Member Agencies provide the primary source of operational revenue through a property tax levy available to park districts and municipalities who have entered into a cooperative agreement or through a general fund. Mission Statement The Special Recreation Association of Central Lake County provides recreation programs and services to individuals with special needs or disabilities, and their families who reside within the boundaries of Member Agency communities. Vision Statement To be a respected leader through the provision of high quality community based recreation experiences that enrich the quality of life for our participants and their families. Association Goals 1. SRACLC will implement and maintain a broad base of recreation programming encompassing sports, cultural arts, social, and leisure independence. 2. SRACLC will assist its cooperative Member Agencies and the populations they serve. 3. SRACLC will operate and provide recreation services utilizing sound fiscal management. 4. SRACLC will comply with federal, state, and local mandates. 5. SRACLC will actively market and promote its services to the community. 6. SRACLC will maintain and assist its Foundation and its fundraising efforts. SRACLC GENERAL INFORMATION ADA Compliance The Special Recreation Association of Central Lake County intends to comply with the Americans with Disabilities Act (ADA) which prohibits discrimination in the provision and inclusion of programs and services to individuals with disabilities. As stated below, some programs, because of the inherent nature of the activity, are suggested for specific disability groups. If you or any person wishes to participate and is not among the suggested groups, please call SRACLC. Every reasonable effort will be made to include the individual in the program. If you have any questions regarding the ADA or if you feel you have been discriminated against in the inclusion of programs, or the provision of services, please contact John Buckner at (847)816-4866. A procedure is available for you to register your complaint and for its prompt resolution. Atlanto-Axial Instability* Individuals with Down Syndrome are at risk of having a condition known as Atlanto-Axial Instability (AAI). It is an SRACLC policy that any individual with Down Syndrome is required to have a doctor’s written note on file stating the person is free of AAI if that person wishes to participate in any program with a potential to cause undue stress upon the neck. Programs with the * sign are identified as those requiring the AAI test. SRACLC will also be offering other programs that are not by the inherent nature of the activity at risk for undue pressure upon the neck. However, the nature of play is active participation. Please take this into consideration when enrolling your child into a program and inform staff if the condition is present. Dietary Restrictions While SRACLC understands certain participants have food allergies and/or dietary restrictions, we cannot accommodate all needs. Please refer to the program description and make the necessary arrangements for these needs unless otherwise noted in the program write up. Behavior Expectations SRACLC encourages and promotes the concept of fun for everyone. However, certain rules have been established to ensure the safety and enjoyment of all people involved. SRACLC participants are expected to demonstrate appropriate behavior during programs. The Association’s basic behavior code insists that participants shall: • Show respect to each other and staff. • Abstain from using foul or abusive language. • Refrain from inflicting bodily harm to any individual. • Demonstrate respect to equipment and facilities. • Have control of bowel and bladder (diapers and catheters are acceptable). A caring, positive approach will be utilized regarding the use of any disciplinary methods. Additional or individual behavior management plans may be developed on the advice of parents, guardians, SRACLC staff, or other professionals. SRACLC reserves the right to dismiss a participant if the above noted procedures are not adhered to, and/or, parent/guardian response to a problem situation is not attempted. Each situation will be evaluated individually and on its own merit. Annual Information Form SRACLC requires all participants to complete a yearly Annual Information Form (AIF). The form provides staff with valuable information which allows SRACLC to conduct its activities in the safest possible manner and to meet the basic needs of participants. If you are new to SRACLC or if the agency receives a registration form and does not have an AIF on file, one will be sent to you to complete and return. If at anytime the form needs to be updated (change in medication, behavior, etc.) please call the SRACLC office. All information is kept strictly confidential and enhances successful participation. Thank you for your cooperation. Freedom of Information Act Request Methods Public May Request Information and Public Records Requests for information should be directed to the Freedom of Information Act (FOIA), Officer(s) of the Association. All FOIA Officer(s) can be reached at the SRACLC Office, 290 Oakwood Rd., Vernon Hills, IL 60061, Phone (847)816-4866. Requests can be submitted in writing or via e-mail to the address listed above. A request form is also available for your convenience at the SRACLC Office. For black and white, letter or legal sized copies, the first fifty pages are free and additional pages will cost $0.15 a page. For color copies or abnormal size copies, SRACLC