The field of aesthetics is one of the fastest growing industries worldwide. For 2015, US News & World
Report ranked skin care professionals as one of the top 25 jobs. The desire for anti-aging and high
technology skin care services has increased at an incredible rate over the past decade and promises to
continue well into the future. This demand has created a strong need for highly trained clinical
aesthetic professionals. Medical spas and other aesthetics practices have become common place. The
fusion of the beauty and medical industries has provided for the emergence of numerous safe and
effective solutions to many common aesthetics needs. Advanced aesthetic procedures including the use
of laser, light, and energy based therapy devices create a specific and often significant enhancement
to one’s physical appearance and self-image.
IMAj Institute has an innovative curriculum that has truly defined “The Future of Aesthetics Education” by
focusing on the in-depth study of clinical aesthetics. IMAj offers a large state-of-the-art campus with
an abundance of tools and resources, yet limits class sizes to allow for individual attention for each

IMAj Institute campus in Scottsdale, Arizona, was established to provide clinical research and
education services in an effort to standardize training for result-oriented face and body clinical
aesthetics procedures. IMAj Institute provides support to numerous manufacturing firms and aesthetic
& medical practices in the United States, England, Germany, Canada, China, Spain, and South
America. Our specialties include:

worldwide. Scottsdale is a popular vacation destination with endless amenities, and is an ideal

IMAj Institute is designed to provide a professional clinical environment for a true work experience
atmosphere. IMAj is equipped with state-of-the-art aesthetic equipment such as lasers, intense pulse
light (IPL), radio frequency (RF), high-frequency micro-current (HFMC) technologies, light emitting
diodes (LED), microdermabrasion, galvanic and high-frequency machines, facial steamers, magnifying
lamps, and facial beds.
IMAj features a variety of clinical training classrooms and a fully
operational medically supervised medical spa for a unique and comprehensive real world training
experience.

IMAj Institute provides a study area with books and information for student use, as well as a student
lounge, covered parking, and outdoor patio area. Students may at any time seek the assistance of
the school’s Director or Admissions Office. IMAj Institute is proud to feature Dermalogica skin care
products as well as the IMAGE International professional grade skin care product line for facials and
anti-aging treatments.
As a medically supervised and ITEC accredited aesthetics school, IMAj
Institute has the unique opportunity to provide instruction and student use of both traditional and
clinical grade skin care product lines.

To be considered for admission as a student to IMAj Institute the student must be accepted by the
Admissions Office and must fulfill the following requirements:
1.
Attend a personal or phone interview and on-campus orientation session at the institute.
2.
Copy of high school diploma or equivalent
3.
Provide two of the following as proof of identification:
a. Valid state identification from any state
b. Valid state driver’s license
c. Valid military identification
d. Valid passport
4. Must be at least 18 years of age, or provide express written consent for enrollment from
a parent of legal guardian.
5.
Complete the enrollment agreement.
6.
Provide 4 passport pictures
7.
Credit Policy:
If an incoming student holds a current and valid license as a
Cosmetologist or Nursing Professional, they may receive 250 hours of credit towards the
completion of an IMAj course of study of 600 hours or more. This institution will inquire
about each veteran‘s previous education and training, and request transcripts from all
prior institutions, including military training, traditional college coursework and
vocational training. Previous transcripts will be evaluated and credit will be granted, as
appropriate.
8.

9.

