Transitional Care (Swing Bed) Program
Culbertson Memorial Hospital features a service known as transitional care, which is a short-term,
extended care program that patients may be eligible for following hospitalization for an acute problem.
If your condition has stabilized, but you are not ready to return home, you may be eligible for this
program.
In order to be admitted to the transitional care unit, you must meet the following criteria:
 Your transitional care must be related to an acute hospital stay
 You must need skilled nursing or skilled rehabilitative services on a daily basis
 You must have spent three overnight stays in acute care during the past 30 days
The following treatments are offered in Transitional Care:
 Occupational therapy
 Physical therapy
 Speech therapy
 Intravenous therapy
 Patient education
(such as learning about living with diabetes or how to care for a colostomy)

receive the special treatment they deserve and can discover how to handle life’s challenges and find joy
once again.
The issues affecting seniors are uniquely challenging. Declining physical ability, loss of loved ones and
isolation can lead to depression, anxiety and other issues. But problems like these are not normal at
any age. Senior Life Solutions fosters true healing. Our goal is to help patients safely return to their
optimal style of living.
Your treatment team is multi-disciplinary in design to ensure the best quality patient care possible for
those who trust us with their behavioral health needs. Along with the patient, family members, and the
family doctor, the team includes a Psychiatrist, Primary Therapist and Care Coordinator.
Program Description - Voluntary six to twelve week confidential Outpatient Geriatric Behavioral
Health Program that offers individualized treatment based on need. Treatments consist of group,
individual, and family therapy as well as medication management for behavioral health diagnosis.
The patients’ care is coordinated with the their primary care physician. Our program is compliant
with all Medicare and State regulations. Anyone can refer a patient when need is suspected.
