Attend a personal or phone interview and on-campus orientation session at the institute. 2. Copy of high school diploma or equivalent 3. Provide two of the following as proof of identification: a. Valid state identification from any state b. Valid state driver’s license c. Valid military identification d. Valid passport 4. Must be at least 18 years of age, or provide express written consent for enrollment from a parent of legal guardian. 5. Complete the enrollment agreement. 6. Provide 4 passport pictures 7. Credit Policy: If an incoming student holds a current and valid license as a Cosmetologist or Nursing Professional, they may receive 250 hours of credit towards the completion of an IMAj course of study of 600 hours or more. This institution will inquire about each veteran‘s previous education and training, and request transcripts from all prior institutions, including military training, traditional college coursework and vocational training. Previous transcripts will be evaluated and credit will be granted, as appropriate. 8. Pay the appropriate registration fees, deposits and kit fees. 9. Completion of the total course hours for all class courses with a percentage grade average of 80% or above. 2. Successfully complete all practical applications with proper documentation. 3. All fees and tuition due to the institute must be paid in full. A Certificate of Graduation will be presented when the student completes the required number of clock hours, achieves an 80% GPA in all classes, and all course requirements have been fulfilled. Transfer Students IMAj Institute may accept transfer students. All students wishing to transfer into the Institute may be required to complete an entrance exam, pay the $250 registration fee and purchase a full student kit. The course fees are based on the number of clocked hours. Transfer students must present to the Admissions Office a transcript exhibiting a minimum grade average of C or a 2.5 cumulative GPA from a licensed school of Cosmetology or Aesthetics. Once a transfer request has been processed by the Admissions Office, the applicant has the right to appeal the decision in writing directly to the school’s Director for additional consideration. Please contact the IMAj Admissions office for more information. Tuition Financing and Payments IMAj Institute accepts cash, check, VISA/MC/AMEX credit card payments. A deposit of at least 50% or confirmation of financing/scholarship(s) of all class fees must be received by the Institute before the first day of the class. All additional balances must be paid in full by the time the student has completed 50% of their program. In the event of a delinquent tuition payment, the student will be temporarily withdrawn from their program until the balance is satisfied, and all books and student kit items will remain the property of the Institute until payment in full is received. This course is Arizona Radiation Regulatory Agency approved, and also includes Laser Safety Officer (LSO) training and certification. Students are required to maintain a grade point of 80% for both academic and attendance. 2. Students who fall below the standards will not be given a certificate of completion. The student will need to bring the grade up by retaking the examination(s) and achieving an average of 80% or greater. Students will be provided 2 additional opportunities to retake a class section and improve with a passing grade of 80%. If a student fails to do so they will be terminated. Tutoring is available – additional charges may apply. 3. Certificate of completion will not be given until class(es) missed are made up at a later date. 4. The practical operations will be evaluated on a one on one basis. 5. If a student fails 2 tests in a single unit, they may, at the Instructor’s discretion, be required to retake that unit. Clocked Hours 1. The school will record appropriate credit for all clocked hours while in attendance at the Institute. 2. The school provides that student’s total hours for each day shall be according to the timecard. Dress Code While attending IMAj Institute it is important to always look and dress professional. A dress code is required for all our students has been adopted; 1. IMAj logo scrub top & pants Mondays- Wednesdays, IMAj T-Shirt w/scrub pants on Thursdays. 2. Tennis/Athletic shoes or nurse shoes. Trim fingernails and no nail polish or facial piercings. 3. Hair longer than shoulder length must be pulled back. 4. The following rules of attendance must be followed by each student; 1. IMAj Institute’s administrative offices and its classrooms are open from Monday thru Thursday from 8:00am to 5:00pm. Students will be given two 15 minute breaks per day. 2. Classes begin promptly according to the class schedule. The school adheres strictly to this policy. Students who are absent must telephone and report their absence to the Institute 1 hour prior to the class start time and leave a message on the answering service or speak with a staff member. Attendance records are maintained by digital time clock. Students & supervising agencies may request copies of student attendance records once monthly. Clocking in late in the morning or after lunch will result in the loss of clocked time. It is the duty of the institute to prepare students for the professional work environment. Tardiness is not tolerated in the work environment and not at the school. If a student is not clocked in by 8:00 am according to the school time clock then they are considered tardy. After 3 tardies the student may be suspended for one day. 3. Absences are highly discouraged. If a student misses a scheduled class day, the student will have to wait until the class is offered again and return for the day that was missed. The student will not receive a certificate of completion until all the course hours are completed, and as long as they have maintained an attendance rate of 80% or better. If a student misses more than 4 days of class during any single unit, they will be required to retake that unit in its entirety. 