can charge the actual cost of copying. The Association may require the requester to disclose whether the request is for a commercial purpose or to decide whether to grant a fee waiver. NOTE: SRACLC’s program goals and objectives are available for review. Please contact the office, we would be happy to provide a copy. 5 GENERAL INFORMATION SUMMER 2015 Medication Dispensing SRACLC staff may dispense medication only if the following procedures have been completed. • Permission for Medication Dispensing form must be completed for every program season and signed by parent/guardian. • Medication must be given to SRACLC staff with the original prescription order (may be a copy) or the original bottle/container with full doctor ordered instructions. Medicine must come in individual dosage amounts as prescribed. • Dosage, time, and potential side effects are clearly noted. • All of the above information must be delivered to the SRACLC Office at least two days prior to the onset of the program. • DO NOT send any medication information to your child’s school with expectations that it be given to SRACLC. • SRACLC staff may under no circumstances perform an injection or any other medically invasive procedure. • SRACLC staff will not administer medications that are not approved through the FDA. • Please call the office; (847) 816-4866; if you have any questions. Late Pick Up Policy Calculated per Programming Season. • Two (2) verbal reminders will be given. Reminders will be documented through a signature of a parent or guardian. • Five (5) minutes will be the maximum amount of time allowed after the advertised conclusion of the program. • After five (5) minutes and after any two (2) documented warnings, $5.00 per each accumulated five (5) minutes will be assessed. The late party will be invoiced and the bill will be treated like any other late program fee. Risk Management 6 SRACLC is committed to provide its recreation programs and services in the safest possible manner and to keep the safety of all individuals at the forefront. Participants and parents must realize there is some degree of risk which is inherent in all recreation activities. In an effort to minimize exposure to unsafe conditions, SRACLC conducts an ongoing risk management program and strictly enforces that safety procedures be adhered to by all participants and staff. Please be aware that SRACLC does not carry medical or accident insurance for injuries sustained during programs or services. The cost of such would make participant fees prohibitive. Each person registering himself/herself or a dependent in a SRACLC program should review and maintain their own insurance coverage. It must be noted that the absence of individual or family medical insurance coverage does not make SRACLC automatically responsible for the payment of medical expenses. The agency providing liability coverage for SRACLC requires the mandatory execution of a Waiver and Release form for all programs. Thank you for your cooperation. Transportation Policy The following procedures are to be followed to maintain safe transportation of SRACLC staff and participants. • All passengers must be seated with seat belts fastened immediately upon entering the vehicle. Wheelchairs must be securely belted and fastened. The vehicle will not depart until these procedures have been followed. • SRACLC staff are responsible for determining whether or not an individual may be transported safely due to behavior or health concerns. • SRACLC staff will not leave any person unattended after any program unless written permission has been received from said individual’s parent/guardian. • All wheelchairs must be in proper working condition. • Persons riding Amigo-type wheelchairs must transfer to a van seat. • No object (i.e. hands, head, trash) is to protrude or be thrown from the vehicle at any time. Windows will be opened upon permission from SRACLC staff. • SRACLC does not provide door-to-door transportation but will offer drop off and pick up locations for selected programs. • If three (3) or more people from one community will utilize an additional drop off and pick up location, SRACLC will try to accommodate this request. • SRACLC reserves the right to refuse or discontinue transportation services. Please realize that these procedures have been implemented to ensure the safety of all participants and staff. Thank you for your cooperation. • SRACLC vehicles will only wait five (5) minutes after the scheduled time before departing to its next location. SRACLC GENERAL INFORMATION Financial Aid Fund The commitment of SRACLC and the member agencies is to provide all individuals considered “residents” the opportunity for participation in SRACLC programs. A Financial Aid Fund has been developed to assist individuals who may be experiencing financial difficulty. All personal information required for financial aid procedures is kept strictly confidential. Per season, a maximum of five (5) programs, plus any Special Olympics training programs and Day Camp, are eligible for assistance. Financial Aid must be requested in writing each program season. Individuals seeking financial assistance should contact John Buckner for more information regarding the process and deferred payment options. Deferred Payments Deferred payments are an option only if a financial aid form is filled out and payments are completed by the end of each given season. If these conditions are not met, individuals will not be eligible to register for any programs until the balance is paid in full, and future deferred