Our primary focus is to support the patient and their family through the dying process. The Pathways Hospice Care Center serves Larimer County and Western Weld County and provides the choice of inpatient care when it is not possible or advisable for patients to remain at home. Initial and on-going stay at the Pathways Hospice Care Center is based upon established Hospice Medicare and insurance acute care guidelines. Pathways Hospice Care Center Staff to help provide expert medical care to relieve pain and suffering. The Hospice Team will collaborate with the patient’s primary care physician to provide patient goal-focused care and coordinate transition of care if the patient’s condition stabilizes. A family conference is available with our Interdisciplinary Team, should you desire to discuss your loved one’s care in greater detail or need to discuss more complex medical or social issues. To better serve your loved one, please communicate directly with the Pathways Hospice Staff Nurse or Care Center Director. We’re here to help. Thank you. The Rooms We offer care 24 hours a day, seven days a week with a Registered Nurse, Licensed Practical Nurse and Certified Nurse Aide. Additional Hospice Care Team staff, including social workers, counselors, chaplains, volunteers and music therapists are available to meet your needs as they arise. The Pathways Hospice Medical Director is available Located on the third floor of McKee Medical Center in Loveland, the Pathways Hospice Care Center rooms have been especially designed with our patients’ needs in mind. Each room has a CD/radio/TV/VCR and phone. Phone calls will be routed to the nurses’ station for answering and then forwarded to the patient’s room. For guests who would like to stay the night, there is a fold-out chair in each room. Debbie Boose Memorial Family Room A patient room at Pathways Hospice Care Center Visitors Visitors are welcome 24 hours a day, with patient and/or family consent. The cafeteria, with vending machines, is located on the ground floor of the hospital and has posted cafeteria hours for meals. McKee Medical Center is a non-smoking facility. Visitors are not allowed to consume alcohol on the premises. Pets may visit with veterinarian authorization. The Debbie Boose Memorial Family Room provides a quiet space for families visiting the Pathways Hospice Care Center. A microwave, refrigerator, TV/DVD/VCR, games, books, high-speed internet access and information on local restaurants and motels are available. View of the courtyard from a west patient room
