They also listed At Risk for Fall as an inclusion term. Both terms will be indexed to direct the user to the code V15.88. A specific fall code should heighten health care provider awareness of the importance of falling as a manageable medical problem, and enhance the likelihood that fall risk evaluation and management services would be considered medically reasonable and necessary services, and therefore covered. 16 “Our patients love the system because they can see their progress and receive objective scores...” “The majority of our patients are over the age of 70 and most have balance issues, so the Biodex Balance System SD gets a lot of use here. We like it for balance assistance and training, and find it works especially well with hip and knee replacement patients as well as those with back problems. The display and printouts help us prove to patients that they aren't putting equal weight on one leg vs. the other and that they do, indeed, have balance problems. After an initial balance assessment, we use the system to train each patient to balance properly, which helps avoid additional back and lower extremity problems down the road - as well as decreasing their risk of falling. Our patients love the system because they can see their progress and receive objective scores, which lets them know they are getting better or what they need to do to improve upon the last visit. I would highly recommend the Balance System SD to other facilities - especially those working with elderly populations.” - Clif Rizer, MSPT, ATC, CSCS Formerly of Boynton Sport and Back Physical Therapy - Boynton Beach, FL “The Biodex has improved our patients’ balance dramatically. We’ve been using the Biodex with patients that have Parkinson’s disease, stroke and hip replacements.” Mercy Rehabilitation Hospital in Daphne, Alabama now offers a high-tech approach for identifying risk factors for falls among older adults. The Biodex Balance System provides fast, accurate fall risk screening and conditioning for older adults. In addition, the system offers closed-chain, weight-bearing assessment and training for lower extremity patients. According to Jeff Nobles, director of rehabilitation services at Mercy, “The Biodex Balance System has improved our patients’ balance dramatically. We’ve been using the system with patients that have Parkinson’s disease, stroke and hip replacements.” The CONDITIONING PROGRAM WHAT PEOPLE ARE SAYING ABOUT THE BALANCE SYSTEM SD FALL RISK SCREENING PROGRAM system challenges the neuromuscular system in an effort to minimize disability and maintain functional performance. Clinicians, like Nobles, can assess neuromuscular control by measuring a patient’s ability to maintain dynamic bilateral and unilateral postural stability on a static or unstable surface. The Biodex Balance System SD features a large, color, touch-screen display with visual biofeedback. Six interactive training modes provide both static and dynamic capabilities. Training modes emphasize specific movement patterns or strategies by placing markers anywhere on the screen. ...“the Balance System helps show the patients their instability... IT PROVES IT TO THEM in black and white, which allows us to begin working with them before they fall.” The Gardens Court is a 120 resident facility located on the east coast of Florida, part of the Life Care Centers of America. Besides The Gardens Court, Life Care operates more than 200 skilled nursing, assisted living, retirement, home care and Alzheimer’s centers in 28 states - from Boston, Massachusetts to Hilo, Hawaii. The Garden Court has the Biodex Balance SD, as do sister facilities Darcy Hall Nursing and Rehabilitation in West Palm Beach and Lakeside Health Center also in West Palm Beach. A patient’s score reflects how many times a target is hit. Training modes include maze control training, stability training, random control training, weight shift training, weight bearing and game-like balance training. A fall screening test protocol compares balance test results to age- and gender-dependent data consistent with American Geriatrics Guidelines for Fall Risk Assessment. The athlete knee injury screening test is consistent with leading sports medicine programs. One of Mercy’s rehabilitation patients who utilizes the balance system said, “The Biodex Balance System is simple, versatile and flexible.” The Balance System has successfully been integrated into the rehabilitation /wellness programs. We use the Biodex Balance System in conjunction with the aquatic training and standard rehabilitation protocols as a cohesive rehab program. The Balance System helps show the patients their instability... IT PROVES IT TO THEM in black and white, which allows us to begin working with them before they fall. The Biodex Balance System is used for patients with a number of common diagnoses including fall risk in older adults as well as sports/orthopedics injuries, ankle sprains and fractures, amputation and joint replacement. In addition, the system is useful in treatment of movement and neurological disorders including stroke, head injury, Parkinson’s disease, muscular dystrophy, multiple sclerosis, balance/coordination /postural weaknesses and cerebral palsy. Advantages of the Biodex Balance System over traditional balance therapy include biofeedback, objective documentation, measurement and reporting of postural stability and limits of stability, fall risk assessment and normative data. Another advantage is the system’s ability to offer both static and dynamic capabilities. “The degree of recovery a patient can achieve is greater on a dynamic system than on a static system,” Nobles added. “Traditional low-tech methods can’t compare to the benefits of Biodex.” The Biodex