payments will not be allowed. Residency Information Resident: Individuals residing within the legal boundaries of a SRACLC Member Agency (Park Districts of Grayslake, Mundelein, Vernon Hills and Villages of Hawthorn Woods, Lake Zurich, Libertyville, and Lincolnshire) are considered “residents” of the Association and pay the in-district fee. Residents receive priority when registering for all programs through the registration deadline. Non-Resident: Any individual whose primary residence is outside of a Member Agency’s legal boundary is considered a non-resident. Types of assistance that SRACLC can provide are: • In-service training for staff, including activity adaptations, behavior management techniques, and disability awareness. • Recruitment of additional staff, if needed. • Consultations or observations at programs. If there are any questions, please contact John Buckner, SRACLC Director, at 847-816-4866. 7 SRACLC SUMMER DAY CAMPS $50.00 Credit All campers enrolled in Sessions 1, 2, and 3 (all six weeks) will receive a $50.00 gift certificate for SRACLC fall programming. The credit must be used towards youth or teen special events. All credits will expire on 9/30/15. Families receiving SRACLC financial aid towards summer camp will not be eligible for the credit. CAMP FEES WILL NOT BE PRO-RATED. Please see page 12 for additional camp details. Counselors plan fun and exciting activities such as high energy group games, themed crafts, water play, singing and dancing, morning field trips, and so much more. Campers will go swimming every Tuesday and Thursday afternoon as well as enjoy outdoor play each day. We will head to Big Bear Lake in Vernon Hills for a picnic lunch, carnival games, camper awards, and a fishing derby. Campers from ALL camps are welcome to attend with their families. This is a great time to get to know the other SRACLC families. Please watch for your invitation and RSVP as Family Day approaches. Campers and families enrolled in sessions 1, 2, or 3 will have the chance to meet their counselors and explore the camp site prior to their first day of camp. Camp Staff The SRACLC Day Camp staff is composed of recreation professionals, educators, teacher’s aides, college students studying related fields, and high school students. The camp site has a supervisor who oversees the day-to-day operations of camp with staff who are certified and/or trained in First Aid and CPR. Thank you for understanding. 12 Once a child/teen is registered for summer camp, SRACLC will send additional information to each family via email. This will include a Parent Camp Guide, Directions & Maps, Authorization for PickUp Form, Medication Dispensing Information and Waiver, Parent Questionnaire, and Annual Information Form. These forms will also be available at www.sraclc.org and can be mailed upon request. SRACLC’s core day camp (Islanders, Explorers, Trekkers, and Troops) is a six week MondayThursday camp. The camp will be offered in three, two-week sessions for the convenience of summer planning. This camp will maintain a consistent location, schedule, and staff. Firecrackers Camp, Friday Field Trips, and August Adventures are separate mini camps. Please note the different drop-offs, pick-ups, and locations. SRACLC encourages and promotes the concept of fun for everyone. However, certain rules have been established to ensure the safety and enjoyment of all people involved. SRACLC participants are expected to demonstrate appropriate behavior during programs. No jeans or jewelry will be allowed. *If you are in a team sport and you do not participate in the District or ITRS Competitions, you will not be allowed to compete at State Level Competitions unless absence was due to illness and accompanied by a doctor’s note. *Any individual or team that makes it to the Sectional or State Level Competitions will be asked to pay an additional fee per athlete. The fee will be dependent upon the cost of transportation, lodging, and supervision and will be the same for every athlete attending the competition. *SRACLC will be enforcing the maximum numbers of players for team sports. This will ensure that all team players will get adequate playing time during the game. B) If a program is cancelled by the Association after the second meeting, a prorated refund will be given on the basis of meetings not held. All requests for refunds must be submitted in writing. Medical Reasons: A full or prorated refund will be granted if a medical condition prohibits participation. All program refund requests are subject to a $5.00 per session, Day Camp and Active Adult are subject to a $50.00 service charge. The service charge is waived if the refund request is made prior to a given program registration deadline. Any service charge will be applied to an unpaid balance. No refund will be given for any special event or program activity that requires a pre-paid purchase by SRACLC unless an alternate can be placed. There can be no exceptions. No refund will be given after the registration deadline unless the program is deemed inappropriate for the participant. Please see above for exceptions. SRACLC realizes that inappropriate behavior or conduct may occur. However, if the behavior persists and is deemed hazardous to any individual, the SRACLC Director reserves the right to suspend participants with a prorated refund given. The Special Recreation Association of Central Lake County continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for this program/activity must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs.
