Target Audience
The primary target audience of the 26th Annual Congress includes Phlebologists, General
Surgeons, Vascular Surgeons, Interventional Radiologists, Interventional Cardiologists,
Dermatologists, Dermatological Surgeons, Internists, Family Practice Physicians, Plastic
Surgeons, Obstetricians/Gynecologists, Physician Assistants, Ultrasound Technologists,
Nurses, allied health and all other healthcare professionals currently or planning to care for
patients with venous disease.
Conference Objectives
After completion of this activity, participants should be able to:
»» Use selective testing modalities to diagnose chronic venous disease and insufficiency
of the abdomen, pelvis, and extremities or other venous pathology including deep vein
thrombosis and vascular malformations
»» Describe and demonstrate the various techniques for treating venous disease
»» Distinguish indications and contraindications for different treatment modalities
»» Measure and compare the outcome of the various techniques in treating venous disease
Evaluations and Certificates
In order to document your attendance and receive your certificate at the 26th Annual
Congress, you will need to complete an evaluation. This will permit the American College
of Phlebology (ACP) to continue to enhance your personalized professional development
and plan the ACP hosted 2013 World Meeting of the International Union of Phlebology and
2014 American College of Phlebology Annual Congress. Once you have completed the
evaluations, you will be able to access your certificate. Computer stations will be set-up onsite
at the conference to provide feedback immediately or, at your leisure when you return home.
Accreditation and Credit Statements
The American College of Phlebology is accredited by the Accreditation Council for
Continuing Medical Education to provide continuing medical education for physicians.
The American College of Phlebology is an approved Continuing Education Provider by the
California Board of Registered Nursing - Provider Number CEP 15857.
Disclosure of Conflict of Interests
The American College Phlebology requests all persons in a position to control the content of
an educational activity to disclose whether or not they have any relevant financial relationships

Advance registration is required to ensure that badges and tickets to sessions and social
activities will be available when you arrive at the Congress. Please use the registration form
provided or register online at www.acpcongress.org. Advance registration closes Sunday,
October 21, 2012. Any registrations received after this date will be processed at the on-site
rate. On-Site registration will be available, however only a limited number of courses and
social activities will be available.

CANCELLATION/REFUND POLICY
Registration fees, less a $175 administrative fee, will be refunded upon written notice of
cancellation to the headquarters office. In order to qualify for a refund of fees, your written
cancellation notice must be postmarked no later than Saturday, October 20, 2012. There will
be no refund of fees for written cancellations received after this date or for lack of attendance
without notification.

A confirmation will be sent to you via e-mail within one (1) week of receipt of your registration
form and PAID IN FULL registration fees. If you do not receive a confirmation, please contact

On the Blue Wave certified beach of the Atlantic, in
the vibrant city of Hollywood, Florida rests The Westin
Diplomat Resort & Spa; easily accessible by Fort
Lauderdale/Hollywood or Miami International Airports. The
resort is also ideally located near shopping and attractions.
The ACP has reserved a block of rooms at the hotel for
Thursday through Sunday. All room reservations must be
made by October 18, 2012. However, the group room
block is limited in size and will be filled on a first-come
first-served basis. Reservations received after the room
block is filled or after October 18, 2012 will be accepted on a space-available basis and
prevailing hotel rates. Don’t wait! Get your reservations in early!
In order to make reservations under the ACP group room block, please call 954.602.6000.
Be sure to identify yourself with the American College of Phlebology meeting when making
reservations. The hotel confirms the following guest room rates: Single $249 and Double
$249. Hotel room rates are subject to applicable state and local taxes, currently 11%.
Check-in time is 4:00 PM and check-out time is 12:00 PM. Guests arriving before 4:00 PM will
be assigned accommodations as they become available.

The nearest major airport is Fort Lauderdale/Hollywood International Airport (FLL), which is
