PUTTING CONTEST

$5,000 Putt for Cash – $10, 2 Putts to Qualify for Finals
We would like to invite you to participate in our 13th Annual “for
the kids” Celebrity Golf Classic. This year’s event has been
scheduled for Monday May 2, 2016 at Oakview Golf Club located
near Slippery Rock, PA. Included in price is golf with a celebrity,
continental breakfast, lunch, dinner and registration gift. Skill
prizes, hole in one prizes, 100 chance board with prizes, auction,
raffles, putting contest, longest drives, and closet to the pin. You
can purchase a maximum of three mulligans to help with your golf
game. The format of play will be two best balls of four.

The CSI CHANCE “for the kids” Raffle
Attached is the Raffle Board for the 2016 Golf Classic. Each
square is $20. All proceeds benefit CSI programs with
kids. There will be many prizes awarded, making your odds
of winning very good. Just circle the number(s) you wish to
buy.
Prizes will be awarded at the Awards Dinner following the
Golf Classic on May 2, 2016, but winners need not be
present.
Payment may be made in advance through CSI or during
registration the day of the event.