Balance System is available to patients on an inpatient and outpatient basis. Mercy Rehabilitation Hospital, Mobile, AL One of our patients, Josephina, a spunky resident of 65+; had a severely broken leg caused by an accident with two large dogs. She underwent surgery in January 2008 where doctors inserted pins and a plate into Josephina’s leg and released her to The Gardens Court for rehabilitation. Josephina was non-ambulatory for over 8 weeks prior to beginning rehab; she was depressed and anxious to “get better”… to quote Josephina “I can walk again, this machine is amazing, a miracle...it is hard sometimes, but that is good, it makes me work and I can see myself getting better.” The Gardens Court Palm Beach Gardens, FL “Jasen puts the focus of his practice on preventing the injuries that often send people to nursing homes, robbing them of their independence.” At The Beltrone Living Center in Colonie, NY, an independent living facility operated by Colonie Senior Services, a new model for physical therapy has been in business for about four months. Physical therapist Hector Jasen operates HectorPT, a fullservice physical therapy office that offers Beltrone residents, and the public, a fully-equipped physical therapy facility that features traditional PT services, and a Balance System that no one else in the Capital District has to offer. “In-house therapy is unique,” says Jasen, “It’s definitely the new trend in retirement communities. Therapists can follow up with clients, and keep track of changes. Sometimes it does not take much for the elderly to decline and this way, things are taken care of right away and a problem does not escalate,” he explained. Neary and Jasen also stressed that the services offered at HectorPT are available to the public, not just to those residing at the Beltrone. Having an office located within an independent living facility is not the only thing that makes HectorPT unique. Jasen puts the focus of his practice on preventing the injuries that often send people to nursing homes, robbing them of their independence. Jean Myers, a Beltrone resident, has fallen three times, and the last one was a bad one. It left her with polyneuropathy, a loss sensation in her feet, and a lot of fear. Three weeks after that fall, she stands on Jasen’s balance machine, and is learning how to become stabilized and strong. Because she does not have good control of her feet, the ability to see how she stands enables her to learn how to correct her balance, and what proper balance feels like. Jasen will then help her to translate that feeling to real-world walking. Already, after only a couple of sessions with Jasen, Myers said she feels steadier on her feet than she has dependent. Even for those facilities that have a concussion program in-place, Play It S.A.F.E.® adds the vital vestibular and objective balance assessment recently found so important in concussion assessment. BENEFITS of the Play It S.A.F.E.® Concussion Management Program • Easy-to-follow algorithm allows coaches, ATC's and PT's to take full advantage of the program. • Evidenced-based protocol facilitates medically supported “return-to-play” decision process. • Instrumented objective balance testing utilizing the Biodex Balance System SD or the portable BioSway allows quick and objective comparisons of test scores. Step 2: Preseason Baseline Testing The ability to quantify balance and cognitive function in athletes before an injury occurs is an important consideration in a comprehensive athletic program. The objective data provided by computerized assessments provides a performance baseline against which post-injury performance can be compared. • Neuro-physical Testing: Get important baseline balance data on all athletes using the Biodex Balance System SD or portable BioSway. The tests are quick and easy to perform. The Clinical Test for Sensory Integration of Balance (CTSIB) helps to determine which sensory system (visual, vestibular, or somatosensory) an athlete relies on to maintain balance. Research has shown that Mild Head Injury (MHI) populations rely more on visual cues from the environment than an able body population. 4 • Neuro-cognitive Testing: Fully complimentary to any computerized cognitive software testing program including: ImPACT™, Axon Sports CCAT, CNS Vital Signs™, CSMI SportsWare™ Concussion, HeadMinder™, SAC, SCAT2** or any program of choice. Step 3: Sideline Assessment Follow the Play It S.A.F.E.® Algorithm and Sideline Assessment to screen for a concussion and remove the athlete from play if needed. (See inset for details.) • HIPPA compliant post-injury Summary Report shows status, progress and outcomes of tests. • Ongoing education and support with print-on-demand protocols, graded symptom checklist and educational materials round out the program and keep you up to date. NOTE: The CDC Heads Up Concussion Materials are included, but are also available free at http://www.cdc.gov/concussion/headsup/youth.html. **The SCAT2 is a free tool for evaluating concussion in athletes, included with the Play It S.A.F.E.