immediate and remote, of such deceased individual; if any of the above-named parties is a dissolved
partnership, corporation or other association, then the unknown successors, trustees and assigns,

NOTICE IS HEREBY GIVEN that Applicant, Continental Resources, Inc., has filed an application in this
cause requesting the Corporation Commission of Oklahoma to enter an order, to be effective as of the
date of the execution thereof or as of a date prior thereto, as follows: (1) approving a multiunit horizontal well in the 640-acre drilling and spacing units formed for the Mississippian, Woodford and
Hunton separate common sources of supply in Section 23-16N-10W and Section 14-16N-10W, Blaine
County, Oklahoma, so as to allow such multi-unit horizontal well to be drilled at the following location:
Surface location: 354 feet from the north line and 876 feet from the west line of Section
26-16N-10W, Blaine County, Oklahoma
Section 23-16N-10W:
first perforation: no closer than 150 feet from the south line and no closer than 330 feet
from the west line of Section 23-16N-10W, Blaine County, Oklahoma
last perforation: no closer than 0 feet from the north line and no closer than 330 feet
from the west line of Section 23-16N-10W, Blaine County, Oklahoma
Section 14-16N-10W:
first perforation: no closer than 0 feet from the south line and no closer than 330 feet
from the west line of Section 14-16N-10W, Blaine County, Oklahoma
last perforation:
no closer than 150 feet from the north line and no closer than 330
feet from the west line of Section 14-16N-10W, Blaine County, Oklahoma
and to complete such multiunit horizontal well in and to produce hydrocarbons from the separate common sources of supply. Applicant intends to run casing in the horizontal portion or lateral of the borehole of the multiunit horizontal well and to cement such casing so as to cover and isolate the first and
last perforations in such horizontal portion or lateral. Applicant is proposing to drill the multiunit horizontal well to a total measured depth of approximately 20,240 feet and is proposing to have a completion interval in such multiunit horizontal well of approximately 9,800 feet. The proposed multiunit horizontal well is necessary in order to prevent waste and to protect the correlative rights of the owners
of oil and gas rights in the units.
NOTICE IS FURTHER GIVEN that the application in this cause requests that the order to be entered
in this matter be made effective as of the date of the execution thereof or as of a date prior thereto.
NOTICE IS FURTHER GIVEN that this cause is set before an administrative law judge for hearing, taking of evidence and reporting to the Corporation Commission of Oklahoma.
NOTICE IS FURTHER GIVEN that this cause will be heard before an administrative law judge on the
Conservation Docket at the Western Regional Service Office of the Corporation Commission, Jim
Thorpe Building, 2101 North Lincoln, Oklahoma City, Oklahoma, at 8:30 A.M. on the 24th day of
October, 2016, and that this notice be published as required by law and the rules of the Commission.

NOTICE IS HEREBY GIVEN THAT the Applicant in this cause is proposing to drill a multi-unit horizontal well, under 52 O.S. Sec. 87.6, et seq., and OAC 165: 5-7-6.2 and OAC 165: 10-3-28 in the lands
involved herein in the Shale Reservoir hereby, with a portion of the completion interval of such multiunit horizontal well to be located in one or more of the 640-acre drilling and spacing units formed for
the Mississippian and Woodford common sources of supply [Order to be entered in Cause CD No.
201604327] involved herein in said Section 5, Township 15 North, Range 10 West, Blaine County,
Oklahoma, and with a portion of such completion interval to be located in one or more of the 640-acre
drilling and spacing units formed for the Mississippian and Woodford common sources of supply
[Drilling and Spacing Order No. 643729] involved herein in Section 8, Township 15 North, Range 10
West, Blaine County, Oklahoma. Applicant is proposing to drill the multi-unit horizontal well, from a
surface location to be described in the final order, to be issued in this cause to a subsurface location
of such well’s entry into and such well’s exit and potential exit from each of the Mississippian and
Woodford separate common sources in Section 5, Township 15 North, Range 10 West, Blaine County,
Oklahoma and Section 8, Township 15 North, Range 10 West, Blaine County, Oklahoma;
the perforations in Section 5 in the horizontal/lateral portion of the borehole shall be no
closer than 165 feet to the north line, no closer than 0 feet to the south line, and no closer
than 330 feet to the east line of Section 5, Township 15 North, Range 10 West, Blaine
County, Oklahoma;
the perforations in Section 8 in the horizontal/lateral portion of the borehole shall be no
closer than 165 feet to the south line, no closer than 0 feet to the north line, and no closer
than 330 feet to the east line of Section 8, Township 15 North, Range 10 West, Blaine
County, Oklahoma;
The operator is currently reviewing the possible surface locations. Therefore, this well could be
drilled North to South, or South to North and could have a surface location in an offsetting section.
Approving a multi-unit horizontal well in the 640-acre drilling and spacing units formed for the
Mississippian and Woodford separate common sources of supply underlying Section 5, Township 15
North, Range 10 West, Blaine County, Oklahoma and Section 8, Township 15 North, Range 10 West,
Blaine County, Oklahoma, so as to allow such multi-unit horizontal well to be drilled as described
above, and to be completed in and to produce hydrocarbons from the above-named separate common
sources of supply, and establishing of a proper allocation factor for allocating the cost of and the production and proceeds from such multi-unit horizontal well to each of the affected units covered hereby; and
Granting such other further relief as may be proper based upon the evidence presented at the hearing
herein.
NOTICE IS FURTHER GIVEN THAT the Applicant in this cause is requesting the following special
relief: The Commission enters an order to be effective as of the date of the execution thereof or as of
a date prior thereto, and to authorize the Applicant or some other party recommended by the Applicant
as operator for the multi-unit horizontal well to test as an exception to the above drilling and spacing
order at the above-described location.

interval of the Mississippian common source of supply is a shale and will present geological testimony
and evidence supporting the request.
[b] Possibly providing that the order be made effective as of the date of the execution thereof or as
of a date prior to the date of execution of the order.

Applicant intends to run casing in the horizontal portion or lateral of the borehole of the multiunit horizontal well and to cement such casing so as to cover and isolate the first and
last perforations in such horizontal portion or lateral.
In addition, Rule 165:10-3-28 provides that no horizontal well shall be closer than 600 feet to an existing wellbore. The Applicant’s proposed location could potentially be closer than 600 feet to an existing
wellbore. Applicant further requests as part of this location exception an exception to Rule 165:10-328.
NOTICE IS HEREBY GIVEN that the application in this cause requests that the order to be entered in
this matter be made effective as of the date of the execution thereof or as of a date prior thereto and
that the authorization and permission requested herein run in favor of Applicant.
