Hospital providers and staff focus on fulfilling patients’ healthcare needs with always improving, excellent, patient-friendly care. Another part of the solution extends beyond the hospital campus to the community, as NVRH works to enable people to take greater control of their own health. I like puzzles. I like solving puzzles. 1000-piece puzzles are the most interesting. They are even more interesting when you do not have a picture of the completed puzzle on the box cover. Fortunately, we at NVRH have a clear picture in our minds of what the picture of health should look like in our part of Vermont’s Northeast Kingdom. Putting the healthcare puzzle together requires responding to our community’s healthcare needs. NVRH is committed to improving the health of people in our community by ensuring medical services are available locally. Over the last several years and in the coming months, we have recruited a new orthopaedic surgeon, urologist, hospitalist, nurse midwife, emergency services director, general surgeon, internist, pediatrician, nurse anesthetist and several new nurse practitioners. The best approach to providing an improved continuum of care is an integrated one: NVRH is working in partnership with a number of healthcare and community agencies, seeking to improve the patient experience and our population’s health while reducing costs. Obtaining a sustainable floor of funding for the hospital, through a capitated or global payment system, will allow the hospital the flexibility necessary to meet both clinical and community health needs. NVRH is initiating a series of projects and seeks public support for these projects by launching a capital campaign. Some projects have been undertaken and others proposed. They include expansion and renovation of St. Johnsbury Pediatrics, Dan Wyand Physical Therapy, Kingdom Internal Medicine, Operating Room #2, the Birth Center, Women’s Wellness, Cardiac Rehabilitation and the Orthopaedic and Fracture Clinic. Additionally, we seek funding for the hospital’s Community Health Improvement Fund. These projects are designed to improve patient experience and access. A leader in the group of area agencies forming an accountable care community, the hospital is addressing the underlying determinants of health in our communities, such as housing, nutrition, substance abuse and employment. The key to meeting the health needs of the population NVRH serves is found in providing the safest, high-quality patient experience and by enhancing the health of the community at large. NVRH’s past success in unlocking the enigmas of healthcare is directly traceable to the insistence on quality and safety by our hospital’s CEO and management team, as well as the hospital’s talented, dedicated providers, employees, volunteers, and community sup porters…and so it will be in the future. Often, the pieces we cannot see to complete that picture, our teammates can. Those teammates include the people we serve, and lead organizations such as Northern Counties Health Care, Northeast Kingdom Human Services, the NEK Council on Aging, Northeast Kingdom Community Action, and many other organizations. Our challenge is to build an environment of health and well-being in the NEK. That environment should look like the best images we envision for beautiful spring, summer, autumn and winter days in Vermont. And it should inspire us to make the world a better place for our neighbors. This annual report will give you some detailed insight into the type of puzzle-solving work that makes NVRH and its partners so well respected around the state and nation for their innovative work to improve the health of the populations we serve. I believe we have the right people working on a very challenging puzzle. I thank all of our Trustees, employees, volunteers, and community partners for making a profound difference in thousands of lives. Your work is truly meaningful. Enjoy their many stories in this report! I’ve seen care in medical offices change over the years. At St. Johnsbury Pediatrics, our office is now a hub of not only medical activity, but also care coordination. We have two Care Coordinators who work collaboratively with providers and staff. They make sure tests are ordered and completed in a timely fashion. They call team meetings that include the patient and family to make sure everyone understands the plan of care—key to ensuring the best outcomes. St. Johnsbury Pediatrics is also teaming with the NVRH Lactation Specialist to improve breast feeding consult services to new mothers. The Lactation Specialist sees mothers and babies in our office at the time of a scheduled well-baby appointment; no need to make a separate appointment at the hospital. This is a big time saver for new mothers trying to juggle multiple medical appointments. It’s a small thing, but it’s very helpful and has been well received. NVRH and all of its medical offices are working hard to keep medical care centered on the patient. It is a good way to practice medical care and we are committed to this model—providing services in the community, offering centralized care, and constantly thinking of “the little things” that help patients receive only the highest quality care. | 3 Collective Health and social problems are complex. No single organization, no matter how well intentioned or funded, can expect to make large scale long-lasting improvements alone. For this, a community needs to move beyond simple collaboration and take a fundamentally different, more disciplined approach. Experts have named this approach collective impact. Leaders from NVRH and key community organizations have come together to create a common set of goals, share data on important health measures, and pool their talents and resources to improve health and quality of life in our region. Paul Bengtson christened the core group of leaders the “A Team,” setting the bar high for standards and The A Team identified people dually eligible for Medicare and Medicaid as some of our most vulnerable residents. NVRH received a grant from the federally funded Vermont Health Care Innovation Program to support vital services and durable goods not covered