Course Certificate Requirements
1.
Completion of the total course hours for all class courses with a percentage grade average of
80% or above.
2.
Successfully complete all practical applications with proper documentation.
3. All fees and tuition due to the institute must be paid in full.
A Certificate of Graduation will be presented when the student completes the required number of
clock hours, achieves an 80% GPA in all classes, and all course requirements have been fulfilled.
Transfer Students
IMAj Institute may accept transfer students. All students wishing to transfer into the Institute may be
required to complete an entrance exam, pay the $250 registration fee and purchase a full student
kit. The course fees are based on the number of clocked hours. Transfer students must present to the
Admissions Office a transcript exhibiting a minimum grade average of C or a 2.5 cumulative GPA
from a licensed school of Cosmetology or Aesthetics.
Once a transfer request has been processed
by the Admissions Office, the applicant has the right to appeal the decision in writing directly to the
school’s Director for additional consideration. Please contact the IMAj Admissions office for more
information.
Tuition Financing and Payments
IMAj Institute accepts cash, check, VISA/MC/AMEX credit card payments. A deposit of at least 50%
or confirmation of financing/scholarship(s) of all class fees must be received by the Institute before
the first day of the class. Please call the IMAj Admissions department at (480) 361-8585 or email us
at [email protected] to speak with an Admissions Advisor for more information about
available tuition financing options, including Vocational Rehabilitation, VA/GI Bills, and student loan
programs from Sallie Mae and Mountain America Credit Union. All additional balances must be
paid in full by the time the student has completed 50% of their program. In the event of a
delinquent tuition payment, the student will be temporarily withdrawn from their program until the
balance is satisfied, and all books and student kit items will remain the property of the Institute until
payment in full is received. Students will be provided with one calendar year to resolve delinquent
payments before being permanently withdrawn from their program.

IMAj Institute offers training programs designed to prepare and educate each student in the
fundamentals of traditional and advanced aesthetics specifically suited for the medical spa
environment. IMAj Institute adheres to the guidelines of the Arizona State Board of Cosmetology,
OSHA, CDC, ITEC, ACCET and the ARRA as the foundation for its curriculum. If you are considering a
career as a skin care professional, IMAj Institute offers unmatched experience and resources to help
guide to a successful career in aesthetics. As the first school in the nation to teach clinical aesthetics,
IMAj Institute sets the standard for the future of the skin care industry.
All classes are taught within a set, structured curriculum, and the training will prepare students to seek
employment in salons, spas, physician offices, laser centers, medical spas, as a skin care product
representative or to become self-employed. It is the philosophy of IMAj Institute to stress excellence
in all treatment applications. Classes are complimented by lectures, videos, demonstrations, guest
educators and hands on application with spa guests.

Clinical Aesthetics
As our most popular course of study, Clinical Aesthetics is the ideal career track program for aspiring
Aestheticians who would like to pursue career opportunities in a more medically oriented environment, such as a
dermatology office, plastic surgery center, medical spa or laser center. The Clinical Aesthetics course features a
strong emphasis on results-oriented anti-aging and skin care treatments, and prepares students to work with
advanced technologies, including Lasers & Intense Pulsed Light.
Traditional Aesthetics
Do your interests lie more in the field of traditional aesthetics and the spa & salon setting? While IMAj is world
renowned for its clinical aesthetics education programs (in fact, we own the trademark for the term “Clinical
Aesthetics”), many students choose IMAj to complete their Aesthetics License training as a result of the
comprehensive nature of the curriculum of this 600 hour program and the ability to obtain training in advanced
spa techniques such as microdermabrasion, LED therapies, dermal planing, advanced chemical peels, cellulite
reduction, and high-frequency micro-current body sculpting & skin tightening.
Aesthetics Essentials
IMAj Institute Aesthetics Essentials course is the most cost-effective means of preparing for your Aesthetician
License. This 600 hour course covers all the essential classroom and clinical instruction needed to successfully
obtain your license and launch your new career and a skin care specialist. As with all of the Aesthetics career
track courses at IMAj Institute, the tuition fees for the Aesthetics Essentials class includes a complete student supply
kit, uniforms, and access to the the IMAj electronic classroom system.
Advanced Aesthetics
Do you currently hold a license as a Cosmetologist, Aesthetician, Nursing Professional or Physician, and would like
to complete training in advanced skin care services that include LED technologies, micro-current,
microdermabrasion, advanced chemical peels, professional make-up application, cellulite reduction, and highfrequency micro-current face & body sculpting? Advanced Aesthetics is designed to provide enhanced skills
training to professionals who would like to expand their service offerings, while minimizing their time away from
their spa, salon or office.

