1. Collaborate early and often – between and among
CCACs, providers and the OACCAC
2. Investment in resources for planning will result in
effective implementation
3. Adapt your business processes to new CHRIS and
HPG-CHP functionalities including some workarounds.
4. Don’t underestimate change management required –
fundamental shift!
5. OBP is the “right concept” – focusing on the outcomes
and shared accountability
