If your favorite dentist isn’t in our network, you may not need to switch – ConnectiCare
welcomes new providers, just ask yours to apply!
What’s covered?
ConnectiCare Dental Plans are designed to cover the important preventive care adults and
children need for good oral health, along with a variety of services to repair or restore.
Each plan is different, based on the plan design your employer chooses. See your benefits
administrator or review your Certificate of Coverage for specific coverage and exclusions
in your company’s plan.
Getting Care
■ In network
Simply make an appointment with a participating provider and go!
•Choosing network providers means no paperwork, no claim forms
•No referrals required – you have open access to participating specialists
•Pre-determination of coverage may be recommended for some procedures,
but your dentist handles everything
■ Out of network
You’re also covered for services if you see a dentist outside the network, but you will pay
a larger share of the costs. You’ll need to pay the dentist at the time of service, and then
submit the dentist’s bill for reimbursement of covered expenses. Not all procedures are
covered, and pre-determination of coverage may be recommended for some procedures.
Check with your benefits administrator or review your Certificate of Coverage for details.
Annual Maximum Look Back Program
This program allows you to access additional benefit dollars, helping you to maximize
your benefits.