Cosmetic Laser Technician Certification
As one of the first schools in the nation to provide Cosmetic Laser Technician Certification training, IMAj Institute
Laser Tech course provides extensive classroom and hands-on training in small supervised class group settings in
applications and technologies, including laser hair reduction, laser tattoo reduction, laser peels, laser collagen
synthesis, laser vascular lesion reduction (i.e. spider veins), high-frequency micro-current facial and body sculpting,
and Intense Pulse Light treatments for facial rejuvenation & acne reduction. This course is Arizona Radiation
Regulatory Agency approved, and also includes Laser Safety Officer (LSO) training and certification. This course
is ideal for Cosmetology, Aesthetic, Nursing and other medical professionals who would like to expand their
career options to include the provision of in-demand non-invasive skin care treatments with energy-based
devices.

Aesthetic Instructor License Training
500 hour Instructor License course that includes training in lesson plan preparation & implementation, as well as
hands-on clinical instruction. Monday – Thursday, 8:00am – 5:00pm for 16 weeks. $5,000.00 enrollment fee.
Cosmetic Laser Technician Supervisor Certification Training
500 hour Instructor License course that includes training in lesson plan preparation & implementation, as well as
hands-on clinical instruction. Monday – Thursday, 8:00am – 5:00pm for 16 weeks. $5,000.00 enrollment fee.
Fast Track Laser Certification Course
The Fast Track Laser Certification Course is designed for students who would like to accelerate their educational
experience and benefit from a flexible and abbreviated class schedule. The Fast Track course consists of the
completion of a self-paced online 40 hour lecture series (includes LSO), followed by 8 days of on-campus
comprehensive hands-on training in a myriad of energy-based devices and applications, including Lasers, IPLs,
and HFMCs. Monday – Thursday, 8:00am – 5:00pm for 2 weeks. $4,500.00 enrollment fee.
Medical Director Certification Training
24 hour ARRA approved Certification Course designed to prepare Physicians to supervise cosmetic medical
facilities. Monday – Wednesday, 8:00am – 5:00pm for 1 week. $2,500.00 enrollment fee.

Academic Evaluation System
The IMAj Institute operates on the following grading system:
80% or Greater = Pass
79% or Lower = Fail
1. Students are required to maintain a grade point of 80% for both academic and attendance.
2. Students who fall below the standards will not be given a certificate of completion. The
student will need to bring the grade up by retaking the examination(s) and achieving an
average of 80% or greater. Students will be provided 2 additional opportunities to retake a
class section and improve with a passing grade of 80%. If a student fails to do so they will be
terminated. Tutoring is available – additional charges may apply.
3. Certificate of completion will not be given until class(es) missed are made up at a later date.
4. The practical operations will be evaluated on a one on one basis.
5. If a student fails 2 tests in a single unit, they may, at the Instructor’s discretion, be required to
retake that unit.
Clocked Hours
1. The school will record appropriate credit for all clocked hours while in attendance at the Institute.
2. The school provides that student’s total hours for each day shall be according to the timecard.
Dress Code
While attending IMAj Institute it is important to always look and dress professional. A dress
code is required for all our students has been adopted;
1. IMAj logo scrub top & pants Mondays- Wednesdays, IMAj T-Shirt w/scrub pants on Thursdays.
2. Tennis/Athletic shoes or nurse shoes. Trim fingernails and no nail polish or facial piercings.
3. Hair longer than shoulder length must be pulled back.
4. Students must wear a name badge, which will be provided by IMAj, at all times while on
campus.

