live independently
with little assistance. Housing in our agency is
not just a home but hope for the future.

Tenancy Requirements
Horizon Housing Development Co. is a
non-profit housing program.
Head of household assistance can only
be provided to individuals with
disabilities and families in which one
adult or child has a disability.
Summary of Eligibility Criteria:
For the non HUD program
Developmental Disabilities:
* autism spectrum disorder
* cerebral palsy
* epilepsy
* intellectual disability
These developmental disabilities must
be diagnosed before one's 18th birthday
or one must meet the functional
definition of a developmental disability
as determined by the St.Louis Regional
Office.
For HUD program
Developmental Disability defined in
section102 of the Developmental
Disability Assistance and Bill of Rights
Act of 2000 (42 USC 15002)
Means a sever chronic disability that is
attributed to a mental or physical
impairment or combination, is
manifested before age 22 and is likely to
continue indefinitely. Results in
substantial limitations in three or more
major life activities. Which include but
are not limited to self care, learning,
mobility, receptive and expressive
language, self direction, capacity for
independent living and economic self
sufficiency.
Reflects needed for a combination in
sequence of special interdisciplinary or
generic services, individualized supports
or other forms of assistance that are of
lifelong or extended duration and are
individually planned and coordinated.
An individual may be considered to
have a developmental disability without
needing 3 or more of the criteria's listed
previously. If individual is 9 years old or
younger, has a substantial developmental
delay or specific congenital or acquired
condition. Without services and supports,
has a high probability or meeting those
criteria later in life.
The person or the head of household must
be a resident of the City of St. Louis.
DD Resources defines Resident as:
1. Where immediate family/guardian
resides; OR
2. Residency of a family member having
responsibility for the person with DD
3. Last residence with family/guardian
prior to admission to state facility; OR
4. If of age, and possessing “legal rights”,
the individual person with DD on their
own volition chooses to reside in that
county.
To sum up the requirements for tenancy,
the person needs to be Developmentally
Disabled and a Resident of the City of St.
Louis.

Our Program
Our program provides permanent
supportive housing in seven
scattered sites with a total of 31
units. Occupancy rates for the
most of the program's existence
have remained at 100%. The
average retention rate for current
occupants is 5 years.
Currently all units are occupied
and there is a wait list maintained
of individuals referred primarily
from case management entities
that specialize in developmental
disabilities. The waitlist gives first
priority to chronically homeless
individuals and families with a
developmental disability and with
the longest history of
homelessness and with the most
severe service needs.
Stable housing is a key success
factor for individuals seeking
employment, maintaining
employment or dealing with the
application process for Social
Security Income (SSI). With our
permanent housing and our
support, individuals are more likely
to establish schedules of daily
activities such as sleep, meals
and recreation around jobs.

understanding of each individual
program participant. The direct
supports provided through
Horizon’s Supportive Housing
Program in conjunction with
supports provided through
partnerships with other community
agencies, including long-term
case management providers,
employment services, and
socialization programs, assist
program participants with breaking
the cycle of homelessness
permanently.