by Medicare or Medicaid. These flexible funds purchase items such as eyeglasses, fuel and other utility assistance, handicapped accessible ramps, and a truck wheelchair lift. 4 | Northeastern Vermont Regional Hopsital Impact The A Team is working toward the creation of an Accountable Health Community, a formal organization of diverse members pooling their collective resources to address regional health needs. The A Team doubles as the Unified Community Collaborative for the region’s Vermont Blueprint for Health and other health reform work. While each organization brings its own set of services and programs to the table, the leaders are committed to unifying, aligning, and focusing their strategic plans and visions to create a true accountable health community. Collective Impact: A Better Way to Work Together in a collective impact approach: Funder and implementers understand that social problems and their solutions arise from the interaction of many organizations within a larger system. Progress depends on working toward the same goal and measuring the same things. The staff devised a brochure featuring Ask Me 3™ questions designed to ensure that patients better understand their health instructions and follow through on treatment plans. Ask Me 3 Ask Me 3™ is a patient education program designed to improve communication between patients and healthcare providers, support patients to become active members of their healthcare team, and promote improved health outcomes. The program encourages patients to ask their healthcare providers three questions: 1. What is my main problem? 2. What do I need to do? 3. Why is it important for me to do this? Besides personal behavior, social circumstances such as income, education, and employment; and environmental exposures, such as living in unhealthy housing or lack of places to be physically active are critical to living a healthy life. Source: N Engl J Med 2007;357:1221-8 Research shows that often the conventional healthcare system neglects the significant role that social needs play in the health of Americans, especially our most vulnerable residents. There is strong evidence linking social needs to health and life expectancy. Healthcare itself plays a surprisingly small role in life expectancy. Social circumstances, environmental exposure and behavior are estimated to account for 60 percent of the risk of premature death. Our 2015 Community Health Needs Assessment identified obesity, mental health and substance abuse, and poverty related issues as our top three community health priorities. At NVRH we understand how unmet social needs add cost to the system and adversely impact health. To meet our mission “to improve the health of the people in the communities we serve,” we look beyond the conventional care delivery role of hospitals and healthcare providers to address the societal and environmental factors of poor health. 8 | Northeastern Vermont Regional Hopsital | 2015 Annual Report Looking for a fun and safe place to exercise? Get out and ride, walk, ski or snowshoe on the four season multi-use Lamoille Valley Rail Trail. NVRH matched funds from a Vermont Parks and Recreation Trail Grant to produce a trail map, and construct trail head signs and informational kiosks. LVRT: 17 miles of healthy smiles between St. Johnsbury and West Danville. Upward Bound helps high school students succeed in their pursuit of higher education by offering mentoring, tutoring, and work-study opportunities to prepare them for college. Lyndon State College offers Upward Bound to students in our region. NVRH hosts students like Mckenna Binford (middle) each summer. The major renovation of Kingdom Internal Medicine focuses on improved efficiency for all. Ten exam rooms allow five providers to practice simultaneously. A larger, more comfortable waiting room and reception area offers privacy for patients during check-in and check-out. Additional provider offices and the enlarged nurses’ station, as well as private office space for the behavioral health specialist and chronic care coordinator are important factors in the overall goal of improved patient and staff experience. Ultimately, all improvements help support KIM’s current standing as a Patient Centered Medical Home, recognized by the National Committee for Quality Assurance. Matthew Prohaska, MD, recently joined Dr. Richard Gagnon at the hospital’s Orthopaedic and Fracture clinic at 41 Sherman Drive. Work has started on the renovation and expansion of the clinic,which will include additional space for offices and exam rooms, wider corridors, a conference room, and a remodeled reception area. orthopaedic and fracture clinic 18 | Northeastern Vermont Regional Hopsital | 2015 Annual Report Belle and Sebastian provide unconditional love to patients at NVRH. Their owner, Alice Kitchel remarks that this volunteer work is extremely rewarding! Membership is open to all adults who are interested in NVRH and who are willing to uphold the purpose of the auxiliary. All volunteers of Northeastern Vermont Regional Hospital are automatically members of the auxiliary; membership consists of over 150 individuals. The auxiliary, through fundraising efforts, supports the patients and staff. Annually, the auxiliary gives back to hospital departments through the Wish List. Departments submit items they feel would help patients and/or staff, and the auxiliary votes on the new equipment or programs to fund. This year, $25,000 was given back to NVRH by way of fulfilling the department Wish List requests. Birth Center Cardiac Rehabilitation Program Care Management Chaplaincy Services Community Connections award winner Paul Bengtson, NVRH CEO was nationally recognized twice in 2015. He received the 2015 Grassroots Champion Award, which is given through the American Hospital Association in conjunction with each state’s hospital association. One hospital leader from each state is honored for his or her work over the previous year in effectively delivering the hospital’s message to elected officials, helping to broaden the base of community support for hospitals and tirelessly advocating on behalf of patients, hospitals and communities.