Attendance
Attendance to all classes is vital to the learning environment of each student. The following rules of
attendance must be followed by each student;
1. IMAj Institute’s administrative offices and its classrooms are open from Monday thru Thursday from
8:00am to 5:00pm. Students will be given two 15 minute breaks per day.
2. Classes begin promptly according to the class schedule.
The school adheres strictly to this policy.
Students who are absent must telephone and report their absence to the Institute 1 hour prior to the class
start time and leave a message on the answering service or speak with a staff member. Attendance
records are maintained by digital time clock. Students & supervising agencies may request copies of
student attendance records once monthly. Clocking in late in the morning or after lunch will result in the
loss of clocked time.
It is the duty of the institute to prepare students for the professional work
environment. Tardiness is not tolerated in the work environment and not at the school. If a student is not
clocked in by 8:00 am according to the school time clock then they are considered tardy. After 3 tardies
the student may be suspended for one day.
3. Absences are highly discouraged. If a student misses a scheduled class day, the student will have to wait
until the class is offered again and return for the day that was missed. The student will not receive a
certificate of completion until all the course hours are completed, and as long as they have maintained
an attendance rate of 80% or better. If a student misses more than 4 days of class during any single
unit, they will be required to retake that unit in its entirety.
4. Students are allowed up to 8 days of absence within the 750 hour program. If a student is absent
in
excess of 8 days during their period of enrollment without obtaining an authorized leave of absence for
a minimum of 2 weeks, the student will be assessed a fee of $25 for each hour missed. This assessment
must be paid in full prior to receiving a diploma or certifications.
5. The school reserves the right to dismiss any student whose attendance, behavior, performance or
professionalism interferes with the learning environment of the school as a first and final disciplinary step.
In the event the student fails to inform the school of consecutive absences in excess of 2 days, the
student’s enrollment will be terminated. A letter of termination will be sent on the 3 rd day to the address
the student provided on the enrollment contract.

Leave of Absence
A leave of absence can be submitted to the Institute in writing for approval. Leave of absences are a
minimum of 2 weeks, cannot exceed half the total program length in calendar days, and must be
requested in advance of the beginning date of the leave of absence, unless unforeseen circumstances
prevent the student from doing so. The student must sign, date and attest to the reason for the leave of
absence. The reason must be specified in order for the institution to have a reasonable expectation of
the student’s return within the timeframe of the leave of absence as requested. If a student does not
request a leave of absence within a timeframe consistent with the institution’s consecutive absence
policy, s/he must be withdrawn. A student may request to extend the leave of absence if needed, to a
maximum of half the total program length, and the request will be considered on a case by case basis.
There will be no additional charge for the leave of absence that has been approved by the institute.
Upon the students return, an addendum will be written with an updated estimated graduation date. In
the event a student re-enters into a class after a leave of absence, the student will have to start over in
the particular unit the leave of absence was taken in.
Termination
IMAj Institute reserves the right to dismiss any student who has exhibited conduct deemed to be, at the
discretion of the school, detrimental to the learning environment and provisions or completion of class
activities. Detrimental conduct may include excessive absence, tardiness or behavior determined by
the school to be inappropriate and unprofessional.
Appeal Process
A student may appeal a determination when not making satisfactory academic progress by submitting a
letter to the Institute’s Director. The letter of appeal should identify how the institute has incorrectly
assessed the student’s academic process as defined by this policy, or any special circumstances that the
student would like to be considered in the application of the policy. All appeal decisions are final and
are documented and retained in the student’s file. Should the appeal be decided in the student’s favor,
documents would be recorded in the student’s file.

Grievance
Students must file any and all grievances in writing and provide them to the Institute’s Director. The
Director will provide a written response and any resolution based on the established policies of the
school within 3 days. If the matter is not resolved to the satisfaction of the student, the student may then
appeal the grievance in writing to the Arizona State Board of Cosmetology at 1721 E. Broadway,
Tempe, Arizona 85282, 480-784-4539, as well as the school’s accrediting agency (pending), ACCET, at
1722 N Street NW, Washington, D.C., 20036, 202-955-1113.
Cancellation and Refund Policy
If the school does not accept the applicant for enrollment into the course, the applicant shall be entitled to
a refund of all monies paid, including any registration fees paid. The contract and student’s enrollment
can be terminated only by written notice of cancellation from one party to the other. Said notice shall be
mailed, postage paid, either registered or certified, return receipt requested, to the other party at the
address given by either party. The refund policy is as follows:
1. If a course is cancelled subsequent to a student’s enrollment, the school will provide a full refund of all
monies paid within 30 business days.
2. If the school closes for business and is no longer offering instruction after a student has enrolled and
prior to the contracted end date, the student is entitled to a percentage based refund of tuition.
3. If either party cancels the Enrollment Agreement after the student enters class, an applicable
percentage based refund will be provided as follows within 60 days or receipt of request to terminate
enrollment, including any Registration Fees paid:

