No promotional rates or other discounts, including internet rates, apply to this offer. The rates, seasonal dates and hotels are subject to change at any time. FPT guarantees hotel rates only for confirmed reservations that are paid in full at the time a hotel increase occurs. Once FPT has been informed that a hotel rate increase will occur, FPT is under no obligation to provide a grace period so that unpaid reservations can be paid in full in order to qualify for a hotel rate guarantee. All taxes (hotel and airline) and fuel surcharges, if applicable, are subject to increase at any time without notice. FPT reserves the right to invoice such increases if and when they occur, regardless of whether or not a reservation is fully paid. ADVANCE NOTICE REQUIREMENTS & AVAILABILITY You are required to provide just one travel date choice, in compliance with these Terms & Conditions. FPT must receive the correctly completed Reservation Form by regular mail or by email at least 45 days before your requested departure date. Please provide more advance notice, if possible. All reservations are subject to availability. When requesting to travel to Cancun, we suggest providing at least 90 days advance notice. When requesting to travel to any destination during President’s Birthday Holiday in February, during March or during Thanksgiving Week, be aware that availability sells out months in advance, so plan accordingly. Please be aware that air availability for travel to Hawaii during the months of June, July and August is very limited. Correctly completed Reservation Forms are processed based on FPT’s availability of coachclass airline seats and hotel rooms as per our contractual rates. Since we have a “free sell” relationship with our suppliers (air carriers, hotels), we will process all correctly completed Reservation Forms as “available” until a supplier informs us that certain dates are “unavailable” under our contract. Availability for specific dates is not confirmed with the supplier until we receive your payment (deposit or full payment). If, at the time of confirmation, our supplier informs us that your requested travel date is not available under our contract, we reserve the right to offer alternate travel dates or alternate destinations/hotels, and, in that rare event, we will contact you in as expedient a manner as possible. BLACK-OUT DATES Travel is not permitted during any Black-Out period and may not begin, end or overlap these dates: • All Destinations: December 16th-January 3rd • All Destinations except Europe: Palm Sunday through the Sunday after Easter (4/13/144/27/14, 3/29/15-4/12/15, etc.) • Europe Destinations: Two weeks before and after Easter (4/6/14-5/4/14, 3/22/15-4/19/15, etc.) • Europe Destinations: June 16-July 5, Aug 27-Sept 14, Jan 4-Jan 14 • Special Events, including but not limited to: Race Weeks in Daytona Beach, Bike Weeks in Daytona Beach, Thanksgiving in New York, New York Marathon, Boston Marathon, Mardi Gras and Jazz Festival and Essence Festival and French Quarter Fest in New Orleans, Ironman Triathalon on the Big Island of Hawaii (October), Air/Sea Show and Boat Show in Ft. Lauderdale DAY-OF-THE-WEEK DEPARTURE/RETURN RESTRICTIONS • Cancun and the Dominican Republic do not permit flights on Friday and Saturday • Hawaii and Europe are offered based on departure and return flights falling on a Monday, Tuesday, Wednesday or Thursday. You may request to depart/return on Friday, Saturday or Sunday but will be subject to a flight surcharge of $50-$100 per person per weekend flight • Las Vegas hotels do not permit Saturday check-in *In general, weekend flight availability is limited under this program *Requests to extend flight beyond minimum number of nights required may result in a flight surcharge if FPT’s cost of the airline tickets is more than if the extended flight request had not been made. HAWAIIAN ISLANDS HOTEL CHOICES, INTER-ISLAND AIRFARE, OUTER ISLAND FLIGHT SURCHARGE AND STOP-OVER OPTION IN LAS VEGAS AND/OR SAN FRANCISCO Refer to the “Airline Ticket Option” chart on page 22 to determine the total number of nights hotel accommodations you must purchase through FPT when traveling to Hawaii. You may select from the hotels on page 15 for 1, 2 or 3 of the total number of nights required, and then select from the hotels on page 16-17 for the remaining number of nights. OR, you may spend all of your required nights at one or more of the hotels shown on page 16-17. Airfare is provided to/from Honolulu on the island of Oahu. If you select hotels on any other islands, you must purchase the inter-island airfare through FPT at the prevailing fare ($99-$129 per person per hop at the time of printing). Or, if applicable, FPT will try to book a flight directly to/from the