Declaration by candidate:
1. I certify that the information on this form is complete and accurate to the best of my knowledge and that
I have referred to the syllabus to ensure that I am registering for the correct subject and option.
2. I understand that I am liable to pay fees for any entry/amendment made after the initial registration.
Once paid, the external exam fees are non-refundable.
3. I have not and will not make an entry for chosen subject/s or different subject/s at any Centre/School/Exam
Board which may result in a timetable clash. The Centre will not facilitate for clash between subjects/
boards, as separate venues will be used during this session.
4. I understand that in order to achieve the “equivalency certificate from the Ministry of Education”
‫ ﺷﻬﺎﺩﺓ ﺍﻟﺛﺎﻧﻭﻳﺔ ﺍﻟﻌﺎﻣﺔ‬a student must satisfy the following criteria:
Achieve at least 5 IGCSEs (or 5 O-Level’s) subjects (at Grade E or above) prescribed by Cambridge
International Examinations. A list of these subjects can be found on www.cie.org.uk .

6. Fees mentioned within the form are neither refundable nor carried forward to next examination
session under any circumstances.
7. I shall collect my original Statement of Entry (hardcopy) 2 weeks before the examination session
begins. I shall also acknowledge the receipt of the same by signing on a copy of the statement.
8. The School reserves the right to disallow me to take the examination if they are not satisfied with
the identification produced during the examinations.

10. I am aware that as a private candidate, my certificate will not be endorsed by Abu Dhabi Education
Council (‫ )ﻣﺠﻠﺲ ﺃﺑﻮﻅﺒﻲ ﻟﻠﺘﻌﻠﻴﻢ‬or by the Ministry of Education (MOE) in the United Arab
Emirates(U.A.E) and I will not be able to continue my studies in any private school in the U.A.E.

Completing the form

This form must be completed and signed by the school & private candidates.

We cannot accept any enquiry requests from individual candidates, parents or over the telephone.

Select the subjects/syllabus options that you wish to take by writing () if you are taking the exam for
the first time in Grade 10GCE in the boxes next to the provided list of subjects.

She will be available between 8:30 a.m. to 3:30 p.m on Sunday,14 December & Monday /15th
December 2014 in Grades 7 - 12 Reception Area.

We will charge an entry fee per syllabus per candidate.
We will charge late entry fees for any syllabus entries submitted after the final entry closing date.
Please note that a late fee will be charged for each amendment; if an entry option is changed twice, then two sets
of late fees will be charged.

The deadlines for submitting this form for Cambridge IGCSE, Cambridge O Level, Cambridge
International AS and A Level entries are:

There is an administrative fee for any requests to amend candidate names received after 10 February 2015.
Therefore, kindly adhere to the stipulated deadline.