Any monies due to the applicant/student shall be refunded within 60 business days of formal
cancellation. The formal written cancellation is required to occur no more than 3 days from the last day
of physical attendance. The postmark on written notification determines the formal cancellation date of
the date said information is delivered in person to the Director.
Enrollment time is defined as the time elapsed between the actual starting date and the date of the
student’s last day of physical attendance in the course. If the school feels the need to terminate a student
due to behavior, professionalism, rudeness or by interrupting the learning process, or unsatisfactory
progress, which is defined as an interruption of the learning process and/or the wasting of school
resources resulting from a lack of reasonable progress by a student, the student forfeits all fees paid to
the school. The school reserves the right to deny enrollment to any student for any reason.

Unless a student is specifically denied enrollment or terminated, the IMAj Institute provides an “open
ended” learning environment that allows students to re-attend any classes or school activities that they
have previously attended at any time without additional cost to the student to facilitate an optimal
educational experience.
Kits and supplies purchased by the student are not refundable once they have been issued to the student.
Registration fees are non-refundable.
Standards of Progress Policy
Standard of progress is assessed on a continual basis. At the end of each unit (monthly) the student must
have completed all requirements for the previous unit before progressing to the next unit. The student is
given until the start day of the new unit to turn in all requirements or complete tests. A student cannot
continue onto the next unit without completing all requirements for the previous unit. All tests muct be
completed with an 80% or above for passing. A student is allowed two chances to pass each test. If a
student does not pass a test in two chances the Instructor will review the curriculum with the student to see
where the lack of understanding may be, then a test with reasoning questions is given. A student cannot
graduate without all requirements (assignments, quotas, hours and testing, as well as being financially
responsible) being met.
Student Records
Upon receipt of written request, copies of student records will be furnished to the requesting student
within 10 business days and in accordance with the Family Education Rights and Privacy Act (FERPA). A
student may request personal pick-up or mail delivery of any requested records. Academic records may
include, but are not limited to, examination grades and enrollment or withdrawal records. All Student
records are maintained in the office of the Director.

Copyright Infringement
Students may face civil and/or criminal liabilities for any unauthorized use and/or distribution of the
Institute’s or its vendors or partners protected materials.







The legal penalties for copyright infringement may include:
Infringer pays the actual dollar amount of damages and profits.
The law provides a range from $200 to $150,000 for each work infringed.
Infringer pays for all attorney’s fees and court costs.
The Court can issue an injunction to stop the infringing acts.
The Court can impound the illegal works.
The infringer can go to jail.
Placement Assistance
IMAj offers job placement assistance to students and graduates by providing a Job Board on the school’s
website that is updated no less than once weekly. Additionally, programs of 600 hours or more include
a professional development class designed to prepare students for the job interview process and best
practices for maintaining a successful career.

Carol Ann has traveled worldwide as an Instructor for Botox Cosmetic, Juvederm and other injectable products,

Mr. Danosky, a former Nuclear Submarine Mechanic and USN veteran is the gentleman behind the scenes. Joe is
currently a Licensed Cosmetologist, Laser Technician, and Laser Safety Officer, and works daily to maintain and
repair all of our lasers and clinical aesthetic equipment. When he is not busy with that, he his diligently
developing and prototyping new equipment. This not only puts our school on the forefront of new technology, but

