detailed on the back, is available to all members and eligible dependents and
cannot be combined with any other discounts.

It’s convenient and easy! The EyeMed Access network provides you with the
choice of thousands of private practitioners as well as optical retailers including
LensCrafters, Pearle Vision, Sears Optical, Target Optical, and JCPenney Optical
locations. Simply cut out this card and present at the time of service.

1. Locate a provider by visiting
healthpartners.com and selecting the Members
section. Next click on “Health Resources” and
then on “Healthy Discounts.”
2. Schedule an appointment with a
simple phone call. Or stop by one of the many
providers who offer walk-in appointments. Inform
the office that you are a HealthPartners member
with an EyeMed discount plan when you wish to
use your discount.
3. Present the ID card below when
you arrive so the EyeMed provider knows you
have a HealthPartners vision discount.

Member will receive a 20% discount on those items purchased at participating
Providers that are not specifically covered by this Discount design. The 20% discount
may not be combined with any other discounts or promotional offers, and the discount
does not apply to EyeMed Provider’s professional services, or contact lenses. Retail
prices may vary by location.
Through the EyeMed Discount Plan, members also save $5 off a comprehensive exam
and $10 off a contact lens exam. This cannot be combined with any other discounts or
coverage.
Limitations/Exclusions:
• Orthoptic or vision training, subnormal vision aids, and any associated
supplemental testing
• Medical and/or surgical treatment of the eye, eyes, or supporting structures
• Corrective eyewear required by an employer as a condition of employment, and
safety eyewear unless specifically covered under plan
• Services provided as a result of any Worker’s Compensation law
• Discount is not available on those frames where the manufacturer prohibits
a discount

Members are responsible for paying the discounted amounts directly to the EyeMed
Provider at the time of service. This is a discount plan, not an insured benefit.