® program. It was developed by experts at the 3rd International Consensus on Concussion in Sport in Zurich, Switzerland in November 2008. It is also available online. DISCLAIMER: The information provided is not intended to be a substitute for professional medical advice but as a guideline to assessing athletes following a concussion. Always seek the care of a physician or other qualified healthcare provider with any questions or concerns you may have about a medical condition. If there is any question/concern about the athlete’s status then recommend not returning to play. 18 • Fully complimentary to any computerized cognitive software testing program including: ImPACT™, Axon Sports CCAT, CNS Vital Signs™, CSMI SportsWare™ Concussion, HeadMinder™, SAC, SCAT2** or any program of choice. Step 4: Follow-up Evaluation • Repeat neuro-physical balance testing and neurocognitive testing in the days following injury to track recovery using preseason baseline testing as a reference. Step 5: Return-to-Play Decision • Doctors, athletes, parents, coaches and administrators feel confident as decisions are based on best practices objective data and clinical protocols. Objective balance testing is critical Research shows that athletes demonstrate decreased stability up to three to five days post injury. This can be the result of ineffective use of one or more of their sensory systems.1 Signs and symptoms will vary following head trauma. With some individuals, signs and symptoms may resolve immediately after a mild head injury (MHI) while others have persistent symptoms. In either case, the recovery period related to MHI appears to coincide with recovery of postural stability as well as normalization of eye-head coordination and return of cognitive function. 1 Recently, there is strong evidence demonstrating the impact of balance deficits on functional performance and increased risk of re-injury. The only mobile Unweighing System that provides quantifiable, dynamic partial weight-bearing therapy without electric motors, weights or air compressors; plus a universally fitting support vest. 26 UNWEIGHING SYSTEM Allows for functional vertical displacement and functional pelvic rotation or stabilization during partial weight-bearing therapy FEATURES: • Dynamic suspension allows for natural vertical displacement of center of gravity • Patented off-loading mechanism - maintains constant force; simply dial in amount to off load • Single-point suspension permits functional pelvic rotation • Unobstructed open access Offset design provides full access for easy patient interaction • Attachment rings - on support vest allow single or two-point pelvic stabilization when desired • Low-effort manual operation - no power cord, no weights, no air compressor • Adjustable and removable patient handrails accommodates diverse patient populations and allows for easy treadmill access • Unweighing Harness - fits a variety of patients • Adjustable height - can be assembled at reduced height for use in rooms with 8-foot ceilings • Adjustable suspension accommodates children and adults • Therapist seats - provide a place to sit during assistive exercise and provides a safer environment for therapist interaction The Biodex Unweighing System enables partial weight-bearing therapy to be conducted with the assurance of patient comfort and safety, and with convenient access to the patient for manual assistance and observation. Unlike the simple patient lift devices, Biodex Unweighing System incorporates a dynamic suspension system that NEW accommodates the vertical UNWEIGHING HARNESS displacement of the center of gravity that occurs during normal gait. Biodex’s patented off-loading mechanism maintains constant force; simply dial the amount to off load. When walking, an individual’s center of gravity typically moves through a vertical range from one to four inches, depending on walking speed. The Biodex system accommodates up to four inches of vertical displacement while maintaining a consistent level of unweighing. SPECIFICATIONS: • Dimensions: 41.5" w x 50" depth x 106" h (105 x 127 x 270 cm) accommodates patients from pediatric up to 6’11" (210 cm) on a standard treadmill; alternate height setting for facilities with low ceilings: 93.5" h (241 cm) accommodates patients from pediatric up to 6’2" (188 cm) on a standard treadmill Another component of normal gait is pelvic rotation. Twopoint suspension systems have a tendency to restrict rotation on the horizontal plane. They respond to pelvic rotation similar to the way a playground swing undulates back to neutral after being twisted. The singlepoint suspension of the Biodex system allows unrestricted pelvic rotation. Patients can train for weightbearing ambulation without compromising proper gait kinematics. Should pelvic stabilization be desired, retention cords can be attached to special attachment rings on the sides of the support vest, secured to the frame, and adjusted for the specific degree of pelvic stabilization. The ability to allow rotation has other advantages. When training with a treadmill, single-point suspension permits functional pelvic rotation and versatility when walking, sidestepping, retro-walking and turning. With a Biodex Unweighing System, the patient can change direction without repositioning the entire patient support system. Patient and clinicians alike know that the comfort of the harness can make or break the therapy session. By sharing the offload burden through the hip straps, gluteal fold straps and the lumbar-thoracic harness, patients are more comfortable using the Unweighing System for extended periods. To solve the common slippage problem associated with traditional vests, the NEW Biodex Unweighing Harness employs combined elements from parachutes, rock climbing, safety and rescue, and even kite sailing gear. A torso wrap, worn beneath the harness, is a simple solution to negate any slipping from clothing issues while providing support and comfort. This one single harness accommodates children with a 25" torso up to adults with a 60" torso. WHAT DO YOU HAVE WHEN YOU COMBINE THE GAIT TRAINER WITH UNWEIGHING? THE BIODEX GAIT TRAINING SYSTEM + “One of the major considerations for stroke patients is to develop symmetry of walking in a functional velocity. The Gait Trainer encourages the patient toward symmetry. My patients have benefited from the symmetry by the use of the target