The management and staff of the Delta Dental of Arizona
Foundation are thrilled to report that the 2013 Golf4Smiles
Tournament raised over $90,000. 100% of these funds go
directly to provide essential oral health services such as dental
screenings, sealants, fluoride varnish applications, dental
supply distribution and oral health instruction to communities
throughout the state of Arizona. With over 200 golfers
participating on two courses at Grayhawk Golf Club
in Scottsdale, the event was bigger and better than
ever. The Foundation would not have realized such
success without the generous support of our sponsors,
volunteers, donors and board members – a very big

Golf4Smiles Tournament for the first time last year and
Senior Vice President and Regional Sales Manager, Jay Baer,
promises to be back. Not only did he learn he must bring a different
chipping wedge to the chipping contest so he can take another crack
at the prize, he is excited to be back in the Valley helping local dentists
achieve their career goals. After 12 years in Columbus, OH managing the
entire sales force for Bank of America, Jay is now running west coast operations
for the Practice Solutions division, which provides flexible and competitive
financing programs for various loan products. From start-up practice loans to
business consolidations loans, Jay provides a level of personalized attention,
convenience and simplicity to dentists so they can concentrate on their patients. You

Dr. Dale Hallberg
Dr. Dale Hallberg feels he has the best dental office staff in
Arizona and quite possibly, the entire world! They are always
ready and willing to help the needy from Give Kids a Smile
to AZDA Foundation’s Donated Dental Services to the Poore
Free Medical Clinic to the Mission of Mercy and even to those
in need that are treated off the street. So the team jumped
at the chance to participate in the Golf4Smiles Tournament.
“I’m always looking for a fun round of golf, but to be able to
contribute to the programs that DDAZ Foundation supports
to boot, it’s a win-win!” said Dr. Hallberg. “I would like to
encourage all the other DDAZ participating dentists and
dental offices to become involved in this great organization
and event.” We will always hold a spot in the tournament for
you, Dr. Hallberg! Thank you for your exuberant support of
oral health and DDAZ Foundation.

Dr. Feldhake occasionally takes a day off from providing
high-level dental care to appreciative patients at his
114th Street and Shea location or his 5310 W. Thunderbird location to play a round
of golf. Everyone at Delta Dental of Arizona Foundation is equally as appreciative
that he saves one day each spring to play in the Golf4Smiles Tournament. “I truly
feel the funds raised at the Golf4Smiles Tournament go to a good cause. There
are very few non-profit organizations where 100% of your donation – without
administrative expenses – goes directly to provide preventive treatment to children.”

Paul Zajas, founder of Dental Elite, a familyowned and operated business that has been

his first appearance at the Golf4Smiles Tournament
in May. “I had a great time, despite the fact that we were
awarded the prize for the highest score. If we were in a bowling
tournament, we would have won!” said Zajas. “Despite that, I’m
proud to support such a worthwhile cause.” He is also proud of the fact

Dental Elite
Risa means “grin” in Spanish and Dr. Nicolas Porter, CEO and Founder of Risas Dental, has given many people reason to smile. Not only
has he participated in the Golf4Smiles Tournament since he started
the company in 2011, making those who support the mission of the Delta
Dental of Arizona Foundation smile, but he built Risas Dental with the goal
of making dentistry affordable for everyone in the community – thus more smiles! To help his
patients find their best smile, Risas Dental conducts “Regalando Risas” (Giving Away Smiles)
every Labor Day and during the opening of a new Risas Dental office. During Regalando Risas,
the company provides one free dental procedure (cleaning, exam and x-ray, filling or an extraction) to patients on a first-come, first-served basis from 8am-noon. That’s a lot of smiles!
Greg Harris, Partner at Lewis
Roca Rothgerber, has worked
closely with Delta Dental of Arizona
since 1999. During that time, he has
led the firm’s participation Golf4Smiles and looks forward
to continued participation in this event. Mr. Harris enjoys
supporting Delta Dental of Arizona because the company

throughout the state. Going to the dentist isn’t a luxury,

Pacific
Dental
Services
Pacific Dental Services has over 50 affiliated dental
practices in Arizona and has a firm belief of giving back
to their community, which is why PDS participated
in the Golf4Smiles Tournament. In addition to
supporting the Delta Dental of Arizona Foundation,
PDS participated in “We Serve Day” where their
dentists, hygienists and staff provided much-needed,
free dental work totaling over $350,000 in August 2013
to patients in various local shelters. Their employees
also staff the “We Serve Bus” benefitting a wide variety
of communities throughout the Valley, including Mesa
Care and the Salvation Army just this fall. Dr. Ian
Young of Tempe Modern Dentistry and Orthodontics,
an affiliated office of PDS, states, “My goal in giving
back is to make every person’s life that I come into
contact with just a little better than before they came
to see me. If we can get people to give just a little back,
we can improve the quality of life for so many.”