4. Students are allowed up to 8 days of absence within the 750 hour program. If a student is absent in excess of 8 days during their period of enrollment without obtaining an authorized leave of absence for a minimum of 2 weeks, the student will be assessed a fee of $25 for each hour missed. This assessment must be paid in full prior to receiving a diploma or certifications. 5. The school reserves the right to dismiss any student whose attendance, behavior, performance or professionalism interferes with the learning environment of the school as a first and final disciplinary step. In the event the student fails to inform the school of consecutive absences in excess of 2 days, the student’s enrollment will be terminated. Leave of absences are a minimum of 2 weeks, cannot exceed half the total program length in calendar days, and must be requested in advance of the beginning date of the leave of absence, unless unforeseen circumstances prevent the student from doing so. The student must sign, date and attest to the reason for the leave of absence. The reason must be specified in order for the institution to have a reasonable expectation of the student’s return within the timeframe of the leave of absence as requested. If a student does not request a leave of absence within a timeframe consistent with the institution’s consecutive absence policy, s/he must be withdrawn. A student may request to extend the leave of absence if needed, to a maximum of half the total program length, and the request will be considered on a case by case basis. There will be no additional charge for the leave of absence that has been approved by the institute. Upon the students return, an addendum will be written with an updated estimated graduation date. In the event a student re-enters into a class after a leave of absence, the student will have to start over in the particular unit the leave of absence was taken in. Termination IMAj Institute reserves the right to dismiss any student who has exhibited conduct deemed to be, at the discretion of the school, detrimental to the learning environment and provisions or completion of class activities. Detrimental conduct may include excessive absence, tardiness or behavior determined by the school to be inappropriate and unprofessional. Appeal Process A student may appeal a determination when not making satisfactory academic progress by submitting a letter to the Institute’s Director. The letter of appeal should identify how the institute has incorrectly assessed the student’s academic process as defined by this policy, or any special circumstances that the student would like to be considered in the application of the policy. All appeal decisions are final and are documented and retained in the student’s file. The Director will provide a written response and any resolution based on the established policies of the school within 3 days. If the matter is not resolved to the satisfaction of the student, the student may then appeal the grievance in writing to the Arizona State Board of Cosmetology at 1721 E. Broadway, Tempe, Arizona 85282, 480-784-4539, as well as the school’s accrediting agency (pending), ACCET, at 1722 N Street NW, Washington, D.C., 20036, 202-955-1113. Cancellation and Refund Policy If the school does not accept the applicant for enrollment into the course, the applicant shall be entitled to a refund of all monies paid, including any registration fees paid. The contract and student’s enrollment can be terminated only by written notice of cancellation from one party to the other. Said notice shall be mailed, postage paid, either registered or certified, return receipt requested, to the other party at the address given by either party. The refund policy is as follows: 1. If a course is cancelled subsequent to a student’s enrollment, the school will provide a full refund of all monies paid within 30 business days. 2. If the school feels the need to terminate a student due to behavior, professionalism, rudeness or by interrupting the learning process, or unsatisfactory progress, which is defined as an interruption of the learning process and/or the wasting of school resources resulting from a lack of reasonable progress by a student, the student forfeits all fees paid to the school. Kits and supplies purchased by the student are not refundable once they have been issued to the student. Registration fees are non-refundable. Standards of Progress Policy Standard of progress is assessed on a continual basis. At the end of each unit (monthly) the student must have completed all requirements for the previous unit before progressing to the next unit. The student is given until the start day of the new unit to turn in all requirements or complete tests. A student cannot continue onto the next unit without completing all requirements for the previous unit. All tests muct be completed with an 80% or above for passing. A student is allowed two chances to pass each test. If a student does not pass a test in two chances the Instructor will review the curriculum with the student to see where the lack of understanding may be, then a test with reasoning questions is given. A student cannot graduate without all requirements (assignments, quotas, hours and testing, as well as being financially responsible) being met. Student Records Upon receipt of written request, copies of student records will be furnished to the requesting student within 10 business days and in accordance with the Family Education Rights and Privacy Act (FERPA). A student may request personal pick-up or mail delivery of any requested records. Academic records may include, but are not limited to, examination grades and enrollment or withdrawal records. The legal penalties for copyright infringement may include: Infringer pays the actual dollar amount of damages and profits. The law provides a range from $200 to $150,000 for each work infringed. Infringer pays for all attorney’s fees and court costs. The Court can issue an injunction to stop the infringing acts. The Court can impound the illegal works. The infringer can go to jail. Placement Assistance IMAj offers job placement assistance to students and graduates by providing a Job Board on the school’s website that is updated no less than once weekly.