Outer Island from/to the mainland, in which case you will be charged an “Outer Island Flight Surcharge” of $75-$100 per person per Outer Island flight segment. The “Stop-Over Option” permits you to “stop-over” in San Francisco on your way to Hawaii and on your way home from Hawaii. With the “Stop-Over Option”, you may substitute up to a total of 3 of your required number of nights to stay in the “Stop-Over” destination by selecting from the San Francisco hotels shown on page 15. Your remaining number of nights in Hawaii must then be selected from the hotels on page 16-17. (A maximum of 3 nights total is permitted at the hotels shown on page 15, regardless of where the hotels are located.) OR, you may spend all of your required number of nights in Hawaii and extend your vacation with a “Stop-Over” in San Francisco in which case you may purchase the extra nights hotel accommodations in your “Stop-Over” city through FPT or go on your own. All “Stop-Overs” must be requested on your original Reservation Form submission and will be charged a “Stop-Over Flight Surcharge” ($129 per person per stop-over at the time of printing), payable to FPT. EXPIRATION DATE This offer expires 2 years from the Issue Date (the “Issue Date” is the date the original Sponsor business gives this brochure to the traveler). All travel must be completed by the expiration date. HOW TO REQUEST YOUR RESERVATIONS Complete one Reservation Form for each set of two round-trip airline tickets OR “Hotel Discount Option” you are requesting. Each Form requires the purchase of one hotel room for the minimum required number of nights if you’re requesting the “Airline Ticket Option” or for up to the maximum allowed number of nights if you're requesting the “Hotel Discount Option”. Be sure your requested travel date does not begin, end or overlap any Black-Out Date (see the list of Black-Out Dates above). Be sure your requested travel date complies with any DayOf-The-Week restrictions associated with your requested destination (see list above). Submit your correctly completed Reservation Form to FPT so that we receive it no less than 45 days before your requested departure date. You may initially submit your Form either way below (do not send it both ways): Regular Mail to: First Priority Travel, P.O. Box 290125, Port Orange, FL 32129-0125 Email: go to www.fptravel.com, click on “Request A Reservation”, enter the Package Code and Serial Number found on the Reservation Form, complete the online Form and submit it. If you do not complete the Form correctly (in compliance with the Terms & Conditions), or if we do not receive your Form with the number of days advance notice required, the Form will be returned to you for correction. If we know your requested travel date is not available, we will return the Form to you for an alternate choice. When you resubmit the Form for any reason, you must, again, comply with all Terms & Conditions, including the advance notice requirement. 25 T ERMS & C OND I TI ONS DEPOSIT AND PAYMENT REQUIREMENTS Do not send any money when you submit your Reservation Form. When we receive your correctly completed Reservation Form, we will determine initial availability. Assuming your request is available, a Quote/Booking Statement will be generated and sent to you in the same way your submitted your Form (by regular mail or by email). The Quote/Booking Statement will outline the travel components intended to be confirmed upon receipt of your payment and will specify the payment requirements. You will be required to pay the full amount shown on the Quote/Booking Statement, so that FPT receives your payment within 15 calendar days from, and including, the date the Quote/Booking Statement was generated, under the following circumstances (please read the “Payment Form” section below): • • • • • If you are requesting to travel within 60 days or less If you are requesting to travel to Mexico or Europe or Dominican Republic If you are requesting to travel to Hawaii during June, July or August If you are requesting the “Hotel Discount Option” If you are requesting to travel during President’s Birthday Holiday in February, or during the month of March or during a holiday period including Memorial Day, July 4th, Labor Day and Thanksgiving Week • If flight availability is very limited, as determined by FPT Otherwise, you will be required to pay a deposit of $125 per person, so that FPT receives it within 15 calendar days from, and including, the date the Quote/Booking Statement was generated. The balance must be then received by FPT no less than 45 calendar days prior to departure (please read “Payment Form” section below). In either case, the deposit or full payment must be received by FPT, in acceptable payment form, by the due date or your reservation is subject to automatic cancellation. If you submit your Reservation Form by email, you must mail the corresponding original, numbered Form with your deposit or full payment or your reservation is subject to automatic cancellation. PAYMENT FORM All deposits and/or payments made to FPT must be in the form of a cashiers check or money order, made payable to First Priority Travel. Payment will also be accepted by bank wire (subject to an additional wire fee of $20 payable to FPT and charged to us by our bank); please call our Customer Service department at 800-404-0177 for bank wire instructions. 