2009 FORE! Wellness Classic
July/August 2009 Volume: 5 Issue: 2 2009 FORE! Wellness Classic The 2009 FORE! Wellness Classic was held on Friday, June 26th with approximately 210 golfers in attendance. The weather was perfect and the atmosphere energized. Guests enjoyed donuts & bagels along with their bag of goodies before heading out to their carts. Lunch at the turn was donated by several area businesses and generously prepared by our volunteers. At the end of play, a barbecue buffet awaited our golfers. Following the meal, trophies and free golf certificates were presented to the 1st place teams on each of the three courses. Some pretty impressive scores secured 1st place for those teams. Trophies were also presented to the course game winners. Tickets were drawn for the 50/50 raffle and Skins were paid out. The day came to an end with the pulling of the final five reverse raffle numbers. Mark your calendars now for the 2010 FORE! Wellness Classic scheduled for Friday, June 25th! Location is to be determined. Heart Run On June 28th, the 3rd Annual Heart Run was held. Amidst the rain drops, devoted bikers took to the roads to raise money for the Cardiac Rehabilitation Unit of Lake Health. Thank you to Denise White for her continued organization of this event. Glitz 2009: Celebrate TriPoint! The 2009 Glitz is less than two months away. The event is being held at Holiday Inn Suites and Conference Center LaMalfa on October 31, 2009. October 31st EMPLOYEE ASSISTANCE - IT’S ALL ABOUT YOU! The Employee Assistance Program exists to aid team members who, from no fault of their own, find themselves in extenuating circumstances. An employee requesting assistance from the EAP can contact Jennifer Shannon in the Human Resources department at x41863 for an application. A committee of fellow team members reviews the circumstances of each application. This is done in an anonymous manner with all name references blacked out. An applicant can request up to $1,000. In 2008, $21,000 was provided to twenty-one employees. There has been a greater need present in 2009 - fifteen team members have received help from the EAP for a total of $14,150. We expect to receive many more requests as the year progresses. The Employee Assistance Fund can not exist without the continued support provided by Lake Health’s dedicated team members. If you have excess PTO, you may donate a specific number of hours which are then calculated into a dollar value and transfered to the Foundation. The EAP is comprised of funds received from PTO and Payroll Deduction, along with 50% of the proceeds from the FORE! Wellness Classic. Please consider a contribution to the Employee Assistance Fund - You may end up helping the person right next to you! Captains Community Connection Once again this year, Lake Health Foundation has partnered with the Lake County Captains in the Captains Community Connection campaign. Area businesses that have a current relationship with the Captains pay a fee to provide each guest at a specific Captains’ game with a coupon which benefits their business in some way. The 2009 Captains Community Connection partners are listed below. Bryant & Stratton College Drs. Levine & Reigle, Inc. Team Members Demonstrating Leadership in Giving Lake Health and Lake Health Foundation are partners in philanthropy. Over the past several years, Team Members have collectively supported a variety of important needs including Employee Assistance. Team Members in crisis due to extenuating, debilitating circumstances were helped when they needed it most through the generosity of co-workers. If you are a past contributor, thank you. For the future, please commit to a contribution to any Lake Health Foundation fund and be recognized as a member of the 2009 Pillars of Commitment. ___________________________________________________________________________ Name (as you wish it to appear in print) ___________________ Phone/Ext. ____________________________________________________ Address City State Zip ____________________________________________________ Email Address Pillars of Commitment Giving Program PTO I would like to donate ______ hour(s) to the 2009 Pillars of Commitment Giving Program.