settings, which prompts them to stretch out with the unaffected leg. Patients capable of evoking an overlearned or central pattern generator response have a much easier time once they approach a one cycle per second time because it does not require focused concentration. In essence, gait training may actually be a process of creating a feeling of security and thereby reduce the tendency to walk slowly. This feeling of confidence, without the struggle to hold TREADMILL TRAINING “One example of neural plasticity is the improvement seen in locomotion of patients with stroke or spinal cord injury through partial body weight supported treadmill training. This rapidly developing form of therapy is based on two fundamental observations. First, that the spinal cord of all vertebrates, including man, contains neuronal circuitry that encoded the locomotor pattern, and which can be activated by stimulation at supraspinal, suprasegmental, or segmental levels or even by intrathecal drug administration. The activation of locomotor circuitry does not therefore require momentto-moment control of individual muscles by the brain. The second observation was that repeated activation of the central pattern generating circuit by treadmill training could result in long lasting enhancement of locomotor output. In human trials, this resulted in improved locomotion months and even years after training was discontinued.” Excerpt from: The Role of Neural Plasticity in Neurorehabilitation Michael E. Selzer, MD, PhD., Associate Dean of Graduate Education University of Pennsylvania Medical College,USA = themselves up, permits them to focus on making even steps. Meanwhile the therapist can increase belt speed as step length and speed of the affected lower limb permits. Attaining a cycle velocity that is faster than 1.6 seconds typically evokes a more symmetrical pattern of leg motions. The last phase involves decreasing the unweighing but maintaining the velocity at an optimal level. Should the gait become asymmetric again, the unweighing could be increased, but the velocity maintained. Make sure the patient does not incline the trunk forward to increase velocity.” Arthur J. Nelson, Ph.D, F.A.P.T.A., Physical Therapy Associates VA HOSPITAL STUDY* SUPPORTS THE CONCEPT OF THE BIODEX UNWEIGHING SYSTEM “If the system does not allow for vertical movements, gait is markedly distorted. The subject may still be able to move on the treadmill, since the walking surface is moving below him, but this unnatural gait is not the goal of therapy.” SPECIFIC RECOMMENDATIONS: - Support up to 40% of patient’s body weight - Allow for 5.5 cm of vertical movement to permit normal gait - Report reliably to ensure the correct degree of unloading - Enable easy adjustment to the amount of body weight support as the subject improves or fatigues during a training exercise Biodex satisfies VA Hospital Recommendations. The stepthrough frame, simple easy to see and use “Smart” display and comfortable, robust design make the BioStep the obvious choice for cross training. The Biodex BioStep Helps Patients and their Therapists Achieve Multiple Rehabilitation Objectives If there is one piece of equipment for all your patients... this is it. The BioStep’s fluid, elliptical motion is forgiving on the knees, ankles, hip and lower back… a natural closed-chain low-impact functional exercise. The BioStep reduces joint impact because the user is not required to lift and strike their feet on the walking surface. By combining lower extremity motion with upper body, the BioStep provides a total body workout in both forward and reverse directions, thereby involving different muscle groups. The Clinical Pro model includes isokinetic resistance, which is totally accommodating and proven to be the most effective resistance for rehabilitation and strengthening. Ideal for Older Adults Low-impact total body exercise is ideal for senior fitness. The low-impact elliptical motion of the BioStep reduces the jarring impact often associated with the stop-start motion so common with other recumbent steppers. The BioStep motion also helps strengthen the muscles important to maintaining function; those muscles specifically associated with mobility, walking and balance. APPLICATIONS: Wellness and Fitness Clubs, Orthopedic/Sports Medicine, Cardiac Rehabilitation, Neurorehabilitation, Senior Centers and more Great for Cardiac Rehabilitation BioStep incorporates both the upper and lower extremities into the cycling motion; patients experience less local muscle fatigue. This allows similar heart rates to be achieved at lower rates of perceived exertion when compared to traditional exercise ergometers. Blood pressures can be checked while the patient continues to exercise by simply releasing one arm while the other three extremities comfortably continue to perform the exercise. Patients who may experience leg claudication during exercise can easily reduce effort in the involved leg, while keeping heart rate elevated, by transferring workload to the non-involved extremities. Recent study validates BioStep as a new exercise device that older adults will find very tolerable. In a recent study by Mendelsohn, Connelly, Overend and Petrella, published in the Journal of Aging and Physical Activity, April 2007, the BioStep was shown to be a valid and reliable aerobic exercise device. If you are looking for an effective device for exercise testing and training in an older population… look no further. PubMed ID #17556784 Wellness/General Aerobic Conditioning If there is one piece of equipment for general aerobic conditioning of your adult population, this is it. BioStep provides a zero impact, safe and comfortable total body workout in both forward and reverse directions.