100% of consumer payments are deposited into a separate Trust Account. REQUESTS TO TRAVEL WITH ANOTHER PARTY You may submit two Reservation Forms together, requesting to travel together. However, you may not submit more than two Reservation Forms with the same requested departure date and destination. VACATION REVISIONS Once a Reservation Form has been received and processed, the primary (first-named) traveler’s name may not be changed. Other revisions may be requested, subject to availability and compliance with all Terms & Conditions. Change requests must be made in writing and sent to: First Priority Travel, P.O. Box 290125, Port Orange, FL 32129-0125. A change fee of $25, in the form of a cashiers check or money order, must accompany the written change request. Written change requests received by FPT within 15 calendar days from the date the original Quote/Booking Statement was generated are not subject to a change fee, on a onetime basis. Once airline tickets have been ordered, changes are no longer permitted and cancellation policies will take effect. CANCELLATIONS & REFUNDS All requests and reservations are subject to automatic cancellation if the deposit or full balance payment, in acceptable payment form, is not received by the due date. If a cancellation occurs for this reason, you may re-instate your ability to request a different travel date, in compliance with all Terms & Conditions and subject to availability, provided you make a request in writing and submit the written request with a rebooking fee of $25, in the form of a cashiers check or money order, to: First Priority Travel, P.O. Box 290125, Port Orange, FL 32129-0125. If you have paid a deposit but fail to pay the balance by the due date specified, all reservations are subject to automatic cancellation in which case, your initial deposit, less a $50 cancellation fee, will be automatically refunded within 14 days.. If a cancellation occurs for this reason, you may re-instate your ability to request a different travel date, in compliance with all Terms & Conditions and subject to availability, provided you make a request in writing and submit the written request with a rebooking fee of $25, in the form of a cashiers check or money order. All cancellations of a confirmed reservation must be made in writing and may be subject to cancellation penalties based on the work-day date FPT receives the written cancellation request (weekends, holidays, and Dec 23rd-Jan 3rd are not considered “work-days”). Written cancellation requests received before FPT has ordered airline tickets may request a refund, less a $50 cancellation fee, or may request an alternate travel date, in compliance with the Terms & Conditions and subject to availability, in which case previously paid money, less a $50 cancellation fee and less a $25 change fee, will be applied. Written cancellation requests received after FPT has ordered airline tickets but no less than 10 work-days (see definition of “work-day” in paragraph above) prior to scheduled departure, may receive a refund less a $50 cancellation penalty charged by FPT and less the cost of the airline tickets and less any hotel cancellation penalties, if applicable. Written cancellation requests received by FPT less than 10 work-days prior to scheduled departure may not be entitled to a refund of any kind. No-shows are not eligible to rebook and are not entitled to a refund of any kind. If, after paying a deposit or full payment, FPT notifies you that your reservation is “not available”, you may request a full refund of all money previously paid to FPT towards this reservation, without penalty, or you may apply all money towards an alternate booking of this offer, subject to availability, without penalty. If you request a full refund due to nonavailability, the refund will be issued within 14 days. RESPONSIBILITY It is your responsibility to ensure that, when traveling to the Caribbean, Mexico or Europe, you and all members of your party possess a valid passport or other identification that you have verified in advance with the airline will be acceptable to the country you are visiting.