X_______ PURCHASE OF EDUCATION SERVICE: This is a contract for the purchase of aesthetics educational services from IMAj
Institute (IMAj). The Laser Technician Course is a continuing education program, &enrollment is limited to licensed
cosmetology or healthcare professionals.
X_______ DRESS CODE: Students are required to wear the IMAj issued top, scrub pants and tennis/athletic/nursing shoes.
X_______ PAYMENT: All payments are to be made according to the above schedule to IMAj. A 50% deposit or confirmation of
student loan approval is due 30 days prior to scheduled class start date. All fees due must be paid in full prior to start date.
X_______ NOTICE OF CANCELLATION: This contract and student’s enrollment can be terminated by either party with written
notice of cancellation to the other party. Said notice shall be mailed, postage pre-paid, either registered or certified, return
receipt requested, to the other party at the address set out on the first page of this contract or delivered to the other party in
person. This cancellation shall be effective three (3) business days after receipt by the party to whom it is directed.
X_______ REFUND POLICY: The contract and student’s enrollment can be terminated only by written notice of cancellation from
one party to the other. Said notice shall be mailed, postage paid, either registered or certified, return receipt requested, to the other
party at the address given by either party. All refunds due to be provided by the school per the refund policy described herein shall be
paid within 45 calendar days of receipt of written request by the student. The refund policy is as follows:
a. If the Institute rejects the applicant for enrollment, the applicant shall be entitled to a refund of all monies paid.
b. If a course is cancelled subsequent to a student’s enrollment, the Institute will provide a full refund of all monies paid.
c. If the Institute closes for business and is no longer offering instruction after a student has enrolled and prior to the contracted end
date, the student is entitled to a percentage based refund of tuition as described herein.
d. If the student “no shows” and never begins class, a full refund of all monies due will be paid per the refund policy described
herein.
e. If either party cancels the Enrollment Agreement after the student enters class, an applicable percentage based refund will be
provided per the terms detailed herein within 45 calendar days of receipt of request to terminate enrollment:
The formal written cancellation shall occur no more than 3 days from the last day of physical attendance. The postmark on
written notification determines the formal cancellation date.

X_______ ABSENCES: If the student misses a scheduled class, the student will have to wait until the class is scheduled again to make
up the days missed. Students will be assessed $25 per hour for any hours missed in excess of 64 total hours over the
duration of their time of enrollment.
X______ GRADUATION REQUIREMENTS: The student must clock the appropriate course hours and maintain a GPA 80% in both
academic and attendance. Please note that the contents of the Student Kit will not be relinquished to the student until they
have obtained their professional license.
X_______ DIPLOMA: A Diploma of Graduation is given only when the student has completed the required amount of course clocked
hours and all other requirements set forth by IMAj.
X_______ CLOCKED HOURS: The student will clock in and out with the school time clock to document the actual amount of
classroom time credited to the student.
X_______ JOB PLACEMENT: IMAj does not guarantee placement for employment. The IMAj will assist the student in seeking out
employment when employment is available.
X_______ TERMINATION BY IMAj FOR CAUSE: IMAj may terminate this Agreement without notice for any breach of this
Agreement. The student’s right to a refund is governed by paragraph 5 below.
X_______ GRIEVANCE: Students must file any all grievances in writing and provide them to the school’s Director of Education. The
DOE will provide a written response and any resolution based on the established policies of the school within 3 days. If the
matter is not resolved to the satisfaction of the student, the student may then appeal the grievance in writing to the Arizona
State Board of Cosmetology at 1721 East Broadway, Tempe, Arizona 85282, 480-784-4539.
X_______ RESPONSIBILTY FOR DAMAGE, INJURY OR COLLECTION CHARGES: IMAj shall not be responsible to the student
for any negligence, carelessness, or lack of skill by the student, or any one or more students practicing any instruction of
techniques. Student agrees to indemnify and hold IMAj harmless from any claim, liability of damages arising our of
student’s failure to perform this agreement of student’s failure to follow any of IMAj rules or policies.
X_______ ATTORNEY’S FEES: In the event IMAj retains counsel to enforce any of the provisions of this agreement, student will pay
for IMAj actual attorneys’ fees and cost.
X_______SCOPE OF AGREEMENT: Student acknowledges having received a copy of this agreement. This agreement contains the
entire agreement between the student and IMAj. There are no other promises or representations other than what is
contained herein.
